This carnival will be held on Monday February 25 th 2019. This activity is complementary to NSWCHS Hunter Regional swimming selection. Venue: Lambton Pool Date: Monday 25 th February 2019 Accompanying staff: P See and A Dennis Time: Students must be at the pool by 8.45am. Event concludes at 2.50pm Cost: Total $5.50 cash only. $3.50 paid at the pool upon entry and $2 zone levy to be paid to Mrs Dennis on the day. Transport: Students are to travel to and from the venue by private transport. Further information and/or student requirements: Please note students competing in the 200m events must be at Lambton Pool by 7.45am ready to compete at 8.00am. All weather venue. Please see attached sheet for program and event order. Parents please note there is no personal injury insurance cover provided by NSW Department of Education for students in relation to school sporting activities, physical education lessons or any other school activity. Parents and caregivers are advised to assess the level and extent of their child’s involvement in the sport program offered by the school, zone, area and state sport school associations when deciding whether additional insurance cover, above that provided by Medicare, is required. The NSW Supplementary Sporting Injuries Benefits Scheme, funded by the NSW Government, provides limited cover for serious injury resulting in permanent loss of prescribed faculty or the use of some prescribed part of the body. PRIVATE VEHICLE USE STAFF, STUDENTS & PARENTS

The transporting of students in the cars of staff members, parents, caregivers, volunteers and other students should only occur in those circumstances where there is no feasible option available to provide alternative transportation. This may only occur under the following conditions: ALL DRIVERS Copies of the following documents are required: 1. Current Driver’s Licence

2. Current Vehicle Registration

3. Current Comprehensive Insurance NOT GREEN SLIP

4. Passengers in vehicle must not exceed the number of seat belts

5. Minimise Risk – have another adult or student present, have students sit in the rear of the vehicle and advise

activity coordinator of travel arrangements.

PARENT DRIVERS

In addition to the list above, we need:

6. A Prohibited Employment Declaration and/or Working with Children Background Check Consent to be

completed and kept on file

STUDENT PASSENGER We need:

Written permission from the parent/carer of the student being transported nominating the driver to transport their child.
