Instructions PageThe Enhanced Work PlanningSurvey Toolfor Surveying Implementation of Integrated Safety Management at the Activity LevelInstructionsNote: Read this page for the steps to plan a survey, put questions together to form a survey, and lastly analyze the data.Step 1: Read the Survey Guidelines on the next sheet to understand why a survey might be used and how to get the most out of a survey.Step 2: Follow the Guidelines to state your purpose for the survey, determine what is to be evaluated, and what you are going to do with the survey results. This step is especially critical. You should consider what you are going to do if you encounter an problem area. Also, when considering the use of a survey, determine if the survey will be repeated periodically. A single survey provides limited information, whereas a sequence of results from repeated surveys (but not very often) provides trend information. The repetition of a survey should (1) use the same questions each time, and (2) not be done very often as annually is recommended.Step 3: The smart thing to do is to start working, at this point, with a statistician from your site so they can work with you through the process.Step 4: Identify the Core Functions and/or Guiding Principles you want to survey. Select those questions from the "List of Questions-Step 4" sheet that you feel will get the information to achieve the purpose of your survey. Edit each question so that the words used are familiar to everybody at your site. Questions on the List of Questions may contain words that are bracketed. For example: (job hazards analysis, hazards list, AJHA) You are to select one of these, or put in a word of your choosing which will be understood by the people taking the survey. Then from the List of Questions, select questions relating to a specific ISM core function or guiding principle (indicated by an "X" in the respective column) to be evaluated.Note: The instructions that follow are generic for any size survey, however, the Data Entry Form-Step 7, Data Analysis-Step 8 and the Results Display Chart-Step 9 were sized to allow only 15 questions. Here is where you need a statistician familiar with Excel. If you want to have a different number of questions, contact Steven Prevette at Flor Hanford at 509-373-9371 for instructions on how to modify each of these sheets.Step 5: Order of the questions in the survey is not critical, but should be somewhat random.Option to Step 6: Conduct a pre-survey or Beta test to see if the survey will accomplish what you want.Step 6: Fully explain the purpose of the survey. Describe management's commitment to the survey. Make it clear to the survey takers what the survey objective is. Conduct the survey.Step 7: Results from the survey should be entered in the "Data Entry-Step 7" program, which is the fifth sheet in this file. Additional instructions are on this sheet. At this point, your statistician should be fully involved in helping to process the data.Note: If you want to go farther with the statistical analysis of the data and be able to look at responses over a duration of time (trending), contact Steven Prevette at Flor Hanford at 509-373-9371. Several examples exist on the site.Step 8: Data analysis is accomplished somewhat automatically when you copy the total results fields from the Data Entry-Step 7 form into the "Data Analysis-Step 8" (sheet 6 in this file). The formulas in the Data Analysis program compute the statistical information. The statistical information is subsequently linked to the "Chart-Step 9" program (sheet 7).Step 9: Statistical information is plotted on the "Chart-Step 9" at which time analysis of the survey information can begin.Step 10: Now the planning done in Step 2 comes into effect. Essentially, look at the results and figure out what to do.

It is the least accurate form of measuring performance.2. A method to gather people's opinions and feelings.3. It gathers information which can be used to plan future actions4. Only one of several tools to evaluate. Another tool is performance measures.Survey Tool Objective: To provide sites with a survey tool that will help them evaluate and improve implementation of ISM at the Activity Level.Discussion: All sites are in the sustaining phase of ISM where continuing improvement becomes an objective. So questions like "How is my site doing?", "Where are there opportunities for improvement?" and "What areas of ISM should we focus on?" then need to be answered. This Survey Tool is used at the working level, the information from which can be used to develop "leading" indicators for the health of your ISM program.Overview of the Survey Tool processFirst, you will need to understand the nature of a survey.Next, determine why we need the survey, AND more importantly, what is going to be done with the information.Then, if you decide to go forward, the Survey Tool is used to customize the survey to get the information wanted. Selecting the questions is the easiest step in this process.Once the survey is conducted, and you have the results, the Survey Tool package gives you a pre-programmed data entry, data analysis and display (chart) program.The last step, which is not included in this tool, is to determine what is to be done with the new information.Before embarking on survey construction, answer a few questions and ponder this advice. First and foremost, ensure you have a purpose for the survey, and that there is no other means to gather this information than to pester a large number of the workforce. Don't ask what you don't really want to deal with. If all the responses are extremely negatively to a question, would you really be willing to let people know this was the result? Know what you want to do with the results. What would you do for action if everyone responded extremely negatively to any of the questions? Be sure you would value the opinion if you were to get it. Involve the analyst early on. Plan ahead for how far down you want to break out the results. It takes at least 25 people to respond to a question to get a reasonable analysis result.This is a true story. We went to help a group develop a survey. They wanted to know the "attitude" of the workers. I told them, let us suppose I made a black box meter that measured everyone's attitude. You are a 6, you are a 3, etc. Now what would you do with that information? They did not know. They did not end up doing the survey.Statistical Concerns:The Survey Tool recommends using a numerical rating scale of five questions where "categorical judgements" are made by the person being surveyed. The scale the Survey Tool questions were designed for is: 1 = Strongly disagree 2 = Disagree 3 = Neutral 4 = Agree 5 = Strongly agree Everybody needs to understand what the scale means. Most people do not think linearly, but more relationally. Measurement scale is bounded, probably artificially (can't go less than the lowest value, or higher than the highest value).Advantages of the Survey Tool method used here for survey analysis are: Use of verbal descriptors avoids the numerical scale problems. Use of 5 categories gives enough fineness of response, without being overly cumbersome. The width of each category can vary with non-linearities of people's responses. The standard deviation of the average response can be calculated based upon the variability of responses, and the number of people making up the average.How the Questions were Constructed:When the Survey Tool questions were made up we tried to avoid: "I am a crook" questions. Those questions such as "I always follow procedures", where certain responses would reflect negatively on me as a person. "I know what I am doing" questions. Typically "I am familiar with company policies on . . ." or "I know what to do in an emergency". Two issues exist: I must admit that I don't know what I "ought" to know, and I may think I know it, but I really don't, So . . . "Of course I know!" Unsure of who the question is referring to. "Management supports safety". Who is management? My own manager? The next person's manager? My manager's manager? Psychologists have found that people tend think their own manager and coworkers are okay, but everyone else is all messed up. Unsure of what the question is asking for. "Management supports safety". What action is it that is indicative of "supports safety"? Expecting me to be able to read other's minds. "My coworkers believe that safety is important." Asking questions that are better asked of an expert. "This company meets all requirements for the Voluntary Protection Program". How would I as an average worker necessarily know this? Asking questions that are better measured by another means. "Our workgroup follows the schedule". Get out a schedule, and mark off what items were met and which were not, and plot the percentage made. Excessively long surveys. Maximum time limit we would recommend is 5 minutes. Don't make the survey painful.We have tried to include Personalized questions that start with "I" or "My". Question targeted to a specific population such as "I", "My manager", "My coworkers", "My manager's manager" are examples. Recognize surveys measure perceptions so phrases like "I believe that" or "I feel that" are helpful when trying to gather emotions and subjective responses. Phrases like "I see that" help to target specific observable items. A mixture of both types of questions through a survey is good. Know your audience. Make sure that the targeted survey responders have the knowledge to answer the survey. Instead of asking if "The preventive maintenance program is effective", ask "Equipment in my work area is ready to use when I need it." The rating scale is always kept in the same direction and consistent. Positive is always the same way. People do seem to treat "disagreement" differently than "agreement". Make the survey easy to take, and easy to turn in. On the bottom of the survey form thank the individual for their response, and tell them when results will be available and what will be done with the results.Do a trial run of the survey on a small group Ensure the group is representative of the population. Watch the person take the survey. Ask them what they interpreted questions to mean. Did any questions trouble them? Any frustrations? How much time did it take?When you do the survey Management has to commit to expend time for workers to answer the survey. Management has to "sell" the value of taking the survey. Try to get as high a return rate as you can. People who choose not to take the survey may have considerably different opinions than those who self-elect to take the survey.Example story of what not to say: At the end of a staff meeting, the manager hands out a survey. "Now you must take this stupid survey. I don't know what it is for, but get it done and turn it in."After the survey is done and analyzed. Be sure to thank the group for participating. Report out results, what questions were significantly high, which were significantly low. Examine any differences between groups. Provide explanation of "what this means". Take actions! Monitor the effects of the actions. Conduct a follow-on survey.
