Physio Direct - Self-referral Form
What is self-referral?
Self-referral is a direct route to physiotherapy. It allows patients toaccess physiotherapy treatment without seeing a GP first.

Can I self-refer?
You must be 16 years of age or over and have musculoskeletalproblems such as back pain, neck pain, painful joints and/or generalsoft-tissue sprains and strains. This includes sports injuries, workrelated injuries and postural problems.
How will I benefit?
Patient satisfaction surveys have shown that patients benefit fromself-referral in the following ways:
• It is a direct route to advice and intervention when it is mostneeded
• Early intervention can speed up healing and recovery• It supports the individual in developing skills in self-care and
self-management
When should I not self-refer?
You should not self-refer but see your GP urgently if you have hadany of the following symptoms since the onset of the problem:
• If you have back pain and feel this is causing bladder or bowelproblems (difficulty passing water)
• If you have suddenly lost any weight without trying• If you have dizziness (with loss of hearing on one or both sides)
blackouts or falls• If you have a previous history of cancer or are currently
undergoing treatment for cancer
Self-referral is not an option if:
• You are under 16 years of age• You have neurological, breathing or continence problems

Making a referral
Complete the attached referral form and return in the post to therelevant address over the page.
Alternatively you can complete a referral online at:

Booking an appointment
Please contact the relevant service five working days after you sendthe form back, this will allow us time to process your referral.If youare registered with a Heywood, Middleton or Rochdale GP pleasecall 0161 716 5888 (Option 2)
