2015 GAHP WORKSHOP American Association of Zoo Veterinarians (AAZV)

TheGreatApeHeartProject(GAHP)hostedaworkshoponSeptember26inPortland,ORatthe2015AmericanAssociaDonofZooVeterinarians(AAZV)MeeDng.TheworkshopwaspartoftheGAHP’s2012NaDonalLeadershipGrantfromtheInsDtuteofMuseumandLibraryServices(NLG-IMLS).Thethree-yearIMLSgrantprovidedfinancialsupporttocreatearobust,online,cardiacdatabasesystemtobeusedbyzooandmedicalprofessionals,researchersandothersubjectmaUerexperts(SME’s)astheycollecDvelyworktoassess,treatandpreventcardiovasculardiseaseingreatapes.TheprimaryobjecDveofthemeeDngwastointroducetheGAHPExecuDveCommiUeetothenewdatabaseaswellastoupdatethemonGAHPprogressandacDviDestodate.TheGAHPdecidedtohostthemeeDnginconjuncDonwithAAZVsothattheinformaDoncouldfurtherbedisseminatedtozooveterinariansandotherparDcipantsintheproject,whilealsogainingperDnentfeedbackfortheproject.

o BenBrainard–Discussedthedrawbacksofusinganalpha-2agonist(inmostcasesthatismedetomidine).StatedthatmedetomidinehasitsplaceasananestheDcagentinapes,parDcularlywhenyouneedareversibleagent.Drawbackisthecardiovascularsideeffectsofhypertensionthatlastapprox.20-30minutesandthenthecompensatoryhypotensiveeffectthatispredominant.

o HelikesTelazol™(Dletamine/zolazepamcombo)asananestheDcinapes.BenBrainarddiscussedtheuseofpremedicaDon’scontainingabenzodiazepineinordertomaximizethedissociateeffectsoftheDletamine.

o ManyzoovetsdonotliketheprolongedanestheDcrecoveryDmesseenwithTelazol.BenBrainardexplainedthatTelazollastssolonglikelyduetothemetabolitesofthezolazepam,whichareacDvelikethediazepam.

o HayleyMurphy:personalexperience-usesalowdoseof2.5-3mg/kgTelazolandfindsthatrecoveriesarequickerwithmostapessikngupwithin2hoursofendofprocedure.

o MedetomidinemaycauseanormalhearttolookdiseasedonechosoBBdoesn’trecommendusingitforexamsthatrequireechoanddon’tuseonapeswithcardiovasculardisease

• QuesDonpresented:Whenisitbeneficialtostarttreatment?o GreggRapoportnotedthattherearedifferencesintheliteratureonwhento

iniDatetreatmentsofCVD.EveryoneonpanelagreedthatthereareaspectsofanabnormalechocardiogramthatweallthinkareworthtreaDng,butdisagreementexistsbetweentheveterinarycardiologistsandthehuman/MDcardiologistsinsomeareas.AllpanelistsagreedthatwhenyouseeaheartwithsystolicdysfuncDon,treatmentwithaceinhibitorsisappropriate.

o MoredisagreementexistswhenthereisnosystolicdysfuncDon,butjustthicknessintheheart.Allagreedthatifcardiacthickeningisduetohypertension,thattreatmenthasbeenshowntobetherapeuDc.However,theliteratureisnotclearonthebenefitsoftreatmentwhenthereisnoclearcauseofthehypertrophy.

o BillDevlin:inhumansthickeningoftheventricleisnotnormalanditistreatedwithanaceinhibitor.Ifwewaitbadthingsaregoingtohappenifthetherapyhaslowriskwhynottreat.SomeuseENALAPRIL5-20mg2xperday.LISINOPRILisaoncedailyACE-I2.5to40mgdaily.

o Ilana,wedon’tknowwhatnormalissowewereverygenerousindescribingandsekngour“normal”forthemalegorillas.LVHisbadandace-inhibitorstreatLVHandace-inhibitorsextendlifeinhumans.Weallcamefromthedimensionthatitishypertensiondrivenbutitmaynotbe;itmaybefamilialanditmaybebeUertreatedwithbeta-blockers.Wejustdon’tknowyet.

o BenBrainardwarnedthatanimalsonaceinhibitorshaveadramaDcallymagnifiedhypotensiveeffecttoinhalantanestheDcsandinhumanstheystopthesedrugsbeforeanesthesiatoavoidprofoundhypotension.

o HayleyMurphy:Pointedoutthatthisdiscussionwasverybeneficial.Thegoalwastopointouttozoocliniciansthatthisisnotanexactscience.WantedthezoovettorealizethatsincealltheSMEsarenotinagreement;itisuptothezoocliniciantoulDmatelymakedecisionsforhis/heranimals.Wantedtoaidinmakinginformeddecisionsandthencliniciansneedtousetheirbestclinicaljudgmentonacase-by-casebasis.

o IlanaKuDnsky–stressedagainthatthereisnodisagreementamongSMEsontreatmentofheartsthathavesystolicimpairment

o MarieUaDanforth:FYI-theILRdatatheyaretalkingaboutisn’t“GAHPdata”thatisinthedatabase,butI’mkeepingtrackofwhichzooshaveILRsinplaceandtryingtorecommendthosewhoinquireaboutILRstogetintouchwithzoosthatarealreadyusingthem(suchasDetroitandLincolnParkZoos,ColumbusandNaDonalZoos,andOmaha).
