Dear GB and JW,
I would be grateful if you could help me with a clinical issue.
I am treating a 13 years old girl affected by MDR-TB. Linezolid was included in her treatment regimen and planned to continue for several months.
In Italy linezolid is available as tablets 600 mg or syrup for oral administration or as intravenous preparation. The girl's weight is 40 kg. Therefore I started linezolid as 600 mg (one tablet) in the morning and 300 mg (syrup) in the evening. Because of vomiting after syrup intake we shifted to evening intravenous administration of linezolid 300 mg (keeping the morning 600 mg tablet).
When we started planning for discharge from hospital we tried to maintain linezolid as 600 mg tablet in the morning only. With this dosage the through concentration of linezolid was 1.2 mg/L.
The question is: is this concentration too low ? (the reference values of the lab are 2 to 8 mg/L - but I guess they refer to bacterial infections).
If insufficient, what alternative administration strategy could I select for linezolid ?

RespuestaDear Alberto,
By adding the peak level of 15.6 mg/L the estimated AUC of 170 mg*h/L was confirmed. Peak level fitted the pk model very well. So the dose of 600mg qdcan be maintained for mic of 0.5-1 mg/L. If mic is <0.25 mg/l the dose can be reduced to 300mg qd. We normally confirm exposure after a dose reduction.

Los fármacos anti-TB con potencialidades para futura revaluación : linezolid, delamanid, bedaquiline, carbapenémicos
Conclusiones• Tenemos una nueva clasificación de las drogas
• Algunos medicamentos han subido en el ranking, otros han caidos
• La evidencia de la eficacia y la tolerabilidad es pobre
• Tenemos algunos estudios nuevos interesantes
• Los centros de referencia necesitan de tener alto nivel y aplicar estándares más altos posibles, por que estos fármacos son difíciles de manejar
• Gestión basada en la evidencia adecuada es necesaria
• Los ensayos (trials) clínicos están detrás del uso clínico de los medicamentos (y directrices clinicas)
• Tenemos la responsabilidad de conducir el proceso, lo que permite el consentimiento informado y la farmacovigilancia, y el establecimiento de centros de excelencia capaces de manejar estos medicamentos de manera segura para el paciente
