PRE-REHEARSAL MEETINGS

1) Pre-briefing meeting with all Stake Holders was conducted in Collectorate Conference Room – Jamnagar on 11.10.2017 under the chairmanship of District Collector. In this meeting, it was decided to organize Industrial Chemical Disaster Mock Drill in Jamnagar District at GSFC (SU) site on 13-10-2017.

2) The scenario of Mock Drill was heavy leakage of Ammonia from ammonia road tanker vapour valve. Tabletop exercise and a brief presentation was made by GSFC officials in the meeting on emergency scenario and facility available with GSFC to tackle the emergency.

3) Briefing meeting with all executives & HoDs of GSFC Ltd. was arranged on 12-10-2017 in Conference Room under the chairmanship of Sr. VP(SU). In the said meeting, Mgr.(S&F) informed that Collectorate office has decided to organize mock drill for the emergency scenario in which heavy leakage of Ammonia from road tanker of GSFC(SU).

4) Prior to organizing the Mock drill, a pre-briefing meeting of Observers

& Govt. Official was held in Learning centre on 13-10-2017. The locations of the observers were decided as under:

5) The observers adjusted their mobile timing for getting correct time of all the operations.

VARIOUS ACTIONS TAKEN BY DIFFERENT EMERGENCY TEAMS

Mock drill was simulated with white cloud of ammonia by operating dry chemical powder fire extinguishers in the presence of observers at about 03.36 PM on Ammonia road tanker No. GJ -12-AW 1057 parked near Weigh Bridge.

Following three persons were assigned the role of victims:

1. Mr. Sohansinh - Tanker Driver 2. Mr. Rakesh Sitapara - Tanker Cleaner 3. Mr. HB Mavadia - Sr. Operator(U/S)

The activities of Off-site mock drill are detailed below in chronological order:

1. 3.36 pm Security Guard of approach gate saw ammonia vapor from road tanker and informed Ammonia control Room about ammonia leakage from road tanker parked near weigh bridge.

2. 3.37 pm On receiving the message following actions were taken by Shift Engineer of Ammonia Control Room :

i. Immediately informed Fire Station, Medical Centre & Security Dept. about the situation.

ii. Informed to SM(U/S) & Chief (Utility & DAP) about the situation. They rushed to the incident site.

3. 3.39 pm Fire fighting crew of GSFC reached at the site and rescued 02 nos. persons who were affected by Ammonia gas in which one was very serious. Thereafter water mist generation to combat ammonia gas was started.

4. 3.39 pm Rescue van & Ambulance of GSFC reached at Incident site.

5. 3.40 pm Two victims were rescued and immediately shifted in GSFC Trauma Ambulance and send to OHC.

6. 3.40 pm Fire fighting operation started by Fire crew and downwind area was virtually covered with the continuous sprinkling of water through different types of fire nozzles & monitors to prevent the spreading of ammonia in surrounding areas.

7. 3:40 pm DIC & IC reached at incident site, assess the situation, discussed the matter with Sr.VP (SU) and declared On-Site Emergency (Level-2).

8. 3.41 pm Message conveyed by IC to DAP A/B Train Shift Engineer to blow emergency siren.

9. 3.41 pm Emergency siren blown. 10 3.41 pm Message conveyed by DIC to Mechanical Team for

providing blind on leaked valve. 11. 3.42 pm

Message conveyed by DIC to Environment cell for environment monitoring in downwind direction.

12. 3:43 pm With continued firefighting efforts, it was difficult to handle ammonia leakage without external help. Hence message passed to Mutual aid scheme control room & District Disaster Control Room for informing to all MAs members for sending their fire crew.

13. 3:43 pm Two victims reported at OHC and attended immediately by doctor. One victim has severe effect of ammonia was sent to GG Hospital - Jamnagar for further treatment.

14. 3.44 pm IC & Site Incident Coordinator discussed about the gravity of incident and quantity withheld in tanker. After healthy discussion following threat zones were found in downwind area:

a) 1100 ppm concentration will remain up to 70 mtrs. area.

b) 160 ppm concentration will remain up to 140 mtrs. area.

c) 30 PPM concentration will remain up to 330 mtrs area.

15. 3.44 pm Safety personnel reached at site with BA sets and other PPEs and people nearby area were cautioned through portable Public Address System to go to safe Assembly Points.

16. 3.45 pm Ammonia leakage became uncontrollable. Looking to the situations it was converted into Off Site Emergency (Level-3) and accordingly communication was made with District authority for evacuation, rehabilitation, Medical assistance and traffic control in surrounding area.

17. 3.45 pm Sr. Fire Supervisor wore BA Set and covered the leaky valve by wrapping gunny bags on it to restrict traveling of ammonia vapor in downwind direction.

18. 3.46 pm Third victim i.e. Operator was injured during emergency handling and was immediately rescued by fire staff of SDCC and shifted to OHC through Ambulance of PHC - Govt. Para Medical staff.

19. 3.46 pm Communication for the incident was briefed to different Government agencies and at our HO by Main Incident Controller.

The Mechanical maintenance & Emergency Task Force team reached at site and started removing of broken valve and blind was fixed on vapour line to stop leakage by wearing Ammonia Splash suit, SCBA set, PVC suit etc.

Fire fighting crew of different Mutual aid scheme members started continues heavy water jet / fogging through different fire fighting branches and portable fire monitor to prevent the ingressing of ammonia in surrounding areas. Reporting time of MAS members & Govt. emergency vehicles at site. 3.57 - SDCC Fire Tender arrived 3.57 - Meghpar Police Station team with Jeep arrived 3.57 - Ambulance with Medical team from Vasai PHC arrived 4.03 - Fire Tender from Sikka Nagar Palika arrived 4.05 - Fire Tender and Security van from Reliance arrived 4.12 - Fire Tender from ESSAR arrived 4.13 - Ambulance with Medical team from Padana

23. 4:29 pm Ammonia leakage was under control and downwind area was inspected through Dragger tubes for safe clearance by Environment engineer.

24. 4:31 pm IC ensure that ammonia leakage is totally under control and informed MIC to declare all clear.

25. 4.33 pm MIC instructed IC to inform for blowing all clear siren. Subsequently All clear siren blown by DAP Plant shift engineer.

26. 4.33 pm MIC informed all Govt. officials and executives at HO that emergency situation is over and site is all clear.

27. 4.34 pm SDM Shri RK Patel & Chief controller of Mock drill arrived at site and gathered information in detail of drill.

28. 4.34 pm MIC also reached at incident site from ECR and collects the information.

29. 4:38 pm Drill declare concluded.

DEBRIEFING MEETING:- 1. After the mock drill, a de-briefing meeting of District Administration

Officials, observers, representatives of various MAS members, officials of GSFC was held under the chairmanship of Shri RK Patel, District SDM – Jamnagar (Rural).

2. Manager (S&F) welcomed Chairman, dignitaries and all the members

before starting the de-briefing session.

3. Dignitaries and all the observers deliberated their observations to the audience narrating good practices and some points for improvement.

4. Sr. VP (SU) in his address expressed a sincere thanks to District

Administration, DISH officials, Police for making this mock drill successful and appreciated all the MAS members for quick and effective response during the mock drill. He specially thanked District Disaster Management authority & DISH office for selecting GSFC for this mock drill. He assured the all present and observers that whatever scopes of improvement have been suggested by the observers would be taken care in future and ensure ZERO observations in future.

5. SDM – Jamnagar (Rual), Shri RK Patel complimented for the

successful mock drill and unified participation of MAS member industries. He also said that active preparedness, planning is very much necessary to tackle any emergency. He appealed all industries and Govt. officials for minimizing any kind of loss on account of any disaster by mutually helping each other.

6. At the end, Manager (S&F) tendered a vote of thanks to District

Administration, Govt. Officials and all MAS members industries for timely helping during the off site emergency mock drill. He also express thanks for all employees of GSFC (SU) for successful completion of off-site mock drill in very short notice.

NOTEWORTHY OBSERVATIONS MADE BY DIFFERENT OBSERVERS:-

1. Entire mock drill was carried out with full coordination with Govt. officials

and all MAS members in organized manner.

2. On receiving the message, Fire crew & Medical Team responded very quickly and rushed to the site within a time. Fire Crew Leader communicated message very fast and effectively to his seniors.

3. Employees were found well trained, energetic and co-operative.

4. On hearing the siren most of the non-technical and some of the technical people gathered at respective assembly points. About 20 persons near Bagging ground floor (Nr. Log. Office), 80 persons at factory gate and about 40 persons of truck and tanker driver cleaner at highway gate assembled. Plant, U/S control room and store were in upwind direction hence not affected hence they were at their duty point.

5. Security persons with red flag and placard indicating incident site were deployed at different locations. Looking to the threat zone up to 330 mtrs in downwind, Factory Gate was selected for safe entry for

Emergency & Rescue vehicles of Mutual Aid scheme members for combating operation of ammonia gas.

6. The security personnel restrict the entry of people and vehicles

7. Emergency was attended from up wind side and most of the resources were deployed at right points.

8. Causalities handling and First aid treatment was quite effective.

9. Nearby areas were evacuated in organized manner.

10. Response of maintenance team was good and they took immediate actions as per standard operating procedures.

11. Environment monitoring was carried out continuously and measured ammonia concentration in air in the downwind and cross wind area by using Dragger tubes as per the instructions.

12. GSFC volunteers of different emergency teams reached at their pre-designated places for help.

13. Coordination between MAS members who participated in mock drill was effective and all have responded in quickest possible time after getting the message

14. Communication & instruction was prompt & effective from ECR by Main incident Controller.

15. Material Deptt. informed Tanker Transport contractor about the incident and instructed to send his team at site for necessary corrective actions.

16. Drinking water and cold drinks were distributed at incident site by Administrative team.

POINTS FOR IMPROVEMENTS:-

1. Security team did not barricade the threat zone area of incident site. 2. Only one contract security guard posted at main gate of highway road

for control of traffic movement & evacuation of tankers/trucks drivers & cleaners from parking area of GSFC. It would have been better if some more guards would have been deployed at such crucial location in emergency.

3. Some fire crew members were found removing of their respiratory PPE while talking which another during attending mock drill.

4. Stretcher of GSFC fire tender found without strip buckle for lifting casualty. For rescue operation strip buckle is must for shifting the victim.

5. Communication like way2sms or group SMS or Whatsup should apply for mass messages to all MAS members & Govt. Officials from DEOC

CONCLUSION:-

The Mock drill proved to be a learning session for all Government agencies and Mutual Aid Scheme member industries. It provided a good platform for cross-checking the effectiveness of emergency mitigation system and difficulties as well as shortcomings brought out during the drill. SDM expressed his full satisfaction for the mock drill and congratulated GSFC and all concerned Govt Officials and MAS team members for successful rehearsal.

In a nutshell it remained quite satisfactory.
