Brain imaging in FTLD diagnosis
• Pattern of atrophy on structural MRI is very helpful for diagnosis of frontotemporal lobar degeneration FTLD.
• FDG-PET may be more sensitive than MRI in early stages, and can be clinically useful in distinguishing FTLD from AD
• Currently, structural MRI and FDG-PET are included in to the diagnostic criteria of FTLD

ผ ปวยชายไทยหมาย อาย 80 ป ภมล าเนา จ.อยธยาอาการส าคญ มปญหาดานความจ า 4 ปประวตปจจบนลกชายสงเกตวาผ ปวยมกจะพดซ าๆ ถามซ าๆ อาการเปนมากขนเรอยๆ คนหาขาวของทงวน มหลงทางไปขางนอก กลบบานไมถก ปสสาวะ อจจาะระในหองนอน เขาหองน าไมถก เดนไปเดนมา อาบน าไมสะอาด นอนไมหลบ คอยเรยกญาตตลอด

- จบการศกษาระดบม.6
- เปนทหารอากาศ เกษยณตอนอาย 60 ป- โรคประจ าตว Type 2 DM, HT, Dyslipidemia
- เคยดมเหลาสงสรรคกบเพอน เลกมา 20 ป - ไมสบบหร- ปฏเสธประวตการไดรบการกระทบกระเทอนรนแรงทศรษะประวตครอบครว- ไมทราบวาบคคลในครอบครวมอาการหลงลม การเคลอนไหวผดปกต หรอโรคทางจตเวชหรอไม

ประวตปจจบนมารดาสงเกตวาผ ปวยท าอะไรชาลง พดชา เดนชา แตยงสามารถชวยเหลอตวเองได ขบรถไปท างานได ไมมหลงทาง เรมมออกจากบานหรอไปไหนมาไหนโดยไมบอก ไมคอยสนใจคนรอบขาง กลนปสสาวะไมได มปสสาวะราดเปนบางครง สามารถกลนอจจาระได ปกตไมมแขนขาออนแรงตดตามการรกษา 6 เดอนมารดาบอกวาอาการแยลง ขบรถไปท างานไมไดแลว มารดาตองไปรบไปสงทจดขนรถ ทท างานไมไดมอบหมายงานใหท างานแลว อารมณยงคงนงเฉย เดนชาพอๆเดม
