    Pur"ose The obective is to help people become aware of key health practices, how important theyare in determining their health and longevity. /elp people identify risk lifestyle practices theymay have. 0rovide motivation through awareness and education for prevention of diseaseand maintenance and enhancement of optimum health.

    Settin#s This health assessment and awareness tool can be used in a variety of settings to promote ahealthy lifestyle.

    1.ne-on-one in a variety of health promotion settings for uick health counseling.

    2.n small groups, either in the home, church or other small group settings.

    3.n a large audience for a uick lifestyle assessment. 3coring and evaluation can be completed using overheads or a slide presentation.

    4.A self-help approach used through the mail or distributed by an organi4ation. Aphone follow-up for counseling would be recommended when possible.
