REHAB TEAM LAUDED FOR ‘ACCOUNTABLE CARE’

One of the new buzzwords in our fast-changing healthcare system is “accountable care.” Efforts by the government and healthcare providers are pushing for a more responsive and responsible healthcare system where providers are accountable

for the care they provide and the outcomes they achieve.

At Montefiore, we have embraced the concept of “accountable care” and are working to provide the best possible experience to our patients and residents. Over the last year, we have instituted many improvements in our staffing levels, dining services and additions to our management team – all in an effort to be more accountable to our healthcare customers.

We have also partnered with two of the best health systems in the world – Cleveland Clinic and University Hospitals – to achieve excellent outcomes for our patients who come to Montefiore for short-term rehabilitation. We are part of both health systems’ ‘preferred partnership programs’ and have Cleveland Clinic and UH doctors and nurse practitioners on-site every day to be responsive to our patients’ needs. We have become active partners for both the Clinic and UH’s ‘Accountable Care Organization’s, a new Medicare program that is pushing for more coordination between hospitals and community-based providers.

We are measuring our outcomes in order to prove the value that we are creating. In data we received from the Cleveland Clinic Connected Care program from 2017, Montefiore had the second lowest readmission rate of all partner facilities on the east side of Cleveland and we achieved the lowest length of stay. This means that patients getting rehab therapy at Montefiore are less likely to have to go back to the hospital for complications during their rehab stay and return home more quickly than patients that rehab at other facilities.

We are very proud of our expert team of physicians, nurse practitioners, nurses, rehab therapists, nursing assistants, social workers, dietitians, volunteers and support staff that demonstrate on a daily basis that an accountable healthcare system works.

“Chai-lights” of My 18 Years at Montefiore!

In May 2000 I joined Montefiore’s team with encouragement from past board officer Patty Inglis. Montefiore had a handful of AOL accounts for email and a little over 300 employees. Fast forward to 2018 – we have more than 5000 emails on our e-blast list and nearly 700 employees between Montefiore and The Weils! The staff inspires me daily with their dedication and compassion.

As a native Clevelander returning home, Montefiore reconnected me to my Cleveland Jewish roots. Sally Wertheim, past Montefiore board chair, was instrumental in creating a position that combined my expertise in human resources with my strengths in strategic planning, board engagement and leadership development – all to help shape the future of Montefiore and enhance the lives of our residents and loved ones.

I have been privileged to serve as a trusted advisor to three remarkable CEOs, each of whom have made and are making significant contributions to our Montefiore and Weils communities. My sincere thanks to the many talented colleagues, past and present, who through teamwork and collaboration help us remain competitive in this ever-changing healthcare landscape.

I have honored several community leaders, staff and volunteers who have given their time, talents and tzedakah, touching countless lives along the way. Sadly, I have also paid my respects to many influencers who enriched my life with their sage advice and support – Clair Barkley, Audrey Frank, Millie Klein, Elmer Paull and Elaine Rocker – just to name a few.

Several years ago, Michael and I proudly established The Carol and Michael Weiss Family Fund to support staff appreciation and will continue to make it part of our charitable giving in the years ahead.

I wish for Montefiore to go from strength to strength for generations to come. It has been my great honor to be part of this extraordinary organization – I will miss everyone and thank each of you for embracing me and my family as part of this special Montefiore “mishpacha!”

CAROL WEISSVICE PRESIDENT OF HUMAN RESOURCES AND PLANNING

Thank You Carol for your leadership, insight,

guidance and compassion over these past 18 years.

We wish you all the best as you and Michael begin the next phase

of your lives in Indianapolis. We will truly miss you!

Have You Had“The Conversation?”Those of us who have journeyed with

loved ones at the end of their lives have an understanding of how difficult that can be. Many of us enter it unprepared to address the needs that arise, leaving us floundering. However, it doesn’t have to be that way.

Montefiore’s Vinney Hospice is partnering with The Conversation Project to offer a program to help people talk about their wishes during end-of-life, and help families and loved ones make sure those wishes are known, understood and followed. We are reaching out to religious and community groups for help by co-sponsoring this two-part program to groups of 10 or more.

Talk about what is important to you with your loved ones who have the power to make a difference as you live into the end of your life. We look forward to the opportunity to give you some tools that will make it a positive and constructive experience. MESSAGE FROMRABBI JOEL CHAZIN

THE WORD IS LOVEThe staff at Montefiore is

exceptional…and we do not often recognize their enormous gifts to the residents, patients and general spirit of Montefiore.

We are blessed because of their untiring efforts every single day to dress, feed, give medication, treat with care, transport and

do so many things to sustain our residents and all of our efforts to be a leading, excellent and compassionate home. Our employees have families of their own, spouses, children or elders whom they must care for before or after they come home from work.

Yet, relentlessly, they arise early and regardless of the weather carry out an implicit and driving commitment to our residents and to our mission. We often do not realize that we have hundreds of employees who sustain Montefiore, 24-hours a day, 7-days a week. Do we thank them in a way that shows our deep appreciation?

A modern saint offered these words, “Spread love everywhere you go. Let no one who comes to you, go away without feeling happier.”

Another reflected, “Love is always building up. It gives new hope to the discouraged; new strength to those who are weak. It helps the despairing to rise and start again. It makes life seem more worthwhile to everyone it touches. Its words are benedictions. Its every breath is inspiration.”

Yes, it is love and untiring commitment that inhabits and inspirits Montefiore. We and those we love all benefit from this and might take the opportunity to rise up, reflect and offer our love in return.

DEBBIE ROTHSCHILD, DIRECTOR OF THE MONTEFIORE FOUNDATION

A REAL MITZVAH!As I sit at my desk to write my newsletter

message, I’m having a hard time separating my professional role as director of The Montefiore Foundation from my personal role as a family member whose loved one was cared for by the Vinney Hospice team just five years ago. My love for Montefiore runs deep on both levels.

My family and I were all shocked and saddened by my mother’s diagnosis of stage 4 cancer in December 2012. It was clear from the diagnosis that treatment was not an option. Although I did not work for Montefiore at the time, we knew where to reach out for help. The team of administrators, nurses, nursing assistants, social workers and clergy were, as my sister and I put it, “like angles from heaven.” They cared for my mom (and cared for us) as they would their own family. My mom passed away peacefully in February 2013.

Judaism teaches us the importance of honoring the memory of our loved ones. One way to keep a loved one’s values and beliefs alive is by committing time and effort to a worthy cause. With my twins, Bradley and Melissa celebrating their B’nai Mitzvah just this past February, the very same weekend as the 5th anniversary of my mother’s passing; there was no question about their mitzvah project.

I am proud to say that they chose to hold bake sales and do a letter writing campaign to support The Montefiore Foundation. Together we are honoring my mother (their grandmother) and raising important dollars to help ensure that other families have access to top quality care for their loved ones.

As any mother would be, I am tremendously proud of my kids as they embrace this special time in their lives. My pride is enhanced as we work together to support the organization we all love so dearly. I know they are fulfilling a real mitzvah; one they will feel good about throughout their entire lives.

Thank you for indulging me in this personal message. And thank you for continuing to support Montefiore.

With Your Support We Will Hit Our Goal! What a great first half to our 2018 Annual Sustaining Campaign! We have already seen

the generosity from well over 500 donors and are on our way to hitting our fundraising goal for this fiscal year. These gifts help fill the $83 gap per resident per day between the true cost of care and Medicaid reimbursements and it enables us to maintain our holiday celebrations and resident activities, such as art, music and massage therapies.

You can make a world of difference. Your dollars ensure that our residents, clients, patients and their families receive the best possible care. Additionally, by registering for our upcoming 4th annual Home Run on Sunday, May 6, 2018, you are helping make that difference. For more information or to make your pledge, please contact Renee Greller at 216.910.2652 or Alyssa Rothstein at 216.910.2636.

Montefiore Launches NewKaddish Memorial Program

Montefiore is now offering a new program to the family members of recently deceased Jewish residents, Montefiore’s Kaddish Memorial Program. To help elevate the neshama (soul) of the departed, linking them to the living, Kaddish (“Mourner’s Prayer”) will be recited for your loved one daily for 11 months, after which it will be recited annually on the Yahrzeit (anniversary of passing) and during Yizkor. A yearly reminder of the upcoming Yahrzeit will be mailed accordingly.

Although many of us wish to say Kaddish for our loved ones, we may find it too difficult, time consuming or not possible to

say it accordingly. But because the Jewish people are one people with one soul, when needed, one Jew may say Kaddish on behalf of another. We will make it our mission to help you fulfill this mitzvah, while still sanctifying your loved one’s soul. A suggested donation to the Rabbi’s Fund of The Montefiore Foundation would be greatly appreciated.

For more information about Montefiore’s Kaddish Memorial Program, please contact Rabbi Akiva Feinstein at 216.910.2794 or [email protected] or make a donation at montefiorecare.org/foundation.

Bradley and Melissa Rothschild raise funds for Montefiore residents for their B’nai Mitzvah project.

THANK YOU ALL!

Mazel Tov to Montefiore resident David Ramser upon his recent completion of the Mishna Berurah (codified Jewish laws for daily living). Although faced with many life challenges, he reached his goal even as he moved into Montefiore. He began learning one page of the Mishnah Berurah daily after his daughter was born, while living on an Air Force base in New Mexico. For the next 20 years he continued learning a page of the Mishnah Berurah each day.
