Q: . I am considering a breast reduction. What happens if, down the road a

bit, I gain or lose weight. How will it affect the results?

A: Minor weight fluctuations do not usually affect post-surgical breast cup

size a lot. You can expect changes similar to your pre-surgery status. Major

weight changes will affect breast size primarily by influencing the size of the

fatty layer around the breast – everyone is different in this regard. The chest

circumference is affected in the same way. This will cause changes in the cup

size and the chest size. In general, after breast reduction surgery there is a

slow change in the shape of the breast even if weight is unchanged. You

should ask to see pre- and post-surgery pictures to get an idea of how this

may apply to you. (Answered by Dr. Ian Bourhill)

Q: I currently get botox injections, and am considering having a facelift.

Would I still need to include botox in my maintenance?

A: A well-performed facelift can result in a more natural youthful appearance

with minimal discomfort. It may turn back the clock, but not the ticking. A

healthy routine with diet and exercise along with "maintenance" of Botox or

Dysport and combination of medical fillers to restore and maintain the youth-

ful aesthetic contours, may help to enhance and prolong your lovely result.

Q: I am in my late 50s, and am noticing that my eyelids are very heavy and

drooping to the point where it seems to be impeding my vision. What can I

do about it?

A: Drooping eyelids are very common as we age and can be caused by several

things. True eyelid drooping or ptosis is when the eyelid muscle becomes

weakened and the eyelids no longer open properly. Drooping can also occur

due to age- related descent of the eyebrow or of the upper eyelid skin. This

should be evaluated by an oculoplastic surgeon to determine the cause and

discuss treatment options. Almost all of these can be treated, and in some

cases may be covered by insurance, if the vision is truly obscured.

Osteoporosis is a diseasein which bones becomeweak and are more

likely to fracture. It is known asa “a silent disease” becausebone loss often occurs withoutyour knowing it. Until the age of30, your body forms new boneto replace the bone that isnaturally broken down by thebody. Your highest bone mass isreached between the ages 20and 25, and it declines afterthat. After menopause,however, women begin to losebone at an even faster rate. Os-teoporosis develops when yourbody cannot replace bone asfast as it is broken down.

In the U.S., 44 million Amer-icans are at risk for osteoporo-sis, while 10 million alreadyhave the disease. Women makeup 80% of the cases. Certainfactors make it more likely thatyou will develop osteoporosis.

The risk factors are: havinga family history, being post-menopausal, having a thinframe, being Caucasian, havinga diet low in calcium andvitamin D, doing little exercise,consuming alcohol, and being

on prolonged steroid therapy.Other factors that can leadto osteoporosis includehormonal conditions such ashyperthyroidism, hyperadrenal-ism, hyperparathyroidism, andanorexia nervosa.

If you have gone throughmenopause, have had afracture, or are consideringtreatment for osteoporosis, abone density test (DXA scan)can help determine your risk offracture. If you are over 65, anddo not have any of these riskfactors for osteoporosis, youshould still have a bone densitytest.

Too much bone loss canlead to fractures, which cancause serious health risks,including disability and prema-ture death.

Prevention of osteoporosisis crucial in today’s world. Youcan take these steps to preventbone loss:

-Get enough calcium andVitamin D, either through dietor supplements (at least1,200mg of calcium; and800IU of Vitamin D daily).-Do weight bearingexercises and stayphysically fit. -Avoid smoking.-Do not drink too muchalcohol.

barry Chase: Snoring occurs mainlyin the deeper stages of sleep, whenthe muscles of the body relax. Indeeper sleep, the lower jaw(mandible) falls back, and thetongue falls back and sits very closeto the soft palate and uvula (thepunching bag -looking tissue hang-ing down in the back of your throat).Instead of a wide airway for air toflow from the mouth into the wind-pipe (pharynx), the airway is closedinto a small slit. As air squeaks by thetongue and the soft palate, the tis-sues vibrate and cause snoring. Tis-sues of the entire pharynx can

vibrate, from behind the nose all theway into the deep throat. Loud snor-ing occurs mainly during inspiration,but snoring can occur during expira-tion also.

IM: Can any of the causes of snoringactually be controlled?

bC: It is hard to control snoring sinceit occurs when we are in deep sleepand don’t have the ability to be per-ceptive of ourselves. However, thereare some steps you can take to atleast reduce some snoring.1- Weight loss. Excess weight pro-motes snoring; weight loss cansometimes reduce it. However,many thin people snore loudly be-

cause the uvula and tongue are verylarge. Weight loss is no guarantee ofsnoring reduction, but is better forgeneral health.2- Position. Many people snoremore, or louder when on their back(supine). Using pillows to promotesleeping on your side may help. 3- Avoid big meals and alcohol closeto bedtime. Alcohol will causegreater snoring as it relaxes thetissues of the throat. It reduces thesleep quality, is a dehydrating agent,and disrupts the even flow of sleepstaging.4- Muscle relaxants, sedatives, andsleeping pills. Any agent or medica-tion that further relaxes the tissuesof the throat will promote snoring.5- Excessive sleepiness and sleepdebt. Being overtired and notsleeping enough will worsen snor-ing. Make sure to get the properamount of sleep, which for adultsshould be seven to eight hoursnightly.

IM: What is the link between heavysnoring and heart disease? Doesheavy snoring indicate a seriousheart problem?

bC: Snoring restricts airflow fromthe mouth into the lungs. The resultcould be a decrease of the oxygenlevels in the blood (oxygen desatu-rations). The condition is called aHypopnea, which is an oxygen

SNORING … IT’S MORETHAN JUST ANNOYING

Growing up in my house, I tolerated a lot of noise at night. Thechiming of the grandfather clock, the fish tank with its loud, gurglingair infuser, and my brother’s TV on all night were the norm. But aboveall, there was the constant sound of my father’s loud snoring. Henapped all the time, even dozing off at red lights. He once fell asleepin an MRI machine, started snoring and the vibrations blurred the MRIimages.

He constantly suffered from cardiovascular disease, diabetes, highblood pressure (hypertension), heart attacks and mini-strokes. Hewatched his diet, took his medications, went to the cardiologist, andexercised as best as he could, but he was fighting an uphill battle. Hisundiagnosed and untreated sleep apnea AND snoring were killing him.He passed away in February 2009 from heart failure.

Snoring is not just annoying; it is pathologic. Snoring is an airwayresistance that results in a decrease of oxygen in the blood. That’swhere the danger starts.

decrease due to restricted airflowduring sleep, and resulting inincreased heart rate and often anarousal out of deep sleep into lighterstage sleep. These desaturations ofoxygen can be dangerous if the levelsdecrease significantly. Oxygen desat-urations may not be acutely danger-ous, unless coupled with high bloodpressure, atrial fibrillations (cardiacarrhythmia), high cholesterol, previ-ous heart attacks or strokes,pulmonary disease, and obesity.Then, the likelihood of heart attacks,according to the National SleepFoundation, increases.

IM: How can sleep problems directlycontribute to risk factors for heartdisease?

bC: Besides a direct decrease ofoxygen to the heart, there is anothersystem activated during snoring andsleep apnea that can negatively affectthe heart. It is the brain’s response toprotect the body by causing certainglands to release hormones such ascortisol and adrenaline. These

hormones can cause physical andemotional stress, including increasingblood pressure and heart rate whilesleeping. Persistent increases of heartrate and blood pressure while sleep-ing is stressful to the heart. If the oxy-gen in the blood falls due to snoringand sleep apnea, and heart rate andblood pressure increases, heartattacks and heart disease can result.

IM: Is it important to try to correctsnoring problems even if they are notbothersome to the snorer? If so why?

bC: I just linked snoring, which is apathologic condition, to contributingto heart disease. In addition, decreas-ing O2 blood levels can cause stresson the body, resulting in the break-down of glycogen in the liver andultimately causing insulin intolerance,leading to Adult onset Type 2diabetes. It is usually easier to pre-vent the problems than try to treatthem later.

IM: How can snoring be treated sothat at the very least heart problems

can be minimized?

bC: Previously mentioned are somesuggestions to reduce snoring.However, they are not treatmentsthat are predictable or measurable. Apatient should first get a sleep test tomeasure the amount and loudness ofsnoring for the entire night. The sleeptest is called a polysomnography(PSG) and is performed in a sleeptesting facility. There are home testdevices as well. Consult yourphysician to determine which test isbest for you. Based on the results ofthe sleep test, therapy can be recom-mended. Types of therapy includeCPAP (continuous positive airwaypressure), dental oral appliancesconstructed by a trained dentist inoral appliance therapy, and surgery.Be strongly cautioned that there aremany oral devices available,especially on the internet, whichclaim to treat snoring but are oftendangerous and damaging.

IM: How does a person know if their

It’s February, the month asso-ciated with love, Cupid'sarrow, flowers, romance, and

all affairs of the heart --- yourheart and that of all your lovedones. The heart is the one organthat keeps you alive and keepsyour love alive.

How can we keep our mostvital organ alive and fullyfunctioning? How do we makeour heart "skip a beat" onlymetaphorically and not literally?

First and foremost, regard-less of your age – the keys to ahealthy heart are diet andexercise. We all know this, butsomehow, most of us seem tohave a difficult time maintainingit. A healthy heart requires aconstant flow of oxygen-richblood. It also needs foods thatdon't clog the arteries orcontribute to arterial plaque,such as fatty foods and hard-to-digest heavy, mucus –formingfoods. You can fudge (excuse thepun) on the diet a little if, whenyou overindulge, you make upfor it by exercising a little longerand harder. Nevertheless, like

any good relationship, diet andexercise are partners thatsupport each other. In addition,one often neglected or unknowncomponent is diaphragmaticbreathing. 21st century living ingeneral is stressful and we oftenforget to practice breathing.

The key to remembering allthis is to keep it simple and startslowly, one effort at a time, sothat you won’t get easily discour-aged or give up. Find a startingpoint that seems agreeable andstick with it. Just put your heartinto it!

So let's get started with yourdiet. There are many herbs youcan use for cooking that areheart-healthy. If you don’t domuch cooking, you can considerusing supplements and herbs.

The herbs are in a group offour. These are Hawthorne,Cayenne, Ginger, and Garlic.Hawthorne dilates the bloodvessels of the heart. Cayenneincreases circulation and bloodflow to to the heart as doesHawthorne. Ginger also relaxesthe blood vessels and garlic low-ers blood pressure and relaxesthe muscles for greater bloodflow. Food wise, although noteveryone is a meat eater, eatingeven a small amount of meatgives you a good source of B12(only found in red meat), whichpromotes energy production. Other heart-healthy foodsare leafy greens and fruits. Andif you don’t want to eat yourvegetables and fruits, you candrink them. Don’t overdo oneparticular fruit or vegetable thatmay be high in sugar, such asbananas and carrots. Modera-tion is the key, whether it’shealthy eating or indulging in adelicious dessert.

For decades, it had beencommonly believed by themedical community that

periodontal disease and chronicgingivitis, which are infectiousprocesses affecting the teethand gums, had a direct correla-tion to an increased incidencein cardiovascular disease.

Gum disease has long beenidentified as being the mostcommon chronic inflammatorycondition in the world.Chronic inflammation withinthe body has been known to in-crease the risk of heart attack,atherosclerosis, stroke, andeven cancer.

We already know that theplaque surrounding the teeth ismade up of a sticky film of pro-teins, food particles, acids, andmost importantly, bacteria,which can enter the blood-stream during certain dentalprocedures and even naturallyoccurring events such as eating.Many people already must takeantibiotics prior to certain

dental procedures if they aremore susceptible to the adverseeffects of bacteria within theircirculation. Since gum diseaseand cardiovascular diseaseproduce similar markers ofinflammation, such as C-reactive proteins -- which areproteins found in the bloodwhose levels rise in response toinflammation -- it is thereforeeasy to conclude that there ex-ists a link between gum diseaseand heart disease...or is there?

No Link, after all?Earlier this year, the Ameri-

can Heart Association hadreleased the following state-ment, which caused a stir in themedical community:

"Despite popular belief,gum disease has not beenproven to cause atheroscleroticheart disease or stroke, andtreating gum disease hasn'tbeen proven to prevent it...Theassociation between periodon-tal disease and atherosclerosiscomes mostly from observa-tional studies, and thereforedoes not demonstrate that gumdisease is a cause of heartdisease." The article furtherwent on to state, "Although acontribution of periodontal

disease to vascular disease isbiologically plausible, they bothshare multiple risk factors thatare prevalent and powerfulpromoters of disease, includingtobacco use, diabetes, andage." And, "Although periodon-tal interventions (brushing,flossing, regular cleanings)result in a reduction in systemicinflammation in short-termstudies, there is no evidencethat they prevent atheroscle-rotic vascular disease or modifyits outcomes."

Not everyone agrees withthese findings, however. Aresponse to this article by theUniversity of Buffalo, School ofDental Medicine, argued thatthe press release was mislead-ing because it was written insuch a way as to conclude thatthere is no association betweengum disease and heart disease,which is simply not true.Researchers are in agreementthat there is a correlationbetween periodontal diseaseand cardiovascular disease, butthat the exact cause and effectrelationship has not yet beenestablished.

between periodontitis (periodontal disease) andcardiovascular disease. As recently as 2010, a joint articlepublished by the American Journal of Cardiology and theJournal of Periodontology stated that: "It seemsreasonable, on the basis of current data, to acknowledgethat because untreated or inadequately controlled mod-erate to severe periodontitis increases the systemicinflammatory burden, periodontitis may independentlyincrease the risk for cardiovascular disease.

So, where Do we Go From here?The correlation between gum disease and heart

disease, although not completely understood, is still tooimportant to ignore. Periodontal disease is so commonlypresent in those with cardiovascular disease that it shouldnot be dismissed as an early warning sign of cardiovascu-lar problems. Because many of the conditions leading toheart disease are present without symptoms -- such ashigh cholesterol, hypertension, or plaque formation --proper diagnosis and treatment is often delayed, resultingin poorer outcomes.

It is, however, easy to notice swollen, painful gumsthat bleed easily. I often ask those with untreated gumdisease, "If your hands bled every time you washed them,you would see your physician. So, why would you ignoregums that bled every time you brushed or flossed?"

ment of periodontal or gum disease should go hand-in-hand with other lifestyle changes that can decrease one'srisk factor for cardiovascular disease. Some of thesechanges include:

Every February, organizations teamup to sponsor American HeartMonth and last year, even the White

House officials formally proclaimed it so.Cardiovascular disease is the leadingcause of death in the u.S. According tothe Center for Disease Control and Pre-vention, one in every three deaths is fromheart disease and stroke, equal to 2,200deaths per day. There are reportedlymore than two million heart attacks andstrokes a year, with at least a half a millionresulting in death. And about 8.6 millionwomen worldwide die of heart diseaseeach year.

The American Heart Association statesthat heart disease kills one in threewomen each year or approximately onewoman every minute. It doesn’t affect allwomen alike, and the warning signs forwomen aren’t the same as those for men.

The one and only legendary icon ofsong and film, Barbra Streisand was“heartbroken,” so to speak, when she be-came aware of the deadly disparity be-tween the amounts of funding dollarsspent for research and treatment ofcoronary illnesses in women, comparedto the amount allotted for men’s heart ail-ments. And, with all her heart and soul,she set out to shift the paradigm rallyingfriends, celebs, and even former Presi-dent Bill Clinton, who has battled his ownheart disease, to support the cause in a

very grand way, Streisand-style. In June of 2012, this leading lady held

a fundraiser at her Malibu home onbehalf of what is now officially called thebarbra Streisand women’s heart Centerat Cedars-Sinai heart Institute in Los an-geles. This event was the finale of her ef-fort to raise money for this center, whichbegan with her pledge of $5 million, con-tinued with her additional pledge of $5million and concluded with another $12million raised from supporters, friendsand celebs, to total $22 million!

In a recent interview on CNN, PiersMorgan asked Streisand about her phil-anthropic career, which he considers tohave been almost as relentlessly produc-tive as anything else she has done. Heasked her why she is so keen on women’sheart disease and why it’s been such apassion. She responded, "I dislike in-equality so much, whether it is gender is-sues or gay rights or whatever. Even in themedical sciences, there is discrimination.So it turns out that more women die ofheart disease now than all cancers com-bined. And even though more womenthan men die of it, heart disease is stillconsidered largely a male problem. I wasjust so shocked by these statistics, and thefact that it actually kills 30% of the popu-lation of the country.”

“Even in scientific research, women arestill treated as second-class citizens. Tome, that’s just unacceptable,” she com-mented.

Barbara is busy, seemingly all the time.

She told us how much she loves makingmovies and recordings, especially hermost recent fun film, "Guilt Trip" withSeth Rogen. She is working on anupcoming 5 Decades of Streisand DVDcompilation as well as a new studioalbum. Between all of this and all her timeadvocating for gun control, for Obamaand other causes, she still manages togive her time generously to the cause ofthe heart. She has handled it either by mak-ing a movie about it or becoming involved in womens’ issues.In this case, when it comes to medical science, she is so surethat gender really DOES matter. She balks at the idea thatwomen are receiving treatment for a life-threatening ailmentbased on research done on men, especially since physiolog-ically, women are different, cardiovascular-wise, from men.Women tend to have blockages not only in their main arteriesbut also in the smaller arteries that supply blood to the heart,which is a condition called microvascular disease.

Both Streisand and Dr. Bairey Merz wholeheartedlybelieve that the promoters of breast cancer awareness did a“magnificent” job, raising millions of dollars to help the dis-ease and bringing awareness into the population. The num-ber of women dying from breast cancer has significantlydeclined because of people speaking out, sharing stories, andraising money for awareness campaigns and medical re-search. She claims that heart disease desperately needs thesame kind of coordinated effort .

“Women need to be educated about it. And the medicalcommunity must be propelled toward change. Just like withbreast cancer, the impetus must come from women them-selves striving to become empowered to reduce their risksfor heart disease. We need to be as good as the breast cancercampaign in order to achieve this,” said Streisand.

Dr. Bairey Merz says that Streisand’s endowment providessustained funding to fill the gap during times when federalgrants and other monies drop off because it becomes politi-cally incorrect to support women’s health issues – even thosethat have nothing to do with reproductive health. “Everythinggets bundled together, and it’s very sad”.

According to the National Heart, Lungand Blood Institute, approximately3,000 people in the U.S. are on the

waiting list for a heart transplant on anygiven day. About 2,000 donor hearts areavailable each year. Wait times vary fromdays to several months, and depend uponthe recipient’s blood type and condition.Guidelines suggest that the donor beyounger than 65 years old, have little or nohistory of heart disease or trauma to thechest, and not be exposed to hepatitis orHIV. The guidelines also recommend thatthe donor heart not be without blood circu-lation for more than 4 hours. A person maybe taken off the list for some time if he orshe has a serious medical event such as astroke, infection, or kidney failure.

Time spent on the waiting list plays apart in who receives a donor heart. For ex-ample, if a donor heart becomes availableand two recipients have equal need, the re-cipient who has been waiting longer usuallywill get the heart.

Patients on the waiting list for a donorheart receive ongoing treatment for heartfailure and other medical conditions. Treat-ing arrhythmias (irregular heartbeats), forexample, is very important because theycan cause sudden cardiac arrest in peoplewho have heart failure.

As a result, many transplant centers willplace implantable cardioverter defribillator(ICDs) in patients before surgery. An ICD isa small device that's placed in the chest orabdomen to help control life-threateningarrhythmias. Another treatment that maybe recommended to waiting list patients isan implanted mechanical pump called aventricular assist device (VAD). This devicehelps the heart pump blood.

I spoke to one young man, David

Peltz, age 36, who is waiting for a hearttransplant. He is one of the many people inneed of organ donations. He is part of thethe www.helphopelive.org website.HelpHOPELive is a nonprofit that raisesfunds to bridge the gap between whathealth insurance will pay and what is actu-ally needed to heal, live and thrive after atransplant.

David, from Brooklyn, worked as an in-spector for New York City and was a first re-sponder at Ground Zero, moving debris forthree weeks after 9/11. Six years ago he wasdiagnosed with a heart disease called di-lated cardiomyopathy. He is now at thepoint in which he can no longer survivewithout a heart transplant.

David summed it up with this comment,“I have no control over my life now, whileneeding this transplant.”

As he waits for a heart, he has had aheart pump implanted. At all times, he hasto wear a computer module around hiswaist and walk around with batteries thatneed to be recharged every eight hours. Heis unable to work now, and is on a fixed in-come through SSD. Medicare only covers80% of his medical bills and does not coverthe exorbitant cost of his many medica-tions.

To help alleviate some of the expenses,his family has held fundraisers to benefitthe New York Heart Transplant Fund, and isalso hoping that he will receive donationsthrough HelpHOPELive. level of snoring is actually harmful tothe heart or other conditions?

bC: This is an excellent question be-cause it means that the person ac-knowledges that there is potential fordanger here. Nothing in medicine ex-ists by itself. Sleep apnea and snoringare among many contributing factorsto heart disease. By participating ina sleep study, you create a baselineof medical data so that, after therapyis rendered, more testing can bedone to check for improvements.Taking into consideration a variety ofcriteria, a doctor can paint a pictureof the relationship between snoring,general sleep, sleep apnea, and yourgeneral health.

IM: Are there any ways to treat asnoring condition without the use ofinvasive devices or surgery?

bC: Yes. The two non-surgicalmethods of treating snoring areCPAP and dental devices. Dental OralAppliance therapy is gaining popu-larity among patients and physicians.They are small retainer type devicesworn on the upper and lower teethduring nighttime only. They hold thelower jaw slightly forward duringsleep preventing the tongue from vi-brating with the soft palate. This alsokeeps the O2 blood levels high. Thedevice is easy to wear and very effec-tive. It is often covered by insurance.However, it should be made by adentist who is trained and experi-enced in oral appliance therapy forsleep apnea and snoring.

One invasive device used to treatsnoring is a Pillar Implant. With thisdevice, 3-4 small metal rods aresurgically inserted into the softpalate to reduce vibration. Theysometimes, but not always, work.Surgery involves removing the softpalate and some tissue of the rear ofthe mouth. This doesn't always work,is very painful, and often has relapse.

IM: If a person snores quite a bit,should they be checked for signs ofheart disease, and how?

bC: Snoring is both a medical maladyand a symptom of other possible con-ditions, and needs to be consideredalong with other signs and symptomsas a danger for heart disease. Youshould see your physician if you areobese, have hypertension, highcholesterol, a diet high in fats, and ahistory or family history of diabetes.Also, if you snore, stop breathing atnight, have excessive daytime sleepi-ness, and awaken with headaches,alert your physician. You will thendiscuss medical testing, including asleep test in a sleep facility orat-home sleep testing. IMaGE

As I now walk through thetown of Great Neck, I takenotice of the many changes

that occurred throughout the years.But one storefront has remainedthe same for over three decades.That‘s DeFranco Spagnolo Salonand Day Spa. While the namechanged from Peter’s Place toDeFranco Spagnolo in 1991, and itrecently moved to a newer, morevisible location down the block, thissalon is one of the few businessesthat has withstood the test of time.

Great Neck has remained abustling town, where fashion trendsand fine dining still dominate Mid-dle Neck Road. Other salons havecome and gone, but DeFranco Spag-nolo has staying power. And in thisuncertain economy, it's a wonderthat the owners opened an addi-tional location in Roslyn.

I had the privilege of sittingdown with legendary style icons,Anthony DeFranco and Rene Spag-nolo, to learn the secret to theirsuccess, the famous celebrities’hairstyles that made them famousthroughout the years, and wherethey see hair styling trends headingfor Spring 2013.

Trends Through the Times

Anthony and Rene began theircareers at a time when hair stylesset the tone for fashion trends. Bigname celebrity stylists graced thewindows of Manhattan’s Madisonand Fifth Avenues and East 57thStreet, including that of Paul Mc-Gregor, Paul Mitchell, Cinandre,Vidal Sassoon and Jean Louis David.In the 70’s and 80’s, hair was allabout glamour. Vidal Sassoon madeprecision cutting a true art and

probably the most popular trend inthe history of hair styling. Who canforget the 1975 movie “Shampoo”,when Warren Beatty cut JulieChristie’s hair into the iconic “bob”style?

Then, Jean Louis David brokethe rules for precision and createdthe unconstructive look, the “di-sheveled” look, as he called it, free-ing women from conformity. Theshaggy cut was an appealing unisexstyle. What high schooler didn’thave a shag?

In the 1983 movie "Flash-dance," Jennifer Beals made the bi-level bouncy look so popular thatpermanent waves were the mostdefining element of the 80’s. Hairstylists were having fun creatingmovement and bounce.

In the 90’s, Jennifer Aniston ap-peared on the hit show, Friends,and every woman wanted her hair.Rumor has it that Jennifer wasn’teven happy with that hairstyle atfirst, and never imagined that she’dbecome famous for her role asRachel Green, and for her hair too.

When asked, “Who do youthink will be the next celebritytrendsetter?,” Rene claimed to loveAnne Hathaway’s short hair in herrole as Fantine in the 2012 film "LesMiserables," and hopes that it willbe the fresh Spring look. His eyeslight up at the thought of stylishcuts re-emerging.

However, Anthony feels thatwomen are more influenced bywhat’s on the runway and in thefashion magazines. Given the pop-ularity of the Victoria’s Secret mod-els and their influence on younggirls, both gentlemen agree thatlong hair is here to stay, at least for

now. They insist that if your hair islong, it must be healthy first!

The hair’s health is the mostimportant factor in its look. Mostwomen’s hair has been abused byblow dryers and flat irons. Womenshould embrace the natural body oftheir hair. It can remain long, but it

should look alive. The way to havestraight hair without damaging it isthe responsibility of your hairstylist.Success starts by going to a rep-utable salon with a highly trainedstaff that uses the best professionalproducts.

a Consultation is Important

At your consultation, youshould discuss your desires with thestylist. The stylist should addressthem as well as the at-home main-tenance. There is no point in a greatstyle, if you can’t recreate it athome. You need the proper prod-ucts, as well as a lesson on how tostyle your hair. Professional prod-ucts that are exclusively available insalons may be more costly than

drugstore brands but they are made with finer ingredientssuch as argon oil and aloe vera and are gentler and saferto use. Popular brands in drug chains are usually ownedby larger companies that spend money on advertisingand packaging rather than on the actual product. Manytimes, the ingredients are cheaper and less effective.

I asked Rene if it is true that washing your hair everyday can cause dryness. He responded, “Not anymore.The new shampoos do not have the strong detergents ofthe past, so they are much gentler and won’t dry outyour hair. In fact, nothing looks better than clean,beautifully styled hair.” However, using a blow dryer orflat iron on a daily basis can cause damage to your hair.

On the controversy surrounding Keratin straighteningand the danger of formaldehyde, Rene explains thatformaldehyde has been in many products, such as nailpolish and lipstick, for decades. If you go to a reputablesalon with properly trained staff, certified in Keratin treat-ments, you are safe. Keratin is a wonderful ingredient tomaintain healthy hair.

Success and the Future

I asked both gentlemen, “How has DeFranco Spag-nolo been able to maintain its success throughout theyears, amidst all of the competition on the North Shore?

DeFranco Spagnolo is one of only 2,500 members ofthe Intercoiffure, an elite, worldwide organization. Eachyear the organization holds an atelier at The Waldorf As-toria Hotel where top hairdressers from Paris, Italy, Spain,Germany and Japan present the newest styles. After at-

tending, Anthony and Rene train their staff or send theirstaff to continuing education courses.

Anthony says, "We also reach out to our clientsregularly through social media and email blasts, but mostimportant is the trust we have developed between ourclients and staff". We worked hard for over thirty yearsto create and maintain solid relationships, whichcontribute most to our success. Rene says, “I still get hugegratification when a woman steps out of my chair feelingmore beautiful than when she arrived. Then I know, I’vedone my job well.”

Both love their work. Anthony says, “The beautyindustry allows me to work closely with my clients and tobe creative each day doing something I love. The day Iam no longer behind my chair, I will help my son Nicholasmake his mark in the beauty industry.”

What’s trending next in hairstyles? Anthony believesthat a look is created from the shoulder and above, andanything past the shoulder is extra. Many of his clientswant to emulate Katie Couric’s hair style. It’s a good, cleanbob, but one of his favorites is Halle Berry. Rene wouldlike to see more styled cuts and less one-length long hairon women. “The old days of glamour are long gone, butif I had my way, the glamour of Marilyn Monroe wouldreturn.” IMaGE

Loving, close, vivacious ---these are all words thatcould be used to describe

the Bavero family, consisting ofCarolyn and Ron Bavero and us-- their four children, Christen,Terry, James, and Joanna. Ourfamily dynamics changeddramatically eleven years ago,when our mother was diag-nosed with pancreatic cancer.Over the next two years, wedeveloped very distinct rolesso that as a family, we couldgive her the best whileshe bravely fought thisdisease.

Since she was the gluethat held us together, wewere left with greatemptiness and sadnesswhen she passed away.We found it difficult tolive life passionately. Theroles that were neces-sary for us to take onduring this difficult timebecame our new norm,essentially they becameour identity. As a family weremained close, though wewere merely surviving, notthriving.

Christen:I was the first to be intro-

duced to Synergy Education bymy best friend who has alwaysbeen involved with and knewintimately the dynamics of myfamily. Before beginning theworkshop, I, as the oldest,always felt a sense of responsi-bility to take care of my familyand I stepped into my mother'sshoes after her death. Ibecame more than a sister butless than a mother. This quicklyled to a life of feelingexhausted and sad as I put offthe passions in my own life tostep in and take care of myfamily. I realized, throughSynergy, that by "taking care

of" or "controlling" my family,I was doing a disservice to myamazing family, who had theirown talents to contribute toour well-being. I was also doing

a disservice to myself, as I wasnot living fully and joyously. AsI shifted, my family dynamicsshifted too. Not only were wehappier, but it sparked andreinvigorated what we wantedto create in our lives. As mybrother James saw the changein me, he then went throughthe workshop, and was quicklyfollowed by my sisters Terryand Joanna, who participatedin the workshop together.

Terry:As a middle child, I became

extremely comfortable withbeing diplomatic. I was the onethat family members would gowhen they wanted to vent and

not want to be judged.However, in an attemptto be everyone’s biggestcheerleader and sup-porter, I did not haveauthentic relationshipswith my siblings, fatherand extended familymembers. This led to alife of feeling lonely andmisunderstood. It alsocaused me to put off

the dreams in my own life toremain close to what my familywanted. After taking theSynergy Education workshop, Ibecame very connected to

how powerful I was and how much people wantedto know the “real” me. I never realized the spacethat I had placed in between myself and others.Now, I can continue to support people in a non-judgmental way while still being true to myself. Asa result, I have completely honest and meaningfulrelationships with those whom I love.

Joanna:I am the youngest in the family and so I had the

tendency to let everyone else do everything for me.I was easily disempowered and used to call my sis-ters, especially Christen, who was eager to controlsituations, to fix everything that happened in mylife. This led to a life of my “playing it small” and notrealizing how much strength that I had as theyoungest of four. In Synergy Education, I realizedhow vast my talents are as an artist, a baker, etc. Ibecame eager to give my gifts to my family and theworld. I no longer felt like I needed to ask permis-sion and I saw that I can stand alongside the womenin my life whom I admired, because I have just asmuch to offer as everyone else.

Moving forward, we now have the opportunityto support each other as we live in contribution tonot only our family, but also the world around us.We now empower each other to not only pursueour individual dreams but to bring our unique tal-

ents passionately to all situations. Synergy Education has given us the awareness

to “call each other out” in a loving way when Chris-ten is being too controlling, or when Terry is notbeing honest, or when Joanna is letting others stepin, instead of living powerfully. We can all laugh at

ourselves rather than becoming defensive and/orbeating ourselves up. Since we come from a bigfamily, most of the others have not yet gonethrough the workshop, but because of the ways inwhich we have shifted, our relationships with ourextended family members have changed for thebetter.

Our family is far from perfect, but it sure is fun.And at the end of the day, can you ask for anythingmore than for those you love to really know you andlove you for everything you are and also for every-thing that you’re not? IMaGE

Your primary care physi-cian, also known as a“general practitioner” or

“internist” is your communitydoctor who not only sees youwhen you’re sick but also playsan important role in keepingyou healthy.

A general myth aboutprimary care physicians is thatthey are visited by a patientwho is feeling sick. However,the role of the primary carephysician is actually quiteexpansive and includes not onlytreating illness, but preventingillnesses from occurring in agenerally healthy patient.

Other roles of the primarycare physician include coordi-nating the entire team ofmedical professionals who areoccasionally on board to pro-vide more specialized care.Conducting baseline testing ofyour heart, blood pressure,physical exam, blood work, lungfunction, vision and hearing

helps the primary carephysician recognize anydeviations from normal, asearly as possible or duringregularly scheduled well visits.Early recognition and interven-tion is essential for preventingdisease or the progression of adisease to a worsening state.

Participating in early detec-tion of medical problemsmeans taking a proactivestance to try to prevent illnessand negative outcomes, instead

gastroenterologist, urologist,gynecologist, etc. Coordinatinga patient’s care opens up linesof communication among all ofthe doctors on the team, andcan track new medicationsadded by other physicians toprevent adverse drug reactionsfrom multiple medications.This type of medicationproblem can typically happen inthe elderly population who areoften followed by a team ofmultiple doctors and prescribedmedications by each individualspecialist. The primary carephysician must always beupdated on all new medica-tions to be sure the drugregimen is safe and does not in-teract poorly with the rest ofthe patient’s medications.

Frequently encountereddiseases that are caught early byprimary care doctors include hy-pertension, diabetes, stroke,obesity, heart disease, anemia,and chronic obstructivepulmonary disease. The primary care physician can also update andcoordinate screening tests appropriate for each agegroup, for example colonoscopy, mammography, bonedensity testing, pap smears, prostate exams as well askeeping all pertinent vaccines up to date.

The primary care physician does not always workalone. Usually there is a team of medical professionalswho provide care to the patients of the practice. Theteam may include a medical assistant, nurse practi-tioner or physician’s assistant. The goal of the team isto know each patient well, discuss goals of care foreach patient and design the most comprehensive ap-proach for a patient’s specific needs. It must be em-phasized that the patient is also a very important partof the team.

Teamwork, dedication to specific needs of eachpatient, defining goals of care, and addressing risks assoon as possible for future disease prevention areamong the many roles of the primary care physician.

I encourage healthy young people, in addition to thosewith pre-existing medical conditions, to visit their pri-mary care physician and start a proactive approach inthe fight against illness and disease. Avoid medical

A Visia imaging session usesmulti-spectral imaging to photo-graphically document six areasaffecting complexion healthand appearance. It measureswrinkles, spots, pores, evenness,porphyrins (signs of bacteria inthe pores), and UV spots. Knowing how and why skin isdamaged helps us to correct thesigns of aging. There are severalways to prevent aging in the skin:Avoid skin pollutants, such assmoking. Protect the skin fromdaily sun exposure, supplementand nourish the skin withprescription topical skin-careproducts that help maintain theskin’s supple and smooth appear-ance. Avoid excessive alcoholintake and poor nutrition.

To treat spots, both visibleand invisible, always wear sun-block. The American Academy ofDermatology recommends that asunscreen or sunblock with anSPF of at least 30 be appliedabout a half hour before you gooutside each day. Sunblocks thatcontain zinc oxide and titaniumdioxide block UVA rays that causefacial wrinkling and leathering.

Antioxidants may offerprotection to the skin and preventexcessive skin aging while nour-ishing and supplementing its

deeper layers. Free radicals causechronic damage to the cells of theskin, and antioxidants can neu-tralize these dangerous mole-cules.

Applying antioxidants directlyto the skin is one way of solvingthe problem. All antioxidants donot contain the same ingredients,so look for vitamins C and E,ferulic acid and phloretin.Vitamin C is vital for producingcollagen which gives skin its firm-ness and elasticity. It is alsonecessary for correcting pigmen-tation problems. It is a water sol-uble vitamin that regeneratesvitamin E and provides UVA/UVBprotection and reduces redness.Vitamin C appears to provide su-perior protection from sun dam-age when used with vitamin E.Ferulic acid is an organic plantcompound found in plant cellwalls. When combined with vita-mins C and E, ferulic acid reducesoxidative stress and helps preventredness and sunburn from UVBrays. Phloretin creates a potentantioxidant and provides ad-vanced photo protection. It alsocontributes to greater skin pene-tration of active ingredients forgradual release and deliverybeneath the skin surface.

the skin. We need to combat rough texture, wrinklesand enlarged pores, first by exfoliating and moisturizingthe surface of the skin, and then building deeper colla-gen layers of the skin that thin with age and even outthe skin tones and hyperpigmentation. Alpha and betahydroxyl acids (AHAs and BHAs) diminish fine lines andwrinkles. The FDA has approved them as effective toreduce wrinkles, spots and other signs of aging and sundamage.

The more commonly used alpha hydroxyl acidsinclude glycolic acid, lactic acid and ascorbic acid: thebeta hydroxyl acids include salicylic acid and benzoicacid. Alpha and beta hydroxyl acids have beendemonstrated to decrease the signs of aging by en-hancing the shedding of the most superficial layer ofthe skin, the epidermis. Some claim they improve thequality of the elastin fibers and the collagen densityin the middle layer of the skin, the dermis. Becauseconcentrations of these compounds vary, it is recom-mended that they be purchased through a physician,so that you can obtain more significant exfoliationand other positive changes. Once the skin is exfoli-ated, the next step is moisturization. A goodmoisturizer should be nongreasy, nonirritating,noncomedogenic and fragrance-free. It is notnecessary to buy an expensive moisturizer; there arevery effective inexpensive ones widely available.

Vitamin A was the first vitamin to be usedtopically for the treatment of damaged skin. It maybe found as retinol or tretinoin (Retin A). These formsof vitamin A induce thickening of the epidermis,increase skin cell proliferation and act as a hormoneto activate deposition of collagen in the dermis toreduce fine lines and wrinkling, roughness and laxity.The key is to use Retin A consistently and alwaysapply sunscreen. Visible results will be achieved inabout six months. Tretinoin applied at night tightensthe skin, reducing the appearance of pores.

Enlarged pores are caused by several factors. Aswe get older, oils and skin cells build up in the pores.Sun damage and loss of elasticity make pores appearlarger over time. To decrease the appearance ofpores, always cleanse daily -- especially at night --with a gentle cleanser to keep pores uncloggedwithout irritating skin. Do not use harsh skin tonersor astringents to minimize pores. They maytemporarily contract pores but may cause overpro-duction of oil and larger pores in the long run.

Anyone who has experi-enced braces knows thateven though pain is

involved, they don’t always gainthe desired results. There is, how-ever, a painless and simple post-orthodontic smile enhancementtreatment that literally fills thegaps left after braces.

Veneers are not commonlythought of as a follow-up treat-ment to orthodontics, but whenbraces leave something to bedesired, patients may go in searchof another solution.

That was the case with arecently treated cosmeticdentistry patient, Mary Kay. Likemany adolescents, she receivedorthodontic treatment. “I hadspaces in between my teeth,”Mary Kay admitted, “so smilingmade me feel self-conscious.”

After years of braces anddiscomfort, the greatlyanticipated moment for herbraces to be removed arrived.Mary Kay found the resultsdisappointing.

The braces were, as Mary Kaysaid, “just to straighten out myteeth,” adding, “they didn’t makemy teeth [look] any better.”Embarrassing gaps remained even

though her teeth were nowperfectly straight. The issue wassimply a case of size and shapediscrepancies, particularly in herupper teeth; however, correctingspaces due to small teeth is notsomething orthodontics canalways solve.

Mary Kay knew that veneerscould close the spaces betweenher teeth, but she had reserva-tions. “I’d heard of veneers,” shesaid, “but also heard that dentistsgenerally shave down your naturalteeth to small nubs for theprocedure. That definitely did notappeal to me.”

While researching an alterna-tive veneer treatment, she discov-ered a specific type of painlessand non-invasive veneer proce-dure that did not require thedrilling down of her teeth. Shewas very concerned that if aveneer should pop off for somereason, she would still have hernatural tooth below and not asmall nub. So, she made it herbusiness to learn the differencesin the techniques used by dentiststo place veneers.

In the world of dentalveneers, there are two mainmethods of preparing a tooth toreceive veneers: “traditionalpreparation” and “no-prep”.Interestingly enough, there is nocost difference between the two

methods.For traditional prep, a dentist

uses a drill to remove significantamounts of healthy tooth struc-ture to create room for theveneers. This technique invadessensitive tissues in the tooth andrequires shots to numb the areaso the patient doesn’t feel theotherwise painful drilling. Inextreme cases, this invasive

technique can cause serious, irre-versible damage to the tooth andmay lead to the need for a verypainful root canal.

“No-prep” methods aim atpreserving as much tooth struc-ture as possible. In extreme cases,a tooth may require somecontouring of the enamel with adrill, but this does not disturb orexpose sensitive tissues nor doesit require a shot or topical anes-thetic for pain. No-prep veneersare generally ultra-thin, so there is

additional medications within safe guidelines. It usually takes30-60 minutes for the effects to begin, so many dentists pre-scribe a starting dose to begin an hour prior to treatment.

Some patients, because of medical conditions or prior reg-ular usage of sedatives, may not be good candidates for oralsedation. For those, dentists who are certified for I.V. Sedationmay be the better choice. The advantages are that I.V. drug’seffects are felt within a just a few minutes and can very closelybe titrated (incrementally dosed as needed) to reach thedesired safe level of sedation. It can also, where indicated, pro-vide a deeper level of sedation with the same beneficial loss ofthe memory of the dental procedures. During sedation ses-sions, another qualified staff member is present to check vitalsigns such as pulse, pulse oxygen level, blood pressure andeven at times , an EKG can be monitored.

So, now you know that going to the dentist does not haveto be the experience you may remember from your childhood.Today’s modern advances can certainly make going to thedentist seem like you never went!

Now that flip flops are a dis-tant memory and nextsummer feels like light

years away, we all tend to neglectour feet. We tuck them away inbig fluffy boots to hibernate allwinter long. This time of year, asthe temperatures drop and the airdries out, so does our skin.

During the winter, we seemany patients start coming incomplaining about dry, flaking skinthat just won't go away. Peopletry lotion after lotion, some evenquite expensive, to try and allevi-ate the problem unsuccessfully.What people don't realize is thatsometimes it's not just a simply acase of dry skin.

Xerosis is the medical term fordry skin and can be very itchy, irri-tating, painful and unsightly.Many times it will not respond toover-the-counter lotions andmight require more involved treat-ment. Severe and chronically dryfeet can be linked to a more seri-ous underlying condition or dis-ease. Evaluation by a podiatristcan help rule out these types ofconditions and, if necessary, guide

you towards appropriate testing orspecialists. However, in mostcases, xerosis is a localized andself-limiting condition that is verysuccessfully treated with prescrip-tion medications that actuallychemically remove the dry skin.

In some cases, patients canalso develop cracks or fissures inthe skin which can be extremelypainful and can lead to more seri-ous issues like bleeding and infec-tion. If you notice these, youshould call a podiatrist and beevaluated as soon as possible.When this occurs, intervention isrequired to remove the dead cal-lused skin and medications may berequired to prevent infection andhelp the skin heal. If you noticeredness, swelling, drainage or pus,this can be serious and should betreated as an emergency.

So, how can you prevent thisterrible problem? Well, first thingis to keep moisturized. Using agood lotion nightly before bed is agreat way for most people to keeptheir feet healthy and smooth. Apumice stone is also a simple andeffective way to remove thosedead skin cells and keep the restsupple and clear. Wearing propersocks during the day is also impor-tant in the winter. Cotton socks inheavy boots can pool sweat andcreate problems as well, so a

blended material that wicks mois-ture is usually advisable.

The best thing to do is haveyour doctor take a look at yourfeet and make recommendationsfor a course of care that will keepyou healthy all winter long. It is al-ways easier to maintain your feetyear-round rather than waiting

until summertime and then at-tempting to resolve problems likedry cracked feet. Even though it'scold and snowy outside, and youlikely won't be parading around inopen- toed shoes anytime soon,taking care of your feet is impor-tant all year! IMaGE____________________________

Think about whatreally matters to you and commit toyour answers. When you meetsomeone that interests you, setgoals and stand by them. Think ofwhat you want that relationship tolook like and begin creating it.

In step four, the author encour-ages everyone to be a “Dating Octo-pus”. Without going out andmeeting a variety of people, youcan’t meet that one special person.A person doesn’t just arrive becauseyou sit home dreaming about himor her. Use your most positive at-tributes to attract the kind of personyou want to meet. There are greattips about how to go about findingyour dates.

The next step is about “Winningthe Dating Game”. Ernest suggestsenhancing all of your greatest attrib-utes since you don’t get a secondchance to make your first impres-sion. Also, you can’t win the gameif you don’t play, so get out there.Present yourself the way you wantto be perceived. Men are visual, sothe way in which you present your-self is the way in which the man willlook at you. It is important for menand women to be respectful, au-thentic, honest, loyal, a good lis-tener, and reasonable.

Steps six and seven focus on all

the Do’s and Don’ts of relationships.I am guilty of many of those Don’ts.The Do’s sound so achievable that Iask myself why I don’t behave thatway as often as I should. The Do’sare doable, so this is a time for re-flection and to try on another kindof behavior

Step eight, “Don’t Get Marriedto Get Divorced”. I love this chapter.Ernest teaches us, very simply, thatif you don’t want to fail, thenDON’T! This chapter focuses onhow to succeed in your marriage.He cites that he wrote this book sothat more marriages would stayalive. With a 60% divorce rate in ourcountry, we need to understandwhy we are failing in marriage andhow we can turn that around. Whatdo a man and women really needand want from each other? Are wemarrying for all the right reasons?You will ask yourself that very ques-tion as you read this chapter.

The final step is to remind your-self that if you are fortunate enoughto find your love, do not forget tocreate a plan for success. If you cre-ate a plan in which both of you areclear on your goals as a couple, yourchances of survival are so muchgreater. Ernest advises couples tocreate a six-year plan, just as execu-tives do to create a successful busi-ness. Decide what you are lookingfor in a marriage and rememberthat mostly everything you need tomake a marriage divorce -proof is al-ready in front of you. You both needto work together to overcome anyobstacles that may arise.
