Welcome to Duke Temporary Service (DTS). We aredelighted to have you working with us. As an employee of DTS,you will be providing temporary assistance to variousdepartments within Duke University and Duke UniversityHealth System. Please remember the work you do on anassignment is always confidential and is not to be discussed with employees or any other individuals outside of DukeUniversity and Duke University Health System.

Your DTS employment representative will assist you with any questions or problems which may arise during youremployment. Job duties, length of assignment, location ofassignment, work schedule and pay rate will be discussed with you prior to starting an assignment.

DTS EMPLOYMENT POLICIES and PROCEDURES

BENEFITS

HOLIDAYSDTS paid holidays are: New Year’s Day, Fourth of July,

Labor Day, Thanksgiving Day and Christmas Day. Eligibility forholiday pay is determined by the number of hours worked priorto the holiday. To qualify for holiday pay, you must work a totalof 400 hours in the six (6) pay periods prior to the holiday.

HOLIDAY PAY POLICIESPlease do not record hours on your electronic timecard unless

you worked on the holiday. If you qualify for holiday pay, theDTS payroll specialist will adjust your paycheck to the correctamount. Holiday pay will be listed under “adjustments” on yourpay statement.

If you work on a DTS holiday: a. and are eligible for holiday pay, you will receive pay forthe actual hours worked plus eight (8) hours of holidaypay at your current pay rate.

b. and are not eligible for holiday pay, you will receive payfor the actual hours worked at your current pay rate. Youwill not receive holiday pay.

If you do not work on a DTS holiday: a. and are eligible for holiday pay, you will receive pay for eight (8) hours at your current pay rate.

b. and are not eligible for holiday pay, you will not receiveholiday pay.

RETIREMENT

EMPLOYEES’ RETIREMENT PLAN (ERP) This plan is a pension plan designed to provide biweekly-paid

employees with a guaranteed monthly income at retirement, paidentirely by Duke. As a DTS employee, you automatically willbecome a member of the plan if you are over age 21 and havecompleted one year of employment, working at least 1000 hours in a fiscal year (July 1-June 30). You become vested, or entitled to receive a benefit from the plan, after completing five years of continuous service. THE DUKE UNIVERSITY FACULTY AND STAFF RETIREMENT PLAN

As a DTS employee, you are eligible for this 403(b) savingsplan which is very similar to a 401(k) plan. This plan is fundedby your voluntary pre-tax contributions. Duke does not make a contribution for non-exempt staff to this plan. You areresponsible for investment decisions in this type of retirementplan. You are eligible to participate immediately in the plan andmay direct your contributions to over 300 mutual funds andannuities. Your contributions grow tax-deferred, untilwithdrawn. If you withdraw funds before age 59 1/2, you may besubject to penalty charges. Please visit www.hr.duke.edu/benefits/retirement/about/savings.php for more information.

PAYPayday is every two weeks in accordance with the regular

biweekly pay period at Duke (refer to your Biweekly Schedule).

OVERTIME PAY POLICYTo be eligible for overtime pay you must work more than

forty (40) hours in one (1) week. You will be paid one and onehalf times your regular rate of pay for any hours worked overforty (40) hours in one (1) week.

DIRECT DEPOSIT Please arrange for your paycheck to be deposited directly

into your bank or credit union account. You may go toDuke@Work (www.hr.duke.edu/selfservice) to sign up for directdeposit. You will need to log in using your NetID and password.You can also view and print your pay statement from thiswebsite.

If you decide to cancel your direct deposit, you must use a paper direct deposit form.

If you choose not to have direct deposit, you must pick upyour paycheck at the DTS office between the hours of 8 am-5 pm,Monday-Friday. Only you or your supervisor can pick up yourpaycheck.

With direct deposit, Duke University Corporate PayrollServices must verify your transit and account numbers. Thisverification process may take 30 days or longer. During theverification period, you must pick up your paycheck at the DTS office between the hours of 8 am-5 pm, Monday-Fridayuntil your direct deposit becomes effective. Once your directdeposit is established, you can visit Duke@Work(www.hr.duke.edu/selfservice) to view and print your paystatement. You will need to login using your NetID andpassword.

If you are unable to establish a checking or savings accountwith a banking institution, the Duke University Credit Unioncan assist you in opening an account. The Duke UniversityCredit Union is located at Erwin Square Plaza, 2200 West MainStreet, Durham, NC. You must take your Duke TemporaryService ID badge with you.

Duke University Corporate Payroll Services and DTS are not responsible for financial service fees associated with delayedpaychecks and direct deposits.

TIMECARDSYou will be paid for the exact hours you work, excluding lunch.

Online Timecard:

You must submit an electronic timecard for each departmentyou work for during the pay period. An electronic timecard canonly be submitted for the current pay period.

Record the hours you worked each day in the spaceprovided. Time worked less than 60 full minutes should beconverted to tenths of the hour (refer to Tenth Conversion Charton the next page).

ELECTRONIC SUBMISSION OF WORK TIME

Login to Duke@Work (www.hr.duke.edu/selfservice) usingyour NetID and password.

Select “MyInfo” tab.Select “MyTime” tab.In “Status” column, click on Enter Time link. Enter time for days worked. Click on “Save” to save timecard so that additional hours can

be added later in the pay period.At end of pay period, click box indicating “I certify that this

is an accurate record of my time for the period indicated.” Click on “Submit” to send timecard to the designated

approver’s Duke@Work account for approval. If you cannot submit your timecard, please contact

the Duke Temporary Service office immediately at 919-681-3132.

For additional information, you may review the “Quick StepGuide” for DTS employees on the Duke Temporary Servicewebsite (www.hr.duke.edu/jobs/dts). The guide provides step-by-step instructions on how to access and submit your work timethrough the Duke@Work website.

LENGTH OF TEMPORARY ASSIGNMENTTemporary employment at Duke is defined as employees hired

to fill short-term assignments for a defined period of time. Temporaryemployees may not work more than 900 hours during the fiscal year(July 1-June 30). Assignments may be of any duration in length aslong as the 900 hour limit per fiscal year is not exceeded.

A temporary employee who reaches 900 hours workedduring the fiscal year, including work with any department acrossDuke, will be unavailable for any temporary assignment until thebeginning of the next fiscal year.

WHAT EVERY DTS EMPLOYEE SHOULD KNOW

TAX FORMS You must complete your Federal W-4 and NC State NC-4

forms online at Duke@Work (www.hr.duke.edu/selfservice). Ifyou do not complete your tax forms, the system willautomatically default to single and 0.

The paper Federal W-4 and NC State NC-4 forms can onlybe used if you are claiming “Exempt from withholding.”

W-2 FORMS You may access your W-2 tax form electronically in January.

You will be able to view and print your W-2 form online usingthe Duke@Work website (www.hr.duke.edu/selfservice).

You will continue to receive paper W-2 forms unless youconsent to electronic-only delivery of your W-2 form.

Electronic W-2 forms will not be available for terminatedemployees who are no longer working with Duke. Your W-2form will be mailed to you using the most recent home addressin the Duke payroll system. Employees who are no longerworking with Duke may contact Duke Corporate Payroll Services(919-684-2642) to provide an updated mailing address.

ATTENDANCE You should call your DTS employment representative and

your department supervisor if you are going to be tardy or absentfrom the assignment. Repeated tardiness or absences can affectfuture assignments or your employment at Duke. Please notifyyour department supervisor and your DTS employmentrepresentative if you need to leave your assignment early due to illness or an emergency. In case of inclement weather, pleasecontact your department supervisor.

Reminder: Any time taken off from an assignmentmust be coordinated with both your departmentsupervisor and your DTS employment representative.

EMPLOYEE EVALUATION As a DTS employee, you are evaluated by your department

supervisor during each assignment. This electronic evaluation isreviewed by your DTS employment representative. Whileemployed at Duke, you may request to review your evaluationsby calling your DTS employment representative.

Since subsequent assignments are based on prior evaluations,it is important to do your best and be conscious of yourperformance. If your performance is unsatisfactory, you will becounseled by your DTS employment representative and ifnecessary the DTS manager. Such counseling could lead totermination of employment.

PARKING INFORMATIONIf you plan to drive to work, you must purchase a parking

permit at Parking and Transportation Services. Parkinginformation, parking locations and the cost of a parking permitare available at the Parking and Transportation ServicesDepartment. The Parking and Transportation ServicesDepartment is located in the Duke Clinic, Red Zone basement,Room 04320.

Please park in the designated areas according to yourparking permit. DTS will not be responsible for traffic or parkingtickets. For additional information, please contact Parking andTransportation Services (919-684-7275) or visit the website athttp://parking.duke.edu.

ID BADGES You will be issued a form to get a photo ID badge. Your ID badge

will be valid for six (6) months. When your ID badge expires, contactDTS to obtain another. There is no charge if the ID badge is expired.However, if you lose your ID badge you will be responsible forpurchasing a replacement. You should always wear your ID badge whileworking on Duke premises. The ID badge must be returned to DTS ifyou terminate employment or transfer from DTS.

WORK-RELATED ACCIDENTSIf you are injured on the job while working through DTS, you

MUST inform your department supervisor when the accident occursso an incident report can be completed. Within that same workshift, you MUST contact your DTS employment representative. Ifthe injury requires emergency medical care, immediately seektreatment at the Emergency Room. If the injury is less severe butstill requires medical treatment, please go directly to Duke’sEmployee Occupational Health and Wellness (EOHW) office. TheEOHW office is located in the Duke Clinic, Red Zone basement,Room 04290.

You must complete an incident report during the shift inwhich the injury occurred and your supervisor should complete a report within 24 hours.

Reminder: If you do not report an on-the-job injury to your department supervisor by the end of the shift inwhich the injury occurred, you will be released fromyour assignment and terminated by DTS.

Reminder: Please make sure that you and yoursupervisor complete an incident report within thespecified time frames even if the injury does not requiremedical care.

BLOODBORNE PATHOGENS VIDEO AND HEALTH SCREENINGS

Duke University Health System has developed a ControlPlan to effectively control the spread of Hepatitis B and otherdiseases. In accordance with this plan, DTS requires allemployees to view a video explaining “Universal Precautions.”Please follow the safety precautions outlined in the video.

You will also receive information on the Duke UniversityHealth System Integrity In Action Compliance Program.

Some job classifications require a substance abuse screenand/or health screening. When required, DTS will scheduleappointments with Employee Occupational Health and Wellnessfor these screenings.

MANDATORY COMPLIANCE UPDATE TRAININGCompliance update training is mandatory for all DTS

employees on an annual basis. You may access the complianceupdate training through the Occupational and EnvironmentalSafety Office website at www.safety.duke.edu. You will need yourNetID and password. Please send any questions about your

You musttest on any training that is marked with an asterisk and anexpired date. Your information will be automatically updatedafter you complete the quiz. With your supervisor’s approval, you may complete training at your worksite or you may use thecomputers at DTS.

Reminder: If you do not update your compliancetraining as required, you may be released from yourassignment and terminated by DTS.

PERSONAL TELEPHONE CALLS Your work location and/or telephone number will not be

released to outside callers. DTS will forward messages to you onlyin case of an emergency.

APPLYING FOR REGULAR EMPLOYMENT AT DUKE You may apply for available job opportunities online at

www.hr.duke.edu. You are considered an external applicant forregular positions until you have worked 500 hours for DTS. Afterworking 500 hours, you may contact the DTS office to verifyyour hours worked for internal applicant status.

TRANSFERRING INTO A REGULAR POSITION ATDUKE

You must be employed a minimum of thirty (30) calendardays with DTS before transferring into a regular position atDuke. If you transfer into a regular position at Duke:

1. You need to notify your DTS employmentrepresentative.

2. You must return your ID Badge and all other Dukeproperty.

3. Your pay rate for a regular position may be different(more or less) than the pay rate you received as a DTS employee.

TERMINATION OF EMPLOYMENT WITH DTSIf you terminate employment with DTS, you must return your

ID Badge and all other Duke property. Please notify your DTSemployment representative in advance if you are planning toterminate employment with DTS.

DTS WORK RULESAs an employee of DTS you are expected to follow certain

rules of conduct and performance. Any one, or several, of thefollowing actions will be justification for DTS to terminate youremployment, including, but not limited to:

1. Unsatisfactory performance of duties.

2. Failure to report to assignments, leaving assignmentsearly, tardiness or excessive absences.

3. Failure to contact your supervisor or your DTSemployment representative when absent, tardy orneeding to leave the job early.

4. Smoking in areas designated “No Smoking.”

5. Failure to maintain a neat, clean appearance; failure towear ID Badge while on assignment; failure to conformto a department uniform or dress code policy.

6. Use of profane or abusive language.

7. Failure to adhere to Duke or department safety policiesor procedures, including failure to immediately report an accident on Duke premises involving an on-the-jobinjury or property damage.

8. Insubordination, including refusal to accept and followinstructions from supervisors.

9. Loud and boisterous conduct which may be discourteousor harmful.

10. Failure to adhere to the Duke policy on non-solicitation.

11. Refusal to accept instructions of security officers, and inemergency situations, civil defense personnel or otherproper authorities.

12. Sleeping on the job.

13. Unauthorized or improper use of Duke property.

14. Unauthorized use of “Duke Identification Badges” or passes; permitting another person to use youridentification.

15. Unauthorized use of computers and/or the internet for non-business purposes.

16. Reporting to work in an unfit condition and/or workingunder, or suspected of working under the influence ofdrugs or alcohol.

17. Commission of any crime on Duke premises such astheft: unauthorized removal of or willful damage toproperty; unauthorized possession of alcohol/weapons/explosives; gambling; being found in possession ofillegal drugs; and charging personal long distance phonecalls to Duke without authorization.

18. Behavior which compromises another’s safety or privacy,or discloses confidential information, includingmedically-related records.

19. Any activity which violates federal or state standardsregulating the provision of professional services, orviolation of regulations affecting continued licensure,commissioning or certification in a profession.

20. Failure to appropriately interact with anyone on Dukepremises (including patients, their families, students,visitors, customers or other employees), when suchbehavior violates another’s privacy or dignity, includingsexual harassment.

21. Falsifying or forgery of any Duke records (such asemployment application, timecards, etc.); improperlyrecording hours worked on timecards.

22. Threats, fighting or physical actions against anotherperson on Duke property.

23. Willful damage to Duke property.

24. Conducting personal business in the workplace orinterrupting other employees from their scheduled work.

25. Use of another’s computer sign-on or computer accesscode or providing another the use of an individual’s sign-on code without proper authorization to gainunauthorized access to confidential or privilegedinformation.

26. Personal use of electronic mail including solicitations,chain letters, sexual or ethnic jokes and slurs, defamatorystatements and idle gossip is forbidden.

27. Any employee who is arrested for a misdemeanor or a felony must notify his or her DTS employmentrepresentative of such arrest no later than seven calendardays after the arrest. If an employee is convicted of amisdemeanor or a felony, he or she must inform his orher DTS employment representative of such conviction(including pleas of guilty and nolo contender) no laterthan seven calendar days after the conviction.
