The main goal of this study is to perform a large, simple, multicenter, randomized,
parallel group trial directly comparing screening colonoscopy with annual fecal
immunochemistry test (FIT) screening in average risk individuals and determine whether
colonoscopy is superior to FIT in the prevention of colorectal cancer mortality measured
over 10 years.

care through telemedicine and online resources).

Narrative Report
I earned my MD degree at the University of Debrecen in Hungary, where I then completed specialty
training in internal medicine and gastroenterology (GI). I earned my PhD for the research I conducted
as a Fogarty Fellow at the University of Pennsylvania. After additional training in US medicine and
gastroenterology, I joined the faculty of Brown Medical School and Rhode Island Hospital. In 2007, I
was appointed chief of gastroenterology at the VA Boston Healthcare System (VABHS), where my
current professional efforts primarily include clinical service, administrative, and leadership activities,
with the remaining time devoted to teaching and research.

Area of Excellence: Clinical Expertise and Innovation. Patient care is an essential and rewarding
part of my work. Since 2007, I have been active clinically at the VA in GI endoscopy and the GI
outpatient clinic, and as an inpatient consult attending. I have also introduced clinical innovations
involving the electronic medical record, patient navigators to improve compliance, incentives to
decrease the colonoscopy cancellation rate, and lifestyle guidance to preserve liver health. My clinical
and procedural skills benefit from the initial 10 years of academic practice that I did in Hungary.
In the past 25 years, I have focused on various aspects of fatty liver disease. As an NIH Fogarty
fellow, I analyzed calcium-mediated growth factor signaling in liver cells. As a GI fellow, I studied the
role of uncoupling protein-2 (UCP2) in fatty liver at HMS and expanded this work to bioenergetics
and metabolic reprogramming in digestive cancers at Brown. My current work includes various
projects aimed at nonalcoholic fatty liver disease (NAFLD), a prevalent condition that may predict
morbidities with profound societal implications. We study the risk of hepatocellular carcinoma in
veterans. We utilize concepts of network medicine to predict disease trajectories in NAFLD and to
improve cost effectiveness of cancer surveillance. Using an international twin cohort, we study
heritability of NAFLD and associated cardiovascular disease. I have written several reviews and
concept papers, and organized regional and national symposia and workshops on issues of the
pathophysiology and clinical management of fatty liver. My goal is to establish a center of
excellence for obesity-associated liver diseases at VABHS.
Teaching and Education: I have taught medical students, residents, and GI fellows from the
beginning of my academic career and continue to do so at HMS and VA-affiliated GI training
programs. I have also mentored several postdoctoral fellows, undergraduates, and PhD candidates now
successful in their own research careers. I have organized many local, regional, and national CME
events. Since 2009, I have been the program director of a highly successful postgraduate series at
VABHS on a variety of major topics in gastroenterology and hepatology, providing an important
educational opportunity for the Veterans Integrated Service Network 1 in New England. As the
recipient of a Fulbright US Scholar Fellowship, I recently worked at the University of Debrecen
School of Medicine in Hungary. This program was sponsored by the US Department of State Bureau
of Educational and Cultural Affairs and allowed me to teach an accredited course on obesity and
associated disorders. The course consisted of 15 two-hour seminars in the English language program
over a total of 8 weeks during the first semester of the 2014/15 academic year.
Significant Supporting Activities: Investigation: Dual NIH funding supported my research at
Brown, where my group made fundamental observations on the pathobiology of mitochondrial
uncoupling proteins in hepatocytes and cancer cells. I serve as the local site investigator for the
extensive VA Cooperative Studies Program (CSP 577 - CONFIRM) on the impact of colonoscopy vs.
fecal immunochemical testing on colorectal cancer mortality. My additional investigations currently
focus on early biomarkers of hepatocellular carcinoma at VABHS, BWH, and Beth Israel Deaconess
Medical Center.
Administrative and Institutional Service: I have been a member of various committees in the HMS
Department of Continuing Education and completed a 1-year leadership program at HBS. The unique
academic environment of my current position as section chief provides me with major opportunities to
advance the cause of digestive disease care and prevention.
