Dear friends of Isabella,

2012 was a challenging year for Isabella.As with any challenge, there are always opportunitiesfor positive change and growth.While diminishedrevenue caused us to reduce some staff positionsduring the year, we added new services to meetgrowing community needs as well as improvedexisting programs.We are ever thankful to ourdedicated and caring staff who work togetherin good and bad times.

Nursing home based services were expanded byincreasing our capacity to care for those needingshort-stay rehabilitation or ventilator-dependent care.The ability of staff to communicate effectively andproblem solve was measurably increased by trainingin coaching communication presented in partnershipwith PHI. This vital training will extend to allIsabella staff and was made possible through agenerous grant provided by the United HospitalFund, the New York Community Trust and theFan Fox & Leslie R. Samuels Foundation.

Within our local community, we becamean anchor partner in the “Together on Diabetes”Program, a groundbreaking effort under the auspicesof the United Hospital Fund. This program offersa comprehensive approach which enables individualswith diabetes to properly manage their conditionthrough education, counseling and access tocommunity resources.

Isabella Visiting Care, Inc. our licensed home careservices agency has more than doubled its size inthe past two years. In addition we have providedprofessional home health aide training to nearly600 community residents in the past four years —all at no cost to the trainee.

Innovation continues to be our watchword.In a time of increased pressure on health careproviders to do more with less due to cutbacksin government-funded programs such as Medicareand Medicaid, the imperative to do well becomesever greater. This is a challenge Isabella takesseriously and more than ever, we need your support.As you read these pages, think about what effortscost, our community’s loss were they to be eliminatedand what you can do right now. Please considerIsabella as you plan your charitable giving.

Person centered care is at the forefront of howwe deliver care in our nursing home and acrossour spectrum of services.We pioneered in thiseffort more than 15 years ago and in 2012 webenefitted from several measures that strengthenour effort in these areas:• With the help of PHI, a national non-profitorganization that provides training in coachingand supervision, Isabella staff received specializedguidance in how best to communicate and managein a variety of challenging situations. This training– which continues in 2013 –enables staff to handlesituations at the point of service in the best waypossible as well as to communicate most effectivelywith residents, family members and each other.• We continued to engage family members andour community partners in helping to create adynamic setting for those in our care. New YorkCares, Inc. and Arts and Minds are just twoof our valued community partners that havecontributed time and creative talent to enrichthe lives of our residents. It would be impossibleto count the many hours that family membersand caregivers have contributed to helping carefor residents in our nursing home – as wellas those in our community based programs.• Isabella’s own staff members have continued thetradition of finding new and festive ways to engagethe residents in activities that enrich daily life.Using the theme of an “Ocean Cruise”, Isabellahosted “Summer Fest”, an annual day-longextravaganza of musical performances, games andculinary delights to entertain and engage residents.• Personal pride was very much on display asresidents delighted in showing off their creativetalents in Isabella’s Annual Art Show.

2012 HIGHLIGHTS

Embracing change, expanding services to meet new needs and

advancing quality care have been abiding hallmarks of Isabella over

the many years. 2012 offered several challenges and opportunities

which helped Isabella strengthen and grow in important ways.

Here are some highlights:

Extraordinary developments are rarely anticipated –but can be a wonderful source of accomplishment and pride.• Superstorm Sandy presented unprecedented challenges forIsabella.While our location above sea level presented no fearof flooding, mass transit shut downs, gas shortages and poweroutages wreaked havoc on staffing schedules. Thanks to theenduring commitment of Isabella staff who remained onduty until relief arrived, we were able to maintain residentcare throughout the emergency. In addition, we were able toaccommodate the transfer of residents from a nursing homein Brooklyn that suffered extensive damage from the storm.• In 2012 Isabella was pleased to be awarded a 5 star rating“Much Above Average” by the Centers for Medicareand Medicaid Services, a federal agency that administersMedicare and Medicaid.

Technology continues to progress as the tool ofcommunication and innovation. During this pastyear Isabella modernized operations by convertingfrom the paper-bound world of yesteryear to theelectronic system of today.We participated in thecreation of the platform that will enable electronichealth records to transfer seamlessly to othermedical providers. Our staff electronically recordsresident care and treatment interventions.Telemedicine remains ever more important asa means for electronically communicating vitalinformation from our home care clients. And a newPhysician Portal facilitates the timely authorization ofcare by community doctors.With the help of Local1199 SEIU, all nursing home staff members havecompleted training to master Microsoft Office skills.

Isabella Visiting Care, Inc. our licensed homecare services agency, more than doubled in sizeover the past two years as it continues to offerhigh quality, cost effective home care to clientsin the greater New York area. Our new programof Platinum Care provides additional servicesto complement special lifestyle and social needs.

In partnership much can be accomplished. Isabella isfortunate to have participated in several vital communitypartnerships in 2012 to promote important community goals:• Under the aegis of the United Hospital Fund, and with othercommunity providers, Isabella is participating in “Togetheron Diabetes” a unique program for seniors with diabetesthat provides education, support, community referrals andindividual guidance to enable them to remain healthy andbetter manage their disease.• In partnership with GrowNYC,Isabella offers YUM Fresh Foodfor Northern Manhattan, ahealthy food access program ledby a coordinated committee ofcommunity senior volunteers.YUM is part of a nationaldemonstration project – “HealthyCommunities though HealthyFood” – managed by UnitedNeighborhood Houses of New York.• Isabella provides leadership to the Upper ManhattanPartnership for Senior Independence, a consortium of8 community organizations that offer case managementservices and program referrals to 900 residentsof Northern Manhattan.

Education remains an importantelement of Isabella’s mission.It enables our staff to remaincurrent with the latest treatmentmodalities; it helps nursing homeresidents recover and return safelyto home; and it provides guidance tocommunity residents so that they canminimize the need for costly medicalcare in the future. Our commitmentto education also provides incentivesto the youth of the community.Here are some of our educationalaccomplishments this year.• Nearly 150 community residentsreceived free career training tobecome home health aides and/orcertified nursing assistants.• More than 400 community residentsattended one or more of the 14educational programs conductedby the Institute for Older Adults.• More than 200 communityresidents attended the AnnualIsabella Health Fair where theylearned about the services of35 community providers.• 60 Community residentsattended an Education Dayon Advance Directives.• 10 Community High Schoolseniors with a record of academicachievement and a commitmentto community service wereawarded Scholarships to helpwith the cost of college.

OUTLOOK FOR 2013

Healthcare will continue to undergo majortransformation in the next few years.While wemust prepare for change, one thing remains constant:our commitment to improve the quality of care for all.

• Short Stay Rehabilitation will continue to be animportant need in our community. In 2013 we willexpand our capacity to meet this need.

• Our existing Ventilator Dependent Unit is fullyutilized at all times and demand for this specializedcare has exceeded our ability to accommodate allrequests. To remedy the situation, we plan toexpand capacity in 2013.

• Home Care will be needed more than ever as thedemand for case management and quality care inthe community rises. To prepare for this, we willopen a new certified home health agency“Isabella Care At Home”.

• Preventing unnecessary hospital readmissions – andcontrolling costs – are important health care goalsin 2013. In partnership with New York PresbyterianHospital,Weill Cornell Medical Center, AllenHospital and St. Luke’s Roosevelt Hospital, Isabella

will offer “Bridge to Home”, a free 30-day programfor Medicare fee-for-service patients that will provideguidance and support before and after discharge toenable them to connect with community health careproviders, secure appropriate social services and learnto properly manage their illness.

• The transition to Managed Care is well underwayfor our community based clients. In anticipationof the upcoming requirement for Managed Careparticipation of all nursing home residents, we willbe educating staff, family members and residentsas to the changes that are anticipated.

• Quality of Care will continue to be at the forefront.We will continue the training created by PHI toadvance the communication and coaching skillsof Isabella staff.

• As part of a grant awarded to the New York Chapter ofthe Alzheimer’s Association, Isabella will be workingto develop innovative programs for nursing homeresidents with dementia, including palliative care.

• We will remain steadfast in our goal to maintainfinancial stability, quality care for all anda willingness to adapt to positive change.

We rely on funds from individual donors, foundationsand government sources to make new programs possible andto expand the existing services we offer on our campus andin our community.We are grateful for the generosity of themany entities that make this a reality.

Our Fall Annual Gala is a generous source of philanthropicfunds for Isabella’s programs. Hundreds of guests gathered atthe Tribeca Rooftop for an evening of dinner and dancing underthe stars. This year’s Gala honored two extraordinarily dedicatedindividuals in the field of health care:

Robert G. Newman, MD, President Emeritus of ContinuumHealth Partners, Inc. and a Professor of Community Medicineat Mount Sinai School of Medicine was honored for his visionand leadership in developing partnerships to meet the growinghealthcare needs of all New Yorkers. Jo Ivey Boufford, MD,President of The New York Academy of Medicine and Professorof Public Service, Health Policy and Management at the Robert F.Wagner Graduate School of Public Service was honored for hercontributions to the field of public health - both on a nationaland international level - and for her expertise in advancingthe contemporary study of public health issues.

Isabella held its fourth fundraiser for community basedprograms in early November. Called “A Night in the Heights”,this festive event, attended by more than 100 guests, offered winetasting, a silent and live auction and spirited entertainment.
