연 목 : 연 는 료 용 지역 변 에 어
지역 단 가 변 에 미 는 향 악하고 하 다.
해 8개 주요 시 에 지역 변 행 역과, 동 시간,

하여 지역 변 비 하 다. 한 포 료 용지

하 다. 행 역(시 , 역시도)과 진료 ( 진료 , 진료 )
비 상 하여, coefficient of variation, extremal quotient,
systematic component of variation 산출하여 시 변 비
하 다. 한 지역 단 별 포 양상과, 8개 시 에 한
지역 도, 지 , 출비 등 료 용지 비
하 다.

결과: 진료 경우 행 역 시 과 비 하여 8개 주요 시
에 coefficient of variation, extremal quotient등 감 하
다. 지역 단 별 coefficient of variation도 시 과 비
하여 감 하 , 지역 도 지 등 료 용 지
는 시 에 비해 높 값 나타냈다. 진료 경우
행 역 역시도 비 하여 결과 차 가 뚜 하지 않았다.
결론: 지역 단 법에 지역 변 가
달 지 , 지역 단 에 료 용 지역 도 단 별
포 양상에 차 가 하 다. 포나 지역 도
같 지역 특징 지역 변 에 미 는 향 도 악하
해 는 차후 연 가 필요할 것 다.

I. 론
1. 연 경
료 용 지역 변 는 지리 지역 단 에
료 용 도 양상 다 게 나타나는 상 다.
집단 료 용에 어 용 재한다 ,
변 (變異, variation)는 료 용 효과 는 효 에 어
에 지 못함 미한다. 료 용 지역 변 ( 하 지역
변 ) 연 는 러한 식에 고 해 다.
1938 Glover는 학 역에 등학생들 편도
차 보고하 , 에 한 사 견해 차 에는
달리 하 어 다고 하 다(Glover, 1938). 연 는
지역 변 에 한 과 후 지역 변 연 본보
가 었다(Wennberg, 2008). 1973 Wennberg는 지역
(small area analysis) 통해 지역 변 가 공 요 에 함
보여주었다(Wennberg et al., 1973). 그는 연 상
지역 변 개 ‘ 당 지 않는 변 (unwarranted
variation)’ 체 하 고(Wennberg, 2010), 변 연 법
립하 지역 변 연 토 마 하 다.
후 지역 변 연 는 상 시 과 질 범 해 고,
료 용 결과, 합병증, 료 에 한 근 등 지역 변 연
주 가 었다(Appleby et al., 2011; Corallo et al., 2012;
Wennberg, 2014). 연 들에 지역 변 는 나
필요 같 특 차 만 는 지 않
(Wennberg, 1987; Volinn et al., 1992; Schäfer et al., 2011;
Ozegowski et al., 2013), 병상 (Wennberg, 1987; Fischer et

al., 2000), 사 (Black et al., 1995; Milcent et al., 2009),
사 진료 식(Wennberg et al., 1982; Keller et al., 1998;
Wright et al., 1999; Coyte et al., 2001; Tu et al., 2012), 료
비스에 한 근도 차 (Black et al., 1995; Gusmano et al.,
2013) 등 변 하는 것 다. 그러나 지역
변 많 여 지 않 상태 남아 다
(Newhouse et al., 2013; OECD, 2014).
료비가 증가하고 료 실 에 한 식 높아지 지역
변 연 에 한 필요 증가했고(Skinner, 2012), 지역 변 연
는 가 차원 연 가 간 비 연 했다(Thygesen et
al., 2015). 변 연 는 료 용 변 시하고
변 시 간 비 통해 변 도 악하고 하 다. 그러
나 어 도 변 가 당한 것 , 어 도 시 타당한
것 가는 여지 남아 었다(Wennberg, 1986; Wennberg,
1987). 가 간 비 연 는 동 시 에 한 지역 변 가 간
비 가능하게 하 다는 에 , 당 지 않는 변 탐색
는 에 진 보하 다고 할 다.
가 간 지역 변 비 연 가 하 지역 단 가 변
에 미 는 향에 주목하게 었다(Thygesen et al., 2015).
변 비 가 주 동 지역 단 에 다 시 간에
루어진 하여, 가 간 연 에 는 다 지역 단 용하여
산출 동 시 에 한 변 비 하게 다. 지역 단 가
변 에 향 미 경우, 각 다 규모 지역 단 가진 가
간 지역 변 비 하는 것 당한가가 가 다(Jackson et
al., 2010).
지역 변 연 에 어 지역 단 택 핵심
다. 지역 단 하는 법에 변 가 달 지

(Volinn et al., 1994), 지역 단 규모 (OECD,
2014) 지역 단 는 변 에 향 미 다(Jackson, 2010). 지
역 지역 단 질 그 체 가 변
(extra-variation) 래하여(Thygesen et al., 2015), 변 원
지역 료 공 에게 돌리는 범하게 할 다
(Bernal-Delgado et al., 2015).
한 지역 단 는 지역 변 연 결과 해 과 책 용에
어 그 공한다. 1990 후 미 Dartmouth
Atlas Project 시작 미 럽지역 등에 가 차원
료 용 변 연 가 루어 다. 들 연 는 극 는
변 에 한 책 끌어 내는 것 목 한다(OECD,
2014). 러한 지역 변 연 실 용 고 해
보 , 지역 단 는 그 범 규모 에 실 료 용
함과 동시에, 료 공 과 직에 향 갖는 책 집행 단
할 연 결과 실 용 가능 높 다
(Bernal-Delgado et al., 2014; OECD, 2014).
상 고 하여 지역 단 가 갖추어야 할 건 내재
합 (intrinsic relevance)과 재 합 (extrinsic relevance)
에 생각해 볼 다. 내재 합 지역 단 가
규모 지리 범 에 탐색하고 하는 료 용양상
함과 동시에 지역 간 규모 차 한 가 변
생 함 미한다. 변 연 에 어 상 지역
단 는 해당 지역에 , 에 없 거주지
료만 할하는 경우 다(Volinn et al., 1994). 지역민 지
역 내 료 용 도가 높 지역 단 가 지역민 료 용
잘 하고 다고 할 다. 내재 합 과 하여 지
역 단 가 실 병원 용 할 도 병원 용 심

지역 단 하는 지역 법 (Wennberg et al.,
1973; Klauss et al., 2005) 용 었다. 한, 지역 단
규모 균등한 포에 가 변 하여 행
역 단 본 하여 가 균질한 새 운 지역단 만
드는 법 시도 다(Thygesen et al., 2015).
재 합 지역 변 연 결과 실에 해 과 용에
한 것 , 료 해당사 과(Ibid), 연 결과 산출
과 용에 어 책 에(OECD, 2014) 용 다. 재
합 에 행 역 주민 생 하는 단
책 집행 는 단 다 지역 변 연 에 지역
단 용 어 다.
우리나 에 료 용 지역 변 연 는 역시도(안 식
등, 1991; 등, 1993; 연 등, 2006; 등, 2010; 신
, 2011; Lee et al., 2012; Lee et al., 2013), 시 ( 용 등,
1994; Hong et al., 2014), 시 ( 등, 2013; 미 등,
2014) 등 주 행 역 그 단 하고 다. 행 역
연 결과 산출과 해 , 결과 책 용 용 는 에
단 재 합 충 시킨다. 그러나 거주 지역 에
료 용 잦 우리나 실 감안하 변 단
내재 합 떨어질 다.
2009 우리나 시 지역 지역 도 평균 37.9%
나타났다( 경 등, 2011). 규모가 각각 32만 상, 80만
상 지역 단 하 도, 지역 도는 각각 51.1%,
67.1% 나타났다(한 보건산업진 원, 2012). 미 과 스 스 등에 는
병원 송 역 평균 지역 도가 70% 상 나타난 다
(Wennberg, 1996; Klauss et al., 2005). 한 미 욕주 캘리포니
아에 거주하는 어 용 상 한 연 에 거주하

는 주에 원 료 는 비 95% 상 나타났다(Yip,
1993). 료 용 거주지 내 용 비 상 낮 우리
나 에 행 역 료 용 변 한다 , 지
역 변 연 에 극 탐색하고 하는 변 요
해당 지역 료 공 향 규 하 는 어 워진다.
행 역 격차도 가 다. 내 지역 변 연 는
주 행 역 그 지역 단 하 지만 행 역 격차가
변 에 미 는 향에 한 추가 탐색 루어지지 않았다.
단 행 역 갖는 약 보 하고 내에 도
료 용 편 한 진료 단 용하 는 시
도가 었다. 진료 편 행 역 하
합과 리 하는 식 루어 고( 용 등, 1994; 우
등, 1994; 미 등, 2004; 신 웅 등, 2007; 경 등, 2011), 병원
심 송 역 악한 연 가 었다( 등, 2013). 그
러나 러한 시도가 행 역 지역 변 단 하는 것과 비
하여 에 어 갖는 효과에 해 는 연 가 없다.
연 에 는 행 역과 지역 내 료 용과 료 용
고 하여 편 한 진료 지역 단 하여, 8개 주요 료
시 시 에 한 지역 변 하고, 지역 단 가 료
용 지역 변 에 미 는 향 하고 한다. 한
지역 단 별 8개 주요 료 시 에 한 지역 도등 료
용 지 평균과 변 비 하여, 각각 지역 단 가
지역민 거주지에 료 용 하는 도 하고
한다.

2. 연 목
연 는 료 용 지역 변 에 어 지역 단
가 변 에 미 는 향 악하고 한다. 해 행
역과 진료 지역 변 지역 단 하 다. 한 지역
단 별 포 지역 도 등 료 용 지 비 하여
지역 단 변 단 합 검증하고 한다. 연
체 목 다 과 같다.
첫째, 행 역과 진료 포 양상 비 한다.
째, 행 역과 진료 에 상동맥 우 식 , 경피 경
상동맥 장 , 고 골 후 , 슬 ,
, 개 , 산 단 , 공 상 등 8개 주요
시 에 한 시 지역 변 하여 비 한다.
째, 행 역과 진료 에 8개 주요 시 에 한 지역 도등
료 용지 포 하여 비 한다.

II. 연 법
1. 연 상 료
연 연 상 내에 2013 1월 1 12월 31 지
상동맥 우 식 (Coronary Artery Bypass Graft Surgery, CABG),
경피 경 상동맥 장 (Percutaneous transluminal coronary
angioplasty, PTCA), 고 골 후 , 슬 , ,
개 , 산 단 (Computed tomography, CT) 는 공
상(Magnetic resonance image, MRI) 시 다. 민건
강보험공단 지원 연 , 별, 주 ( 상 격 DB), 시 , 진
단 ( 상 진료 DB), 료 주 (요양 DB) 악하 다. 건강보험청
드 한 질병사 통해 시 과 진단 악하 다.

11개 시 들 다(OECD, 2014). 들 시
상 한 장 첫째, 비용과 용량과 책
에 주요 료 시 포 하고 다는 , 째, 상 시 들
민감 료, 공 민감 료 같 료 용 지역 변
에 한 개 틀(Wennberg et al., 2002) 내 범주들 포 하
에 시 별 변 양상 차 찰하 용 하다는 , 째, 시
마다 상 료 역 범 가 달 다양한 지역 단
용할 다는 , 째, 산출 시 변 OECD 보고
에 참여한 다 가들 결과 비 하여 우리나 시 변
객 견지에 악할 다는 다.
시 산출시 지역별 시 건 는 주 지 하
다. 연 상동맥 우 식 , 경피 경 상동맥
장 경우 20 상, 그 시 15 상
하 다. 시 산출시 생 출생아 모 하는 개
경우 산모 연 연 포함하 다(Appendix, Table 6.1). 통
계청 2013 주민등 료 용, 행 역별, 5 단
연 별, 별 주민등 악하 다. 개 경
우 2013 통계청 동향 사 출생 료 용하여 지역별 산모
연 에 출생아 악하 다. 지역 단 별 , 연
차 에 는 차 보 하 해 2013 주민등
직 시행하 고 직 용하여 지역
변 산출하 다.

2. 지역 단
연 에 는 2차 료 용과 3차 료 용
지역 하여 료 용 변 에 해 지역 단 가 미 는
향 악하고 하 다. 2차 료 담당하는 역 2차
단 하여 시 과 진료 (Secondary catchment area) 비
하 3차 료 담당하는 역 3차 단 하여 역
시도 진료 (Tertiary catchment area) 비 하 다.
가. 2차 단 (시 , 진료 )
2차 단 사용하고 하는 행 역 단 는 163개 시 과 57개
진료 다. 우리나 행 역 체계는 특별시, 역시, 특별
시, 도, 특별 도 시, , 가지 체계 는 (지
법, 2014), 시 2차 단 하 다.
행 역 단 가운 는 지역 단 에 포함하지 않았다.
특별시나 역시는 통 달 비스 도달범 가 공간
어 주민 근 에 큰 차 가 없고( 경 등, 2011), 거주
지가 집해 에( 지 등, 2008) 료 용 역 시
단 단 보는 것 하다고 단하 다. 한 시 단
는 연 에 하고 하는 57개 진료 편 본단 지
역 단 크 에 별도 단 간주하는 것보다 진료
과 비 에 합하 에, 진료 료 용 변
에 해 갖는 효과 하 에 하다는 단 하에 시
행 역 단 용하 다.
2차 단 시 과 비 하고 하는 진료 우리나 원
료생 해 2012 10월에 2013 9월 지 원

용 용하여 만들어 다( 등, 2015). 진료
163개 시 본 단 하고, 지역 도 40%, 경 15
만, 병합 거리 60 하여 57개 단 편
었다(Appendix, Figure 6.1).
나. 3차 단 ( 역시도, 진료 )
3차 단 사용하고 하는 행 역 단 는 16개 역시도
다. 역시도는 우리나 가지 행 역 체계 가운 (지
법, 2014) 상 체계에 해당하 , 2차 3차 료 가운 3차 료
에 하는 행 역 간주할 다. 2013 우리나
역시도는 17개 지만, 특별 시 역시에
병합하여 16개 역시도 3차 지역단 하 다.
특별 시 경우 가 약 12만 다 역시도
평균 약 4%에 해당한다. 가 다 역시도
포에 벗어난 도가 커 , 포 에 가
변 생시킬 가능 다. 한 지역 도에 어 도 다
역시도에 비해 낮 값 보 는 (한 보건사업진 원,
2013), 는 지역 단 내 합 충 시키지 못할 뿐만 아
니 료 용 평가하는 지 에 도 지역 변 곡할
다. 연 에 는 특별 시 역시 포함시 다.
3차 단 역시도 비 하고 하는 진료 2014 1월
에 6월 지 상 합병원 원 용하여 만들어 다(
등, 2015). 지역 도 50%, 경 50만 , 병합 거리
180 하여 12개 단 편 었다(Appendix,
Figure 6.2).

3. 법
지역 단 가 료 용 지역 변 에 미 는 향 악하
해, 지역 단 별 시 변 통계 산출하 다.
다 과 같 루어 다. 첫째, 시 포에 해
새 운 지역 단 가 갖는 효과 악하 해 , 커 도 추
(Kernel density estimation) 시행하 다. 째, 시 변
도에 어 진료 갖는 효과 추 하 해 변 통계
산출하 다. 변 통계 는 Extremal quotient (EQ), Interdecile ratio
(IDR), Coefficient of variation (CV), Systematic component of
variation (SCV) 하 다. SCV는 지역 변 에 어 지역
간 차 (systematic variation) 지역 내 차 (random variation)
가지 요 다고 하고, 연 과 별에 지역 내
차 에 작 변 거한 계통 변 값 다(Appleby
et al., 2011). SCV는 1982 McPherson 연 에 그 개 과
법 시 후(McPherson et al., 1982), 동 한 는 변
식 용 어 다(Westerling, 1995; Murthy et al., 2003;
Rodríguez-Martínez, 2012; OECD, 2014). 연 에 는 McPherson 연 에
시한 식 용하 다.

마지막 지역 단 별 료 용 하는 도 비 하
해 시 별 지역 도(Localization index) 지
(Inflow index), 출비(Net patient flow) 산출하 다.
지역 도(Localization index)는 지역민 료 용 가운
거주지역 내에 어나는 료 용 비 다(Wennberg et al.,
1996; Klauss et al., 2005; Kilaru et al., 2015). 내에 는
Relevance index Localization index과 같 미 , 지역
도, 는 체충 는 용어 사용해 나(한 보건연
원, 1982; 경, 2011) Relevance index는 본래 지역 료에
특 병원 차지하는 비 미한다(Griffith, 1972; Thomas
et al., 1981; Delamater et al., 2013). 연 에 는 지역민
특 료 에 한 도가 아닌 지역( 지역 내 체 료
)에 한 도 하고 하므 Localization index 는
용어 사용하 다.
앞 하 듯 , 우리나 경우 거주지 내 료 용 비
높지 않다. 거주지 내 료 용 비 높지 않 도, 주 지
한 료 용 지역 변 악 지역별 집단
료 용 악한다는 에 는 미가 다. 그러나
변 ‘ 당 지 않는(unwarranted) 변 ’ 규 하고
요 료 공 량, 공 행태 한 에 찾는 변
연 에 , 지역 거주 거주지 내 료 용 비 낮다 , 지역
산출 변 원 각 지역별 료 공 에 ‘ ’
돌리는 에는 어 움 다. 실 결과 해 에 어
미가 는 변 하는 어 지역 단 지역 도가
낮 것 직하지 않 지역 단 에 해 도 러한
에 한 검증 필요하다.

지 (Inflow index)는 지역에 어난 료 용 가운
타 지역 거주 료 용 비 그 지역 다 지역
하는 도 나타낸다(Klauss et al., 2005). Net
patient flow는 지역에 료 해 는 과 에 료
는 지역민 비 , 1보다 큰 값 우 함 보여
주 , 1보다 작 값 출 우 함 보여 다(Ibid; Kilaru et
al., 2015).

연 에 는 행 역과 진료 에 주요 시 지역
변 비 하는 한편, 시 별 지역 도 지
출비 하고 평균과 변 하여, 지역 단 가 변
에 미 는 효과 하고 한다. 료 해 SAS
9.4, R-statistics 용하 다.

III. 연 결과
1. 지역 단 별 포
지역 단 별 포 비 해 보았다. 2차 지역 단 경우,
시 평균 는 313,751 고 진료 평균 는
897,219 다. 시 단 에 는 가 10만 미만 지
역 단 가 86개 체 지역 단 약 52.7% 차지하나 57
개 진료 에 는 약 15만 가 10만
미만 지역단 는 없다. CV 경우 시 2.87, 진료
에 1.67, EQ 경우 시 964, 진료 73 진료
에 모 감 한 값 보 다. 3차 지역 단 경우, 역시도
평균 는 약 300만 , 진료 평균 는 약 400만
다. CV 경우 역시도는 0.98, 진료 0.89 감 하 고 극
단값비 경우 21에 20 감 하 다.

2. 시술 지역 변
진료 시 포에 한 향 하 해 진료 과
행 역에 해 각각 시 별 Kernel 곡 비 하 다.(Fig.3.1-2)
2차 지역 단 경우, 상동맥 우 식 과 고 골 후
과 슬 , 개 에 진료 과 163개 시 간
드러진 차 보 는 진료 에 시 산 도가 낮게
나타났다. 3차 지역 단 경우, 진료 과 시도 경우
비슷한 양상 보 개 과 공 상 경우 시도에
산 도가 어드는 양상 보 다.

지역 도 경우 2차 지역 단 에 는 진료 에 시 과
비 하여 8개 시 에 체 약 2 도 증가하 다. 3차
지역 단 경우, 진료 역시도 비 하여 사한 값
나타냈다.

도 증가하 고 슬 경우 4 지 증가한 값
나타냈다. 진료 경우 역시도 비 하여 비슷하거나 감 하

IV. 고찰
연 에 는 행 역과, 지역 내 료 용과 료 용
고 하여 편 한 진료 비 하여, 지역 단 가 료 용
지역 변 에 미 는 향 악하고 하 다. 한 각각
지역단 에 포 양상 비 하고, 지역 도등 료
용 지 용하여 지역 단 가 지역 거주 료 용
하는 도 하여, 지역 단 단 합
악하고 하 다.
상동맥 우 식 , 경피 경 상동맥 장 , 고
골 후 , 슬 , , 개 , 산 단
, 공 상 등 8개 시 에 한 시 포 지역
단 별 비 하 다. Kernel 곡 경우, 진료 에 는 시 과
비 하여 8개 시 모 에 시 산 도가 감 한 양상
보 진료 경우는 역시도 비 하여 비슷
한 양상 보 다.
진료 에 는 EQ, IDR, CV가 시 과 비 하여 시
에 감 하는 양상 보 다. SCV 경우에는 진료 시 과
비 하여 고 골 후 , 슬 , 개 , 산
단 에 는 감 한 값 나타냈지만 상동맥 우 식 ,
경피 경 상동맥 장 , , 공 상에 는
증가한 값 보 다. 진료 역시도 비 하여 IDR
감 하 나, EQ, CV, SCV 경우 시 에 는 감
하나 시 에 는 증가하는 결과 보 다.
지역 단 별 포에 는, 2차 지역 단 경우, 163개
시 과 비 하여 57개 진료 에 CV가 2.87에 1,67 감 하
고 EQ는 964에 73 감 하 다. 3차 지역단 경우, 16

개 역시도 비 하여 12개 진료 에 CV는 0.98에 0.89
감 하 고 EQ는 21에 20 감 하 다.
료 용 지 비 에 는, 지역 도 경우 진료 에
8개 시 체에 시 과 비 하여 약 2 도 증가하 고 진료
경우 역시도 큰 차 보 지 않았다. 지 는
진료 에 시 과 비 하여 평균 1.7 도 증가하 고, 진료
역시도 비 하여 비슷하거나 감 한 값 나타냈다.
출비 경우, 우 한 지역 비 진료 경우
시 과 비 하여 11%p 도 증가하 나 진료 경우 시
에 만 증가한 값 나타냈다.
시 CV는 상동맥 우 식 과 슬 에 높
값 보 다. 러한 경향 연 결과 사하다
(Brownlee et al.,2011; Dieppe et al., 1999; OECD, 2014). 상
동맥 우 식 경우 료 에 어 사 병
원 우하는 가 크고(Tu et al., 2012), 한 요
한 결 요 다(Brownlee et al., 2011). 슬 경우,
증 하게 립 어 지 않 (Dieppe et al., 1999)
들 에 한 견해가 다양한 것 알 다(Hawker,
2006).
시 CV 지역 단 에 변 살펴보 , 진료 에
는 시 과 비 하여 시 에 변 통계가 감 하 다.
것 시 단 에 균등한 포에 가 변
(extra-variation) 감 에 것 도 겠지만, 체 지역
단 감 에 결과 다. 진료 과 시 경우
변동 계 감 는 상동맥 우 식 과 고 골 후
에 드러지게 나타났다. 것 들 시 다 시 과 비
하여 시 건 가 고, 상 료 에 주 시 루어지

것 보 러한 경향 특 상동맥 우 식
에 하 다. 본 연 에 시 주 지 산
출 었 에도 하고, 새 운 지역 단 용했 상
시 에 변 가 드러지게 감 했다는 사실 들 시 시
료 거주지 내 가용 여 에 큰 향
드러낸다 할 다.
연 동 한 시 상 지역 변 악한 OECD
가 간 지역 변 비 연 (OECD, 2014)에 시 해 가들
과 지역 변 비 하 해, 보고 동 한 법 OECD

용하여 시 과 CV 산출하 다(Appendix, Table
6.2). 시 별 CV는 지역 단 가운 가장 높 값 갖는 보 시
단 했 , 상동맥 우 식 , 슬
과 에 평균 보다 다 높게 나타났다. 진료
용할 경우 과 비슷한 다. 후 하겠지만 포
변 가 2차 지역단 경우 에 비 하여 높게 나타
났고(Appendix, Table 6.3), 것 료 용 지역 변 에 미쳤
향 감안하 우리나 지역 변 가 과 비 하여 높
아닌 것 볼 다.
SCV는 진료 과 행 역 비 하 , 시 에 증가
감 가 재 결과 나타냈다. 그러나 SCV 용해 지역
단 새 운 지역단 비 시도했 행 연 경우
(Thygesen et al., 2015)에도 새 운 지역단 에 EQ IQR같
지 는 감 하 에도 하고 SCV는 큰 차 보 지
않았는 , 것 새 운 진료 하는 것 , 진료
간 포 변 한 지 지역 단 내
체 변 는 아니 고 할 다.

SCV 도 단하는 SCV가 3 변 가 진
료 식 나 학 재량에 한 것 가능 높 , 5.4
경우 변 가 높고, 10 경우 변 가 매우 높 것 알
다(Appleby et al., 2011). 에 슬
시 11.9, 진료 5.5 8개 연 상 시
가운 하게 5.4 는 값 나타냈고 3차 지역 단 에 도
다 시 에 비해 높 값 보 다. 그 에 상동맥 우
식 , 경피 경 상동맥 장 3 는 값 나타냈다. 슬

행 연 결과 사하다(Ibid).
다 나 비 하여 우리나 지역 단 규모 질
악하고 우리나 지역 단 포 OECD 보고
(OECD, 2014) 상 가들과 비 해 보았다(Appendix, Table
6.3). 13개 가 단 별 평균 는 13만 에 160만 지
포 보 다. 우리나 지역 단 평균 는 31만 에
426만 지 포 보 고, 진료 약 90만 해 지
역 단 평균 123만 에 비 근 한 값 나타냈다. 지
역 단 간 차 도 나타내는 변동 계 는 0.26에
1.44에 는 포 보 고 CV 평균 약 0.9 나타났다. 우
리나 경우 시 과 진료 에 CV가 다 가들에
값보다 큰 2.87과 1.67 나타났다. 규모가 지역 변 에 미
는 향 악하 해 향후 들 가들 시 변
규모 변 계 해 볼 필요가 것 다.
지역 도는 8개 시 용하 , 시 과 비 하여 진료
에 모 뚜 하게 증가하는 , 는 원 료용 지역 도
고 하여 편 한 진료 여타 시 에 도 지역별 지역 도
높 는 용 보여 다. 진료 경우 역시도 큰

차 보 지 않았고 시 에 도 차 보 다. 것
역시도 단 에 는 규모 시 단 비 해 는 상
지역 도가 높아 진료 편 큰 효과 갖지 않는다고 해
할 다. 그러나 역시도, 진료 에 평균 지역
도가 과 비 하여 낮 것 감안한다 , 우리나 지역
도 는 지역 규모 크게 하 도 그 값 높 는
한계가 다는 것 드러낸다고도 할 겠다.
지 경우 진료 에 는 시 과 비 하여 고
골 후 과 산 단 하고는 증가하는 양상
보 고, 진료 역시도 사한 값 나타내었다.
지 한 지역 도 마찬가지 규모가 커질 증가하
는 경향 보 다. 출비 경우 지 사한
경향 보 다.
진료 시 과 비 하여 8개 시 에 해 높 지역 도
보 다. 것 진료 과 시 규모 차 에 는 것
가에 해 는 여지가 지만, 하 듯 지역 변
극 목 해당 지역 료에 당 지 않는 변
(unwarranted variation) 찾아내는 고, 러한 원 탐색
해 는 높 지역 도 갖는 것 요하 에 행
단 에 여러 시 에 걸쳐 상 낮 지역 도값 갖는
다는 한 것 그 체 미 갖는다고 볼 다.
연 는 첫째, 지역 단 가 지역 변 에 미 는 향에

용하여 지역 단 가 단 갖추어야 할 내 합
탐색하고 에 해 비 하고 하 다는 , 째, 내에 지역

시 경우, 민건강보험 료 탕 산출한 지역 변 가 실
료 용 지역 변 다 다는 , 째, 진료
시 에 비해 지역 변 통계 가 감 에 어 지역 단 단
별 변 가 미 향 어 도 지 내지 못하 다
는 등 들 겠다.
연 는 행 역과 진료 비 통해, 지역 단 가 료
용 지역 변 에 미 는 향 하는 한편, 지역 도
등 료 용 지 용하여 지역 단 가 지역 거주 료
용 하는 도 하고 하 다. 연 결과, 진료
경우 시 과 비 하여 8개 주요 시 지역 변 가
감 하 고, 지역 단 별 변 가 감 하 지역 도
등 료 용지 에 도 시 에 비해 높 값 나타냈다.
연 결과 통해 지역 단 법에 료 용
지역 변 가 달리 나타날 지역 단 에 료 용
지역 도 단 별 포 양상에 차 가 하 다.
포 지역 도 같 지역 특징 지역 변 에 미 는
향 도 악하 해 는 차후 연 가 필요할 것 다.

Effect of Regional Characteristics on Regional Variations
in Medical Services Utilization

Objectives: This study aimed to evaluate the effect of the
regional units of analysis on measuring regional variations
in medical services utilization. For this purpose, we
compared the regional variations in the rates of the eight
major procedures in the administrative units and the
catchment areas which were organized on the criteria of
transporting time, population size, and localization index.
And we attempted to examine the intrinsic relevance of the
regional units on the basis of population distributions and
medical utilization indices.
Methods: To compare regional variations in the regional
units of analysis, we selected the eight medical procedures
considering cost, frequency, and policy relevance. The
age-and-sex-standardized rates of the eight procedures
(coronary artery bypass graft surgery, percutaneous
transluminal coronary angioplasty, surgery after hip fracture,
knee replacement surgery, caesarean section, hysterectomy,

CT exam and MRI exam) were calculated from the Korean
National Health Insurance database for the 2013 period. We
compared regional variations in procedure rates between
the administrative units and the catchment areas by
measuring the coefficient of variation, the extremal quotient,
and the systematic component of variation. Population
distributions and utilization indices (localization index,
inflow index, and net patient flow) of regional units were
also compared.
Results: Compared with the administrative units (regions),
the secondary catchment areas showed reduction in the
coefficient of variation both in the procedure rates and the
population numbers. Regarding utilization indices, the
secondary catchment area generally showed higher
localization index and inflow index in the eight procedures.
In the case of tertiary catchment areas, the change was
not appreciable.
Conclusions: We confirmed that regional variations in
medical services utilization could vary according to the
regional units of analysis. The degree of localization of
medical services use and the variation in distribution of
populations differed by the regional units. To find how
regional characteristics like population distributions and
localization index affect regional variation, we need further
studies.
