ACCIDENTS
1. The Victorian Workcover Authority (VWA) must be notified if any workplace accident
occurs
and if;
Any person is killed or seriously injured
2. If a reportable accident occurs, the scene of the accident should not be interferedwith unless;
Permission has been obtained from the VWA or
A person is trapped and must be released or
The scene is hazardous and attention is needed
3. When operating plant in a public place, such as a street, these steps may be necessary
to protect people;
Erect warning signs and
Erect barriers or fences or
Have a qualified person on duty (flag person) to warn the public
4.The responsibility of seeing that pedestrians are clear of the machine lies with the
operator.
Pedestrians have right of way at all times.
5. A machine has an operating fault which makes it unsafe to use. (for example, the
brakes are faulty or it has a fuel leak.)
Stop the machine
Warn people of the danger, by placing a clearly visible warning sign or tag on
it
Report to an authorized person

ACCIDENTS
Causes and Prevention
Accidents associated with machinery have many different causes. Common causes and ways
of preventing accidents are shown in the chart below:
CAUSE AND PREVENTION
Contact with overhead wires leading to
electrocution
Always take care to inspect for
overhead wires before lifting a length of
pipe or any other object into an upright
position.
Careless use of equipment leading to
crushing of people and property
Always look out for personnel on the
work site before you start work. Keep
alert at all times when machines are
operating.
Alerting equipment to improvise as a
crane leading to accidents caused by
poorly secured loads
There are strict Regulations covering
the operation of equipment as a crane.
These should be followed at all times.
Taking short cuts and not thinking
about the possible dangers leading to
unforeseen accidents
Always remain alert to the possibility of
danger and know the capabilities of your
machine.

WORKING NEAR POWERLINES
Operators working around buildings must also be alert, watching for temporary electrical
wiring, extension leads and bare cables/bus bars from which overhead electric cranes are
powered.
Always take the time to check for powerlines in the area
Always presume conductors to be alive unless it is positively known that the power
has been disconnected
Use non conducting tag lines to control loads. A dry fibre rope is preferred as some
synthetic ropes may conduct electricity
WORKING BENEATH POWERLINES
Care must be taken when working beneath powerlines to ensure that the boom and/or load
does not contact the powerlines.
If a machine comes into contact with live powerlines;
If the machines does touch electric wiring and you suspect that it is alive then you should
not move until the power is turned off. If there is nobody available to turn off the power
and you need to get off the machine, you must make sure that your body does not provide
a path for electric current.
Attempt to move the machine away from the powerlines
Warn people to stay clear
If possible stay on the machine
If you must leave the machine, jump well clear making sure that you are not incontact with the machine and the ground together at any times
Hop rather than walk away from the machine to avoid the possibility of electric
shock as a result of step voltage.
Notify the power authority
Have the machine checked before further use
LEAVE THE MACHINE BY LUMPING WELL CLEAR, WITH FEET TOGETHER AND NO
PART OF YOUR BODY IN CONTACT WITH THE MACHINE.

FIRE PREVENTIONDo not smoke while refuelling or when near batteries. Always have a fire extinguisher onhand and know how to use it (this is a legal requirement). Inspect and service the
extinguisher as recommended on the instruction plate.
If there is a warning sign attached to controls or starter switch do not start the
machine or move any controls. Before operating your machine, inspect for trash or leaks
which could cause a fire.
Do not carry flammable liquids such as starting fluid (ether), gasoline or fuel on the
machine in loose containers. Ensure that you know the type and location of the appropriate
fire extinguisher for fires involving machines. Most machine fires will be of the A, B & Eclass. Foam extinguishers (blue or red with blue band) are suitable for wood/paper andfuel/oil fires and Dry Powder (red with white band) and Carbon Dioxide (red with black
band) are suitable for electrical fires.
The guide below is available from FPAA and provides necessary information for users of
extinguishers.
