Medicare Summary Notice (MSN) and Explanation of Benefits (EOB)Both of these documents are important tools for understanding your Medicare and UnitedHealthcare benefits.

Medicare Summary Notice (MSN) Explanation of Benefits (EOB)Who is it from? Medicare UnitedHealthcareHow is it distributed? By mail By mail

What are the different types?

There are different MSNs for Part A, Part B, and Durable Medical Equipment (DME)

There are different EOBs for the Senior Supplement (Medical) Plan and the Prescription Drug Plan (PDP)

When will you receive one? Each calendar quarter

For Senior Supplement: as claims are processed, for each date of service with a providerFor PDP: monthly, for each month you have a Part D prescription filled or when a formulary change affects a drug you are taking

What key information does it include?

• Amount charged by provider, facility, or supplier

• Amount Medicare approves under its fee schedule

• Amount paid by Medicare• Maximum remaining amount you may be

billed by the facility, provider, or supplier• Amount you can expect to owe when you

receive the provider’s bill (do not pay this amount until you receive and review the corresponding UHC EOB and provider bill)

• Amount charged by provider• Charges not covered by health plan• Amount Medicare and other insurance will pay

the provider for the covered services, it is labeled “Provider Responsibility” (it does not include any amounts that are paid by the plan)

• Amount applied toward the Trust plan’s $300 deductible

• Member coinsurance and any Medicare Part A deductible amount after plan deductible is met

• Amount paid by UnitedHealthcare• Amount you can expect to owe when you receive

the provider’s bill

Is progress toward the PDP coverage gap tracked?

Not applicable

Yes, your PDP EOB lists your current coverage stage (Initial, Coverage Gap, or Catastrophic), as well as the dollar amount of your year-to-date total drug costs, and the remaining dollar amount of total drug costs to be paid in the current stage before you move to the next coverage stage

How can you access an electronic copy?

1. Create an account at mymedicare.gov2. Search your claims3. View a specific claim4. Click on the ‘View MSN’ link located below

the Claim Number on the Claim Details pageNote: If your claim has been adjusted, you must call 1-800-MEDICARE (1-800-633-4227) to order a copy of the MSN.

1. Register for an online account at uhcretiree.com/delta2. Search your claims3. View a specific claim in the Claims List by clicking

‘More info’ in the far right column4. Click on ‘Explanation of Benefits’ next to

‘View Benefits’

Reviewing your MSN and EOB together is important to help ensure your claims are correct and to keep track of your medical expenses. UnitedHealthcare has created a document to help you understand your EOB and compare it to your MSN. It can be found at the Trust website insurancetrust4deltaretirees.com in the Message Center on the Home Page and is titled, “Understanding Your UnitedHealthcare Explanation of Benefits.” The MSN shows you how much Medicare pays, and the EOB shows you how much the Senior Supplement Plan pays. Be sure to compare them to your provider bills to verify that the billed amounts and payments make sense to you. Both the MSN and EOB are helpful for understanding what you owe. You should not pay any provider bills until you receive the corresponding MSN and EOB. If you have questions about any of the information on an MSN, EOB, or provider bill, call a Personal Health AdvocateTM for assistance at 1-877-325-7265, Option 2.

Helpful Legal Tools for Trust Medical Plan MembersIn our February newsletter, we provided information about naming an Authorized Representative to assist with your Trust coverage, as well as a brief summary of a Durable POA for Health Care and a Durable POA for Finances. Did you know that all Trust Medical Plan Members have access to an Online Will benefit with legal tools that include templates to create both of these Durable POA documents? This important benefit is provided by UHC and Consolidated Legal Concepts, Inc. (CLC) and can be accessed at deltaretirees.clcmembers.com.

When you visit the CLC website, you can perform a search for documents specific to your state of residence. In some states, an Advance Directive can be used in place of a Durable POA for Health Care. Once you select a document, you will be prompted to register by entering your email address and selecting a password. This will allow you to save your documents to be accessed later, if needed. You will complete your document by answering a series of questions.

Questions about Durable POAs, Advance Directives, and special situations should be discussed with an attorney. If you do not already have a relationship with an attorney, the program offered by UHC and CLC also provides you with referrals to attorneys in your area. You can request a no-cost 30 minute consultation with an attorney to discuss estate planning including Durable POAs and Advance Directives, and when you retain one of these attorneys for services, you will receive a 25% discount off of regular professional fees. Call 1-877-717-9517, Option 2, or TTY 711, for more information.

Don’t Forget to Appoint a Personal Representative with MedicareMedicare allows a personal representative to be appointed for the life of an individual or for a limited period of time. You can also limit the information the representative can access to one or more topics including eligibility, claims, enrollment, premium payments, or a topic you identify.

To appoint a personal representative, you must complete a ‘Medicare Authorization to Disclose Personal Health Information’ form. The form can be found at medicare.gov/medicareonlineforms/authorizationform/onlineformstep.asp and either filled out online or printed and completed by hand. To print and complete the form by hand, look for the ‘PDF Version’ in the yellow box on the right side of the website. If you do not have internet access, you can call 1-800-MEDICARE (1-800-633-4227) to request the form. Follow the instructions on the form for mailing the completed, signed form to Medicare.

An individual that is not incapacitated may revoke any authority given to a personal representative at any time by communicating in writing with Medicare at the address provided on the Authorization form.

Experience SilverSneakers and Don’t Let Arthritis Keep You from Exercising

As a Trust Medical Plan Member you have access to the SilverSneakers Fitness Program to help you stay physically fit and active, enjoying the best possible quality of life. SilverSneakers is available to you at no additional cost. When you enroll in SilverSneakers, you have a fitness membership that includes group exercise

classes, workouts, health education seminars, social events and much more. To get started visit silversneakers.com or call 1-888-423-4632 (TTY 711), Monday - Friday, 8 a.m. to 8 p.m. Eastern.

But if you suffer from arthritis, you might be hesitant to make your first trip to one of the many SilverSneakers fitness centers. It’s important to discuss your exercise plans with your doctor, but for most arthritis sufferers, moderate activity can help ease the discomfort.

Start out with some warm-up exercises for your arthritis-affected joints. Then try a 20 minute workout and gradually increase your workout time. If 20 minutes is too much, try two 10-minute sessions with a break in between. Gradually increase your time until you reach 30 minutes. This is the recommended time period for arthritis exercise. Keep a log of your workouts so you can remember exercises that you enjoy and avoid exercises that caused you discomfort. Once you find an exercise you enjoy, do it! And invite a friend to join you so you can encourage each other.

Pace yourself – you may feel like you’re not accomplishing much at first, but your body needs time to adjust to activity. If you overdo it, your body will let you know. You may need to use an ice pack briefly after your workout or consult your doctor if the pain persists for more than a couple of hours. But don’t give up! The long-term result will be less pain and stiffness to your joints, and of course, a healthier and more active lifestyle.

For more information about SilverSneakers, please call 1-888-423-4632 (TTY 711), Monday - Friday, 8 a.m. to 8 p.m. Eastern.
