CephalosporinC誘 導 体Cephradineの 骨 関節 感染 症 に対 す る

臨床 研 究 と2,3の 考 察 につ いて(第1報)

CephradineはFig.1の よ うにCephalexinと 類 似 の 構

造 を有 す るセ フ ァ ロ ス ポ リンC誘 導 体 で あ り,そ の分 子

式 はC16HlgN3O4S,分 子 量349.4は で あ る。

また,本 抗 生 物 質 は,白 色 の結 最性 粉 末 で あ り,水 お

よび 各 種pH緩 衝 液 に対 す る溶 解 度 はTable1の とお り

で あ るが,有 機 溶 媒 に対 して は,エ ー テ ル,ク ロ ロホ ル

ム,ベ ンゼ ン,n-ヘ キ サ ンに は 不 溶 で あ り,ア セ トンお

よび エ タ ノー ル に は,わ ず か に 溶 解 し,プ ロ ピ レン グ リ

コー ル に は容 易 に溶 解 す る と報 告 され てい る。

な お,本 抗 生 物 質 の抗 菌 ス ペ ク トラ ム は,Table2お

よび3の とお りで あ り,大 部 分 の べ ニ シ ジナ ー ゼ 産 生 ブ

ドウ球 菌 を含 む グ ラム 陽 性 菌,お よ びE)Pseudomonas属

を除 くグ ラム 陰性 菌 に 対 して 広 範 囲 の抗 菌作 用 を 示 し,

そ の作 用 は殺 菌 的 で あ るが,抗 真 菌 作 用 は み られ な い。

症例お よび検 出菌

本抗生物質 を,整 形外科領域 におけ る骨,関 節感染症

20症 例 に投与 した。 その疾患名,年 齢,性 別はTable4,

5お よび6に 示 した とお りで ある。 なお,本 抗生物質投

与に先立 って,各 患 者か ら,本 剤使用 につ き承諾 をえて

い る。

また,本20症 例 は,す べて病巣か ら菌 を検 出 した もの

であるが,19病 巣 か ら黄 色 ブ ドウ球 菌 を,1病 巣 か ら

は,大 腸菌 と黄色 ブ ドウを認 めてい る。 ただ し,こ れ ら

が(Table4お よび7)感 染原 因菌 である との確証 はえ

ていない。 さらに,そ の各種抗生 物質 に対す る感 受性は

Table8に 示 した とお りであ る。

投与 量および投与期間

Cephradineは,200mgを1カ プセル(力 価)と し,

毎6時 間お きに投与す るの を原 則 と したが,重 症者 で

は,さ らにそ の景 を増 し,ま た,小 児 にお いては,そ の

体重 に よ り投与量 を加減 してい る。

また,投 与期間 は10日 か ら38日 にわた っ て い る。 ま

た,Cephradine投 与 中は,他 の抗生物質の使用 は当然

なが ら中止 した。

効果判定基準

効果判 定基準は,種 々 の条件 が重 な るため,一 定の基

準 を うるこ とは困難で あったが,整 形 外科 的疾患の特異

性 を考慮 し,著 者は次の よ うな方法 に よって効果 を判定

した。

す なわち,臨 床所見(発 熱,腫 脹,発 赤,疼 痛,分 泌

液 ない し膿 汁の増減,お よびその粘稠度 な ど),一 般 検

査所見(血 沈,C.R.P.,白 血球数 な ど),お よび菌検

査所 見の消長の3項 目につ い て検 討 し,各 項 目におい

て,ま った く所 見な い し検査結果 が正常 とな った もの,

お よび著 しく改 善 した もの を(+3),相 等度 に改善 した

もの を(+2),改 善 が軽度 に留 った もの を(+1),ま っ

た く改善 をみ なかった もの を(+o)と した。

次 いで,各 項 目におけ る点数 を合計 し,+gか ら+?を

著効(Excellent),+6か ら十4を 有効(Good),十3か

ら+1を や や 効(Fair),+oを 無 効(Poor)と した

(Table9)。

効 果

以上 の結果,前 述 した20症 例 にお いて,著 効7,有 効

5,や や効5,無 効3と い う成績 を得 た(Table10)。

副 作 用

20症 例 中,2例 に消化 管障 害 をみた。すなわ ち,第3

例 に食 欲不振,第4例 に下痢 をみた。後者は,本 剤投与

後10日 目に生 じた もので,Cephradineの 投薬 を中止 し

たが,前 者は使用 を続行 し,な ん らの増悪 をみ な か っ

た。 また,こ の2症 例 と も,上 述 の 消 化 管 障 害 が,

Cephどadineに よ り生 じた もの との確証 はない。

そのほか,血 液検査,尿 検査,肝 機 能検査の所見 で,

とくにCephradineの 投与前 に比較 し,悪 化 した ものは

み られなか った(Table11,12)。

考 案

さて,以 上 の成績 を,種 々 の条件 か ら検討 す るこ とに

す る。

著効7例 は,す べて黄色 ブ ドウ球 菌だけが検出 され た

症例 であ り,Cephalexinに 対す る感 受性は帯 であった。

有効5例 は,う ち3例 が,Cephalexin感 受性什 であ

り,他 の2例 は什 であ った。

やや効 の5例 では,Cephalexin感 受性 はすべ て升 で

あ り,無 効3例 においては,Cephalexin感 受性は,大

腸菌 を検 出 した第4例 をふ くみ,+に す ぎなか った。

以上 か ら,本 抗 生物質の治療成績 には,類 似構造 を有

す るCephalexinへ の感 受性が,重 要 な条 件の1つ と考

え られ る。

ついで疾患 自体 か ら検討す る と,著 効例 の うち,5例

は骨 接合術 ない し開放性骨 折の1次 閉鎖 の術 後感染 であ

り,ほ かの2例 は単 に軟 部感 染創 であって,整 形外科領

域 の感 染症 と しては,比 較 的,容 易 にco就ro1が 可能な

ものであ った。

有 効例5例 は,軟 部 の術 後感染2例,骨 折 に対 す る術

後感 染1例,褥 創1例,急 性血行性骨髄炎1例 であ り,

著効群 に比較す る と,治 療の困難 な症例(第5例 お よび

第17例)を 含ん でいる。

やや効 の5症 例 は骨折 に対す る術後感染1例,軟 部感

染 を伴 な う骨 折1例,慢 性血行性骨髄炎1例,軟 部感

染創1例,牽 引の 日的 で大腿骨 に鋼線 を刺入 した個所 に

感染 した もの1例 であ った。 この うち,軟 部感染創 は血

行不良部位 に発生 した ものであ り,ま た,鋼 線刺入 部感

染症は当然,異 物が存在 したわ け で あ る。 血行不良部

は,組 織の修復,喰 菌作用 が低 下す るの は 当然 である

が,か つて著者が,動 物実験,お よび臨床 実験か ら証 明

したように,抗 生物 質の分布濃度 も低 い こ とが 考 え ら

れ,こ れ が治療成績 に影響 を有 した もの であろ う。

また,鋼 線刺入部感染創 は,鋼 線抜 去後,急 速 に治癒

に赴いてい るが,こ れは感染異物 に対 して抗生物質の効

果は十分に発揮で きない こと と,抗 生物 質療法のあ る限

界 を物語 ってい ると考え られ る。

無効の3例 は,褥 創2例,骨 折 の術後感染1例 で,前

2者 は下半身 マ ヒを伴 なってお り,こ れ が感染創 の治療

成績が不良であ ったことに大 きな影響 をお よぼ していた

と思 われ る。 さらに,第4例 は大腸菌 を も検 出 した症例

であるが,Table2お よび3に 示 す よ うにCephradine

のMICが 大腸 菌に対 しては,黄 色 ブ ドウ球 菌 よ りは

るかに高いこ とか らも裏付け られ るよ うに,こ の大腸菌

の存在が,治 療成績 を悪化 させた理 由の1つ であろ う。

また,術 後感染症例(第20例)も,血 行の不良 な部 分で

あったことが,臨 床成績 が不 良であ った大 きな理 由であ

ろうと考え られ る。

なお,本20症 例 にお いて,骨 手術 や,開 放性骨折 の術

後感染はexogenous osteomyelitisと して,取 扱 ってい

る点を附記 したい。

ま と め

骨関節感染症20症 例 におけ るCephradineの 臨床的検

討 を行ない,著 効7例,有 効5例,や や効5例,無 効3

例 とい う成績 を得 た。

なお,検 出菌 および病巣 の状 態か ら,上 述 の治療成績

を検討 した。

A CLINICAL STUDY ON CEPHRADINE, A NEW DERIVATIVE OF

CEPHALOSPORIN C (Part 1)

Cephradine was applied clinically in 20 ceses of bone and joint infections, and the results obtained were

remarkably effective in 7 cases, effective in 5 cases, slightly effective in 5 and not effective in 3 cases.

These treament results were investigated in view of detected bacteria and lesional conditions.
