Open enrollment is your annual opportunity to enroll or make changes to your elections in the benefit program offered by UST Global.
All enrollments or changes made during the open enrollment period will be effective January 1, 2017.

What’s New for 2017?
Aetna will be our insurance provider for Medical and Dental. Aetna was
our carrier for many years prior to 2016, and we are moving back to Aetna
for 2017.
• Still three medical plans to choose from – Plus Plan, Choice Plan,
Premier Savings Plan. No changes to the existing benefits.
• Medical network – Aetna Choice® POS II (Open Access) –
Nationwide PPO plan
• Dental Plans – still choice of two, Enhanced or Basic
• Dental network – Aetna Dental® PPO II
• Flexible Spending Accounts and Commuter Benefits. You will need
to re-enroll in these benefits for 2017, offered by Aetna PayFlex.

Premier Savings Plan: How does it work?
The Premier Savings Medical Plan is a High Deductible Health Plan (HDHP)
which is HSA compatible. How does an HDHP work?
• Preventive care is covered at 100%
• Meet your deductible first – Deductible is the amount that you must pay
before the plan begins to pay out ($1,500 for individual / $3,000 for
Employee +1 or more dependents)
• Note: If family coverage is selected, the entire family deductible must
be met before plan pays benefits for any individual. The deductible may
be satisfied with a single claim for one person or by a combination of
claims from all family members.
• After the deductible is satisfied, you will pay coinsurance for most
services. Coinsurance when using a network provider is 20%.
• The out-of-pocket maximum is designed to protect you from catastrophic
costs ($2,500 for individual / $5,000 for family)
• If you have established an HSA account, you can use the funds in it to pay
your deductible and coinsurance expenses.

Premier Savings Plan: How does my HSA Work?

A Health Savings Account (HSA) allows you to set aside funds
on a pre-tax basis. Use the funds in your HSA to pay for
eligible medical, dental and vision expenses (i.e. deductible
and coinsurance).
Features of a Health Savings Account (HSA):
• HSA account is owned by you, so the money is always yours
to keep even if you should you change health plans, change
employers or transfer out of U.S.
• Money in the HSA may be used to pay for expenses as they
are incurred, or saved for future expenses
• Unused amounts roll over to the next year, no “use it or
lose it” rule
IRS limits: The maximum amount that you can contribute to
an HSA in 2017 is:
• $3,400 for those enrolled with individual coverage.
• $6,750 for those enrolled with family coverage.

Premier Savings Plan: Establishing your HSA Account

The HSA account continues to be offered through HSA Bank
Those enrolled in the Premier Savings Plan are eligible to
establish an HSA account. These plan members will receive
a Welcome kit in the mail from HSA Bank with instructions
on how to complete the enrollment process
If you enroll in the HDHP, you are not eligible to contribute
to both an HSA and FSA Health Care account. You must
contribute to one or the other.

If you are in an active course of treatment with a nonparticipating provider Aetna may cover the treatment for a specified period of time at the preferred benefit level.
Active course of treatment is when you have begun a program of planned services with your doctor to treat a diagnosed condition.
Third trimester of pregnancy, ongoing treatment plans such as chemotherapy, radiation, recent
surgery, members taking growth hormone treatment.
You need to complete the TOC form and send/fax to Aetna for medical review.
Guidelines and criteria must be met. A written notification will be provided to physician and member informing you whether or not your request under the Transition of Care guidelines has been approved.

Critical Illness and Cancer Plan
Unum’s Critical Illness can help protect your finances from the expense of a
serious health problem, such as a stroke, heart attack, or cancer. A lump sum
benefit is payable to you at the first diagnosis of a covered condition.
Plan design*:
• Late entrants can enroll for $10,000 of employee coverage, $5,000 of
spouse coverage, and $2,500 of child coverage (automatically included
with the employee coverage). No medical underwriting required
• If you choose not to enroll at this time, you will have to wait until the next

• Premiums are based on age-banded rates. They do not increase as you age.

Group Hospital Indemnity
Unum’s Group Hospital Indemnity Insurance provides financial protection by
paying a benefit due to a hospitalization and in some cases, for treatment
received for an accident or sickness, even if that treatment occurs outside the
hospital. Family coverage is available.
Plan design:
• Hospital admission: Lump sum $1,500 benefit
• Hospital confinement: $100 per day, maximum of 15 days per calendar year
• Emergency room treatment: $150 per calendar year (accidents only)
• No medical underwriting required if you elect coverage during open
enrollment. If you choose not to enroll at this time, you will have to wait
until the next annual enrollment period to enroll and medical underwriting
will be required. Portability is included

Flexible Spending Account - Rules
The Flexible Spending Accounts are “use it or
lose it” plans.
New plan year starts January 1, 2017.
If you enroll, you must participate the entire
year.
Qualified expenses must be incurred in 2017.
You may claim 2016 plan year expenses up

Open Enrollment: Action required
• When you login to the enrollment system and select Open
Enrollment, you will notice you are not enrolled for Medical and
Dental plans for 2017. This is due to the shift to Aetna
• You must choose your plan and enrollment tier for 2017 and
submit elections before 11.59PM PT on December 12
• If you take no action by December 12, your current choices of
plans and election tiers will be rolled over into 2017, and you
will not be allowed to make any changes unless you have a
Qualifying Life Event

Open Enrollment: Other actions to consider
• This is also your time to enroll or change your elections in the other
plans offered by UST Global.
• Contribute to an HSA (participants in Premier Savings plan only).
• Add or delete coverage for you or your dependents.
• Review personal information for accuracy and update as necessary
• New ID cards will be mailed to your home address – make sure it is
accurate!
• Update your Life Insurance Beneficiary.
• This is the person or persons that will receive your life insurance
benefit in the unfortunate event of your death
• This can be changed at any time during the year.

year unless you experience a qualifying life event as defined by the IRS

IRS Qualifying Life Events / Change in Status
An IRS Change in Status Event is an event set forth by the IRS/Sect. 125. It
allows employees to make mid-year election changes when a change in a
participant’s status occurs. If you do not experience any of these events you
will not be able to change your elections during the year.
Qualified changes in status include:
• Marriage, Divorce, or Legal separation
• Arrival or departure of a dependent from another country
• Birth or adoption of a child
• Change in child’s dependent status

Benefit changes must be completed within 31 days of the event, and will
be effective as of the date of the event. Enrollment changes will not be
permitted after the 31 days.

Important note to employees recently landed
in the U.S.
• Your benefits are in effect as of the day of
arrival, but you must complete the online
enrollment process for 2016 and confirm
your elections within 31 days of your arrival
date.
• In addition to completing your online
enrollment for 2016, you must also make
open enrollment elections for 2017.
• This also applies for any new dependents
joining you – i.e. spouses or newborns.
If their arrival date is before
January 1, 2017 you must enroll
them for 2016 AND 2017!

Accessing the Enrollment System
Step 1: Log into the online benefits system at:
www.ustglobalbenefits.com and enter Code: UST
Step 2: Enter your Username and Password in the Enroll/Change Benefits
section on the home page and click Log In.
Username: UST_<Employee ID>
Password: your password
Step 3: Choose your plans
Medical, Dental, Vision, Voluntary life, Flexible
Spending Accounts and more
Step 4: Add/update your eligible dependent(s)
Step 5: Choose/Update your beneficiaries

2017 Insurance ID Cards
New ID cards will be issued by Aetna
One combined card for Medical, Rx and Dental coverage
• If you are covering dependents, a separate card will be provided for
each dependent with their name on the ID card
• ID cards will be mailed to your home address – please make sure the
enrollment system has your most current information
• ID cards should be received by January first week
• If you do not receive new ID cards, you may download a soft copy
online, or use the Aetna Mobile App
• Providers will also be able to verify your coverage with Anthem by
your social security number.
There are no ID cards issued for the VSP vision plan
• Providers can verify eligibility using your SSN
