2011 Annual Report
Sense of homeDiscover ing persona l g row t h
Sense of homeAfter 85 years, Graceworks keeps discovering how we can best help others experience personal growth in challenging or changing circumstances.
From our beginnings in chaplaincy with local institutions, to providing emer-gency assistance, we began helping families through youth and family programs, adoptions, and support ministries.
Sixty-five years ago we opened a home for older persons which has grown to include nearly 750 residents in Bethany Village. Our Consumer Credit Counseling Service helps individuals dis-cover how to manage personal finances and navigate financial challenges.
Graceworks led the way in Ohio in helping adults with developmental disabilities transition from institutional living to small, loving homes. We cre-ated caring communities of apartment homes for older adults and adults with disabilities and limited income. We tapped the caring creativity of senior adults who make friendly visits to other seniors in their homes.
We continue to discover ways to help individuals age in their homes, through our new Health at Home, Hands at Home and Friends at Home programs.

Throughout Graceworks, individuals ex- perience Faithful Caring that helps them grow during times of challenge or change, as in the stories in this annual report:
• Mary encourages personal growth through arts and crafts in a community of adults who are older or have disabili-ties and limited income.
• Chris thrives in a residence with a group of friends with developmental disabilities, where he explores ways to enjoy life through sports, fitness, dancing, music, church and work.
• Bea continues to learn, grow and inspire others at age 94, through arts, crafts and caring ministries in Bethany Village.
• Bill got back on his feet – and behind the wheel – after three heart attacks and triple bypass surgery, with help from nurses and therapists in Graceworks’ new Health at Home program.
These individuals experience a sense of home that helps them grow and discover new possibilities for their lives, new beginnings.
We are thankful for many faithful congregations, donors, volunteers, staff and other friends who commit them- selves to inspiring and supporting others through Graceworks Lutheran Services. We recognize their commitments in this annual report.

25 years ago as an affordable apartment community for adults who are older or have disabilities, with limited income. Graceworks operates 18 such communi-ties in southwestern Ohio, southeastern Indiana and northern Kentucky.
Encouraging residents’ artistic abilities is one way to help inspire a sense of home in Graceworks Housing Services communities, where individuals con-tinue to grow with each other. The art class, including ceramics, quilting and crafts, has helped residents uncover talents and enjoy time with each other.
Terry carefully applies fleshtone paint to a fired mold of ceramic, in the
shape of a miner with two children and shows it to the teacher, Mary. “I’m glad this class started,” says Terry. “It’s relaxing and fun.” Jamel, in his wheel-chair, paints the whiskers on a ceramic dog. “I’ve had shepherds, huskies and pit bulls as pets,” he says.
Mary, Terry and Jamel are residents of Martin Luther Community, a Graceworks Housing Services com- munity which opened in Dayton

A shining spirit

“We always try to encourage our resi-dents to do things for each other.” says Site Manager Carol Buggs. “Mary is very spiritual and walks the talk. She knows how to bring others out. They have found they have some artistic abil-ity. It has caused some of the residents to really shine!”
Class participants, especially those with limited physical abilities and experience, inspire Mary.
“What they’re feeling inside is what they do through art,” Mary says softly.
“Everyone has some artistic ability hidden within them that you can bring out, and they feel good when they can express themselves. I like to see people use their talents, not waste their talents.”
Carol sees personal growth in some residents through the art class and other interactions. One man has pro-gressed from using a wheelchair to a walker and sometimes walking unas-sisted, as he lost body weight, says Carol. “He tells me he feels so blessed
to be here because of the many things that have transpired in his life since he’s been here.”
Building community is especially impor-tant in helping residents during times of difficulty and loss, says Carol.
“Community means just that: We are one. We come together to help one another, to be a force for those who don’t have a family, perhaps, to become family for them.”
Mary appreciates how Carol and the staff work to help individuals grow individually and in community. “She is spiritual-minded and she encourages me. She’s always thinking of residents and what we need. Her whole staff is just beautiful.
“It’s like a family to me. I have my own space, but I feel like we’re a big family here. I love being here,” says Mary.
“And we love you living here,” Carol answers.

“He has always

around other
people in school,
in church, at the
workshop. To
Chris, nobody’s
a stranger”
– Janice
At Chris’ daytime Goodwill workshop in Cincinnati, Janice noticed James.
“He was always happy, and was always clean.” She learned that James lived in one of 28 residences where Graceworks Enhanced Living provides services for adults with developmental disabilities in Butler, Greene, Hamilton and Montgomery counties. She called Graceworks to arrange a visit.
“I came here and James opened the front door and smiled and said, ‘Come in.’ It was great!” It felt like home.
When her father died, Janice knew she and her son, Chris, couldn’t
continue as just the two of them at home living together. She and Chris, a non-verbal young adult with develop-mental disabilities, might not be able to manage emergencies. But beyond that, she knew her sociable, active, caring son needed to be around more people. She wondered where he could find good care and friends his own age and abilities, where he could enjoy his favorite activities, dancing and sports and music.
Joyful living
5
Seven years later, Janice and Chris couldn’t be happier. The staff and Chris’ roommate James and seven other resi-dents relish Chris’ fun-loving energy.
“Chris is the life of the party,” says Shannon, a Graceworks caregiver for 11 years in the home. “Chris is the initiator. Guys might be hanging around and tired of watching Family Feud. Chris will go and put on a CD, usually rap. He’ll break the monotony. Or he’ll get up and get everybody exercising. Chris is usually the leader. Chris will motivate them to keep on going.”
During weekdays, Chris enjoys going to the Jackson Center workshop in Cincinnati, where he often sorts materi-als for packaging for local companies.
“He loves the Jackson Center,” says Shannon.
Chris lives with his mother on the weekends, going on outings with his friends and church with his mom. “He has always enjoyed being around other people in school, in church, at the work-shop,” says Janice. “To Chris, nobody’s a stranger.”
The home manager, René, appreciates not only Chris’ energy and fun-loving spirit, but also his sensitivity to others.
“Sometimes, when he senses you’re not having the best of days, he’ll come up and just rub your back or give you a hug. He helps other individuals if he can.” Chris smiles, reaches out to René with a hand signal, “R” for René. “Yes, that’s my name,” says René, smiling back.
He has become more independent and more of a leader, just as his mother hoped when he moved to his new home.
“He has grown and developed,” she says. “It worked out just fine.”

contest (placing in two), but also helped others to finish their entries.
“Bea’s positive outlook and how she continues to thrive is inspiring,” says Larisa McHugh, Bethany Music Therapist and coordinator of Bethany’s Art and Writing Show.
“I just like to help people,” says Bea.
She and her husband, Paul, met and married in western Pennsylvania, trav-eled while he served in the U.S. Navy during World War II and lived in the Buffalo, N.Y. area while he worked as an

engineer for Westinghouse, Inc. Their sons Ron and Karl were born there. The family moved to Kettering in 1956. She loved serving with the Cub Scouts, the Flowering Acres Garden Club, the Kiwanis Club and Good Shepherd Lutheran Church. They enjoyed living near her brother, the Rev. Kay Glaesner, a Lutheran pastor in Springfield.
Bea worked at the Kettering YMCA for 20 years. She volunteered in the Kettering Medical Center for 22 years, visiting patients, and wrote for the Kettering-Oakwood Times.
“My husband and I were both brought up in the church and learned about serv-ing from our parents.”
After they moved to Bethany Village in 1987, they quickly became involved in helping residents in their new com-munity. As Stephen Ministers, they faithfully visited other residents and took to heart the Stephen Ministry program purpose: “To equip the saints for the work of ministry for building up the body of Christ.”
“To reach out to other people, help other people, that’s what we both wanted to do,” says Bea. Paul passed away in 1999.
Bea loves being in a community where she can continue to grow and learn. She is a faithful member of a weekly crafts class led by Julia Wheeler, Life Enrichment Coordinator. During a recent week, the laughs spread throughout the class as smoothly as the peanut but-ter they were applying to pine cones for homemade bird feeders.
“The most important part about living in Bethany Village is the people and the employees,” says Bea. “We moved here because we thought Bethany was the best for our needs and I still do. It has lots to offer and a good Christian atmosphere.
“The Lord’s given me all this time and I feel very grateful. I have a good family, good friends and a good place to live.”

became independent, the more I valued the care I got from Graceworks,” says Bill.
He remembers feeling helpless when he was discharged from the hospital to the Bethany Rehabilitation Center. Bill had never before been hospitalized. He had managed Sears department stores as a career. He hated being so reliant upon others. He was determined to recover independence.
“Our children picked Bethany, where they thought we’d get the best care, which we did,” says Mary Ellen, a retired registered nurse.
Bill lay on his back and prayed for strength. He longed to return to
the healthy life he had lived for more than 80 years before three heart attacks and triple-bypass surgery in the sum-mer of 2011, a pain-free life full of tennis, scuba diving and long walks with his wife, Mary Ellen. He became frustrated with needing help with simple tasks, exhausted from just brushing his teeth.
His prayers were answered with help from the new Graceworks Health at Home service and the Bethany Rehabilitation Center. “The more I
Care with heart

“The care was first class,” says Bill. He clicked off the names of a dozen nurses, therapists and resident care technicians.
“All of them were cheerful and capable. They did a lot of personal things that made the day not so dreary.” They encouraged him, worked exercises with him and taught him.
When he was ready to go home in six weeks, there was no doubt which agency he would choose to provide home nursing and therapy care.
Graceworks’ new Health at Home program provides health services to older individuals in their homes. Health at Home became Medicare-certified in 2010 after completing a Joint Commission survey.
Graceworks has also begun Hands at Home, providing personal care services to older individuals in their homes.
“If the home health care would be any-thing like what we had at Bethany, there was no need to shop around,” Bill says.
Bill received visits from Denise, a regis-tered nurse; Joanne, a physical therapist; and Florence, a physical therapy assistant.
“They made me enjoy it,” says Bill. “Nobody ever said to me, ‘Bill, you’re 82 and you can’t expect to be able to do what you did before.’ They recognized my needs and my progress. When the doorbell would ring, I’d look forward to it.”
“His attitude and his family support were big factors,” says Joanne. Mary Ellen adds, “And all the prayers from our church.”
With hard work, he progressed from a walker to a cane, then to walking with-out assistance. “He was so willing to work with every suggestion we made,” says Denise. “To be able to drive, to go to church were so important to him.”
Seven months after his operation, he was healthy enough to drive with Mary Ellen to Atlanta and Florida to visit their children.
“From Bethany to Health at Home, through the whole organization, the quality of care was professional and experienced and their spirit was up- lifting,” says Bill. “They always made me feel like I was the only patient they had all day. I am grateful for their kindness.”

Graceworks Lutheran Services inspired and supported a sense of home for tens of thousands of individuals who were experiencing challenging or changing circumstances in 2011. Through a wide range of residential and community care services, we seek to help older adults, individuals with disabilities, and persons facing financial or personal challenges experience stability, security, belonging and community – a sense of home – in their lives. During 2011, Graceworks provided $4.7 million of uncompensated charity care that was not paid by user fees or government funding.
Bethany Village Over 750 older adults lived at Bethany Village. About 450 enjoyed the active lifestyle and wellness programming in the residential area, another 60 people lived in assisted living residences, and approximately 250 people lived in the Linden House and received full-time skilled nursing and memory supportive care.
Bethany Rehabilitation Center A total of 710 older adults experienced skilled nursing care and individualized rehabilitation programs as they recovered from surgery or hospitalization. Sixty percent of the people served returned to their previous homes after recovery.
Consumer Credit Counseling Service Nearly 2,300 people experienced one-on-one financial counseling, and Graceworks helped 915 people manage their debts through the Consumer Credit Counseling Service debt management program. Graceworks housing counselors assisted 20 families to become homeowners and helped 257 families avoid foreclosure. Nearly 580 individuals received bankruptcy pre-filing counseling and 300 individuals received bankruptcy pre-discharge financial education. Approximately 1,100 individuals learned how to manage their finances through financial education classes or workshops. Another 3,600 individuals learned about Consumer Credit Counseling Service through outreach presentations and media events. An additional 2,200 individuals benefited from responses by Consumer Credit Counseling Service to financially-related inquiries.
Friends at Home A total of 35 older adults, residents of Bethany Village and the community at large, received companionship services that supported them during challenging or changing circumstances through the efforts of 32 volunteers and 726 hours of service. The Friends at Home program is supported by a grant from the Lutheran Services for the Elderly Endowment Fund at the Evangelical Lutheran Church in America Foundation.
Hands at Home Since its inception in April 2011, Hands at Home provided 102 residents of Bethany Village with personal care and private transportation services. Hands at Home caregivers assist with household tasks such as laundry, light housekeeping, and meals as well as bathing, dressing and medication reminders and companionship for older adults or respite for family caregivers.

Health at Home Graceworks provided health and therapy services for 131 individuals in their homes in the first calendar year since Graceworks achieved accreditation and Medicare certification. A total of 66 of the individuals were residents of Bethany Village and 65 were residents in the surrounding Dayton-area community.
Graceworks Enhanced Living More than 150 individuals with developmental disabilities lived in safe, caring environments in 28 homes in Butler, Greene, Hamilton and Montgomery Counties, including a new residence that opened in Greene County. The Graceworks Enhanced Living day program in Hamilton County provided recreational day habilitation services for 31 adults with developmental disabilities.
Graceworks Housing Services More than 700 income-eligible elderly or disabled residents experienced affordable housing in 18 Graceworks apartment communities in Ohio, Indiana and Kentucky. On-site employees worked to establish caring and stable environments in well-maintained and conveniently located apartment buildings. Service coordinators helped residents find and obtain needed services to allow them to age in place. Graceworks continues to refinance properties, which enables us to complete renovations and major repairs.
Homelessness Prevention and Rapid Rehousing Program Thirty-three families, consisting of 70 individuals at imminent risk of homelessness, received financial assis-tance with rent and rent arrears to prevent them from losing their housing. Program ser-vice ended June 30, 2011, fulfilling the terms of a contract with Montgomery County.
Interfaith Care Team Ministry Thirty-three families received life-skills education, family mentorship, advocacy, and support through case management services in their homes in targeted neighborhoods. Three hundred twenty-eight individuals received material and financial assistance and referrals to support their basic and emergency needs. The program closed June 30, 2011 when Montgomery County funding concluded.
Once Around Shop and Emergency Assistance Program A total of 5,888 individuals received food, clothing or financial assistance for shelter, utilities and medi-cations through the Preble County Emergency Assistance program. Thousands more received affordable clothing and household items at the Once Around Shop in Eaton.
Senior Companion Program A total of 187 older persons were able to maintain independence in their own homes with assistance through friendly visits and respite care. Elderly persons in Montgomery, Miami, Preble, Greene and Clark Counties received visits from 40 trained volunteers who gave 43,954 hours of faithful service. As part of the Corporation for National and Community Service Senior Corps, Senior Companions offer companionship, friendship and light assistance to mostly elderly persons who strive to maintain an independent lifestyle.

Consumer Credit Counseling Service offers online financial assessment, bankruptcy education
Now, anyone may get an assessment of personal financial health and behavior through a free online financial tool: mymoneycheckup.org.
“We want to make it as easy as possible for people who need help with their personal finances to find the help they need,” says Melodee Sheils, Director, Consumer Credit Counseling Service. The online money check-up helps you identify whether you should
“continue on your path,” “proceed with caution” or “stop and make a change” in each of four areas of personal finance: budgeting and credit management, saving and investing, planning for retirement and managing home equity.
The assessment is offered by the National Foundation for Credit Counseling (NFCC) to help consumers identify whether they need to seek advice from member organizations like Graceworks Consumer Credit Counseling Service.
Online bankruptcy education
Consumer Credit Counseling Service now offers online bankruptcy pre-filing counseling and pre-discharge financial education. The secure web-based courses became available through Graceworks’ membership in the NFCC. Persons can access bankruptcy counsel-ing by visiting graceworks.org/CCCS.
“Our thorough organization and analysis of the client’s financial situ-ation in addition to answering their questions places the client at ease, and provides them with the knowl-edge to make an informed decision,” says Melodee.
“The online service is a valuable and convenient resource. The finan-cial counseling helps individuals filing for bankruptcy modify how they manage money, leading to greater personal economic security, increased peace of mind and ulti-mate community improvement. We will continue to provide in-person

or telephone counseling and in-person classroom financial education for those who feel more comfortable with or pre-fer a more personal approach.”
Individuals are required to receive coun-seling from an approved organization before filing for bankruptcy, and then again before their debts are discharged. CCCS is approved by the Executive Office of the U.S. Trustee and issues bankruptcy certificates for both the Southern and Northern Districts of Ohio.
CCCS provides one-on-one counseling for personal financial management, debt and housing matters, as well as educational workshops. Clients can schedule an appoint-ment at our offices in Dayton, Springfield and Sidney by calling 937-643-2227 or 1-800-377-2432.
Graceworks launches Hands at Home personal care services
Graceworks Lutheran Services expanded its personal care services on April 1, 2012 to serve the community as well as Bethany Village residents. Personal care services are offered under Hands at Home, LLC, managed by Robyn Nelson, former Wellness Clinic Manager at Bethany. Graceworks supports seniors aging in place and recognizes that many seniors can be successful staying in their homes if additional services can be provided. Some may need help with bathing or preparing meals or simply companionship while the caregiving spouse takes a break to socialize outside the home. Personal care services have been available to Bethany residents but are now being offered to the community as well.
Hands at Home operates under Graceworks at Home, the home care corporation affiliated with Graceworks Lutheran Services. In-home skilled nursing and therapy services are also offered under Graceworks at Home as Health at Home, LLC, our Medicare-certified home health agency.
For new information on Health at Home and Hands at Home, visit graceworksathome.org.

Grand Cottages in Bethany Village
Nine new Grand Cottages are now under construction to replace 12 of the original 1972 Bethany Village cottages which were torn down in November 2011. The appliances, cabinets, doors, windows and fixtures were removed and donated to the Habitat for Humanity ReStore.
“Today’s retirees want new construction, fresh approaches to daily living, bright and open floor plans and smart spaces,” says Bill Serr, President and CEO of Graceworks Lutheran
Services, which owns and oper-ates Bethany. The project builder is Oberer Companies.
The new Grand Cottages are 50 percent larger with attached two-car garages, and include more amenities, comfort and safety features than the cottages they will replace. Construction was scheduled to start in March 2012 and to be completed by the end of the year.
“Bethany continues to remodel and enhance other cottages and residences, but new construction might not be possible again for several years,” says Bill. In 2008-09, Bethany added 60 new residences and a Village Center for events, dining, fitness, classes and activities. Bethany is home for approximately 750 adults of age 62 and older in a continu-ing care retirement community on 100 acres in Centerville, Ohio.
For information, illustrations of the Grand Cottages and testimonials from Bethany residents please visit bethanylutheranvillage.org, or contact the Bethany Marketing Department at 937-436-6850.
