JOB SUMMARY

Hampshire Hospitals is looking for a general paediatric consultant with an interest, or willingness to develop an interest, in neonatal care. Within the role the direct clinical care activities will include neonatal unit, general paediatric ward, working in the children’s area of the emergency department, general outpatients and general paediatric on call. As with all new paediatric consultants there will be a requirement to do a small number of resident night shifts to support the middle grade rota. This post is based primarily at RHCH, Winchester with close links with colleagues in Basingstoke and potential requirements to cover duties in Basingstoke in due course. There will be an opportunity to use or develop other sub-speciality interests commensurate with the needs of the department. This is an exciting opportunity to join a large team of paediatric consultants serving one of the largest populations of children in the Wessex Region. The post has arisen due to the increasing demands on unscheduled attendances and to support the longer term plan to co-locate the well-established paediatric assessment areas within the emergency departments. Hampshire Hospitals NHS Foundation Trust is the major provider of hospital services to the population of North and mid Hampshire and surrounding areas. The hospitals in Basingstoke and Winchester provide a comprehensive range of acute services as well as being a tertiary referral centre for colorectal, liver, peritoneal carcinomatosis, haemophilia and pseudomyxoma surgery. Services are provided across Basingstoke, Winchester and Andover sites. The trust is involved with the local STP to develop effective ways of working across the region between primary and secondary care and the provision of acute services for the population. This may involve changes in the current method of delivery, for example change in sites for acute working and outpatient clinics.

The Trust covers a population of 600,000 incorporating Basingstoke & North Hampshire Hospitals (BNHH), Royal Hampshire County Hospital (RHCH) in Winchester, Andover War Memorial Hospital (AWMH) and community hospitals. We provide one hospital service across multiple localities, delivering services as close as possible to the patient. BNHH and RHCH are 23 miles apart, situated in the middle of the M3 corridor, with excellent transport links to London and the south coast.

There are excellent post graduate medical facilities on both hospital sites. The trust is closely linked with Southampton University Medical School and the paediatric department has a major role in delivering the paediatric component of the undergraduate medical curriculum.

The paediatric department is fully integrated with the community paediatric department. Both sites provide established and well recognised training for trainees from the Wessex deanery both for acute general and community paediatrics (ST1-ST8 paediatrics, GPVTS and Foundation Year.) Paediatrics at HHFT.

The two main hospitals, Royal Hampshire County Hospital and North Hampshire Hospital provide both acute paediatric and Level 2 neonatal services.

1. The children’s wards (Northbrook ward Winchester, 18 inpatient beds, G2 ward Basingstoke; 28 inpatient beds) look after children aged 0-16 years and provide inpatient care for a range of specialties including medical, surgical, ENT and orthopaedic admissions. There are appropriate facilities to provide high dependency care and arrangements to support intensive care prior to transfer to the regional PICU. The consultant paediatricians operate a “consultant of the week” pattern of service so that there is always at least one consultant on site between 08.30 and 20.30 hrs on weekdays and 09.00 – 15.00 at the weekends and bank holidays. There is CAMHS support including out-of-hours advice from a consultant child psychiatrist.

2. Paediatric Assessment Units (RHCH - Sophie’s Place, BNHH - Charlie’s Day Unit)

The assessment units are adjacent to the main paediatric wards. Local GPs have immediate access to an expert Paediatric opinion on any child requiring urgent assessment. Children referred for a paediatric opinion from the Emergency Department are also reviewed. Some medical day case services are managed within the units. Current plans associated with investment into both emergency departments would see the assessment units co located within the emergency departments to provide a single point of access for all children attending the main hospital sites, staffed by paediatricians and the emergency department with input from primary care and other hospital specialities.

3. Emergency Departments

Both main hospitals run full emergency departments supported by all main hospital specialities with co located acute adult medical assessment wards. There is a minor injuries unit in Andover. Over 25,000

children present to the units each year. Basingstoke is a designated Tier 2 trauma centre. There are 12 general emergency consultant physicians with currently one paediatric emergency medicine certified consultant. The ED consultants and teams will contribute to the paediatric areas within the emergency departments in association with child health doctors, nurses, play specialists and allied health professionals.

4. Outpatients: There is a dedicated paediatric outpatient area adjacent to each paediatric ward with

approximately 8-22000 attendances each year at each site. There are daily general paediatric clinics, special interest clinics plus visiting specialist clinics in cardiology, neurology, nephrology, endocrinology, genetics, cystic fibrosis, and surgery. The department actively participates in all the relevant regional specialty networks.

5. Neonatal Care: Level 2 neonatal intensive care is provided as part of the South Central Neonatal

Network. These serve consultant lead obstetric units with delivery of around 3,000 at each site / year, accepting babies for short term intensive care and gestations above 27 weeks. As is the case across the UK there are active discussions occurring about the provision of intensive care and changes to level 2 (local) neonatal units. The team in HHFT is concentrating on providing excellent family centred high care and special care for the babies in conjunction with a successful and recently developed transitional care ward. We are engaged in discussion with the neonatal network about future provision of intensive care services.

SECTION 4: ABOUT THE ROLE AND RESPONSIBILITIES OF THE POST HOLDER

Neonatal unit – ward rounds and contribution to leadership and development of the neonatal service, transitional care ward and neonatal follow up clinics of premature and unwell newborns.

General Paediatrics - the consultant will participate in the consultant of the week duties, covering the general paediatric ward, ambulatory ward (Sophie’s place) and acute admissions. They will contribute to the consultant on call rota for evenings (resident consultant until evening handover) and weekends (2.5 PA /day of resident duties and on call for emergencies).

Night shifts to support the middle grade rota, working with an SHO and a colleague consultant on call at home

Emergency Department support – the trust has recently developed clinical areas within each main ED to provide clear separation for the care of children. The child health department aims to support the current ED workforce by providing senior doctor support and paediatric nursing at the busiest times, ensuring children are seen by senior decision makers in a timely fashion. General ED consultants and their teams including emergency nurse practitioners will also contribute to the care of children within the children’s areas.

General Paediatric clinics will be included within the job plan and there will be an opportunity to develop or contribute to specialist clinics depending on the needs of the department.

The post holder will also be expected to:

JOB PLAN

Job plans for individual consultants will be closely integrated and complementary to those of other members of the consultant team.

The posts will be EWTD compliant with 10PAs.

Appointees will be expected to undertake 8 programmed activities per week on average of patient related activity (direct clinical care, DCC)

SECTION 5: TERMS AND CONDITIONS OF SERVICE

This post will be offered under the terms and conditions of service for Consultants [England 2003]. The Terms and Conditions referred to in the contract can be viewed in full via: http://www.nhsemployers.org/~/media/Employers/Documents/Pay%20and%20reward/Consultant_Contract_V9_Revised_Terms_and_Conditions_300813_bt.pdf Basic Pay and Pay Thresholds Basic pay is £76,001 and, subject to satisfactory job planning and review, will rise through annual threshold increases to £103,490. Part-time Consultants will be paid pro rata to the thresholds described above, based on the number of agreed weekly Programmed Activities. The starting salary for new consultant appointments will be at the minimum of the pay scale in accordance with the terms and conditions. Previous service at Consultant level or equivalent may be taken into account when determining the starting salary; please contact the Consultant Recruitment Advisor for further information. The trust has excellent facilities for Private Practice and we encourage Consultant to pursue their practice within the hospital setting. The appointee must follow Trust agreed procedures when seeking to commence private practice. Any private practice commitments must not prejudice the basic service requirements contained in the job plan. Appointees expressing an interest in a part time post in order to undertake private practice will normally be expected to hold a 6 programmed activity [PA] contract. Public Transport and Car Parking Unrestricted car parking is available on the BNHH and AWMH sites. Core hours parking restrictions exist on the

RHCH site only. You may wish to apply for an exemption from this restriction, if you meet one of the criteria set out in the Policy for Parking Management. Park and ride facilities are available for this site. Relocation Expenses Assistance may be given to newly appointed Consultants with part of the cost of their removal expenses; providing that the removal is necessary to comply with HHFT requirements concerning the place of residence. Please note that applicants are encouraged to take the opportunity prior to interview to discuss removal expenses should they be successful. Different Trusts offer different packages and prior discussion will ensure clarity. Residential Criteria Consultants will be required to reside within a distance to allow safe discharge of their on call duties.
