for students at KSG, HSPH, HMS, HU, HBS, and schools across the country.

While serving as vice chair of Faculty Council I was named chair of Faculty
Council Committee on Community Service which led to recommendation of
creation of Academic Society Committee on Community Service. I served as first
chair, co-founded Division of Service Learning in 2001, and served as the
Division's inaugural director. We were one of the first schools in the country to
have an officially recognized Division of Service Learning.
1999 - Expanded
advising systems
at HMS
In 1999, proposed to the Dean of Education an office devoted to study skills and
other advising issues and proposed that the inaugural director be Dr Laurie
Raymond. The office was officially funded and continues to operate and serve as a
national model. Successfully proposed the formal recognition and expansion of the
role of the resident advisor program in Vanderbilt Hall, Created the UHS working
group in response to concerns in a past LCME review. This program continues
today and is responsible for the significant improvement in student/UHS
interactions.

core course - Transition to the
Wards
In response to student requests for better preparation for transitioning from
preclinical to clinical education, successfully proposed the creation of a Transition
to the Wards course. Led the team that designed the initial course and participated
as faculty. The course became a core requirement and has now been reconfigured as
a core component of the Primary Clinical Experience (PCE) in each hospital. This
was presented at national meetings. Received excellent student evaluations.

medical education
Orchestrated initial donation of mannequin simulator to HMS, was appointed initial
chair of the executive steering committee, helped secure funding and build the
current 5-room simulation center at HMS. Co-created and co-directed many of the

at HMS simulation courses. Receives excellent evaluations from students. Served as model
for many other medical schools both in USA and abroad, hosting many site visits.

Worked with the Student Health Coordinating Board to turn the lessons learned
from our successful management of a student crisis into a "Crisis Manual for
Harvard University." First presented for adoption in 2001 and eventually adopted
as part of the University-wide crisis manual.

courses/offerings
at HMS
Developed the first mandatory pre-matriculation reading assignment with the
objectives of fostering cultural competence and addressing early educational
exposure to disparities in health outcomes. Led follow-up orientation event,
organized first class-wide mandatory diversity event, developed elective pre-
matriculation program in cultural competence, developed elective preclinical
experiential program and was a member of the original Cultural Competence
Steering Committee.

Healer's Art at
HMS
Co-directed the adoption of the Healer's Art at HMS. Course focused on reflections
on why we become physicians. Offered for 7 consecutive years, on average 40-50
students per year, 100% attendance and excellent evaluations by the students. Co-
presented nationally with Dr Remen and others.

course
Co- created and co-direct the Mentored Clinical Casebook course. Unique
educational program, presented nationally, published and replicated nationally.
Inspired numerous students to publish reflection essay on the value of this
experience.

Co-created Harvard Medical School MEDscience Curriculum for High School
Students, a program to foster biomedical literacy in novice high school students
using simulation-based teaching. Concept has been published and presented
nationally. In 2008 was expanded to be a semester long high school course that is
being taught in 3 different MA school systems. We have taught over 200 students.
Preliminary outcomes show positive impact on interest in science, technology,
engineering, and math (STEM), self confidence in ability to succeed in STEM. In

Encounter
presented our work on the use of simulation for novice students which led to the co-
creation of a 3-day section of ITP devoted to "Introduction to the Clinical
Encounter," including vital signs, case scenarios, and team training. During the
initial offering course this section had the highest evaluation of the entire ITP
course.

Walking Epidural
In 1991, pioneered the technique that was to be called the "walking epidural," so
named when it was televised nationally. Presented at national meetings, adopted
broadly, formally published 1993. Changed the standard of care from labor
epidurals that caused motor weakness to the standard of providing pain relief
without motor weakness. Trained international fellows and hosted international
visitors to share the technique, inspired large-scale international studies (England
and Canada), introduced to China 2000, story of introduction to China published in
International Journal of Obstetrics and Gynecology by Huixia Yang. Fellows
continued to conduct and publish follow-up studies, including the 2006 study on the
impact of ultra-low dose epidural and the need for anesthetic interventions to
answer the question whether the walking epidural increased workload.

Published study proving the safety of epidural use during trial of labor. This
changed standard of care allowing women having vaginal birth after cesarean
section to receive epidural pain relief for labor. Published with fellow Dr Calvin

Obstetric Anesthesia and Perinatology, published 1994. Prior to this study,

Paradigm shift in the discussion from epidurals causing cesarean sections (C/S) to
the pain of labor causing C/S. Initiated the concept that dysfunctional labor causes
pain and it is the dysfunctional labor that increases the risk of C/S, not the epidural
used to treat the pain. Presented at national meeting, invited to join American
Society of Anesthesiologists task force on guidelines, invited to NIH for consensus
discussion 1997, received support from NIH to continue this line of inquiry,
published formally 2000, 2001, carried on by fellows. Led to seminal work by Hess
et al. on relationship between pain and C/S.

I was one of 11 Task Force members who reviewed the literature, wrote the guidelines, and presented
these at several national meetings for adoption. I played a specific role in arguing for low-dose labor pain

Narrative Report My career has focused on developing clinical innovations, beginning first in the field of obstetric
anesthesia and then adding health services delivery. In parallel, I have been continuously involved in
teaching and educational leadership. I have designed many educational innovations and made significant
contributions to administration and institutional service at HU, HMS and BIDMC along with community
service and education.
Clinical innovation: In 1990, I pioneered the technique of ultra-low dose epidural anesthesia for labor
along with the concept that it is possible to provide labor analgesia that does not cause maternal muscle
weakness. The concept came to be called the “walking epidural.” I published the first paper on this idea,
was interviewed frequently by the national press, and presented it at meetings here and abroad. The
concept spawned several multi-center clinical trials and was written into the guidelines of the American
Society of Anesthesiologists’ Task Force on Obstetric Anesthesia, of which I was a member. This led to
the final resolution, at an NIH consensus meeting, of the debate about the relationship between epidural
pain relief, length of labor, and cesarean section. The conclusion adopted, based on my ultra-low dose
data, was that providing pain relief while avoiding muscle weakness had no adverse impact on anesthesia
or the outcome of labor. In addition, this work led to the seminal paper on the impact of pain itself on the
outcome of labor – completely altering the premise of the association between pain relief and labor
outcome. In 2005, I presented this technique to anesthesiologists at national meetings in China after which
it was adopted and is continuing to spread across the country. This led to several publications on the
technique and now to a textbook that I co-edited that is the first scholarly textbook to be published in
Chinese on Obstetric Anesthesia.

In 1990, I founded a mobile health clinic for the underserved communities of Boston. This clinic became
the model for numerous mobile clinics across the country – some modeling on our work and some using
us as design consultants. Since the clinic opened it has served as both a non-traditional health care
delivery system and a unique educational venue for teaching cultural competence to medical students and
residents. The program, as a model for both health care delivery and education, has been published and
presented in numerous national meetings. In 2009, I published the first paper on the cost-effectiveness of
this model for health disparities prevention and have begun to build a collaborative research network
utilizing a Web platform to present the scope of the national mobile health sector and its impact on
resource-poor communities and the national health care system. I was awarded a contract by the
Department of Health and Human Services, Office of Minority Health to partner with them and convene
an expert committee to consider the role that Mobile Health Clinics could play in addressing issues of
health equity for vulnerable populations.
Educational leadership: I have been teaching medical students and residents since I began my tenure as
faculty at BIH in 1983. I have received excellent reviews for this work since the beginning. I have also
created educational innovations that have endured and been copied nationally. In particular, my work
helping to incorporate simulation into the HMS curriculum and now into graduate, college and high
school biomedical education is a novel contribution to the field of bioscience education. I have published
this work and presented nationally and internationally. Other educational innovations I have co-created
and directed include: the Transition to the Wards Course, Mentored Clinical Casebook Project – which
has been copied by numerous other schools (note that this is different from purely narrative medicine as in
this program the basic sciences are integrated along with the narrative and the personal reflections), and
the creation of the Office of Advising Resources – which I spearheaded but do not run. Most recently I
was invited by the Institute for Healthcare Improvement to present our work on how a mobile clinic-based
curriculum can teach medical students about the social determinants of health.
Administrative and institutional service: I have held many service roles in my hospital, the medical
school, and the university. I have devoted my principal service and leadership in depth within Harvard
University and related to Student Affairs, Diversity, Education, and Service Learning. Additionally, I am
pleased that HRSA invited me to present our experience with Mobile Health Clinics to their grantees and
staff as they initiated a program that funds mobile clinics to make it possible for communities throughout
the U.S. to replicate this unique health service model.
