  • Purpose of the Manual

    This manual was created to be used in homes to enhance

    youths knowledge of independent living skills to better

    prepare them for living on their own. Please note that there

    is not a copy write on the workbook. It was created to be

    copied by those using it and to be distributed as individuals

    deem necessary.

    We would like to thank the different agencies that

  • 13. Scenario: You now have a job at Wal-Mart. A co-worker

    starts a rumor that you are stealing money from the cash

    register, which is not true. List two appropriate ways you can

    deal with this situation.

    14. After the situation with your co-worker simmers down, you

    now have problems with your boss. Your boss always seems to

    give you more work than the others, work that should not be

    given to someone in your position, frequently keeps you over

    your scheduled time and you feel, is generally treating you

    unfairly. List two appropriate ways that you could deal with

    the situation.

  • RESUMES

    1. Read the Best Resume Tips sheet.

    2. Look over the sample resumes provided, noting the format.

    3. Create a resume on the template provided, based on the

    Dos and Donts and the sample resumes or go to Pikes Peak

    Workforce to complete a resume.

    The sample job applications you filled out would be a

    BIG help here!*

    4. Use the Action Verbs to help you come up with the best

    words to describe you and your work experience.

  • DOS AND DONTS OF THE RESUME

    Do: Do keep your resume between one and two pages in length.

    Do ask a counselor or friend for an opinion on your resume.

    Do update your resume whenever you change jobs, earn awards,

    etc.

    Do underline and capitalize to draw attention to impressive

    aspects of your resume.

    Do type with neat margins so the resume can be easily read.

    Do carry several copies of your resume when you go on interviews.

    Do list any prestigious affiliations or charities that you help.

    Do use 8 X 11 unlined bond paper.

    Do list your address (es) and phone number(s) at the beginning.

    Do use staples to attach a two-page resume. (No tape or paper

    clips).

    Do list grade point average if it is above B.

    Dont: Dont use full sentences in the phrasing of your resume.

    Dont make salary demands.

    Dont be modest about your background and activities.

    Dont have typographical errors or ugly smudges on your resume.

    Dont list overly ambitious job objectives.

    Dont list the names of references unless they are important

    people or ones the employer will recognize.

    Dont list memberships in groups that are controversial (i.e.

    abortion rights).

    Dont use outrageous gimmicks to draw attention.

  • JOB INTERVIEW STRATEGIES FOR TEENS

    Interview Preparation

    Know yourself. The more self aware you are the more comfortable

    and confident you will be in job interviews. Make a list or your

    strengths and abilities. Make another list of your achievements.

    Practice saying positive things about your skills and abilities.

    Know about the job you will be interviewing for.

    Think about yourself in relation to the job. Read through the job

    description thoroughly so you know what they are looking for and the

    skills you possess that match.

    Understand what employers are looking for.

    Practice. Ask friends or family members to conduct practice

    interviews with you.

    Plan to dress nicely and appear well groomed. Dress more

    conservatively than you normally would.

    Be prepared to be interviewed even when youre not expecting to.

    If youre cruising the mall filling out job applications, dont be

    surprised if some employers want to interview you on the spot.

    Set realistic expectations about salary. You should be aware of

    what the current minimum wage is so youre not surprised (and so you

    do not ask for less than minimum wage).

    Know your hours you can work and prepare to be flexible. Consider

    school, homework, extracurricular activities and sports anything

    that takes up your time.

  • JOB INTERVIEW STRATEGIES FOR TEENS

    During and After the Interview

    At the Interview:

    Punctuality and reliability are a matter of show and tell.

    Show your punctuality by arriving 5 - 15 minutes early.

    Remember you are making an impression from the first

    moment you walk in the door. Chances are the receptionist or

    the first person you see will tell the hiring manager if your

    behavior before the interview isnt as respectful and optimistic.

    Dont chew gum!

    Dont downplay your previous experience, no matter how lowly

    it seems.

    Avoid using um and like in the interviews. The best way to

    get past overusing these pause words is practice.

    Make eye contact. It is extremely important for connecting

    with your interviewer.

    Be yourself.

    Be memorable.

    Show your enthusiasm. Employers list lack of enthusiasm as

    their No. 1 turnoff in interviewees. The best way to show

    enthusiasm? A big smile throughout the interview a natural

    smile.

    Project confidence. The scary world of job interviewing is new to

    teens, but overcoming the fear and appearing confident is a great

    way to stand out.

    Ask questions. Asking questions shows your enthusiasm for the

    job, so have a couple prepared, but dont ask about things like

    salary or vacation time.

    Close the sale. Conclude the interview by thanking the

    interviewer and making a statement that conveys your enthusiasm

    for the position, also find out when they will be making their

  • hiring decision and how they will communicate that to you or if

    you need to follow up with them.

    After the Interview:

    Write a thank-you note. Its just common courtesy to thank

    people for their time and since very few tees exercise this little

    gesture, youll stand out if you do it.

    If you havent heard a hiring decision by the time you

    expected to, call the interviewer to check on your status.

    Dont make a pest of yourself, but do follow up.

    In the fast pace world of today grabbing a bite to eat at a fast

    food restaurant is very tempting. Before doing so here are some

    fast food facts to think about (www.fatcalories.com and

    At Burger King:

    1 Cheeseburger = 350 calories, 17g of fat

    1 Large Fry = 500 calories, 30g of fat

    1 Medium Dr. Pepper = 200 calories, 0g of fat

    Total for one meal = 1050 calories, 47g of fat

    At McDonalds:

    1 Breakfast Bacon, Egg and Cheese Biscuit = 480 calories, 31g of fat

    1 Breakfast Hash Browns = 130 calories, 8g of fat

  • 7. You are going to plan and price a menu for a week. Follow

    these instructions:

    a. When making your menu, consider time and practice

    (i.e. how much time will you have to prepare meals?)

    b. Following the Meal Plate for proper nutrition, plan a

    menu for a week including breakfast, lunch and dinner.

    Recipes

    Baked Chicken and Potatoes

    o Rinse 2 pieces of chicken in cool water and put in baking pan. o Chop potato into bite sized chunks. o Take 1 teaspoon of margarine and cut into 6 pieces. Divide among potatoes and chicken. o Sprinkle chicken and potatoes with seasonings. o Bake at 350, for 45 min. to an hour. Chicken should not be pink. Potatoes should be tender

    when you poke them with a fork. Serve with vegetable

    o Option: Cook all the chicken. Freeze the rest and use for Chicken Broccoli Stir Fry.

    Spanish Rice

    o Make a cup of rice following directions on pkg., for 1 cup cooked rice. Stir in 2 Tablespoons of salsa. This can be made in a microwave. Serve with side of refried beans and sour cream.

    Spaghetti

    o Cook 2 3 oz. spaghetti according to pkg. directions. Stir in some of the spaghetti sauce. o Save the rest of the sauce for the ravioli in a container or zip lock not in the can. o Option: Can use some of the hamburger in sauce Fry until browned, no pink showing,

    season with salt, pepper, and other seasonings if desired. Add cooked hamburger to warm

    sauce. Serve with garlic bread and salad.

    Grill Cheese

    o Butter 1 side of 2 pieces of bread. Put one slice, butter side down in a frying pan. Top with cheese and other slice of bread, butter side up. Fry on medium hate until golden brown, turn

    over and brown the other side. Serve with tomato soup and/or French fries.

    o Option: Can add slice of sandwich meat.

    Chicken Broccoli Stir Fry

    o Cook chicken in boiling water until cooked, about 30 min. (until no pink remains) or use leftover baked chicken (remove skin and bone if needed).

    o Make 2 cups of rice by following directions on pkg. o Rinse 1 cup frozen broccoli with hot water. o Put 1 tablespoon margarine or oil in a frying pan; add rice, chicken and broccoli. Cook and stir

    until warm, about 5 min. or less.

    Beef Burritos

    o Brown ground beef in frying pan, until no pink is showing. Drain grease into empty can. o Add chili powder to ground beef, making it as hot as you like it. (Start with 1 teaspoon). o Warm tortillas. Serve with salsa. Sour cream optional.

    o Cut steak into long strips. o Put steak in frying pan. Add tablespoon of margarine. Sprinkle with garlic and chili powder.

    Fry until meat is cooked.

    o Season meat with garlic powder, salt, pepper, and Worchester Sauce (optional). Put 1 tablespoon margarine in frying pan, cook cubed steak, on medium heat for 7 min. Turn and

    o Option: Can add small can of mushrooms at start of frying if desired, may save half can to fry with hamburger).

    o Baked potato (med. lg.) scrub potato with water. Poke with fork 2-3 times on both sides of potato. Bake at 425, for 40 min. or microwave for 5 min. (depends on size of potato for more

    or less time). Serve with margarine and sour cream (optional).

    Bean Burritos

    o Heat beans in frying pan 93-5 minutes) or microwave. Spoon into warm tortillas. Serve with Salsa and sour cream (optional). Serve with veggie.

    o Cook according to package directions. Dont over cook, they will break. o Warm spaghetti sauce in sauce pan on low heat. Mix two together, gently. o Another option: stir in 2 tablespoons margarine, season with salt, pepper, garlic, parmesan

    cheese. Serve with garlic buns and a salad.

    Hamburgers

    o Pre-heat oven according to directions on French fry package. Bake as directed. o Shape hamburger meat into a 2-3 inch ball, making sure its pressed tightly together. Flatten

    with the palm of your hand until approximately inch in thickness.

    o Season with salt, pepper; garlic/onion powder (optional), Worchester Sauce (optional). o In frying pan, on medium heat, cook for approximately 5 7 minutes and then flip to cook

    other side, pressing burger down to release grease. Check center for pinkness, cook until no

    pink is showing.

    o Option: Can save some of the hamburger and use in the spaghetti sauce. Can also add some saved mushrooms and fry with burger if desired.

    Veggie Omelet

    o Pre-cook veggies in frying pan with small amount of margarine. May also add mushrooms, optional.

    o Using 2-3 eggs, in mixing bowl, mix eggs 2-3 tablespoons milk, salt, pepper, (onion and/or garlic powder optional).

    o Add egg mixture to veggies in frying pan, cook until bottom of omelet is brown (top side may still be a little runny), flip carefully, and brown the other side. May put a slice of cheese on one side of

    Unacceptable aggressive driving behavior includes tailgating, weavinginandoutoftraffic,passingontherightortheshoulderof the highway, cutting people off, failure to allow others to merge orpass, obscenehandgestures,honking,yellingorflashinglights, exceeding the speed limit, bumping or ramming another vehicle or brandishing weapons.

It is based on current laws, legislation and department policies and is subject to change. It is not a book of laws and should not be used as a basis for any legal claims or actions. It is a book of information only and does not supersede Colorado Revised Statutes.Trafficregulations in cities, towns, counties and federal territories maygobeyondstatelaws,aslongastheydonotconflictwith state law.     1. THE DRIVERS LICENSE1.1 PERSONS REQUIRED TO HAVE A DRIVERS LICENSE: Any person who operates a motor vehicle, motor-driven cycle or moped on the public streets and highways in Colorado is required to be at least 16 years of age and have a valid drivers license.RESIDENT:PerC.R.S.42-1-102(81),anypersonwhoownsor operates a business in Colorado or who has resided within the state continuously for 90 days or who has obtained gainful employment within this state, whichever shalloccurfirst, is consideredaColorado resident. (SeeNewResident3)SPECIAL EXEMPTIONS C.R.S. 42-2-102 AND 24-60-1106The following persons need not obtain a Colorado License provided they are 16 years of age or older and possess a valid license from their home state or state of last assignment.COLORADO RESIDENTS: Any resident of this state who is employed in another state, where the laws of that state require licensing to drive in order to engage in a regular trade or profession, does not need a Colorado Drivers license as long as such other license to drive is valid and the employment is not terminated. This special exemption applies only if the other state is a member of the Interstate Drivers license Compact Agreement.MILITARY: Anyone who is serving as a member of the Armed Forces of the United States on active duty; this also includes the spouse and children of such members.FOREIGN MILITARY PERSONNEL: On duty or assigned to temporary duty with the United States Armed Forces; this also includes the spouse and children of such members.STUDENTS: Any nonresident who is temporarily residing in Colorado for the PRINCIPAL purpose of furthering his/her education and is considered a nonresident for tuition purposes.

    NON-RESIDENT ALIENS: Foreign tourists, instructors, and businesspersonsmaydriveanyprivate(noncommercial)vehicle with their personal Drivers license. 1.2 PERSONS WHO CANNOT BE ISSUED A COLORADO DRIVERS LICENSE Anypersonwhodoesnotsubmitproofofage,proofof

    identity, or both, as required by the department. AnypersonunabletopasstheDriverslicenseexamination. Anypersonwithalicensefromanotherstatethatdoes

    notsurrenderthatlicense.(C.R.S.24-60-1106) Anypersonundersuspension,revocation,ordenialof

    driving privileges in this or any other state. AnypersonwhosepresenceintheUnitedStatesisin

    violation of federal immigration laws. Anypersonadjudicatedtobementallyincompetent,an

    alcoholic, or an habitual user of narcotic drugs. EmancipatedminorswithoutanAffidavitof

    Liability.(DR2460) Any personwho has unpaid fines or outstanding

    judgmentsfortrafficviolations. Anypersonunder16yearsofage.1.3 OBTAINING A COLORADO DRIVERS LICENSEApplicationsandwrittentestingareonafirstcome,firstserved basis. You should allow yourself ample time before closing to complete the process. No appointment is needed for thewritten test,however, toallowsufficient time tocomplete the test and any processing, no written test will be givenafter4:15p.m.atthoseofficeswith8:00a.m.to5:00p.m. hours of operation. Drive tests are by appointment only.Toallowsufficienttimetocompletethetestandanyprocessing, the last appointment for a drive test is 4:00 p.m. at thoseofficeswith8:00a.m.to5:00p.m.hoursofoperation.Trytoarriveattheofficeasearlyaspossible.Generally,Mondays, Fridays and days after holidays are the busiest.     2. ProvideyourSocialSecurityNumber(SSN). IfyoustatethatyoudonothaveanSSN,youmust

    provide documentation from the Social Security Administration supporting your statement.

    3. Supply a Colorado residence address.4.Passallrequiredexaminations:(physical,vision,written,

    drivingtest).DRIVING RECORD ANALYSIS: The Motor Vehicle Division examines your driving record and present license status to determine eligibility before issuing a new license.PHYSICAL APTITUDE ANALYSIS: This is a determination, by the drivers license employee, of any physical ailment or disability which may have an effect on your driving. A ConfidentialMedicalExaminationReportisrequiredformany physical ailments or disabilities such as epilepsy or paralysis.VISION SCREENING: Your vision will be tested to determine if you can see well enough to drive safely. If the test discloses that you need corrective lenses to meet the minimum visual standards, your license will show this restriction. Failure to meet established vision standards will result in initial rejection and referral to a vision specialist for correction and recommendations at the applicants expense. The minimum vision standard is 20/40.BASIC WRITTEN TEST: This covers various aspects of driving knowledge and safety. The test questions will cover the contents of this handbook including road signs, driving undertheinfluence,aswellasotherdrivingrules,safetyrules and legal items.DRIVING TEST: This is the most important part of the examination because it provides you with the opportunity to demonstratethatyoucandrivesafely.(SeeDriveTest13)5. Pay the required fee. A separate fee is required for

    the Instruction Permit and for the Drivers license. A surcharge will be added for any motorcycle endorsement, license, or extension.

    1.4 CLASSES OF COLORADO DRIVERS LICENSESColorado Drivers licenses are issued in various classes according to your age group and the type of vehicle for which you qualify.All classes of licenses are valid for chauffeur purposes. Chauffeurisdefinedas:Everypersonwhoisemployedfor the principal purpose of operating a motor vehicle and every person who drives a motor vehicle while in use as a public or common carrier of persons or property. At a minimum, a valid, basic Class R license is required for theoperationoflow-powerscooters(enginesizeof50ccorlessor4476wattsorless,ifelectric.40MPHmax)andlow-speed electric vehicles.

    CLASSR(BasicLicense)Anymotorvehiclewhichisnotconsidered a motorcycle or commercial vehicle as described below.CLASSA,BandC(CommercialDriverslicense-CDL)Adriver must have a Commercial Drivers license to operate the following types of vehicles: Anymotor vehiclewith a gross vehicleweight

    rating(GVWR)orcombinationvehicleweightrating(GCWR)of26,001poundsormore.

    Anyvehiclethattransportsquantitiesofhazardousmaterials that require warning placards under the Department of Transportation regulations.

    Anyvehiclethatisdesignedtotransport16ormorepassengers, including the driver.

    TheGVWRorGCWRof the vehicle(s) is not theweightof thevehicle(s),but theweightrating thatthemanufacturerassignedtothevehicle(s).

    For further information including requirements, qualifications, endorsements and restrictions on the Commercial Drivers license, please obtain a Commercial DriverManualatanydriverlicensingofficeinthestate,oratanyPortofEntryoffice.For information on adding a motorcycle endorsement to either a basic or Commercial Drivers License, please pick up a Motorcycle Operators Handbook from any driver licenseofficeordownloadacopyfromtheInternet.1.5 PARENT CONSENT FOR MINORS/AFFIDAVIT OF LIABILITY AND GUARDIANSHIPIfyouareunder18,anAffidavitofLiability(DR2460)must be signed by a parent, stepparent, grandparent with Power of Attorney, legal guardian or spouse 18 years of age or older. If the signer is a grandparent with Power of Attorney(POA),theoriginalPOAmustbepresentedanda copy of the POA must be surrendered upon application for the permit. If the signer is a legal guardian, original court documents showing guardianship, custody or adoption must be presented. Spouses must show the marriagecertificate.The affidavit must be signed in front of the examiner or a notary public.Whoever signs the Affidavit agrees to take legal responsibility for your actions as a driver. If the signer decides later not to accept responsibility for your driving, that person may withdraw their signature and your permit/license will be canceled.

    2. TYPES OF INSTRUCTION PERMITS AND LICENSESINSTRUCTION PERMITS: An Instruction Permit allows limited driving privileges for people learning to drive. Regardless of age, if you are driving with an Instruction Permit, you must be accompanied by a person 21 years of age or older, who holds a valid Colorado license, and is riding in the seat beside you.

    AGE GROUPS: See the following categories for specific requirements and restrictions other than those listed previously.MINOR AGE GROUP - 15 years to 21 years of ageDRIVER EDUCATION PERMIT - available to customers 15 years to 15 years, 6 months. Expires3yearsafterissuance. MustpresentacompletedAffidavitofLiabilityand

    GuardianshipandacompletedAffidavitofCompletionof a Driver Education Classroom Course indicating the minor has completed and passed the classroom portion of a state-approved driver education course within 6 months prior to applying for the instruction permit.

    Whileholdingthistypeofpermit,youmayonlydrivewithyourdrivereducationinstructorortheperson(s)whosignedtheAffidavitofLiability(DR2460)untilyou are 15 years, 6 months. At 15 years, 6 months, youmaydriveonlywiththeperson(s)whosignedtheaffidavitofLiability(DR2460)untilyouare18.This restriction is automatically lifted when you reach 18 years of age.

    Whileyouareunder18,iftheperson(s)whosignedthe DR 2460 does not hold a valid Colorado drivers license, they may appoint an Alternate Permit Supervisor to supervise you while you are driving. The Alternate Permit Supervisor must hold a valid Colorado Drivers License and be 21 years of age or older. Drive time with an Alternate Permit Supervisor will count toward the 50 hour total required on the logsheet.Theperson(s)whosignedtheDR2460mayalso allow any person 21 years of age or older, who holds a valid license, to supervise you while you are driving but that driving time does not count toward the 50 hour total required on the log sheet.

    DRIVER AWARENESS PERMIT - available to customers 15 years, 6 months to 16 years of age. Expires3yearsafterissuance. Must present a completedAffidavit of Liability

    andGuardianship (DR2460) anddocumentationindicating completion of an approved driver awareness program.

    While holding this type of permit youmay onlydrivewiththeperson(s)whosignedtheAffidavitofLiability(DR2460).Thisrestrictionisautomaticallylifted when you reach 18 years of age.

    While you are under 18, if the person(s) whosigned the DR 2460 does not hold a valid Colorado drivers license, they may appoint an Alternate Permit Supervisor to supervise you while you are driving. The Alternate Permit Supervisor must hold a valid Colorado Drivers License and be 21 years of age or older.     permit for at least TWELVE MONTHS and be at least 16 years of age before obtaining a Drivers license.

    Minorsunder16years6monthsofageatthetimethey apply for their drivers license, must complete behind-thewheel (BTW) training prior to beingissued the license. The BTW must be either 6 hours of behind-the-wheel training with an approved driver education instructor or 12 hours of BTW training with their parent/guardian/alternate permit supervisor, if there isnt an approved driving school, offering at least 20 hours of BTW training per week, within 30 miles of the permit holders residence. If 12 hours are completed with the parent/guardian/alternate permit supervisor, the 12 hours are in addition to the 50-hour requirement of the log sheet, for a total of 62 hours.

    Minorsunder18mustpresentacompletedandsignedDrive Time Log sheet when applying for their Drivers license.

    Minors18andolderdonothavetoholdaninstructionpermit for any required amount of time but do have tofirstpurchaseaninstructionpermitbeforetakinga drive test.

    ADULT AGE GROUP- 21 years of age and overINSTRUCTION PERMIT - Expires 3 years after issuance. Adults do not have to hold an instruction permit for any requiredamountoftimebutdohavetofirstpurchaseaninstruction permit before taking a drive test.ADULT LICENSE - Expires on the birthday 5 years from the year of issue. TEMPORARY DOCUMENT:Driverslicenseofficesnolonger have instant photo equipment. Drivers license officeswill issue apaper temporary license, permit orID card, which is valid for 30 days, while your photo is processed. Because you will receive your photo document in the mail, it is important that your address is correct. If you do not receive your photo document in the mail after30days,youshouldreportittotheofficewhereyouoriginally applied.MOTORCYCLE ENDORSEMENT: See the Motorcycle Handbook.IDENTIFICATION CARDS: Any Colorado resident not holding a valid license may apply for a Colorado IdentificationCard.SeeResident,1.1.)PerC.R.S.42-2-114and 42-2-302, Colorado law prohibits a person from holding bothavaliddriverslicenseandanidentificationcard. Youmust present identification establishing age,

    3. NEW RESIDENT / RENEWAL Some offices in the metro area are limited service offices. If you do not have to take any tests, you may be able to save time by going to one of these offices. Check the list of offices in the Service Center Guide in the back of this handbook for locations, services provided, and hours of operation.3.1 NEW RESIDENT WITH A VALID LICENSEIf you are 16 years of age or older and have a valid license in your possession which was issued by another U.S. state, territory or possession, you will normally not be required to take the written test or the drive test.The drivers license employee has a responsibility to require a drive test, even if you turn in a valid license, if there is a problem with your physical aptitude analysis or vision screening. You must surrender your valid out-of-state drivers license (C.R.S.24-60-1106)andpresentidentificationshowingproofof lawful presence in the U.S. along with your surrendered out-of-state drivers license.

    Ifyouareundertheageof18,anAffidavitofLiabilitymustbesigned.(SeeParentConsentforMinors,1.5)3.2 RENEWAL OF A COLORADO DRIVERS LICENSE OR INSTRUCTION PERMITTo renew a Colorado drivers license or instruction permit, you will need to present your old license or permit, pass a visiontest,paytherequiredfeeandbefingerprintedandphotographed.Once you obtain a Colorado Drivers license or Instruction Permit, you will not normally have to repeat the written or driving test unless: Youapply foradifferent classof licenseoraddan

    endorsement. Youreceivetrafficticketswhichresultinsuspension. Thelicenseorpermitiscanceled,revokedordenied. Youletthelicenseorpermitexpireformorethanoneyear.

    You may renew an Adult Colorado Drivers license at any time prior to the expiration date. Minor licenses cannot be renewed before the day of the 21st birthday.You may be eligible to renew your drivers license or ID card over the Internet, or by mail, if you are at least 21 years of age and you have been issued an adult, non-CDL drivers license or adult ID card. Internet or mail renewal is available everyotherrenewalcycle.CommercialDriverLicense(CDL)holders are NOT eligible for this service. If you wish to have your name changed or a new photograph taken, you must appearinpersonataDriversLicenseoffice.Internet renewal is available for 21 through 65 years of age. DUPLICATE DRIVERS LICENSE4.1 LOST, STOLEN, OR MUTILATED LICENSE OR PERMITIf your drivers license is lost, stolen or mutilated and you are 21 years of age or older, you will be required to renew the license for thestandardrenewal fee (C.R.S.42-2-117(1.5)).Duplicates are only available tominor drivers,Commercial Drivers license holders and all instruction permit holders. To replace or renew a lost, stolen or mutilated drivers license or instruction permit, you must appear inperson at anydrivers licenseoffice,presentacceptable identification andpay the required fee. SeeSection 1.3, Obtaining a Colorado Drivers license, for the listofacceptableidentification.4.2 CHANGE OF NAME AND/OR ADDRESSColoradolaw42-2-119(1)(a)requiresthatyounotifytheMotor Vehicle Division in writing within 30 days of a name change or a change of address.

    CHANGE OF NAME: If you are changing your name, you must appear at adrivers licenseoffice inperson,withlegal,original,orcertifieddocumentation. (i.e. recordedmarriagecertificate,divorcedecree,courtorder)Atthetimeyou change your name, you will be required to renew your drivers license for the standard renewal fee.CHANGE OF ADDRESS: For emergency notification purposes, to receive a renew-by-mail application or to receive your license in the mail, it is important to keep the Motor Vehicle Division informed of your current address.     5. EXTENSION OF A COLORADO DRIVERS LICENSEThe Motor Vehicle Division examines your driving record, drivers license status and the type of license you have before issuing an extension. Please include your full name, date of birth and license number on any extension requests. For non-CDL licenses:MILITARY: Only one three-year extension is given for Colorado residents and their dependents on active duty outside the state. You may request an extension card prior to the expiration date of the license. The request must include acopyofyourvalidMilitaryIdentificationCard.CIVILIAN: The charge for a one-year extension is $3.00. Add $1.00 if you have a motorcycle endorsement. Ifyouareout-of-statewhenyourlicensewillexpire,

    you may obtain a one-year extension. Ifyouareoutofthecountrywhenyourlicensewill

    expire, you may obtain up to 2 one-year extensions. If you areunable to renewyour licensedue to a

    medical condition, you may obtain a one-year extension. The request must include a statement from your doctor.

    For CDL licenses: Due to medical and testing requirements for CDL drivers, only a 30-day extension is available. Please contact either of the above addresses.

    6. KEEPING YOUR LICENSELICENSE/DRIVING PRIVILEGE STATUS: Having a drivers license is a privilege. Protect that privilege by driving with care and consideration for others.VALID DRIVING PRIVILEGE: The individual may apply for or hold a Colorado Drivers license. There are no restraint actions against that privilege.VALID DRIVERS LICENSE: The Drivers license is considered legally in force and authorizes the individual to drive.

    CLEARANCE: A formal indication from a state, on letterhead,oranofficialstateform,thattheapplicanthasvalid driving privileges and is clear to apply for a Colorado Drivers license.REINSTATEMENT: A procedure to restore an individuals driving privilege. Reinstatement of driving privileges does not always reinstate the Drivers license.Reinstatement of the driving privilege may be processed even with outstanding tickets, however, a new permit or license may not be issued until the individual can show clearance of any such tickets.The following are actions that affect your driving privilege or driverslicense.Theseactionsmaybeforaspecificamountoftimeand/oruntilcertainconditionsaresatisfied.YoumayNOT drive while under any of these actions.CANCELLATION: An action to void a valid Driver License. This action does not require reinstatement.DENIAL: A restraint action taken when an individual does not have a Colorado Drivers license. This action may require reinstatement.SUSPENSION: A restraint action that temporarily invalidates the Drivers license and withdraws the driving privilege. This action requires reinstatement.The following actions require the individual to reinstate their driving privilege, pass the written test, purchase an Instruction Permit, pass the driving test, and purchase a new Drivers license.CANCELLATION AND DENIAL: A restraint action to cancel the Drivers license and withdraw the driving privilege.REVOCATION: A restraint action that invalidates the Drivers license and revokes the driving privilege.6.1 REEXAMINATIONA reexamination (vision,written anddriving)will berequired for one or more of the following reasons: Involvementin2accidentswithin3years. Involvementinafatalaccident. Incidentreportfromanylawenforcementagency. Medicalreportfromadoctor. Afamilymembersubmittingawrittenrequest.

    The notice of re-exam allows you 20 days to complete the re-exam (pass or fail). Failure to complete the re-examwithin 20 days will result in cancellation and denial of your driving privilege.After you have taken the examination, your license may be returned, you may be issued a license or permit with restrictions, or your driving privilege could be canceled and denied depending on the results of the examination.6.2 COLORADO POINT SYSTEM As a driver, you start out with a clean record, no points against you. If you accumulate too many points against your driving record within a certain period of time, you

    will lose your drivers license through a point suspension. Eachtimeyouareconvictedofatrafficviolation,thecourtnotifies theMotorVehicleDivision.Aconvictionoccurswhen you plead guilty, pay the ticket, or accept a plea bargain. Each type of conviction causes a certain number of pointstobeplacedonyourtrafficrecord.Pointsassessedagainst your record are not erased when you get a new license or get reinstated.The number of point accumulations for suspension are:MINOR DRIVER UNDER 18 YEARS OF AGE: 6pointsinany12consecutivemonths. 7ormorepointsfortheperiodofthelicense.MINOR DRIVER BETWEEN 18 AND 21: 9pointsinany12consecutivemonths. 12pointsinany24consecutivemonths. 14pointsintheperiodofthelicense.ADULT DRIVER: 12pointsinany12consecutivemonths. 18pointsinany24consecutivemonths.CHAUFFEUR POINTS: 16pointsinany12consecutivemonths. 24pointsinany24consecutivemonths. 28pointsinany48consecutivemonths.All chauffeur violations must have been received in a motor vehicle in use as a public or common carrier of persons or property.6.3 YOUR DRIVING PRIVILEGE MAY BE SUSPENDED, REVOKED OR CANCELED IF YOU: Meetorexceedtheminimumpointaccumulationfor

    Give false information on your driver s licenseapplication.

    Failtosettleajudgmentagainstyouasaresultofanaccident while operating a vehicle.

    Lendyourlicensetosomeoneelseormisuseitinanyway. Alterordefaceyourlicense. Failtoappearforareexaminationrequestedbythe

    Motor Vehicle Division. Areconvictedofmanslaughterasaresultofamotor

    vehicle accident. Failtopayafineforatrafficviolationfromthisor

    any other state. Failtoregister,inColorado,allvehiclesownedwithin

    30 days of becoming a resident.

    Failtopayorderedchildsupport. Fail toprovidevalid evidenceof insurancewhen

    requestedbyalawenforcementofficer.If you are convicted of driving while under restraint, your driving privilege will be suspended again effective for ONE YEAR from the date you would have been reinstated.

    7.
