Delta Air Lines employees will have the option to choose this program effective January 1, 2011. Why did Delta Air Lines choose to sponsor the DeCare Dental program as an option for its employees? Delta Air Lines was looking for innovative ways to enhance their dental benefits, with the primary objective of providing employees and their families’ access to quality dental care at an affordable cost. Open enrollment for Delta Air Lines employees will take place in October 2010. As a participating DeCare Dental network dentist, your office will be listed as a participating dentist for Delta Air Lines employees who select this option. As you know, the greatest dental value is delivered by dentists like you who consider each patient’s unique oral health needs when developing a treatment plan. CDT 2011DeCare Dental has placed theClaims Administration Guidelines(Guidelines) on our websitewww.decaredental.com thatreflects data code set requirementsset forth under the AdministrativeSimplification Provisions of theHealth Insurance Portability andAccountability Act of 1996 (HIPAA).It is the policy of DeCare Dental tocomply with these requirements.Please feel free to print theGuidelines and keep them in a convenient location for easy access.If you would like a hard copy of theGuidelines and are unable to printthem, please contact a NetworkRepresentative at 1-800-658-4187.

The American Dental Association(ADA) has finalized the code set,identified as CDT 2011. We recommend you obtain a copy of the CDT 11 code set from the ADA,as it will identify new codes as well

as code revisions. DeCare encouragesall dentists to refer to their copy ofthe ADA CDT 2011 manual for specific code information.

In all cases, specific group contractprovisions, limitations and exclusionstake precedence over the ClaimsAdministration Guidelines. Sincecertain contractual items (e.g. timelimits, frequency of procedures, agelimits, etc.) can vary among groups,they have not all been listed withtheir associated procedure codes.Therefore this document should notbe interpreted as comprehensiveand encompassing all possible limitations and exclusions. Dental offices should contactDeCare Dental’s Customer Serviceto determine the specific limitationsand exclusions for each group.

98 percent of claims within 10 business days, which is significantly better than the industry average. In fact, in 2009 we processed the averageclaim in 1.59 days.

2.Accurate, weekly payment. We have a greater than 99 percent claim payment accuracy rate. (Our actual performance in 2009 was 99.93 percent.) We send payment to your office directly, every week. No need for you to follow up on bills or worry about assignment of benefits.

We offer the highest level of customer service and have representatives available to take your calls Monday through Friday. If you happen to leave a message, we will return your call within 24 hours.

Tips and RemindersIn order for a claim to be paid to the correct dentist in an accurate and efficient manner, please remember the following important items.

Billing Address: The billing address must match the address thatwas submitted by the billing dentist on their DeCareDental Networks application.

Electronic Claims Submission: Submitting claims electronically is easy and efficient. When submitting claims electronically, use the submission code 07035 (also called apayor ID). If you need a copy of the Electronic Claims Submission Guide, please call a DeCareDental Networks Representative at 1-800-658-4187.

When filing claims, don’t forget to submit theoffice's Usual & Customary (UCR) charges.
