Accessing Your 2020 Form1095-C or 1095-B

Log in to hertzbenefits.com. On the top line, click Log In > Enroll for Benefits

This links to the Businessolver website where you elected to receive your 1095 electronically. Enter your User Name and Password, then click on Login.

This provides access to the Menu screen. Click the Personal Documents box.

Then you will see a list of Personal Documents available.

Click on the 1095 Employee Statement for the appropriate year.
