If groundwater has been impacted, describe pro posed monitoring plan (# of wells or sample points, sampling schedule, analytical methods, etc.):

Describe reclamation plan. Discuss existing and new grade recontouring; method and testing of compaction alleviation; and reseeding program, including location of new seed, seed mix and noxious weed prevention. Attach diagram or drawing. Use additional sheet for description if required.

Attach samples and analytical results taken to verify remediation of impacts. Show locations of samples on an onsite schematic or drawing.

Is further site investigation required? Y N If yes, describe:

Final disposition of E&P waste (landtreated and disposed onsite, name of licensed disposal facility, recycling, reuse, etc.):

OGCC Employee:

Based on the topographical setting of the release area (high arid mesa) and Division of WaterResources data, it is not anticipated groundwater has been impacted.

No reclamation activities are necessary as no surface disturbance occurred.

Analytical data and a map depicting the sample locations (soil and water) from the impacted areas areattached.

The water utilized for the flushing activities will be contained and recovered with a vac truck. Therecovered water will be transported and transferred to one of the WPX production water storagefacilities. The excavated material, that slightly exceeds pH and SAR concentrations will be transportedto the KP 11-16 well pad (COGCC facility #335557) where it will be incorporated into the cut slope ofthe pad during reclamation activities.
