Patient-Centered Medical Home
• When visiting primary care, students will
be assigned to a primary care provider
who is part of a care team.
• What does this mean for students?
Students will see members from the
same care team with each appointment.
• This approach encourages
communication and partnerships
between individual patients, their
primary care physicians, other team
members (counselors, psychiatrists,
nutritionists, etc.), and when
appropriate, the student’s family.
Received accreditation as a Medical Home in 2014 and
re-affirmed in August 2017 by the
American Association Ambulatory Health Care (AAAHC)
