Welcome

I am delighted to welcome you on behalf of The Queen’s Nursing Institute to the Transition of Care Conference for nurses and all their colleagues working in community and primary care settings who support young people. The conference is the culmination of a two year programme on Transition of Care. We will be joined today by a range of wonderful speakers from a wide field of expertise, who are dedicated to improving the lives of young people transitioning to adult services.

Kath Evans will share NHS England’s perspective and talk about raising the bar and enhancing young adult care experiences. She will be joined by two young people who will share their own experiences. We will also hear from another young adult, Hannah Phillips, who as project advisory group member has been up and down the country with the project, giving presentations about her transition pathway.

Nigel Mills from Great Ormond Street Hospital will discuss the challenges that GOSH clinicians face meeting the NICE recommendations for young people starting transition. We will also hear from the QNI’s Transition Project Manager, Dr Candice Pellett and Sandra Lawton, the project’s Chair, on the learning resource and film.

In the afternoon session, we will hear a parent’s perspective from Feisal Jassat. Multi-disciplinary working will be discussed by Carrie James, a Health Transition Case Manager and her team. And last but by no means least, we will hear how to prepare young people for transition from Dr Su Madge.

We are very grateful to all speakers for sharing their time and wisdom with us. Please do also take the time to view the poster presentations during the breaks.

We are also very grateful to the Burdett Trust for Nursing for funding this programme of work. I hope that you will find the day to be thought-provoking, inspiring and enjoyable.

Christopher Berns, Health Transition Clinical Support WorkerChris is a Health Transition Clinical Support Worker with an interest in young people’s engagement. In 2012-14 he completed an apprenticeship in Children and Young Peoples Workforce whilst working within the Transition Team. In 2015 Chris successfully applied for a Clinical Support Worker role working and became an official member of the Health Transition Team’s workforce. As part of this role, he helps to facilitate a Youth Group for Young People aged 14 -25 with physical conditions known to the Health Transition caseload. Chris has recently completed the FRIENDs Teen and Adult Emotional Resilience training and looks forward to applying this with the young people he works with.

Kath Evans, Experience of Care Lead, Maternity, Infants, Children & Young People, NHS EnglandKath is a registered general and children’s nurse and her career has included clinical, educational, managerial and service improvement roles. She is Experience of Care Lead for Maternity, Infants, Children and Young People at NHS England where she is committed to ensuring the voices of children, young people, families/carers and maternity service users are heard in their care and in the design, delivery and commissioning of services. Jordan Jackson, Clinical Support Worker, Health Transition TeamJordan is a Clinical Support Worker currently working on a bank contract with the Health Transition Team. He has a wide range of experience and shares his personal perspective of disability to support others, when he was studying at a local college he took up the additional role as a Student Disability Officer. He enjoys using his knowledge to help and inspire young people with physical disabilities to achieve their ambitions and transition successfully into adulthood. As part of his job role he helps to facilitate a youth group for Young People with physical conditions known to the Health Transition caseload. Jordan has completed a Children and Young People’s Workforce qualification and recently trained in a specialist FRIENDs Teen and Adult Emotional Resilience course. Jordan plans to roll out a series of workshops in the new year incorporating his additional learning, to put this into practice.

Carrie James, Queen’s Nurse, Health Transition Case ManagerCarrie is a Queen’s Nurse with an interest in service development and improvement, focusing on community services for Children and Young People. Since 2009 Carrie has worked as a Case Manager for Children’s Specialist Services based in the community, focussing on supporting Young People with Physical Disability/ Long Term Conditions through their transition into adulthood. Cited as an example of good practice in the NHS Confederation report 2012 “Children and young people’s health and well-being in changing times”. The team work with a range of Transition Networks to help influence and improve public policy and government legislation. Carrie is also a member of the Queen’s Nursing Institute Transition Advisory Group. The Health Transition Team led by Carrie has been shortlisted for several national awards in 2011, 2016 and 2017. More recently, the team were finalists for three national awards in 2016, Nursing Times - two categories Child and Adolescent Health and Team of the Year, Health Service Journal (HSJ) for Specialist Service Redesign (Highly Commended). Earlier in 2017, Carrie a was finalist in the RCNi Awards Child Health Category.

Feisal Jassat, Parent and Lay Member, North Durham CCG Feisal has worked in both the NHS and Local Government for over 30 years. His NHS career began in operating theatres where he worked as a paramedic supporting both anaesthetists and surgeons in theatre procedures. He moved into public health and developed his public health career by working in local government pursuing healthy public policy. He moved to work for Durham County Council in 2006 where he led and managed the Overview and Scrutiny

Sandra Lawton OBE, QN, Nurse Consultant Dermatology, The Rotherham NHS Foundation TrustSandra is a Nurse Consultant in Dermatology at The Rotherham NHS Foundation Trust and Past Chair of BDNG (British Dermatological Nursing Group) 1998-2000. She is also a specialist advisor for the CQC. Sandra has worked in dermatology for 30 years previously at Nottingham University Hospitals NHS Trust and developed the role of dermatology liaison sister in 1990, the first post of its kind in the UK. Her areas of interest include paediatric dermatology, care of children and their families with atopic eczema, nurse led services and vulval skin conditions. She has contributed to the field of dermatology through her publications, presentations and research at national and international level. She was awarded Public Servant of the Year in 2003, the title of Queen’s Nurse in 2007, Stone Award presented by the British Dermatological Nursing Group in 2010, Alumni Laureate Award from The University of Nottingham in 2011,Fellow of Queen’s Nursing Institute in 2012 and an OBE for services to nursing in 2014. In 2015 she was also named one of The Most Inspirational People in Nottinghamshire.

Dr Susan Madge, Consultant Nurse and Associate Director of the Department of Adult Cystic Fibrosis at the Royal Brompton Hospital in LondonDr Susan Madge is a consultant nurse and Associate Director of the Department of Adult Cystic Fibrosis at the Royal Brompton Hospital in London. She received her training at University College London (UCL) and Great Ormond Street Hospital, London, where she began working in CF. Following an MSc in Psychology, Dr Madge completed her PhD in Behavioural Sciences at UCL. Dr Madge has now been working with children and adults with CF for nearly 30 years. Her areas of interest include adolescent and adult CF issues, fertility and pregnancy and end of life care; she is also involved in writing guidelines for CF care at both a national and European level. Dr Madge teaches and lectures to multidisciplinary audiences and runs a highly successful multidisciplinary course in managing the care of children and adults with CF.

Nigel Mills, Transition Improvement Manager, Great Ormond Street Hospital, LondonNigel Mills is the Transition Improvement Manager at Great Ormond Street Hospital (GOSH), leading a Trust-wide project to improve the transition experiences of young people and their families. Nigel qualified as a paediatric nurse in 2000 and has worked exclusively with young people since 2001. He worked initially on the dedicated adolescent ward at Chelsea and Westminster Hospital before moving to Teenage Cancer Trust Ward at University College Hospital. Nigel joined Great Ormond Street Hospital in 2007 as Clinical Nurse Specialist for Adolescent Medicine and helped to set up the GOSH Young People’s Forum; the Trust’s advisory, consultative and engagement group for young people aged 11 to 25. Nigel sits on the advisory board of the Association for Young People’s Health and is a member of the All Party Parliamentary Group for Young People’s Health.

Dr Crystal Oldman CBE, Chief Executive, The QNIDr Crystal Oldman joined the QNI in November 2012 as Chief Executive. Crystal worked in the NHS for 18 years, the majority of which was in the field of community nursing, working with some of the most deprived communities in West London. She has a passion for learning, teaching and research to support best practice and in 1994 joined Buckinghamshire New University as a senior lecturer, to develop a variety of community nursing programmes. Crystal enjoyed an academic career for a further 18 years, completing her Higher Education career as Dean of Enterprise and Business Management in the Faculty of Society & Health, where her role included the development of partnerships with external agencies to promote research and assist in workforce development. Crystal’s interest in leadership and management inspired her doctoral studies into the knowledge, skills and attributes of middle managers and this research was completed in 2014. She was awarded a CBE in 2017 for services to community nursing and her leadership of the QNI.

Hannah Phillips, QNI Project Advisory group memberHannah is a blogger, patient and national speaker. As a child she spent time in hospital and was cared for at the Evelina Children’s Hospital on happy children’s wards surrounded by peers who no doubt aided one another’s recovery. At the age of 16, without warning, Hannah’s hospital stays started to take place in adult wards, a place she describes as a ‘big white room’. Motivated by her own experience of social isolation on adult wards, Hannah is now doing all she can to make sure that no other young person has to go through what she did during this mid-way phase to adulthood. Hannah works collaboratively with medical professionals, such as NHS England, Board for Children with Complex Needs, Evelina Children’s Hospital and the Queen’s Nursing Institute to help make the transition journey an easy and enjoyable one for young adults.

Dr. Candice Pellett OBE, Queen’s Nurse, Transition Project Manager, The QNICandice is currently working as a Transition Project Manager at the QNI. She is an experienced District Nurse with the Specialist Practitioner Qualification in District Nursing and holds the Practice Educator and Independent Nurse Prescribing qualifications. Her previous career was in banking! Candice became a Queen’s Nurse when the title was re-introduced in 2007 and she has been very involved in contributing to national policy documents over the past ten years. She has also worked on secondment with the University of Nottingham as a Practitioner/ Lecturer on the pre-registration nursing programme. In 2014 Candice was awarded the OBE for “Services to Nursing and Healthcare”. Also that year she was awarded The Queen Elizabeth the Queen Mother Award for Outstanding Service by The QNI. Michelle Shore, Senior Paediatric Occupational Therapist, Walsall Healthcare NHS TrustMichelle is a Paediatric Occupational Therapist and member of the Royal College of OT Specialist Section for Children, Young People and Families. She qualified at the University of Derby in 2005 and has remained within the NHS since this time, currently holding a position in the Walsall Healthcare NHS trust. Michelle specialises in enabling inclusion for children and young adults with a physical disability into mainstream education. She works as part of a multidisciplinary team alongside Walsall’s Health Transition Team and has a keen interest in supporting young adults on their journey to adulthood.

Lucy Watts MBE, Patient Leader, Ambassador, young people and palliative care advocate, speaker and blogger. Lucy Watts MBE is an accomplished Patient Leader, Blogger and Speaker. Lucy is passionate about making a difference. Disabled with a complex, life-limiting condition, she uses her time, knowledge and experiences to benefit others. Holding multiple roles within charities and organisations, as well as sitting on committees and taking on various other projects, Lucy received an MBE in the 2016 New Years Honours, aged only 22, for services to young people with disabilities.
