Pain is a highly unpleasant physical sensation caused by any injury or illness. Physical part of pain results from nerve stimulation that is mediated by specific

nerve fibers that carry the pain impulses to the brain. Based on the severity, pain is two types namely acute and chronic pain. Generally acute pain is

managed with analgesics and anaesthetics where as severe pain in managed by using morphine as pain killer. Though this medicine is effective pain killer

but has common side effects like drowsiness, constipation, nausea etc. Hence recently scientists have discovered the venom procured from Caribbean

Sea snails which is a possible alternative to opioid-based painkillers such as morphine.

Key words: Analgesic, Caribbean Sea snails, morphine, pain, pain killer.

INTRODUCTION

Pain is the most common reason for physician consultation in all 1,2developed countries . It is a response or signal sent out to the brain to

indicate a problem in the body Hence pain is defined as an unpleasant

sensory and emotional experience arising from actual or potential tissue

damage. In 1968, Margo Mc Caffery first explained pain sensation and

explained as it is subjective, protective and modified by developmental,

behavioural and cultural factors.

Causes of Pain:

1. Inflammation, Infection, ischemia and tissue necrosis or burn.

2. In skeletal muscle, pain is developed from ischemia, haemorrhage

etc.

3. Some body organs such as kidney, liver and brain are characterize by

pain receptors.

4. In stomach and intestines the pain is generated from the inflammation

of the mucosa or by the muscle spasm.

Types of pain:

Chronic pain: It is long lasting and usually does not reduce with most

treatments. It is associated with conditions such as osteoarthritis or

fibromyalgia.

Acute pain: It occurs suddenly but is temporary. It is associated

with anxiety and emotional stress, and resolves as the injury heals.

Acute and chronic pain can be classified as (Figure-1):

.

3Pain can be of two types

Nociceptive pain: It is caused due to an injury to the body tissue;

this includes damage to muscle or bone such as bruises, cuts,

fractures, burns, cancer and surgery. Pain reduces once the injury

heals.

Neuropathic pain: It is caused due to dysfunction or damage to the

nerves, spinal cord or brain. It is characterized by a burning, tingling

or stabbing sensation. Pain can last for a long period of time, even

after the inju

Another types of pain is Psychogenic pain: It is caused by psychological

factors. Pain usually occurs due to tissue or nerve damage, but increases

and is prolonged because of stress, fear, anxiety or depression.

Based on transmission, pain is two types likely fast pain and slow pain.

Fast pain felt about 0.1 sec after a pain stimulus is applied whereas slow

pain felt after 1 sec or more.

Pathways of pain sensation:

The pathways of pain sensation are as follows:

1. Pathways from skin and deeper tissues

2. Pathway from face: Pain sensation is carried by trigeminal nerve

3. Pathway from viscera: Pain sensation is carried from thoracic and

abdominal viscera are transmitted by sympathetic nerves and

from trachea and pharynx by glossopharyngeal nerves.

4. Pathway from pelvic region: Pain sensation is carried by sacral

parasympathetic nerves.

Mechanism of pain :

It is described in

Pain medications for relief pain:

Analgesics are mainly use to relieve pain selectively without blocking the

conduction of nerve impulses, markedly altering sensory perception, or

affecting consciousness. They are classified into anti-inflammatory drugs,

which alleviate pain by reducing local inflammatory responses; and the

opioids (narcotics), which act on the brain. The opioid analgesics are

used for either short-term or long-term relief of severe pain whereas the

anti-inflammatory drugs are used for short-term pain relief like headache,

muscle strain, arthritis etc.

Opioids are powerful analgesic and mainly all the natural and synthetic

chemical compounds which are related to morphine, they act either as

agonists (cellular activators) or antagonists (substances that block the

actions of agonists) by affecting opioid receptors in brains and spinal

cord.

Anti inflammatory agents such as Acetylsalicylic acid, or aspirin is widely

used as mild analgesic. The two other major types of anti inflammatory

agents include acetaminophen (a derivative of phenacetin) and the

aspirin-like drugs, or nonsteroidal anti-inflammatory drugs (NSAIDs),

which include compounds such as ibuprofen, naproxen, and fenoprofen.

All these drugs mainly act by inhibition of the synthesis of prostaglandins

that induce the responses in local tissue that include pain and

inflammation.

Common side effects of opioids:

Common side effects of opioid administration include sedation,

dizziness, nausea, vomiting, constipation, physical dependence,

tolerance, and respiratory depression. Physical dependence and

addiction are clinical concerns.

Alternate source of Opioids:

To overcome the side effects of opioids, recently scientists are invented

natural products which can relief pain i.e. snail venom. Research

evidences are claim that snail venom is an alternate to opioid analgesic

and more powerful than morphine. Even the research also said that many

of the most promising chemicals for chronic pain relief come from

venomous animals. There are five compounds that are used to control

chronic pain and all based on a protein found in cone snail venom.

Cone snails are gastropod mollusks common on coral reefs and in tropical

tidal zones. They are genus of Conus and belong to the family Conidae.

Their venom is located in a gland within a tooth. The raw form conotoxins

are broken down by the body's enzymes and therefore cannot be taken

orally. Ziconotide is the first painkiller based on cone snail toxins which is

injected directly into the lower spinal cord but in very small doses, the

effect is 100 times as large as morphine or gabaentin pain relief drugs

because the cone-based drugs target different receptors to opiates and

produces the same addiction response without any side effects.

Cone snails use different venoms for hunting prey and defending

themselves against predators. The most venomous cone snail is Conus

geographus which is very dangerous and its LD50 is 0.001-0.003

mg/kg but only 0.0002-0.0005 mg resulted severe paralysis in human.

Even though some compounds found in cone snail venom are proteins

which are isolated and have great potential as pain-killing drugs.

Conantokin-G is a toxin derived from the venom of C. Geographus and

only 15 -20 of the venom's 100-200 toxic peptides are used for

feeding.

Recently the venom of cone snail, Conus magus is used for paralyzed the

fish before eating. This venom is made of hundreds of different peptides

which are harmful to the human but one particular peptide in the venom

can dramatically reduce pain by directl

CONCLUSION

Though the snail venom is poisonous to the human but its few peptides

have great potential to reduce pain which is more potent that morphine.

But still human trials are ongoing for the effectiveness and safety study.
