Events Examples
Failure to Start: used for instantaneous trip prior to completion of startsequence or unit did not start in the startup time defined by dispatcher.

Events Examples
Forced Outage Automatic Trip (FOA): A trip from a state of operation
when a component failure or other condition which causes the unit to beshut down automatically by the control system.
- Unit tripped on high lube oil temperature, due to a shorted transmitter. Replaced
transmitter and the unit returned to service.
Forced Outage Manual Trip (FOM): Shutdown from a state of operationwhen a component failure or other condition which resulted in a decision bythe appropriate personnel to manually trip/remove the unit from service. This
event type can also be used in cases where human actions result in the
tripping of a unit from a state of operation.
- A rise in vibrations was experienced and the unit was shut down to investigate. A
failed vibration sensor was discovered, replaced and the unit returned to service.- A crane operator accidentally damaged the lube oil skid while removing the
generator housing from unit#2 for major repair work. After replacement of the
damaged pumps and piping, unit#1 was restarted.
