II. GENERAL PRODUCT INFORMATIONThrough the advancement of internal fixation hardware, the surgeon has been provided a means of correcting deformity and reducing pain for many patients. While the implants used are largely successful in attaining these goals, it must be recognized that they are manufactured from metal and polymeric materials and no implant can be expected to withstand the activity levels and loads as would normal, healthy bone after fusion occurs. The surgeon must evaluate each situation individually based on the patient’s clinical presentation in making any decisions regarding implant selection.Surgeons must be familiar with the applicable operative technique and instructions for use for each product. This package insert and the immediate package label contain essential warnings, precautions, and contraindications for each surgery. Additionally, the surgical technique should be referenced for detailed information about implant selection, relevant product details, proposed surgical instructions, and/or assembly use. The surgeon should contact Wright for the proposed product-specific surgical technique.In using fusion implants, the surgeon should be aware of the following:• The correct selection and sizing of the implant is extremely important. Selection of the proper
size, shape, and design of the implant increases the potential for success. The implants require careful seating and adequate bone support. Proper implant selection must consider design, fixation, patient weight, age, bone quality, size, activity level, preoperative level of health, and also the surgeon’s experience and familiarity with the device. Implant longevity and stability may be affected by these variables. Surgeons should inform the patients about these factors.
• In selecting patients for surgery, the following factors can be critical to the eventual success of the procedure:1. Patient’s occupation or activity. If the patient is involved in an occupation or activity which
includes substantial lifting or muscle strain, the resultant forces can cause failure of the fixation, the device, or both. The implant will not restore function to the level expected with normal healthy bone, and the patient should not have unrealistic functional expectations.
2. Condition of senility, mental illness, or alcoholism. These conditions, among others, may cause the patient to ignore certain necessary limitations and precautions in the use of the implant, leading to failure or other complications.
3. Foreign body sensitivity. Where material sensitivity is suspected, appropriate tests should be made prior to material selection or implantation.
A. PATIENT SELECTIONUse of internal fixation hardware requires consideration of the following general indications:• Good condition of the patient• Good neurovascular status• Adequate skin coverage• Possibility of a functional musculotendinous system• Adequate bone stock to receive implant• Availability of post-operative therapy• Cooperative patient
B. INDICATIONSThe GRAVITY Suture Anchor Systems are indicated for use:• In the repair of shoulder instability secondary to Bankart lesion, rotator cuff tear, a slap lesion,
acromioclavicular separation, biceps tenodesis, deltoid tear/separation, or capsular shift or capsulolabral reconstruction;
• In the repair of elbow instability secondary to biceps tendon detachment, tennis elbow, or ulnar or radial collateral ligament tear/separation;
• In the repair of hand/wrist instability secondary to tear or separation of the scapholunate ligament, ulnar collateral ligament, or radial collateral ligament;
• In the repair of knee instability secondary to tear or separation of the medial collateral ligament, lateral collateral ligament, patellar tendon, or posterior oblique ligament, or secondary to iliotibial band tenodesis;
• In the repair of foot/ankle instability secondary to tear or separation of the Achilles tendon, lateral stabilization tendons/ligaments, medial stabilization tendons/ligaments, midfoot tendons/ligaments, or metatarsal tendons/ligaments, Tendon Transfers, tendon reattachments, and ligament reconstructions of the midfoot, forefoot, and hindfoot procedures associated with flatfoot reconstruction, hindfoot deformity, midfoot reconstruction, lateral/medial ankle reconstruction or instability, Hallux Valgus or Varus, and MTP instability including:• Achilles reattachment/reconstruction • Flexor Digitorum Longus Transfer • Flexor Hallucis Longus Transfer• Extensor Hallucis Longus Transfer• Brostrom Procedures• Peroneal Tendon Relocation• Capsule Repair• Deltoid reconstruction / reattachment• Plantar Plate Repair• Spring Ligament Repair
C. CONTRAINDICATIONSNo product specific contraindications.
D. POTENTIAL COMPLICATIONS AND ADVERSE REACTIONSIn any surgical procedure, the potential for complications exists. The risks and complications with these implants include:• Infection or painful, swollen or inflamed implant site• Fracture of the implant• Loosening or dislocation of the implant requiring revision surgery• Bone resorption or over-production• Allergic reaction(s) to implant material(s)• Untoward histological responses possibly involving macrophages and/or fibroblasts• Migration of particle wear debris possibly resulting in a bodily response• Embolism
E. PRECAUTIONSFollowing the instructions for use provided in product literature can minimize the potential for complications or adverse reactions with any implant.It is the responsibility of each surgeon using implants to consider the clinical and medical status of each patient and to be knowledgeable about all aspects of implant procedure and the potential complications that may occur. The benefits derived from implant surgery may not meet the patient’s expectations or may deteriorate with time, necessitating revision surgery to replace the implant or to carry out alternative procedures. Revision surgeries with implants are common. The patient’s mental status must also be considered. Willingness and/or ability to follow post-operative instructions may also impact the surgical outcome. Surgeons must balance many considerations to achieve the best result in individual patients.IF EXCESSIVE LOADING CANNOT BE PREVENTED, AN IMPLANT SHOULD NOT BE USED.
The main goal of surgery with this implant is to establish soft tissue to bone union. Abnormal or excessive forces could lead to delayed union, non-union, or failure of the implant.Abnormal force loading and subsequent wear may be caused by:• Uncorrected instability• Improperly sized implant• Inadequate soft tissue support• Implant malposition• Excessive motion• Uncorrected or recurrent deformity• Patient misuse or overactivityProper fixation at the time of surgery is critical to the success of the procedure. Bone stock must be adequate to support the device.Some preventative measures to consider to minimize the potential for complications:• Follow guidelines for indications and contraindications provided above• Identify prior pathology• Stabilize collapse deformities• Bone graft pre-existing cysts• Use a properly sized implant• Avoid K-wires and sutures through the implantAvoid flawing implant surfaces to minimize the potential for early fatigue failure.
If complications develop, possible corrective procedures include:• Implant removal
• Synovectomy• Bone grafting of cysts• Replacement of the implant• Removal of the implant with fusion of the joint
Recommendations Regarding Device Fragments• Use medical devices in accordance with their labeled indications and Wright Medical
Technology’s instructions for use, especially during insertion and removal.• Inspect devices prior to use for damage during shipment or storage or any out-of-box defects
that might increase the likelihood of fragmentation during a procedure.• Inspect devices immediately upon removal from the patient for any signs of breakage or
fragmentation.• If the device is damaged, retain it to assist with Wright Medical Technology’s analysis of the event.• Carefully consider and discuss with the patient (if possible) the risks and benefits of retrieving vs.
leaving the fragment in the patient.• Advise the patient of the nature and safety of unretrieved device fragments including the
following information:a. The material composition of the fragment (if known);b. The size of the fragment (if known);c. The location of the fragment;d. The potential mechanisms for injury, e.g., migration, infection;e. Procedures or treatments that should be avoided such as MRI exams in the case of metallic
fragments. This may help to reduce the possibility of a serious injury from the fragment.
Clinical results depend on surgeon and technique, pre-operative and post-operative care, the implant, patient pathology and daily activity. It is important that surgeons obtain appropriate informed consent and discuss the potential for complications with each patient prior to surgery. This may include a review of alternative, non-implant procedures such as soft tissue reconstruction or arthrodesis.Concerning Magnetic Resonance EnvironmentsThere are inherent risks associated with the use of implants in the MR environment; including component migration, heat induction, and signal interference or distortion near the component(s). Heat induction of implants is a risk related to component geometry and material, as well as the MR power, duration, and pulse sequence. Since MR equipment is not standardized, the severity and likelihood of occurrence are unknown for these implants.These implants have not been evaluated for safety and compatibility in the MR environment. These implants have not been tested for heating, migration, or image artifact in the MR environment. The safety of this in the MR environment is unknown. Scanning a patient who has the device may result in patient injury.
F. HANDLING AND STERILIZATIONIMPLANTSThe implants described in this package insert are provided sterile. Implants in sterile packaging should be inspected to ensure that the packaging has not been damaged or previously opened. If the inner package integrity has been compromised, contact the manufacturer for further instructions. The implants should be opened using aseptic OR technique; they should only be opened after the correct size has been determined.This product is for single use only. An implant should never be re-sterilized after contact with body tissues or fluids.
Devices labeled for single-use only should never be reused. Reuse of these devices may potentially result in serious patient harm. Examples of hazards related to the reuse of these devices include, but are not limited to: significant degradation in device performance, cross- infection, and contamination.
G. STORAGE CONDITIONSAll implants must be stored in a clean, dry environment and be protected from sunlight and extremes in temperature.

DESCRIPTIONThe GRAVITY Titanium Suture Anchor System features sterile, single-use devices for use in soft tissue reattachment procedures. The implants are available in a range of sizes and feature threaded titanium anchors loaded with ultra-high molecular weight polyethylene (UHMWPE) suture.

II. ALLGEMEINE PRODUKTINFORMATIONENDie Fortschritte auf dem Gebiet der internen Fixierungsvorrichtungen ermöglichen es den Chirurgen, bei vielen Patienten Deformationen zu korrigieren und Schmerzen zu lindern. Obwohl die für diese Zwecke verwendeten Implantate größtenteils erfolgreich eingesetzt werden, ist zu beachten, dass sie aus Metall- und Polymermaterialien hergestellt werden, und dass erwartungsgemäß kein Implantat den Aktivitäten und Belastungen so gut standhalten kann wie ein normaler, gesunder Knochen nach der vollständigen Fusion. Die Implantatauswahl erfolgt nach eingehender Untersuchung des Patienten durch den Chirurgen und in Übereinstimmung mit den individuellen anatomischen Gegebenheiten.Der Chirurg muss mit den entsprechenden Operationstechniken und Gebrauchsanweisungen für die verwendeten Produkte vertraut sein. Diese Packungsbeilage und die Beschriftung der Verpackung enthalten Warnhinweise, Vorsichtsmaßnahmen und Kontraindikationen für die jeweiligen Operationen. Diese sind unbedingt zu beachten. Des Weiteren ist die Operationstechnik mit den genauen Angaben zur Implantatauswahl, den relevanten Produktinformationen, empfohlenen Operationsschritten und/oder Einzelheiten zur Montage der Komponenten zu beachten. Die vorgeschlagene produktspezifische Operationstechnik ist bei Wright erhältlich.Bei der Verwendung von Fusionsimplantaten sollte sich der Chirurg folgender Punkte bewusst sein:• Die richtige Auswahl und Größenanpassung des Implantats sind äußerst wichtig. Die Auswahl
der korrekten Größe, Form und Ausführung des Implantats erhöht die Wahrscheinlichkeit für einen erfolgreichen Eingriff. Die Implantate erfordern eine sorgfältige Platzierung sowie adäquate knöcherne Abstützung. Zur sachgemäßen Implantatauswahl gehört die Beachtung der folgenden Faktoren: Ausführung und Fixation, Gewicht, Alter, Knochenqualität, Körpergröße, Aktivitätsniveau und präoperativer Gesundheitszustand des Patienten sowie Erfahrung und Vertrautheit des Chirurgen mit dem Produkt. Diese Variablen können sich auf die Langlebigkeit und Stabilität des Implantats auswirken. Der Chirurg sollte den Patienten über diese Faktoren aufklären.
• Bei der Patientenauswahl für die Operation können die folgenden Faktoren den letztendlichen Erfolg des Eingriffs entscheiden:
1. Berufliche Tätigkeit oder Aktivität des Patienten. Beinhalten die beruflichen oder sonstigen Aktivitäten des Patienten das Heben schwerer Lasten oder starke muskuläre Beanspruchungen, können die daraus resultierenden Kräfte zum Versagen der Verankerung, des Implantats oder beidem führen. Das Implantat kann die Funktion nicht bis zu dem Grad wiederherstellen, der von normalem, gesundem Knochen zu erwarten ist, entsprechend sollte der Patient keine unrealistischen Erwartungen hinsichtlich der Funktion haben.
2. Senilität, Geisteskrankheit oder Alkoholismus. Diese und andere Erkrankungen können dazu führen, dass der Patient bestimmte notwendige Einschränkungen und Vorsichtsmaßnahmen beim Gebrauch des Implantats außer Acht lässt. Dies kann zu Implantatversagen oder anderen Komplikationen führen.
3. Empfindlichkeit gegenüber Fremdkörpermaterialien. Bei Verdacht auf Empfindlichkeit gegenüber dem verwendeten Material sollten vor der Materialauswahl oder Implantation entsprechende Tests durchgeführt werden.
A. PATIENTENAUSWAHLBei der Verwendung von Implantaten für die interne Fixation müssen die folgenden allgemeinen Voraussetzungen gegeben sein:• Guter allgemeiner Gesundheitszustand des Patienten• Guter neurovaskulärer Status• Ausreichende Hautabdeckung• Vorhandensein eines funktionellen Muskel- und Sehnensystems• Ausreichende Knochensubstanz für die Aufnahme des Implantats• Verfügbarkeit einer postoperativen Therapie• Kooperationsbereitschaft des Patienten
B. INDIKATIONENDie GRAVITY Nahtankersysteme sind in den folgenden Situationen indiziert:• Bei der Reparatur einer Schultergelenkinstabilität (Zustand nach Bankart-Läsion, Ruptur der
Rotatorenmanschette, SLAP-Läsion, Separation von Akromion und Clavicula, Bizepstenodese, Riss bzw. Separation des Deltoideus, Kapselverschiebung oder Rekonstruktion der Kapsel und des Labrums);
• Bei der Reparatur einer Ellenbogeninstabilität (Zustand nach Ablösung der Bizepssehne, Tennisellenbogen oder Riss bzw. Separation des seitlichen Ellen- oder Speichenbands);
• Bei der Reparatur einer Hand(gelenk)instabilität (Zustand nach Riss bzw. Separation des Lig. scapholunare, des seitlichen Ellen- oder Speichenbands);
• Bei der Reparatur einer Kniegelenkinstabilität (Zustand nach Riss bzw. Separation des Innen- oder Außenbands, der Patellarsehne oder des schrägen Kniekehlenbands oder Zustand nach Tenodese des Lig. iliotibiale);
• Bei der Reparatur einer Fuß-/Sprunggelenkinstabilität (Zustand nach Riss bzw. Separation der Achillessehne), der lateralen gelenkstabilisierenden Sehnen/Bänder, der medialen gelenkstabilisierenden Sehnen/Bänder, der Sehnen/Bänder am Mittelfuß oder der Sehnen/Bänder an den Metatarsalien, bei Sehnentransfers, Sehnenrefixationen, Bandrekonstruktionen am Mittelfuß, Vorfuß und Rückfuß, Operationen im Zusammenhang mit der Korrektur von Plattfuß- und Rückfußdeformitäten, der Rekonstruktion des Mittelfußes, der Rekonstruktion oder Stabilisierung des lateralen/medialen Sprunggelenks, Hallux-valgus- oder Hallux-varus-Korrekturen und bei MTP-Instabilitäten einschließlich:• Refixation/Rekonstruktion der Achillessehne • Transfer der Flexor-digitorum-longus-Sehne • Transfer der Flexor-hallucis-longus-Sehne• Transfer der Extensor-hallucis-longus-Sehne
• Bandrekonstruktion nach Broström• Relokation der Peronäussehnen• Kapselrekonstruktion• Refixation/Rekonstruktion der Deltoideussehne• Rekonstruktion der Ligg. plantaria• Rekonstruktion des Ligamentum calcaneocentrale plantare
C. KONTRAINDIKATIONENKeine produktspezifischen Kontraindikationen.
D. MÖGLICHE KOMPLIKATIONEN UND UNERWÜNSCHTE WIRKUNGENBei jedem operativen Eingriff können Komplikationen auftreten. Risiken und Komplikationen bei der Verwendung dieser Implantate sind u. a.:• Infektion oder schmerzhafte, geschwollene bzw. entzündete Implantationsstelle• Fraktur des Implantats• Lockerung oder Dislokation des Implantats, wodurch ein Revisionseingriff erforderlich wird• Knochenresorption oder -überproduktion• Allergische Reaktion(en) auf das/die Material(ien) des Implantats• Ungünstige histologische Reaktionen möglicherweise unter Beteiligung von Makrophagen
und/oder Fibroblasten• Migration von Abriebpartikeln mit möglicher körperlicher Reaktion• Embolie
E. VORSICHTSMASSNAHMENDurch Beachtung der im Lieferumfang der Implantate enthaltenen Gebrauchsanweisung kann das Risiko potenzieller Komplikationen und unerwünschter Nebenwirkungen auf ein Minimum reduziert werden.Es obliegt der Verantwortung jedes Chirurgen bei der Verwendung von Implantaten den klinischen und medizinischen Zustand jedes Patienten zu berücksichtigen. Der Chirurg muss mit allen Aspekten der Implantation und den möglichen Komplikationen umfassend vertraut sein. Vorteile und Nutzen der Versorgung mit Implantaten entsprechen möglicherweise nicht den Erwartungen des Patienten oder können sich im Laufe der Zeit verringern und einen Revisionseingriff erforderlich machen, um das Implantat zu ersetzen oder Alternativmaßnahmen durchzuführen. Revisionseingriffe bei Implantaten sind häufig. Der Geisteszustand des Patienten muss ebenfalls berücksichtigt werden. Die Bereitschaft und/oder Fähigkeit zur Befolgung postoperativer Anweisungen kann den Erfolg der Operation ebenfalls beeinflussen. Der Chirurg muss viele Aspekte abwägen, um bei jedem Patienten das beste Ergebnis zu erzielen.FALLS EINE ÜBERMÄSSIGE BELASTUNG UNVERMEIDBAR IST, IST VON DER VERWENDUNG EINES IMPLANTATS ABZUSEHEN.Dieses Implantat dient hauptsächlich dazu, eine Verbindung zwischen Weichgewebe und Knochen herzustellen. Anomale oder übermäßige Kräfte können zu verzögerter Frakturheilung, Non-Union (Pseudarthrosenbildung) oder Implantatversagen führen.Anomale Belastung und daraus resultierende Abnutzung kann folgende Ursachen haben:• Nicht korrigierte Instabilität• Implantat der falschen Größe• Unzureichende Weichteilabstützung• Fehlpositionierung des Implantats• Exzessive Bewegung
• Nicht korrigierte oder rezidivierende Deformität• Falsches Verhalten des Patienten oder ÜberaktivitätDie ordnungsgemäße Fixation zum Zeitpunkt des Eingriffs ist maßgeblich für den Erfolg des Verfahrens. Die vorhandene Knochensubstanz muss die adäquate Abstützung des Implantats gewährleisten.Nachstehend sind einige vorbeugende Maßnahmen aufgeführt, die das Komplikationsrisiko auf ein Minimum reduzieren:• Befolgung der Richtlinien für die vorstehend genannten Indikationen und Kontraindikationen• Identifizierung von Pathologien in der Vorgeschichte• Stabilisierung von Kollapsdeformitäten• Auffüllung vorbestehender Zysten mit Knochentransplantat• Verwendung der richtigen Implantatgröße• Vermeidung von Kirschnerdrähten und Nähten durch das ImplantatVermeidung von Beschädigungen der Implantatoberfläche, um das Risiko vorzeitiger Materialermüdung zu minimieren.

Empfehlungen zu Produktfragmenten• Die Medizinprodukte stets gemäß den in der Begleitdokumentation angegebenen Indikationen
verwenden und die Gebrauchsanweisung von Wright Medical Technology befolgen, insbesondere beim Einbringen und Entfernen.
• Die Produkte vor der Verwendung auf evtl. beim Transport oder während der Lagerung entstandene Beschädigungen sowie auf Defekte, die die Wahrscheinlichkeit einer Fragmentierung während des Eingriffs erhöhen könnten, untersuchen.
• Die Produkte sofort nach der Entfernung aus dem Patienten auf Anzeichen von Bruch oder Fragmentierung untersuchen.
• Beschädigte Komponenten bitte zur eingehenden Untersuchung und Analyse an Wright Medical Technology senden.
• Die Risiken und Vorteile, die mit der Entfernung bzw. dem Zurücklassen von Fragmenten im Körper des Patienten einhergehen, sorgfältig abwägen und mit dem Patienten (sofern möglich) besprechen.
• Der Patient ist über Art und Typ der in situ belassenen Implantatfragmente und die damit einhergehenden Sicherheitsrisiken aufzuklären. Dazu gehören u. a. die folgenden Informationen:a. Die Materialzusammensetzung des Fragments (soweit diese bekannt ist);b. Die Größe des Fragments (soweit diese bekannt ist);c. Die Lage des Fragments;d. Potenzielle Verletzungsmechanismen, z. B. Migration, Infektion;e. Zu vermeidende Eingriffe oder Behandlungen, wie z. B. MRT-Untersuchungen im Fall von
Metallfragmenten. Dadurch kann das Risiko schwerer Verletzungen durch das Fragment reduziert werden.
Die klinischen Ergebnisse hängen vom Chirurgen und von der Operationstechnik, der präoperativen und postoperativen Versorgung, dem Implantat, der Pathologie des Patienten und dessen täglichen Aktivitäten ab. Es ist wichtig, dass der Chirurg vor jeder Operation eine Einwilligungserklärung des Patienten einholt und den Patienten über die potenziellen Komplikationen aufklärt. Dazu kann ein Überblick über alternative Behandlungsmethoden ohne Implantate gehören, z. B. Weichteilrekonstruktion oder Arthrodese.Verhalten in einer MRT-UmgebungPatienten mit Implantaten sind im MRT-Umfeld inhärenten Risiken ausgesetzt. Dazu gehören: Migration oder magnetisch induzierte Erwärmung der Komponenten, Signalstörungen oder Bilddistorsionen in der Umgebung der Komponente(n). Das Risiko einer magnetisch induzierten Erwärmung von Implantaten hängt von Geometrie und Material der Komponenten sowie Feldstärke, Scandauer und Impulssequenz des MRTs ab. Da MRT-Systeme nicht standardisiert sind, ist eine Aussage zu Ausmaß und Wahrscheinlichkeit einer magnetisch induzierten Erwärmung dieser Implantate nicht möglich.Diese Implantate wurden nicht im Hinblick auf ihre Sicherheit und Kompatibilität im MRT-Umfeld geprüft. Diese Implantate wurden nicht auf Erwärmung, Migration oder Bildartefakte bei MRT-Untersuchungen getestet. Die Sicherheit dieses Implantats im MRT-Umfeld ist nicht bekannt. Eine Untersuchung an einem Patienten, der dieses Implantat trägt, kann Verletzungen des Patienten verursachen.
F. HANDHABUNG UND STERILISATIONIMPLANTATEDie in dieser Packungsbeilage beschriebenen Implantate werden steril geliefert. Steril verpackte Implantate sind vor dem Gebrauch zu überprüfen, um sicherzustellen, dass die Verpackung nicht beschädigt ist oder bereits geöffnet wurde. Wenn die Integrität der inneren Verpackung beeinträchtigt ist, wenden Sie sich bitte für weitere Anweisungen an den Hersteller. Die Implantatverpackungen
unter Anwendung aseptischer OP-Techniken öffnen; die Verpackung grundsätzlich erst nach Bestimmung der definitiven Implantatgröße öffnen.Dieses Produkt ist nur für den einmaligen Gebrauch vorgesehen. Implantate, die in Kontakt mit Körpergewebe oder Körperflüssigkeiten gekommen sind, unter keinen Umständen resterilisieren.Als Einmalprodukte gekennzeichnete Produkte unter keinen Umständen wiederverwenden. Eine Wiederverwendung dieser Produkte kann die Gesundheit des Patienten potenziell ernsthaft schädigen. Beispiele für die mit einer Wiederverwendung dieser Produkte verbundenen Gefahren sind u. a.: deutliche Beeinträchtigung der Produktqualität, Kreuzinfektion und Kontamination.
G. LAGERBEDINGUNGENAlle Implantate müssen in einer sauberen trockenen Umgebung aufbewahrt und vor Sonnenlicht und extremen Temperaturen geschützt werden.

II. ALGEMENE PRODUCTINFORMATIEDankzij verbeteringen in middelen voor interne fixatie heeft de chirurg nu de beschikking over een middel voor het corrigeren van deformaties en het verlichten van pijn voor veel patiënten. Hoewel de gebruikte implantaten over het algemeen succesvol zijn bij het verwezenlijken van deze doelen is het een feit dat zij uit metalen en polymere materialen worden vervaardigd en dat van geen enkel implantaat verwacht kan worden dat het bestand is tegen de activiteitenniveaus en belastingen die door normaal, gezond bot weerstaan worden nadat fusie heeft plaatsgevonden. Bij besluitvorming over implantaatselectie moet de chirurg ieder geval individueel evalueren op basis van de klinische presentatie van de patiënt.Chirurgen dienen vertrouwd te zijn met de betreffende operatietechniek en de gebruiksaanwijzing voor elk product. Deze bijsluiter en het etiket op de binnenverpakking bevatten essentiële waarschuwingen, voorzorgsmaatregelen en contra-indicaties voor elke operatie. Daarnaast moet de operatietechniek worden geraadpleegd voor gedetailleerde informatie over implantaatselectie, relevante productgegevens, voorgestelde operatie-instructies en/of assemblage. De chirurg moet contact opnemen met Wright aangaande de voorgestelde productspecifieke operatietechniek.Bij gebruik van fusie-implantaten dient de chirurg zich van het volgende bewust te zijn:• De juiste selectie en maat van het implantaat zijn zeer belangrijk. Selectie van de juiste maat
en vorm en het juiste ontwerp van het implantaat verhoogt de kans op succes. De implantaten moeten zorgvuldig geplaatst worden en voldoende botondersteuning hebben. Bij het selecteren van het juiste implantaat moeten ontwerp en fixatie, en gewicht, leeftijd, botkwaliteit, grootte, activiteitenniveau en preoperatieve gezondheidstoestand van de patiënt worden overwogen, alsmede de ervaring en vertrouwdheid van de chirurg met het hulpmiddel. De levensduur en stabiliteit van het implantaat kunnen door deze variabelen worden beïnvloed. Chirurgen dienen de patiënten over deze factoren te informeren.
• Bij de selectie van patiënten voor de operatie kunnen de volgende factoren cruciaal zijn voor het uiteindelijke succes van de ingreep:
1. Beroep of activiteit van de patiënt. Indien de patiënt een beroep uitoefent of een activiteit beoefent die veel tillen of spierinspanning vereist, kunnen de hieruit resulterende krachten het falen van de fixatie, het hulpmiddel of van beide tot gevolg hebben. Het implantaat herstelt de functie niet volledig tot het niveau dat van normaal gezond bot mag worden verwacht en de patiënt moet geen onrealistische verwachtingen hebben met betrekking tot functieherstel.
2. Seniliteit, psychische aandoening of alcoholisme. Deze condities kunnen er onder andere de oorzaak van zijn dat de patiënt bepaalde noodzakelijke beperkingen en voorzorgsmaatregelen tijdens het gebruik van het implantaat negeert, wat tot falen of andere complicaties kan leiden.
3. Gevoeligheid voor vreemde voorwerpen. Wanneer materiaalgevoeligheid wordt vermoed, moeten vóór de materiaalkeuze of implantatie de van toepassing zijnde tests worden uitgevoerd.
A. PATIËNTSELECTIEBij gebruik van middelen voor interne fixatie dienen de volgende algemene indicaties te worden overwogen:• goede conditie van de patiënt• goede neurovasculaire status• voldoende huidbedekking• mogelijkheid van een functioneel spier- en peesstelsel• voldoende botmassa om voor een implantaat in aanmerking te komen• beschikbaarheid van postoperatieve therapie• bereidwillige patiënt
B. INDICATIESDe GRAVITY titaan hechtankersystemen zijn geïndiceerd voor gebruik bij:
• Herstel van schouderinstabiliteit secundair aan een Bankart-laesie, een gescheurde rotatorcuff, een SLAP-laesie, acromioclaviculaire separatie, tenodese van de biceps, deltoideusscheur/-separatie, kapselverschuiving of capsulolabrale reconstructie.
• Herstel van ellebooginstabiliteit secundair aan loslating van een bicepspees, een tenniselleboog of een scheur/separatie van het collaterale ligament van de ulna of de radius.
• Herstel van hand-/polsinstabiliteit secundair aan een scheur of separatie van het scafolunate ligament, het collaterale ligament van de ulna of het collaterale ligament van de radius.
• Herstel van knie-instabiliteit secundair aan een scheur of separatie van het mediale collaterale ligament, het laterale collaterale ligament, de patellapees of het posterieure oblique ligament, of secundair aan tenodese van de iliotibiale band.
• Herstel van voet-/enkelinstabiliteit secundair aan scheur of separatie van de achillespees, laterale stabilisatiepezen/-ligamenten, mediale stabilisatiepezen/-ligamenten, middenvoetspezen/-ligamenten of metatarsale pezen/ligamenten, peestransfers, heraanhechting van pezen en ligamentreconstructies van de middenvoet, voorvoet en achtervoet, procedures in verband met platvoetreconstructie, misvorming van de achtervoet, middenvoetsreconstructie, laterale/mediale enkelreconstructie of -instabiliteit, hallux valgus of varus en metatarsofalangeale instabiliteit waaronder:• heraanhechting/reconstructie van achillespees • transfer van flexor digitorum longus • transfer van flexor hallucis longus• transfer van extensor hallucis longus• procedures volgens Broström• relocatie van kuitbeenpees• kapselherstel• reconstructie/heraanhechting van deltaspier
• herstel van plantaire ligamenten• herstel van sprongligament
C. CONTRA-INDICATIESGeen productspecifieke contra-indicaties.
D. MOGELIJKE COMPLICATIES EN BIJWERKINGENBij elke chirurgische ingreep bestaat de kans op complicaties. De risico’s en complicaties met deze implantaten zijn onder meer:• infectie of pijnlijke, gezwollen of ontstoken implantatieplek• fractuur van het implantaat• losraken of dislocatie van het implantaat waardoor revisiechirurgie noodzakelijk is• botresorptie of overproductie van bot• allergische reactie(s) op implantaatmateriaal• ongunstige histologische reacties met mogelijke macrofagen en/of fibroblasten• migratie van slijtagedeeltjes met mogelijke lichamelijke respons als gevolg• embolie
E. VOORZORGSMAATREGELENDe kans op complicaties of bijwerkingen bij elk implantaat kan worden geminimaliseerd door de gebruiksaanwijzing in de productliteratuur te volgen.Het is de verantwoordelijkheid van elke chirurg die implantaten gebruikt, om rekening te houden met de klinische en medische toestand van elke patiënt, en goed geïnformeerd te zijn over alle aspecten van de implantatie-ingreep en de mogelijke complicaties die kunnen optreden. Het is mogelijk dat de voordelen van implantaatchirurgie niet voldoen aan de verwachtingen van de patiënt of na
verloop van tijd afnemen, waardoor revisiechirurgie noodzakelijk is om het implantaat te vervangen of alternatieve ingrepen uit te voeren. Revisiechirurgie doet zich regelmatig voor bij implantaten. Ook moet rekening worden gehouden met de geestelijke gesteldheid van de patiënt. De bereidheid en/of het vermogen om de instructies na de operatie op te volgen, kan het resultaat van de chirurgie ook beïnvloeden. Chirurgen moeten veel aspecten in overweging nemen voor het behalen van de beste resultaten bij elke patiënt.INDIEN OVERMATIGE BELASTING NIET KAN WORDEN VOORKOMEN, MAG GEEN IMPLANTAAT WORDEN AANGEBRACHT.De voornaamste doelstelling van een operatie met dit implantaat is om aanhechting van weke delen op bot te bewerkstelligen. Abnormale of overmatige krachten kunnen leiden tot vertraagde aanhechting, nonunion (niet-genezende fractuur) of falen van het implantaat.Abnormale lastdraging en daaropvolgende slijtage kunnen worden veroorzaakt door:• niet-gecorrigeerde instabiliteit• implantaat van verkeerde afmeting• onvoldoende ondersteuning van weke delen• slechte positie van het implantaat• overmatige beweging• niet-gecorrigeerde of terugkerende vervorming• verkeerd gebruik of overmatige activiteit van patiëntDe juiste fixatie ten tijde van de operatie is cruciaal voor het slagen van de ingreep. De botmassa moet voldoende zijn om het hulpmiddel te kunnen ondersteunen.Sommige preventieve maatregelen ter overweging voor het minimaliseren van de kans op complicaties:• volg de richtlijnen voor indicaties en contra-indicaties hierboven
• identificeer eerdere pathologie• stabiliseer door inzakking veroorzaakte vervormingen• plaats een bottransplantaat in bestaande cysten• gebruik een implantaat van de juiste afmeting• vermijd K-draden en hechtdraden door het implantaatVermijd het veroorzaken van gebreken op de implantaatoppervlakken om de mogelijkheid van vroegtijdig falen door moeheid tot een minimum te beperken.
Indien zich complicaties voordoen, zijn mogelijke corrigerende procedures onder andere:• verwijdering van het implantaat• synovectomie• plaatsen van bottransplantaten in cysten• vervangen van het implantaat• verwijdering van het implantaat met fusie van het gewricht
Aanbevelingen met betrekking tot fragmenten van hulpmiddelen• Gebruik medische hulpmiddelen zoals vermeld op het etiket en in de gebruiksaanwijzing van
Wright Medical Technology, in het bijzonder tijdens het inbrengen en verwijderen.• Inspecteer de hulpmiddelen vóór gebruik op transport- of opslagschade en op gebreken bij het
uitpakken die de kans op fragmentatie tijdens een ingreep doen toenemen.• Controleer hulpmiddelen onmiddellijk na verwijdering uit de patiënt op tekenen van breuk
of fragmentatie.• Bewaar een beschadigd hulpmiddel om Wright Medical Technology te helpen bij de analyse van
de gang van zaken.
• Overweeg zorgvuldig de risico’s en voordelen van verwijdering van het fragment t.o.v. het fragment in de patiënt achterlaten en bespreek dit (zo mogelijk) met de patiënt.
• Adviseer de patiënt over de aard en veiligheid van niet-verwijderde fragmenten van hulpmiddelen, met inbegrip van de volgende informatie:a. De samenstelling van het materiaal van het fragment (indien bekend);b. De grootte van het fragment (indien bekend);c. De positie van het fragment;d. Mogelijke mechanismen die tot letsel kunnen leiden, bijvoorbeeld migratie en infectie;e. Te vermijden ingrepen of behandelingen, zoals MRI’s in het geval van metalen fragmenten.
Dit kan de kans helpen verkleinen dat het fragment ernstig letsel veroorzaakt.De klinische resultaten hangen af van de chirurg en de technieken, van de preoperatieve en postoperatieve verzorging, van het implantaat, en van de pathologie en dagelijkse activiteiten van de patiënt. Het is belangrijk dat chirurgen de benodigde geïnformeerde toestemming verkrijgen en vóór de ingreep de kans op complicaties met elke patiënt bespreken. Dit kan het doornemen van alternatieve, niet-implantatie-ingrepen omvatten, zoals reconstructie van weke delen of artrodese.Betreffende de MRI-omgevingEr zijn inherente risico’s verbonden aan het gebruik van implantaten in de MRI-omgeving, waaronder migratie van componenten, warmteontwikkeling en signaalstoring of -vervorming in de buurt van de component(en). Warmteontwikkeling in implantaten is een risico verbonden aan de configuratie en het materiaal van de componenten, alsmede aan het vermogen, de duur en pulssequentie van de MRI. Aangezien MRI-apparatuur niet is gestandaardiseerd, is de ernst en waarschijnlijkheid van warmteontwikkeling onbekend voor deze implantaten.De veiligheid en compatibiliteit van deze implantaten in de MRI-omgeving zijn niet geëvalueerd. Deze implantaten zijn niet getest op opwarming, migratie of artefactvorming in de MRI-omgeving.
De veiligheid van dit implantaat in een MRI-omgeving is onbekend. Een patiënt met het hulpmiddel kan letsel oplopen bij een MRI-scan.
F. HANTERING EN STERILISATIEIMPLANTATENDe in deze bijsluiter beschreven implantaten worden steriel geleverd. De implantaten in steriele verpakking moeten worden geïnspecteerd om te garanderen dat de verpakking niet beschadigd of eerder geopend is. Neem contact op met de fabrikant voor verdere aanwijzingen indien de integriteit van de binnenverpakking aangetast is. De implantaten moeten worden geopend met een aseptische OK-techniek; ze mogen pas worden geopend na vaststelling van de correcte maat.Dit product is uitsluitend voor eenmalig gebruik bestemd. Een implantaat mag na contact met lichaamsweefsels of -vocht nooit opnieuw gesteriliseerd worden.Uitsluitend voor eenmalig gebruik gelabelde hulpmiddelen mogen nooit opnieuw worden gebruikt. Deze hulpmiddelen opnieuw gebruiken kan mogelijk resulteren in ernstig letsel aan de patiënt. Voorbeelden van gevaren die met het opnieuw gebruiken van deze hulpmiddelen gepaard gaan, zijn onder meer aanzienlijke verslechtering van de werking van het hulpmiddel, kruisbesmetting en contaminatie.
G. OPSLAGCONDITIESAlle implantaten moeten in een schone, droge omgeving worden bewaard en tegen zonlicht en extreme temperaturen worden beschermd.

BESCHRIJVINGHet GRAVITY titaan hechtankersysteem omvat steriele hulpmiddelen voor eenmalig gebruik bij procedures voor het opnieuw aanhechten van weke delen. II. INFORMATIONS GÉNÉRALES SUR LE PRODUITGrâce aux progrès réalisés dans le domaine du matériel pour les fixations internes, le chirurgien dispose de moyens pour corriger les déformations et réduire la douleur chez de nombreux patients. Bien que les implants soient pour la plupart utilisés avec succès, il faut reconnaître qu’ils sont fabriqués en métal et en matériaux polymères et que l’on ne peut s’attendre à ce qu’un implant puisse endurer les niveaux d’activité et les charges que supporterait un os sain normal après fusion. Pour chaque prise de décision concernant le choix d’un implant, le chirurgien doit évaluer chaque situation individuelle d’après l’état clinique du patient.Les chirurgiens doivent être familiarisés avec les techniques opératoires et le mode d’emploi appropriés à chaque produit. Cette notice et l’étiquette de l’emballage contiennent les avertissements, précautions et contre-indications essentiels pour chaque intervention chirurgicale. Consulter par ailleurs la technique chirurgicale pour obtenir des informations détaillées sur la sélection de l’implant, des détails pertinents sur le produit, des instructions proposées sur l’intervention chirurgicale et/ou l’utilisation de l’assemblage. Le chirurgien doit prendre contact avec Wright pour obtenir des informations sur la technique chirurgicale proposée spécifique au produit.Dans le cadre de l’utilisation d’implants pour la fusion osseuse, le chirurgien doit être informé de ce qui suit :• Il est extrêmement important de sélectionner correctement les implants de taille appropriée.
La sélection d’un implant de taille, de forme et de conception appropriées augmente les chances de succès de l’intervention. Les implants exigent une mise en place précise et un soutien osseux adéquat. Le choix adéquat d’un implant doit prendre en compte le design et la fixation, le poids, l’âge, la qualité osseuse, la taille, le niveau d’activité physique et l’état de santé préopératoire du patient ainsi que l’expérience du chirurgien et sa connaissance du dispositif. La longévité et la stabilité de l’implant peuvent être affectées par ces variables. Le chirurgien doit informer les patients de l’existence de ces facteurs.
• Lors de la sélection des patients candidats à la chirurgie, les facteurs suivants peuvent avoir une importance capitale pour la réussite de la procédure :
1. Profession ou activité du patient. Si la profession ou l’activité du patient inclut le soulèvement de charges et des efforts musculaires importants, les forces résultantes peuvent entraîner la défaillance de la fixation, du dispositif ou de ces deux éléments à la fois. L’implant ne rétablit pas la fonctionnalité qu’on peut attendre d’un os sain normal et le patient ne doit pas s’attendre à des résultats fonctionnels irréalistes.
2. Sénilité, maladie mentale ou alcoolisme. Ces conditions peuvent, entre autres, conduire le patient à ignorer certaines limitations et précautions nécessaires pour l’utilisation de l’implant, ce qui peut entraîner un échec ou d’autres complications.
3. Sensibilité aux corps étrangers. Lorsqu’on soupçonne une sensibilité aux matériaux, il convient d’effectuer les tests appropriés avant la sélection ou l’implantation du matériau.
A. SÉLECTION DES PATIENTSL’utilisation de matériel de fixation interne nécessite de prendre en considération les indications générales suivantes :• Bon état général du patient• Bon état neurovasculaire• Couverture cutanée adéquate• Possibilité de système musculo-tendineux fonctionnel• Stock osseux adéquat pour recevoir l’implant• Disponibilité d’un traitement postopératoire• Coopération du patient
B. INDICATIONSLes systèmes d’ancre de suture en titane GRAVITY sont indiqués pour :
• La réparation d’une instabilité de l’épaule secondaire à une lésion de Bankart, une déchirure de la coiffe des rotateurs, une lésion SLAP, une entorse de l’articulation acromioclaviculaire, une ténodèse du biceps, une déchirure/rupture du deltoïde ou un déplacement capsulaire ou bien la reconstruction du bourrelet et de la capsule ;
• La réparation d’une instabilité du coude secondaire au décollement du tendon du biceps, à une épicondylite des joueurs de tennis ou à une déchirure/rupture du ligament collatéral ulnaire ou radial ;
• La réparation d’une instabilité de la main ou du poignet secondaire à une déchirure ou rupture du ligament scapho-lunaire, du ligament collatéral ulnaire ou du ligament collatéral radial ;
• La réparation d’une instabilité du genou secondaire à une déchirure ou rupture du ligament collatéral médial, du ligament collatéral latéral, du tendon patellaire ou du ligament oblique postérieur ou secondaire à une ténodèse de la bandelette ilio-tibiale ;
• La réparation d’une instabilité du pied ou de la cheville secondaire à une déchirure ou rupture du tendon d’Achille, la stabilisation latérale de tendons/ligaments, la stabilisation médiale de tendons/ligaments, de tendons/ligaments du médio-pied ou de tendons/ligaments métatarsiens, transferts ou refixations de tendons, et interventions de reconstruction des ligaments du médio-pied, de l’avant-pied et de l’arrière-pied, interventions associées à une reconstruction du pied plat, une déformation de l’arrière-pied, une reconstruction du médio-pied, une reconstruction ou instabilité latérale/médiale de la cheville, un Hallux Valgus ou Varus, et une instabilité MTP, notamment :• Refixation/reconstruction du tendon d’Achille • Transfert tendineux du muscle long fléchisseur des orteils • Transfert tendineux du muscle long fléchisseur de l’hallux• Transfert tendineux du muscle long extenseur de l’hallux• Procédure Broström• Relocalisation de tendon péronier

C. CONTRE-INDICATIONSIl n’existe aucune contre-indication spécifique au produit.
D. COMPLICATIONS ET RÉACTIONS INDÉSIRABLES POSSIBLESToute procédure chirurgicale implique des risques de complications. Les risques et les complications associés à ces implants comprennent :• Infection ou douleurs, enflure ou inflammation du site d’implantation• Fracture de l’implant• Descellement ou déboîtement de l’implant nécessitant une intervention de révision• Résorption osseuse ou ossification excessive• Réactions allergiques aux matériaux prothétiques• Réactions histologiques indésirables pouvant impliquer des macrophages et/ou fibroblastes• Migration de particules d’usure pouvant entraîner une réaction physiologique• Embolie
E. PRÉCAUTIONSIl est possible de minimiser les risques de complications ou de réactions indésirables associés à tout implant en suivant le mode d’emploi fourni dans la documentation sur le produit.
Il incombe à chaque chirurgien utilisant des implants de tenir compte de l’état clinique et médical de chaque patient et de s’informer de tous les aspects de la procédure d’implantation ainsi que des risques de complications possibles. Les avantages de l’implantation chirurgicale peuvent ne pas répondre aux attentes du patient ou diminuer avec le temps, et une reprise chirurgicale peut s’avérer nécessaire afin de remplacer l’implant ou d’effectuer une autre intervention. Les reprises de prothèse sont des interventions courantes. Il convient également de tenir compte de l’état psychologique du patient. La volonté ou la capacité de suivre des instructions postopératoires peuvent également avoir une influence sur le résultat de l’intervention chirurgicale. Le chirurgien devra examiner de multiples facteurs afin d’obtenir le meilleur résultat possible pour chaque patient.SI L’APPLICATION DE CHARGES EXCESSIVES NE PEUT PAS ÊTRE ÉVITÉE, NE PAS UTILISER D’IMPLANT.L’objectif principal de l’intervention chirurgicale avec cet implant est d’unir les tissus mous à l’os. Des forces anormales ou excessives risquent de provoquer un retard de soudure osseuse, une pseudarthrose ou l’échec de l’implant.Une mise en charge anormale et l’usure qui en résulte peuvent être produites par divers facteurs :• Instabilité non corrigée• Un implant de taille inappropriée• Soutien inadéquat des tissus mous• Mauvais positionnement de l’implant• Mouvements excessifs• Déformation non corrigée ou récidivante• Usage non conforme ou activité excessive du patientLa fixation adéquate de l’implant au moment de l’opération est essentielle pour la réussite de l’intervention. La masse osseuse doit être adéquate pour soutenir le dispositif.Mesures de prévention à envisager pour minimiser les complications possibles :
• Suivre les instructions relatives aux indications et contre-indications énoncées ci-avant• Identifier les pathologies antérieures• Stabiliser les déformations par tassement• Effectuer une greffe osseuse sur les kystes préexistants• Utiliser un implant de dimensions appropriées• Éviter les broches de Kirschner et les sutures à travers l’implantÉviter d’endommager les surfaces de l’implant pour minimiser les possibilités de défaillance dues à une fatigue précoce.
Si des complications se produisent, diverses interventions correctives sont possibles :• Retrait de l’implant• Synovectomie• Greffe osseuse des kystes• Remplacement de l’implant• Retrait de l’implant avec fusion de l’articulation
Recommandations concernant les fragments de dispositif• Utiliser les dispositifs médicaux conformément à leurs libellés d’indication et aux modes d’emploi
de Wright Medical Technology particulièrement pendant l’insertion et le retrait.• Inspecter les dispositifs avant l’utilisation pour vérifier qu’ils n’ont pas été endommagés
pendant le transport ou le stockage et qu’ils ne présentent aucun défaut à l’ouverture de l’emballage susceptible d’augmenter la probabilité de fragmentation au cours d’une procédure chirurgicale.
• Inspecter les dispositifs immédiatement après les avoir retirés du corps du patient pour vérifier qu’ils ne sont ni rompus, ni fragmentés.
• Si le dispositif est endommagé, le conserver pour aider Wright Medical Technology à analyser le cas.
• Prendre en compte les risques et les avantages d’une récupération du fragment plutôt que son abandon dans le corps, et (si possible) en discuter avec le patient.
• Informer le patient de la nature et de la sécurité des fragments de dispositif non récupérés en mentionnant les informations suivantes :a. La composition matérielle du fragment (si elle est connue) ;b. La dimension du fragment (si elle est connue) ;c. L’emplacement du fragment ;d. Les mécanismes potentiels de lésion, par ex. migration, infection ;e. Les examens ou les traitements devant être évités tels que les examens d’IRM dans le cas de
fragments métalliques. Cela peut diminuer le risque de lésion grave due au fragment.Les résultats cliniques dépendent du chirurgien et de sa méthode, des soins pré- et postopératoires, de l’implant, de la pathologie et de l’activité quotidienne du patient. Il est important que le chirurgien obtienne un consentement éclairé et discute des complications possibles avec chaque patient avant l’intervention. Cela peut comprendre le passage en revue des autres procédures sans implant, comme la reconstruction des tissus mous ou l’arthrodèse.À propos des milieux d’imagerie par résonance magnétiqueIl existe des risques inhérents associés à l’utilisation d’implants dans l’environnement IRM, notamment la migration de composants, l’induction de chaleur, l’interférence ou la distorsion de signaux près du ou des composants. Le risque d’induction de chaleur des implants dépend de la géométrie et de la nature du matériau, ainsi que de la puissance, de la durée et de la séquence d’impulsion de l’IRM.
Comme l’équipement d’IRM n’est pas standardisé, l’importance et la probabilité de survenue sont inconnues pour ces implants.La sécurité et la compatibilité de ces implants n’ont pas été évaluées en milieu IRM. L’échauffement, la migration et l’artefact d’image de ces implants n’ont pas fait l’objet de tests en milieu IRM. La sécurité de cet implant en milieu IRM est inconnue. Scanner un patient porteur du dispositif peut conduire à des lésions du patient.
F. MANIPULATION ET STÉRILISATIONIMPLANTSLes implants décrits dans cette notice sont fournis stériles. Les implants sous emballage stérile doivent être inspectés afin de s’assurer que l’emballage n’a pas été endommagé ou précédemment ouvert. Si l’intégrité de l’emballage interne a été compromise, prendre contact avec le fabricant pour obtenir des instructions supplémentaires. Les implants doivent être retirés de leur emballage en utilisant une technique aseptique de bloc opératoire ; mais seulement après avoir déterminé la taille correcte.Ce produit est exclusivement à usage unique. Ne jamais restériliser un implant ayant été mis en contact avec des tissus ou des liquides organiques.Les dispositifs indiqués pour un usage unique ne doivent jamais être réutilisés. La réutilisation de ces dispositifs peut potentiellement provoquer des lésions graves chez le patient. Parmi les exemples de dangers associés à la réutilisation de ces dispositifs, on citera : dégradation significative des performances du dispositif, infection croisée et contamination.
G. CONDITIONS DE STOCKAGETous les implants doivent être conservés dans un environnement propre et sec et protégés de la lumière du soleil et des températures extrêmes.

II. INFORMACIÓN GENERAL DEL PRODUCTOGracias a los avances en el utillaje para fijación interna, el cirujano cuenta con medios para corregir deformidades y reducir el dolor en muchos pacientes. Aunque los implantes utilizados han tenido mucho éxito en la consecución de estas metas, hay que recordar que están hechos de metal y materiales poliméricos, y que no se puede esperar que ningún implante soporte las cargas y los niveles de actividad que podría soportar un hueso normal sano después de la fusión. El cirujano debe evaluar cada situación de forma individual, basándose en la presentación clínica del paciente, para tomar cualquier decisión referente a la selección del implante.Los cirujanos deben estar familiarizados con las técnicas quirúrgicas aplicables y las instrucciones de uso de cada producto. Este prospecto y la etiqueta del envase interno contienen advertencias, precauciones y contraindicaciones esenciales para cada intervención. Además, debe consultarse la técnica quirúrgica para obtener información detallada sobre la selección del implante, los detalles importantes del producto, las instrucciones quirúrgicas propuestas y el uso del conjunto. El cirujano debe ponerse en contacto con Wright para obtener información detallada sobre la técnica quirúrgica propuesta específicamente para el producto.Al utilizar implantes de fusión, el cirujano debe tener en cuenta lo siguiente:• La selección y el tamaño correctos del implante son extremadamente importantes. La selección
del tamaño, forma y diseño correctos del implante aumenta las posibilidades de éxito. Los implantes requieren un asentamiento cuidadoso y un soporte óseo adecuado. Para la selección correcta del implante, debe tenerse en cuenta el diseño, la fijación, el peso y la edad del paciente, la calidad del hueso, el tamaño, el grado de actividad, el estado de salud antes de la intervención, así como la experiencia del cirujano y su familiaridad con el dispositivo. Todas estas variables pueden influir en la duración y la estabilidad del implante. Los cirujanos deben informar a los pacientes de estos factores.
• Al seleccionar a los pacientes para cirugía, los factores que se indican a continuación pueden ser de vital importancia para el éxito final del procedimiento:
1. La profesión o la actividad del paciente. Si el paciente tiene una profesión o realiza actividades físicas en las que levanta pesos considerables o realiza esfuerzo muscular, las fuerzas resultantes pueden provocar el fracaso de la fijación, del dispositivo o de ambos. El implante no devolverá la función al nivel esperado con un hueso normal sano y el paciente no debe tener expectativas funcionales poco realistas.
2. Existencia de senilidad, enfermedad mental o alcoholismo. La existencia de estas afecciones, entre otras, puede hacer que el paciente ignore ciertas limitaciones y precauciones necesarias para el uso del implante, lo que puede conducir a su fracaso o a otras complicaciones.
3. Sensibilidad a cuerpos extraños. Si se sospecha una posible intolerancia al material, es preciso realizar las pruebas pertinentes antes de elegir o implantar un material.
A. SELECCIÓN DEL PACIENTEEl uso de dispositivos de fijación interna exige que se tengan en cuenta las siguientes indicaciones generales:• Buen estado del paciente• Buen estado neurovascular• Adecuada cobertura cutánea• Posibilidad de un sistema musculotendinoso funcional• Existencia adecuada de hueso para recibir el implante• Disponibilidad de tratamiento postoperatorio• Paciente que coopera
B. INDICACIONESLos sistemas de anclaje de sutura GRAVITY están indicados para utilizarse:
• En la reparación de la inestabilidad del hombro debida a una lesión de Bankart, un desgarro del manguito de los rotadores, una lesión «slap», una separación acromioclavicular, una tenodesis del bíceps, un desgarro o separación del deltoides, un desplazamiento capsular o una reconstrucción capsulolabral;
• En la reparación de la inestabilidad del codo debida a un desprendimiento del tendón del bíceps, codo de tenista, o un desgarro o separación de los ligamentos colaterales del radio o el cúbito;
• En la reparación de la inestabilidad de la mano o la muñeca debida a un desgarro o separación del ligamento escafolunar, el ligamento colateral cubital o el ligamento colateral radial;
• En la reparación de la inestabilidad de la rodilla debida a un desgarro o separación del ligamento colateral medial, el ligamento colateral lateral, el tendón rotuliano o el ligamento oblicuo posterior, o como consecuencia de una tenodesis de la banda iliotibial;
• En la reparación de la inestabilidad del pie o el tobillo debida a un desgarro o separación del tendón de Aquiles, los tendones/ligamentos de estabilización lateral, los tendones/ligamentos de estabilización medial, los tendones/ligamentos del mediopié o los tendones/ligamentos metatarsianos, transferencias de tendones, refijaciones de tendones, y reconstrucciones de ligamentos del mediopié, antepié y retropié, procedimientos asociados con la reconstrucción del pie plano, deformidad del retropié, reconstrucción del mediopié, reconstrucción o inestabilidad del tobillo lateral o medial, hallux valgus o varus, e inestabilidad metatarsofalángica, incluido:• Refijación o reconstrucción del tendón de Aquiles • Transferencia del flexor digitorum longus • Transferencia del flexor hallucis longus• Transferencia del extensor hallucis longus• Procedimientos de Bostrom• Reubicación del tendón peroneo• Reparación capsular

D. POSIBLES COMPLICACIONES Y REACCIONES ADVERSASEn todo procedimiento quirúrgico, existe la posibilidad de que se presenten complicaciones. E. PRECAUCIONESSi se siguen las instrucciones de uso que se proporcionan en el folleto del producto, se puede reducir al mínimo la posibilidad de que se presenten las complicaciones o las reacciones adversas que pueden ocurrir con cualquier implante.
Todo cirujano que utilice implantes tiene la responsabilidad de considerar el estado clínico y médico de cada paciente, y de familiarizarse con todos los aspectos del procedimiento de implante y las posibles complicaciones que se puedan producir. Las ventajas derivadas de la cirugía del implante pueden no cumplir las expectativas del paciente o deteriorarse con el paso del tiempo, obligando a una cirugía de revisión para reemplazar el implante o a llevar a cabo procedimientos alternativos. Las operaciones de revisión son frecuentes con los implantes. También debe considerarse el estado mental del paciente. La voluntad y/o capacidad de seguir las instrucciones postoperatorias también pueden afectar al resultado quirúrgico. Los cirujanos deben considerar muchas cuestiones para lograr el mejor resultado para cada paciente en particular.SI NO PUEDE EVITARSE LA CARGA EXCESIVA, NO SE DEBE USAR UN IMPLANTE.El objetivo principal de la cirugía con el implante es establecer una unión entre el tejido blando y el hueso. Las fuerzas anormales o excesivas podrían dar lugar a unión retardada, falta de unión o fracaso del implante.La carga anormal y el desgaste subsiguiente pueden deberse a:• Inestabilidad no corregida• Implante de tamaño incorrecto• Sostén inadecuado del tejido blando• Mala posición del implante• Movimiento excesivo• Malformación no corregida o recurrente• Mal uso o exceso de actividad por parte del pacienteLa fijación correcta en el momento de la intervención es de vital importancia para el éxito del procedimiento. Debe existir una adecuada cantidad de hueso para sostener el dispositivo.
Algunas medidas preventivas a considerar para reducir al mínimo la posibilidad de que se presenten complicaciones:• Siga las recomendaciones para las indicaciones y contraindicaciones previamente descritas• Identifique las patologías previas• Estabilice las deformaciones por colapso• Trate con injertos óseos los quistes preexistentes• Utilice un implante del tamaño correcto• Evite alambres de Kirschner y suturas en todo el implanteEvite causar desperfectos en las superficies del implante para reducir al mínimo las posibilidades de fallo por debilitamiento precoz del material.
Si se desarrollan complicaciones, los posibles procedimientos correctivos incluyen:• Extracción del implante• Sinovectomía• Injerto óseo de quistes• Reemplazo del implante• Extracción del implante con fusión de la articulación
Recomendaciones acerca de los fragmentos del dispositivo• Use los dispositivos médicos de acuerdo con las indicaciones que figuran en sus etiquetas y con
las instrucciones de uso de Wright Medical Technology, especialmente durante su inserción y su extracción.
• Inspeccione los dispositivos antes de usarlos para comprobar la ausencia de daños producidos durante el transporte o el almacenamiento, o de cualquier defecto evidente al desembalarlos que aumente la probabilidad de fragmentación durante una intervención.
• Inspeccione los dispositivos inmediatamente después de retirarlos del paciente para ver si hay señales de rotura o fragmentación.
• Si el dispositivo ha sufrido algún daño, consérvelo para que sirva de ayuda al análisis que Wright Medical Technology efectuará del caso.
• Considere detenidamente los riesgos y las ventajas de recuperar el fragmento frente a la opción de dejarlo en el paciente, y coméntelos con este (si es posible).
• Informe al paciente acerca de la naturaleza y seguridad de los fragmentos del dispositivo que no se recuperan, incluida la siguiente información:a. La composición material del fragmento (si se conoce);b. El tamaño del fragmento (si se conoce);c. La localización del fragmento;d. Los posibles mecanismos de lesión como, por ejemplo, migración, infección;e. Los procedimientos o tratamientos que deben evitarse, tales como exploraciones por
resonancia magnética en el caso de fragmentos metálicos. Esto podría ayudar a disminuir el riesgo de sufrir lesiones graves causadas por el fragmento.
Los resultados clínicos dependen del cirujano y de la técnica, de la atención pre y postoperatoria, del implante, de la patología del paciente y de la actividad diaria. Es importante que los cirujanos obtengan el consentimiento informado correspondiente y expliquen a cada paciente, antes de la operación, la posibilidad de que se presenten complicaciones. Esto puede incluir una revisión de procedimientos alternativos, sin implantes, como la reconstrucción de tejido blando o artrodesis.
Información sobre los entornos de resonancia magnéticaExisten riesgos inherentes asociados al uso de implantes en un entorno de RM, incluidos la migración del componente, la inducción térmica y la distorsión o interferencia de señales en zonas próximas a los componentes. La inducción térmica de los implantes es un riesgo relacionado con la geometría y el material de los componentes, al igual que con la potencia, la duración y la secuencia de impulsos de la RM. Dado que los equipos de RM no están estandarizados, se desconocen la intensidad y probabilidad de estas manifestaciones con estos implantes.No se ha evaluado la seguridad ni la compatibilidad de estos implantes en el entorno de RM. Tampoco se han evaluado el calentamiento, la migración ni los artefactos de imagen de estos implantes en el entorno de RM. Se desconoce la seguridad de este implante en el entorno de RM. El escaneado de un paciente que tenga este dispositivo implantado puede provocar lesiones al paciente.
F. MANIPULACIÓN Y ESTERILIZACIÓNIMPLANTESLos implantes descritos en este prospecto se suministran estériles. Los implantes suministrados en su envase estéril deberán inspeccionarse para comprobar que dicho envase no ha sufrido ningún daño y que no se ha abierto con anterioridad. En caso de duda sobre la integridad del envase interno, solicite instrucciones al fabricante. Los implantes deberán abrirse mediante técnica quirúrgica aséptica; y sólo deberá procederse a su apertura una vez que se haya determinado que su tamaño es el correcto.Este producto es exclusivamente para un solo uso. Nunca se debe volver a esterilizar un implante que haya estado en contacto con tejidos o líquidos corporales.Los dispositivos etiquetados para un solo uso nunca deben reutilizarse. La reutilización de estos dispositivos podría provocar lesiones graves al paciente. Los riesgos relacionados con la reutilización de estos dispositivos son, entre otros: degradación significativa del rendimiento del dispositivo, infección cruzada y contaminación.
G. CONDICIONES DE ALMACENAMIENTOTodos los implantes deben almacenarse en un ambiente limpio y seco, y deben protegerse de la luz del sol y de las temperaturas extremas.

II. INFORMAZIONI GENERALI SUL PRODOTTOGrazie ai progressi dei dispositivi per fissazione interna, il chirurgo ha a disposizione strumenti atti a correggere le deformità e alleviare il dolore in molti pazienti. Sebbene gli impianti utilizzati siano altamente efficaci nel raggiungere tali obiettivi, va comunque considerato che sono realizzati in metallo e materiali polimerici, e che non si può pretendere che un impianto sopporti gli stessi livelli di attività e carichi normalmente tollerati da ossa sane e normali dopo la fusione. Nella fase decisionale di scelta dell’impianto, il chirurgo deve valutare ciascuna situazione singolarmente, in base alla presentazione clinica del paziente.Il chirurgo deve avere acquisito familiarità con le tecniche operatorie pertinenti e conoscere le istruzioni per l’uso di ciascun prodotto. Il presente foglietto illustrativo e l’etichetta della confezione più interna riportano avvertenze, precauzioni e controindicazioni essenziali per ogni intervento chirurgico. Oltre a questo è necessario fare riferimento alla tecnica chirurgica per ottenere informazioni dettagliate sulla scelta dell’impianto, i dati specifici del prodotto, le istruzioni consigliate per l’intervento e/o l’uso dell’unità. Il chirurgo è tenuto a contattare Wright in merito alla tecnica chirurgica proposta, specifica per il prodotto.Per usare gli impianti di fusione, il chirurgo deve essere a conoscenza di quanto segue.• La corretta scelta del tipo e delle dimensioni dell’impianto è di fondamentale importanza. La
scelta della misura, forma e struttura corrette dell’impianto aumenta le probabilità di successo. Gli impianti devono essere posizionati con cura e richiedono un adeguato sostegno osseo. Una scelta adeguata dell’impianto deve tenere conto di aspetti quali struttura, fissazione, peso ed età del paziente, qualità dell’osso, dimensioni, livello di attività, condizioni di salute pre-operatorie nonché dell’esperienza e della familiarità del chirurgo con il dispositivo. Queste variabili possono condizionare la longevità e la stabilità dell’impianto. I chirurghi devono avvertire i pazienti in merito a questi fattori.
• Nella selezione dei pazienti per l’intervento chirurgico, i seguenti fattori possono rivelarsi critici per il successo dell’intervento:
1. Occupazione o attività svolta dal paziente. Se il paziente svolge una professione o un’attività che comporta il sollevamento di pesi o un notevole sforzo muscolare, le forze risultanti possono causare il cedimento della fissazione, del dispositivo o di entrambi. L’impianto non restituisce un livello di funzionalità identico a quello atteso da un tessuto osseo sano e normale, pertanto il paziente non dovrà nutrire aspettative funzionali irrealistiche.
2. Senilità, malattie mentali o alcolismo. Queste e altre condizioni possono indurre il paziente a non tenere conto di alcune limitazioni e precauzioni necessarie nell’uso dell’impianto, dando luogo a cedimenti o ad altre complicanze.
3. Sensibilità a corpi estranei. Se si sospetta una sensibilità a determinati materiali, si devono eseguire gli opportuni test prima della selezione del materiale o dell’impianto.
A. SELEZIONE DEI PAZIENTIL’uso di dispositivi per fissazione interna richiede di tenere in considerazione le seguenti indicazioni generali:• buone condizioni del paziente• buono stato neurovascolare• adeguata copertura cutanea• possibilità di un sistema muscolo-tendineo funzionale• tessuto osseo adeguato per accogliere l’impianto• disponibilità di terapia post-operatoria• paziente collaborante
B. INDICAZIONII sistemi di ancoraggio delle suture GRAVITY sono indicati per l’uso nei casi seguenti:
• riparazione di instabilità della spalla secondaria a lesione di Bankart, lacerazione della cuffia dei rotatori, lesioni del labbro superiore antero-posteriore (SLAP), separazione acromio-clavicolare, tenodesi del bicipite, lacerazione/separazione del deltoide, o spostamento della capsula articolare o ricostruzione capsulo-labrale;
• riparazione di instabilità del gomito secondaria a distacco del tendine del bicipite, gomito del tennista o lacerazione/separazione del legamento collaterale ulnare o radiale;
• riparazione di instabilità della mano/polso secondaria a lacerazione o separazione del legamento scafo-lunato, del legamento collaterale ulnare o del legamento collaterale radiale;
• riparazione di instabilità del ginocchio secondaria a lacerazione o separazione del legamento collaterale mediale, del legamento collaterale laterale, del tendine patellare o del legamento obliquo posteriore, o secondaria a tenodesi della bandelletta ilio-tibiale;
• riparazione di instabilità del piede/caviglia secondaria a lacerazione o separazione del tendine di Achille, stabilizzazione laterale di tendini/legamenti, stabilizzazione mediale di tendini/legamenti, tendini/legamenti del mesopiede, o tendini/legamenti metatarsali, trasferimenti tendinei, riattacco dei tendini, e ricostruzione dei legamenti di mesopiede, avampiede e retropiede, procedure associate alla ricostruzione del piede piatto, deformità del retropiede, ricostruzione del mesopiede, ricostruzione o instabilità laterale/mediale della caviglia, alluce valgo o varo, e instabilità metatarsofalangea, inclusi:• riattacco/ricostruzione del tendine di Achille • trasferimento del flessore lungo delle dita • trasferimento del flessore lungo dell’alluce• trasferimento dell’estensore lungo dell’alluce• intervento di Broström• rilocazione del tendine dei muscoli peronieri• riparazione della capsula articolare
• ricostruzione/riattacco del deltoide• riparazione della placca plantare• riparazione del legamento calcaneo-scafoideo plantare
C. CONTROINDICAZIONINon esistono controindicazioni specifiche per il prodotto.
D. POSSIBILI COMPLICANZE E REAZIONI AVVERSEIn qualunque intervento chirurgico esiste la possibilità che insorgano complicanze. Tra i rischi e le complicanze connessi all’inserimento di questi impianti vi sono:• infezione o sito d’impianto dolente, gonfio o infiammato• rottura dell’impianto• allentamento o spostamento dell’impianto con necessità di intervento correttivo• riassorbimento o sovrapproduzione di materiale osseo• reazioni allergiche ai materiali impiantati• risposte istologiche negative con possibile coinvolgimento di macrofagi e/o fibroblasti• migrazione di detriti da usura che possono dare luogo a reazione corporea• embolia
E. PRECAUZIONICome per ogni tipo di impianto, l’osservanza scrupolosa delle istruzioni per l’uso contenute nella documentazione in dotazione al prodotto contribuisce a minimizzare le probabilità di complicanze o reazioni indesiderate.È compito del singolo chirurgo che utilizza gli impianti valutare le condizioni cliniche e mediche del paziente e conoscere nei dettagli i vari aspetti dell’intervento di impianto, nonché le possibili
complicanze che possono insorgere. I benefici derivanti dall’intervento di impianto potrebbero non rispondere alle aspettative del paziente o venire meno nel tempo, richiedendo un intervento correttivo per sostituire l’impianto o eseguire procedure alternative. Gli interventi correttivi su pazienti impiantati sono frequenti. Si dovrà inoltre tenere conto delle condizioni mentali del paziente. Anche la disponibilità e/o la capacità di seguire le istruzioni post-operatorie può incidere sull’esito dell’intervento. Il chirurgo dovrà bilanciare molti elementi di valutazione per raggiungere i migliori risultati in ogni paziente.SE NON È POSSIBILE EVITARE UN CARICO ECCESSIVO, SI SCONSIGLIA DI ESEGUIRE L’IMPIANTO.L’obiettivo principale di questo intervento di impianto è la fusione fra tessuto molle e osso. Forze anomale o eccessive potrebbero dar luogo a fusione ritardata, mancata fusione o cedimento dell’impianto.Un carico di forza anomalo con conseguente usura può essere causato da:• instabilità non corretta• impianto di dimensioni errate• sostegno del tessuto molle inadeguato• errata posizione dell’impianto• movimento eccessivo• deformità non corretta o ricorrente• uso improprio o attività fisica eccessiva da parte del pazienteLa corretta fissazione durante l’intervento chirurgico costituisce una premessa imprescindibile ai fini del buon esito della procedura. È necessaria la presenza di una struttura ossea di qualità adeguata a sostenere il dispositivo.Al fine di ridurre al minimo il rischio di insorgenza di complicanze, è opportuno adottare delle misure preventive quali:
• seguire le linee guida relative a indicazioni e controindicazioni riportate in precedenza• individuare patologie pregresse• stabilizzare le deformità con cedimento• eseguire un innesto osseo su preesistenti cisti• utilizzare impianti di dimensioni adatte• evitare fili di Kirschner e suture passanti attraverso l’impiantoPer ridurre al minimo il rischio di cedimento precoce da affaticamento dei materiali, evitare di danneggiare le superfici dell’impianto.
In caso di insorgenza di complicanze, le possibili procedure correttive adottabili sono:• espianto• sinovectomia• innesto osseo su cisti• sostituzione dell’impianto• rimozione dell’impianto con artrodesi
Raccomandazioni relative ai frammenti del dispositivo• Utilizzare i dispositivi medici conformemente alle indicazioni riportate in etichetta e alle
istruzioni per l’uso fornite da Wright Medical Technology, particolarmente in fase di inserimento ed estrazione.
• Esaminare i dispositivi prima dell’uso per escludere eventuali danni da trasporto o conservazione o eventuali difetti del prodotto nuovo che possano aumentare le probabilità di frammentazione durante l’intervento.
• Esaminare i dispositivi immediatamente dopo la rimozione dal paziente per rilevare eventuali segni di rottura o frammentazione.
• Se il dispositivo è danneggiato, conservarlo per consentire a Wright Medical Technology di analizzare l’evento.
• Considerare con attenzione e discutere con il paziente (se possibile) i rischi e i benefici dell’asportazione del frammento o della sua permanenza nel corpo.
• Informare il paziente sulla natura e la sicurezza dei frammenti di dispositivo non estratti, menzionando tra l’altro:a. il materiale di cui è composto il frammento (se noto);b. le dimensioni del frammento (se note);c. la posizione del frammento;d. i meccanismi che potrebbero provocare lesioni, ad es. migrazione, infezione;e. procedure o trattamenti da evitare, quali la risonanza magnetica nel caso di frammenti
metallici. In tal modo si può ridurre l’eventualità di lesioni gravi prodotte dal frammento.I risultati clinici dipendono dal chirurgo e dalla tecnica, dalle cure pre- e post-operatorie, dall’impianto, dalla patologia e dalle attività quotidiane del paziente. È importante che il chirurgo ottenga il consenso informato del paziente e che parli con il paziente delle possibili complicanze prima dell’intervento. In tale ambito il chirurgo potrà esporre i metodi alternativi all’impianto, come la ricostruzione del tessuto molle o l’artrodesi.Informazioni sull’imaging a risonanza magneticaEsistono rischi intrinseci associati all’uso di impianti in un ambiente di risonanza magnetica, incluse la migrazione dei componenti, l’induzione termica e interferenze o distorsioni del segnale nei pressi dei componenti. L’induzione termica degli impianti è un rischio che dipende dalla geometria e dal materiale dei componenti, nonché da aspetti inerenti la RM quali la potenza, la durata e la sequenza
degli impulsi. Poiché l’apparecchiatura RM non è standardizzata, la gravità di questi problemi e la probabilità che si verifichino con questi impianti non sono note.La sicurezza e la compatibilità di questi impianti negli ambienti di risonanza magnetica non sono state valutate. Non sono stati condotti test relativi al riscaldamento, alla migrazione o agli artefatti di immagine di questi impianti negli ambienti di risonanza magnetica. La sicurezza di questi impianti nell’ambito dell’imaging a risonanza magnetica non è nota. Sottoporre a risonanza magnetica un paziente portatore del presente dispositivo potrebbe causare lesioni al paziente.
F. MANIPOLAZIONE E STERILIZZAZIONEIMPIANTIGli impianti descritti in questo foglietto illustrativo sono forniti in condizioni sterili. Gli impianti confezionati in condizioni sterili devono essere controllati onde accertarsi che l’involucro non sia stato danneggiato o aperto. Nel caso in cui l’integrità della confezione interna sia stata compromessa, rivolgersi al produttore per le istruzioni da seguire. Gli impianti devono essere aperti usando una tecnica asettica da sala operatoria, solo dopo aver determinato la misura corretta da usarsi.Il presente prodotto è esclusivamente monouso. Non risterilizzare mai un impianto che sia entrato in contatto con tessuti o fluidi corporei.I dispositivi classificati come esclusivamente monouso non devono mai essere riutilizzati. Il riutilizzo di questi dispositivi può avere gravi conseguenze per il paziente. Esempi di pericoli correlati al riutilizzo di questi dispositivi includono, senza limitazioni: un notevole degrado delle prestazioni del dispositivo, l’infezione crociata e la contaminazione.
G. CONDIZIONI DI CONSERVAZIONETutti gli impianti devono essere conservati in un ambiente pulito e asciutto e devono essere protetti dalla luce solare e da temperature estreme.

II. INFORMAÇÕES GERAIS SOBRE O PRODUTOGraças aos progressos nos equipamentos de fixação interna, foi oferecido ao cirurgião um meio de corrigir deformações e reduzir as dores em muitos doentes. Embora os implantes utilizados sejam muito bem-sucedidos na concretização destes objetivos, é preciso reconhecer-se que são produzidos em metal e materiais poliméricos e portanto, não pode esperar-se que qualquer implante suporte níveis de atividade e cargas idênticos aos de um osso saudável normal após a ocorrência de fusão. O cirurgião deve avaliar cada situação individualmente com base na apresentação clínica do doente antes de tomar quaisquer decisões relativas à escolha do implante.Os cirurgiões têm de estar familiarizados com as técnicas operatórias aplicáveis e com as instruções de utilização para cada produto. Este folheto informativo e o rótulo da embalagem incluem advertências, precauções e contraindicações fundamentais para cada cirurgia. Além disso, a técnica cirúrgica deve ser consultada para informações detalhadas sobre a escolha do implante, detalhes relevantes sobre o produto, instruções cirúrgicas propostas e/ou utilização da unidade. O cirurgião deve contactar a Wright para conhecer a técnica cirúrgica específica do produto proposta.Ao recorrer a implantes de fusão, o cirurgião deve estar ciente do seguinte:• A seleção e o dimensionamento corretos do implante são extremamente importantes. A seleção
do tamanho, forma e modelo adequados do implante aumenta o potencial de êxito. Os implantes exigem um posicionamento cuidadoso e um suporte ósseo adequado. Uma escolha do implante adequada deve considerar o design, a fixação, o peso, a idade, a qualidade óssea, o tamanho, o nível de atividade e o estado de saúde pré-operatório do doente, bem como a experiência e familiaridade do cirurgião com o dispositivo. A longevidade e a estabilidade do implante podem ser afetadas por estas variáveis. Os cirurgiões devem informar os doentes relativamente a estes fatores.
• Ao selecionar doentes para cirurgia, os seguintes fatores podem ser fundamentais para o futuro êxito do procedimento:1. Ocupação ou atividade do doente. Se o doente exercer uma ocupação ou atividade que lhe
imponha levantar pesos ou fazer esforços musculares substanciais, as forças resultantes podem
produzir o insucesso da fixação, do dispositivo ou de ambos. O implante não restabelecerá a função ao nível esperado com ossos normais saudáveis e o doente não deverá alimentar expectativas irrealistas em relação à sua funcionalidade.
2. Situação de senilidade, doença mental ou alcoolismo. Estas situações, entre outras, podem fazer com que o doente ignore determinadas limitações e precauções necessárias relativas à utilização do implante, levando a falhas ou a outras complicações.
3. Reação a corpos estranhos. Se houver suspeita de sensibilidade a materiais, devem ser feitos testes apropriados antes da seleção ou da implantação do material.
A. SELEÇÃO DE DOENTESA utilização de equipamentos de fixação interna requer a ponderação das seguintes indicações gerais:• bom estado do doente;• bom estado neurovascular;• cobertura de pele adequada;• possibilidade de um sistema musculotendinoso funcional;• reserva óssea adequada para receber o implante;• disponibilidade de terapêutica no pós-operatório;• doente cooperante.
B. INDICAÇÕESOs sistemas de âncora de sutura GRAVITY estão indicados para utilização:• na reparação de luxação do ombro após lesão de Bankart, rotura do “cuff” do rotor, lesão SLAP,
separação acromioclavicular, tenodese do bíceps, rotura/separação do deltóide, ou desvio capsular ou reconstrução capsulolabral;
• na reparação de instabilidade do cotovelo após deslocamento do tendão do bíceps, cotovelo de tenista, ou rotura/separação do ligamento colateral ulnar ou radial;
• na reparação de instabilidade da mão/pulso após rotura ou separação do ligamento escafolunar, ligamento colateral ulnar, ou ligamento colateral radial;
• na reparação de instabilidade do joelho após rotura ou separação do ligamento colateral medial, ligamento colateral lateral, tendão patelar, ou ligamento oblíquo posterior, ou após tenodese da banda ílio-tibial;
• na reparação da instabilidade do pé/tornozelo secundária a laceração ou separação do tendão de Aquiles, estabilização lateral de tendões/ligamentos, estabilização medial de tendões/ligamentos, tendões/ligamentos da zona média do pé ou tendões/ligamentos metatársicos, transferências de tendões, refixações de tendões e reconstruções de ligamentos das zonas média, anterior e posterior do pé, procedimentos associados a reconstrução de pé chato, deformação da zona posterior do pé, reconstrução da zona média do pé, reconstrução ou instabilidade lateral/medial do tornozelo, hallux valgus ou varus e instabilidade da articulação metatarsofalângica, incluindo:• refixação/reconstrução do tendão de Aquiles; • transferência do flexor digital longo; • transferência do flexor longo do hálux;• transferência do extensor longo do hálux;• procedimentos de Brostrom;• mudança de posição do tendão peroneal;• reparação capsular;• reconstrução/refixação do deltóide;• reparação da placa plantar;• reparação do ligamento calcaneonavicular.
C. CONTRAINDICAÇÕESNão existem contra-indicações específicas do produto.
D. POSSÍVEIS COMPLICAÇÕES E REAÇÕES ADVERSASEm qualquer intervenção cirúrgica, existe o potencial de ocorrerem complicações. Entre os riscos e complicações com estes implantes, incluem-se:• infeção ou local do implante doloroso, inchado ou inflamado;• fratura do implante;• afrouxamento ou luxação do implante, exigindo uma intervenção cirúrgica de revisão;• reabsorção óssea ou produção excessiva de osso;• reação(ões) alérgica(s) ao(s) material(is) do implante;• respostas histológicas desfavoráveis, envolvendo possivelmente macrófagos e/ou fibroblastos;• migração de partículas de desgaste, resultando possivelmente numa resposta orgânica;• embolia.
E. PRECAUÇÕESA observância das instruções de utilização apresentadas na literatura do produto pode minimizar o potencial de complicações ou de reações adversas com qualquer implante.É da responsabilidade de cada cirurgião utilizar implantes que considerem o estado clínico e médico de cada doente e ter conhecimentos sólidos sobre todos os aspetos do processo de implantação e as possíveis complicações que podem ocorrer. Os benefícios obtidos com uma intervenção cirúrgica de implantação podem não satisfazer as expectativas do doente ou podem deteriorar-se ao longo do tempo, exigindo uma intervenção cirúrgica de revisão para substituir o implante ou realizar procedimentos alternativos. As intervenções cirúrgicas de revisão com implantes são comuns. O estado mental do doente também deve ser considerado. A vontade e/ou a capacidade do doente em
respeitar as instruções do pós-operatório também podem ter influência sobre o resultado cirúrgico. Os cirurgiões têm de ponderar muitas considerações para obter o melhor resultado em doentes individuais.SE NÃO FOR POSSÍVEL EVITAR UMA CARGA EXCESSIVA, NÃO SE DEVE UTILIZAR UM IMPLANTE.O objetivo principal de uma cirurgia com este implante é estabelecer uma união de tecido mole com osso. Forças anormais ou excessivas poderão provocar atraso na união, não-união ou falha do implante.Um apoio de força anormal e o subsequente desgaste podem ser causados por:• instabilidade não corrigida;• implante de dimensão inadequada;• apoio inadequado de tecidos moles;• mau posicionamento do implante;• movimentos excessivos;• deformação não corrigida ou recorrente;• utilização inadequada ou excesso de atividade do doente.Uma fixação correta durante a intervenção cirúrgica é fundamental para o êxito do procedimento. A reserva óssea tem de ser adequada para sustentar o dispositivo.Algumas medidas preventivas a considerar para minimizar o potencial de complicações:• seguir as diretrizes para as indicações e contra-indicações acima apresentadas;• identificar patologias anteriores;• estabilizar deformações por colapso;• tratar quistos pré-existentes com enxertos ósseos;• utilizar um implante de dimensão adequada;
• evitar fios de Kirschner e suturas através do implante.Evite defeitos nas superfícies do implante para minimizar a eventualidade de falha prematura devida a fadiga.
Caso se desenvolvam complicações, entre os possíveis procedimentos corretivos incluem-se:• remoção do implante;• sinovectomia;• enxerto ósseo de quistos;• substituição do implante;• remoção do implante com fusão da articulação.
Recomendações relativas a fragmentos do dispositivo• Use os dispositivos médicos de acordo com as indicações constantes da documentação e as
instruções de utilização da Wright Medical Technology, especialmente durante a inserção e a remoção.
• Inspecione os dispositivos antes da utilização para detetar danos que possam ter ocorrido durante o envio ou armazenamento ou quaisquer defeitos ao retirar da embalagem que possam aumentar a probabilidade de fragmentação durante uma intervenção.
• Examine os dispositivos imediatamente depois de serem removidos do doente para detetar quaisquer sinais de quebra ou fragmentação.
• Se o dispositivo estiver danificado, guarde-o para auxiliar a Wright Medical Technology na análise da ocorrência.
• Considere cuidadosamente os riscos e benefícios de recuperar ou deixar o fragmento no doente e discuta-os com o doente (se possível).
• Aconselhe o doente acerca da natureza e segurança de fragmentos de dispositivo não recuperados, incluindo a seguinte informação:a. a composição material do fragmento (se conhecida);b. o tamanho do fragmento (se conhecido);c. a localização do fragmento;d. os potenciais mecanismos conducentes a lesões como, por exemplo, migração ou infeção;e. procedimentos ou tratamentos que devem ser evitados, tais como exames de RMN, no caso
de fragmentos metálicos. Isto pode ajudar a reduzir a possibilidade de ocorrência de uma lesão grave devido ao fragmento.
Os resultados clínicos dependem do cirurgião e da técnica, dos cuidados no pré-operatório e no pós-operatório, do implante, da patologia e da atividade diária do doente. É importante que os cirurgiões obtenham um consentimento informado apropriado e debatam o potencial de complicações com cada doente antes da intervenção cirúrgica. Isto pode incluir uma análise de procedimentos alternativos, não relacionados com implantes, como a reconstrução de tecidos moles ou a artrodese.Relativamente a ambientes de ressonância magnéticaExistem riscos inerentes associados à utilização de implantes num ambiente de RMN, incluindo migração de componentes, indução térmica e interferência ou distorção do sinal próximo do(s) componente(s). A indução térmica dos implantes é um risco relacionado com a geometria e o material do componente, bem como a potência, duração e sequência de impulsos da RMN. Visto que o equipamento de RMN não é normalizado, a gravidade e probabilidade de ocorrência são desconhecidas para estes implantes.A segurança e a compatibilidade destes implantes não foram avaliadas em ambiente de RMN. Estes implantes não foram testados em ambiente de RMN em relação ao seu aquecimento, migração ou artefacto de imagem. A segurança deste implante num ambiente de RMN é desconhecida. Submeter um doente com este dispositivo a exame pode resultar em lesão.
F. MANUSEAMENTO E ESTERILIZAÇÃOIMPLANTESOs implantes descritos neste folheto informativo são fornecidos estéreis. Os implantes em embalagem estéril devem ser inspecionados de forma a garantir que a embalagem não foi danificada nem previamente aberta. Se a integridade da embalagem interna tiver sido comprometida, contacte o fabricante para obter instruções. Os implantes devem ser abertos utilizando técnicas asséticas de bloco operatório e devem ser abertos apenas após ter sido determinado o tamanho correto.Este produto destina-se a uma única utilização. Um implante nunca deve voltar a ser esterilizado após contacto com tecidos ou fluidos corporais.Os dispositivos identificados como exclusivamente para utilização única nunca deverão ser reutilizados. A reutilização destes dispositivos poderá causar lesões graves no doente. Exemplos dos perigos relacionados com a reutilização destes dispositivos incluem, entre outros: degradação significativa do desempenho do dispositivo, infeção cruzada e contaminação.
G. CONDIÇÕES DE ARMAZENAMENTOTodos os implantes têm de ser armazenados num ambiente limpo e seco e protegidos da luz solar e de temperaturas extremas.

II. ÜRÜNE AİT GENEL BİLGİLERDahili fiksasyon donanımının ilerlemesi yoluyla, cerraha birçok hasta için deformiteyi düzeltme ve ağrıyı azaltmanın bir yolu sunulmuştur. Kullanılan implantlar bu hedefleri yakalamakta büyük oranda başarılı olurken, metal ve polimerik materyallerden yapıldıkları ve bu nedenle füzyondan sonra implanttan normal sağlıklı kemikten beklenen aktivite seviyeleri ve yüklere dayanmalarının beklenemeyeceği unutulmamalıdır. Cerrah implant seçimi konusunda karar verirken hastanın klinik görünümüne bağlı olarak her durumu ayrı ayrı değerlendirmelidir.Cerrahlar, her ürünün kullanma talimatı ve uygulanabilir ameliyat tekniğine aşina olmalıdır. Bu prospektüs ve mevcut paket etiketi her cerrahi için temel uyarılar, önlemler ve kontrendikasyonlar içerir. Ayrıca cerrahi tekniğe implant seçimi, ilgili ürün ayrıntıları, önerilen cerrahi talimat ve/veya tertibatın kullanımı hakkında ayrıntılı bilgi için başvurulmalıdır. Önerilen ürüne özel cerrahi teknik için cerrah Wright ile bağlantı kurmalıdır.Füzyon implantları kullanırken, cerrah aşağıdaki hususlara dikkat etmelidir:• İmplantın doğru seçilmesi ve doğru büyüklükte olması son derece önemlidir. İmplantın uygun
büyüklük, şekil ve tasarımının seçilmesi, başarı olasılığını artıracaktır. İmplantlar, dikkatli yerleştirme ve yeterli kemik desteği gerektirir. Uygun implant seçiminde tasarım, fiksasyon, hastanın kilosu, yaşı, kemik kalitesi, cüssesi, faaliyet düzeyi, ameliyat öncesi sağlık düzeyi ve ayrıca cerrahın cihaz konusundaki deneyimi ve aşinalığı göz önünde bulundurulmalıdır. İmplant ömrü ve dayanıklılığı bu değişkenler tarafından etkilenebilir. Cerrah hastayı bu faktörler konusunda bilgilendirmelidir.
• Cerrahi müdahale için hasta seçerken, aşağıdaki faktörler prosedürün nihai başarısında kritik öneme sahip olabilir:1. Hastanın mesleği veya faaliyeti. Hasta büyük oranda kaldırma gerektiren veya kasları zorlayan
bir meslek veya faaliyetle uğraşıyorsa, ortaya çıkan güçler fiksasyonun, cihazın veya her ikisinin başarısızlığıyla sonuçlanabilir. İmplant, işlevi normal bir sağlıklı kemikten beklenen seviyeye çıkaramaz ve hasta bu konuda gerçekçi olmayan işlevsel beklentilere kapılmamalıdır.
2. Yaşlılık, ruhsal hastalık veya alkolizm durumu. Bu durumlar ve diğerleri hastanın implantın kullanımında gerekli olan bazı kısıtlamalar ve önlemleri ihmal etmesine ve sonuçta implantın başarısız olmasına veya başka komplikasyonlara yol açabilir.
3. Yabancı cisim hassasiyeti. Malzemeye karşı hassasiyetten şüphelenilen durumlarda, malzemenin seçimi veya implantasyon öncesinde uygun testler yapılmalıdır.
A. HASTA SEÇİMİDahili fiksasyon donanımının kullanımı aşağıdaki genel endikasyonların ele alınmasını gerektirir:• Hastanın iyi genel durumu• İyi nörovasküler durum• Yeterli deri örtüsü• İşlevsel bir kas ve kiriş sistemi olasılığı• İmplantı alacak yeterli kemik stoğu• Ameliyat sonrası tedavi sunma imkanı• İşbirliği yapan hasta
B. ENDİKASYONLARGRAVITY Sütür Ankor Sistemlerinin şunlar için kullanılması endikedir:• Bankart lezyonu, rotator kaf yırtığı, slap lezyonu, akromiyoklaviküler ayrılma, biseps tenodezi,
deltoid yırtılması/ayrılması veya kapsüler shift veya kapsülolabral rekonstrüksiyona sekonder omuz instabilitesi tamirinde;
• Biseps tendonu ayrılması, tenis dirseği veya ulnar veya radiyal kollateral ligaman yırtığı/ayrılmasına sekonder dirsek instabilitesinin tamirinde;

C. KONTRENDİKASYONLARÜrüne spesifik bir kontrendikasyon yoktur.
D. OLASI KOMPLİKASYONLAR VE ADVERS REAKSİYONLARHer cerrahi prosedürde komplikasyon olasılığı vardır. Bu implantlara ilişkin riskler ve komplikasyonlar şunları içerir:• Enfeksiyon veya ağrılı, şiş veya enflamasyonlu implant bölgesi• İmplantın kırılması• İmplantın revizyon cerrahisini gerektiren gevşemesi veya yerinden çıkması• Kemik rezorpsiyonu veya aşırı oluşumu• İmplant malzemesine/malzemelerine karşı alerjik reaksiyon/reaksiyonlar• Makrofajlar ve/veya fibroblastlar içerebilen uygunsuz histolojik tepkiler• Parçacık aşınma kalıntılarının yer değiştirmesi ve bir vücut tepkisine yol açabilmesi• Emboli
E. ÖNLEMLERÜrün literatüründe verilen kullanma talimatının uygulanması, herhangi bir implantın komplikasyon veya advers reaksiyon yaratma olasılığını en aza indirebilir.İmplantları kullanan her bir cerrah, her bir hastanın klinik ve tıbbi durumunu değerlendirmek ve implant prosedürünün tüm unsurları ve ortaya çıkabilecek olası komplikasyonlar hakkında bilgi sahibi olmaktan sorumludur. İmplant cerrahisinden elde edilen faydalar hastanın beklentilerini karşılamayabilir veya zamanla azalabilir ve bu nedenle implantı değiştirmek veya alternatif prosedürler uygulamak üzere revizyon cerrahisi gerekli olabilir. İmplantlarda revizyon cerrahileri yaygındır. Hastanın ruhsal durumu da göz önüne alınmalıdır. Ameliyat sonrası talimatları uygulama isteği ve/veya kabiliyeti de cerrahi sonucu etkileyebilir. Bu implant ile cerrahinin temel hedefi yumuşak dokuyu kemiğe birleştirmektir. Anormal veya aşırı güçler gecikmiş kaynama, kaynamama veya implant başarısızlığına neden olabilir.Anormal güç yüklemesi ve müteakip aşınma şunlardan kaynaklanabilir:• Düzeltilmemiş instabilite• Uygun büyüklükte olmayan implant• Yetersiz yumuşak doku desteği• İmplant malpozisyonu• Aşırı hareket• Düzeltilmemiş veya yineleyen deformite• Hastanın hatalı kullanımı veya aşırı faaliyetiProsedürün başarısı için cerrahi müdahale sırasında uygun fiksasyonun sağlanması kritik öneme sahiptir. Kemik stoğu, cihazı desteklemek için yeterli olmalıdır.Komplikasyonlar olasılığını en aza indirmek üzere ele alınacak bazı önlemler:• Yukarıda verilen endikasyonlar ve kontrendikasyonlara yönelik kılavuz ilkelere uyun• Önceki patolojiyi tespit edin• Kolaps deformitelerini stabilize edin• Önceden mevcut kistlere kemik grefti uygulayın• Uygun büyüklükte bir implant kullanın• İmplant içinden K-telleri ve sütürler geçirmekten kaçınınErken yorgunluktan kaynaklanan başarısızlığa yol açması durumunu en aza indirmek için implant yüzeylerine zarar vermekten kaçının.
Komplikasyon ortaya çıkması halinde olası düzeltici işlemler şunları içerir:
• İmplantın çıkarılması• Sinovektomi• Kistlere kemik grefti uygulanması• İmplantın değiştirilmesi• Eklem füzyonu ile implantın çıkarılması
Cihaz Parçalarıyla İlgili Tavsiyeler• Tıbbi cihazları, özellikle takma ve çıkarma işlemi sırasında, etiketlerinde yer alan endikasyonlara ve
Wright Medical Technology’nin kullanım talimatlarına uygun olarak kullanın.• Cihazları kullanmadan önce inceleyerek, geldiği haliyle veya nakliye veya saklama sırasında,
bir işlem sırasındaki fragmantasyon olasılığını artırabilecek herhangi bir hasar veya kusur olup olmadığına bakın.
• Cihazları hastadan çıkarıldıktan hemen sonra inceleyerek, herhangi bir kırılma veya fragmantasyon belirtisi olup olmadığına bakın.
• Cihaz hasarlıysa, Wright Medical Technology’nin olayı incelemesine yardımcı olmak üzere saklayın.
• Parçanın hastadan alınmasıyla veya bırakılmasıyla ilişkili riskleri ve yararları dikkatle değerlendirin ve hastayla görüşün (mümkünse).
• Aşağıdaki bilgiler çerçevesinde, hastaya çıkarılmayan cihaz parçalarının niteliği ve güvenliği hakkında tavsiyelerde bulunun:a. Parçanın malzeme bileşimi (biliniyorsa);b. Parçanın boyutu (biliniyorsa);c. Parçanın konumu;d. Olası zarar görme mekanizmaları (örneğin yer değiştirme, enfeksiyon);
e. Parçaların metal olması durumunda, MRG tetkikleri gibi kaçınılması gereken prosedürler veya tedaviler. Bu işlem parça nedeniyle ciddi zarar görme olasılığının azaltılmasına yardımcı olabilir.
Klinik sonuçlar, cerrah ve kullanılan teknik, ameliyat öncesi ve sonrası bakım, implant, hasta patolojisi ve günlük faaliyete göre değişiklik gösterir. Cerrahların her cerrahi işlem öncesinde hastadan bilgilendirilmiş olur alması ve olası komplikasyonları tartışması önemlidir. Bu da yumuşak doku rekonstrüksiyonu veya artrodez gibi alternatif, implant dışı prosedürlerin gözden geçirilmesini içerebilir.Manyetik Rezonans Ortamlarıyla İlgiliMR ortamında implantların kullanılmasıyla ilişkili olarak işlemin tabiatı gereği riskler mevcuttur; bunlar arasında bileşen yer değiştirmesi, ısı indüksiyonu ve bileşen(ler) yakınında sinyal enterferansı ve distorsiyonu vardır. İmplantların ısı indüksiyonu, bileşen geometrisi ve materyaline ve ayrıca MR gücü, süresi ve puls dizisine bağlı bir risktir. MR ekipmanı standardize olmadığından bu implantlar için oluşma olasılığı ve şiddeti bilinmemektedir.Bu implantlar MR ortamında güvenlik ve uyumluluk için değerlendirilmemiştir. Bu implantlar MR ortamında ısınma, göçme veya görüntü artefaktı için test edilmemiştir. Bu implantın MR ortamında güvenliliği bilinmemektedir. Cihazın bulunduğu bir hastanın taranması hastanın yaralanmasıyla sonuçlanabilir.
F. KULLANIM VE STERİLİZASYONİMPLANTLARBu prospektüste tanımlanan implantlar steril olarak sağlanırlar. Steril ambalajdaki implantlar, ambalajın hasar görmediği veya önceden açılmadığından emin olmak üzere kontrol edilmelidir. İç ambalajın bütünlüğünde bir bozulma varsa, ek talimat almak için üretici ile irtibata geçin. İmplantlar aseptik ameliyathane tekniği kullanarak açılmalıdır; ancak doğru büyüklüğün tespit edilmesinden sonra açılmaları gerekir.
Bu ürün sadece tek kullanımlıktır. Bir implant vücut dokularıyla veya sıvılarıyla temas ettikten sonra asla tekrar sterilize edilmemelidir.Tek kullanım etiketli cihazlar asla tekrar kullanılmamalıdır. Bu cihazların tekrar kullanılması hastanın ciddi zarar görmesine neden olabilir. Bu cihazların tekrar kullanılmasıyla ilgili tehlikelere örnek olarak verilenlerle sınırlı olmamak üzere şunlar vardır: cihaz performansında önemli derecede azalma, çapraz enfeksiyon ve kontaminasyon.
G. SAKLAMA KOŞULLARITüm implantlar temiz ve kuru bir ortamda saklanmalı ve güneş ışığı ve aşırı sıcaklıklardan korunmalıdır.
