C leared openings created as part of construction activities authorized under this permit must be effectively and revegetaled. 6. All imported fill material must be fr ee of hazardous or toxic materials and must not contain debris, trash, or rubbish. 7. Upon completion ollhe authorized structures within the terms of th1s permit any existing slrudures to be removed from the lot and other construction debris must be disposed of in a proper manner, in compliance with applicable state and lederal solid waste laws and rules. a Soil disturtlance must not occur wflen the ground is frozen or saiUrated. T opsol must not be removed from the site except for that neressary for construction activities authorized in this permit. Topsoil must be stockpiled at least 100 fieet from any water body. 9. T ernporary and permanent sedimentation control measures must be implemented to effectively stabilize all areas of disiUrtled soil and to catch sediment from runoff water before it leaves the conslruction site so that sediment does not enter water bodies, drainage systems , wa ter crossings, wetlands or adjacent properties. Clearing and construction activities, except those necessary to establish sedimentation control de vices. shall not begin until all erosion and sedimentation control devices (including dit ches, culverts, sediment traps, settl ing basins, hay b ales, silt fences, et c.) have been installed and stabilized. Once in place, such d evices shall be maintained to ensure proper functioning. 1 0. Effective. te mporary stabilization of all di sturbed and stocKpiled soil must be completed at the end of each w ork day. All t emporary sedimentation and erosion control de vices shall be removed after construction activity has ceased and a cover of healthy vegetation has established itself or other ap propriate permanent control measures have been effectively implemented. P ennanent soi l stabilization shall be complet ed within one w eek of inactivity or completion of construction. 11 . All exterior lighting must be located and installed so as to oduminate only the target area to the extent possible. Exterior lighting must not produoe a strong, dauling light or reftection beyond lot lines onto neighboring properties, water bodies, 01 roadway so as to impair driver vision 01 to create nuisance conditions. 12 . The scenic character and healthful condition of the area covened under this permit must be mcmtained. The area must be kept free of Iiiier , trash. junk cars and other vehicles, and any other materials that may constiiUte a hazardous or nuisance condition. 13 . n the permittee(s) holds (hold) interest in this parcel of land via a sales contract or other binding agreement the perrllJttee(s ) shall submit a copy of the transferal deed to the Commission upon final execution of such deed. 14. Once construction is complete, the p ermi ttee(s) shall submit a self-certification form, notifying the Commission that al l Conditi ons of Approval of this permit have been met. The permitt ee(s) shall submit all information req uested by the Commission demonstrati ng compliance with the terms of Illis penni!. 15 The p ermitt ee(s) shall secure and com ply with al l applicable li censes. permits, authorizations, and requirements of all fed eral, state, and l ocal agenci es including but not limaed to: Alr and Water Pollution Cont rol Regulations; Subsurface Wast ewater Disposal System approval from the Local Plumbing In spector and/or Ma ine Department of Health and Human Services, Subsurface Wastewater Program. 16 . All conditions of previously issued Commission permits shall remain in effect excep t as specifically modified by this per mit. In addition, all conditions of any Subdivision Permit authorizing the creation of the permittee's parcel as they pertain to said parcel shaU remain in effect. This permit is approved only upon the above stated conditions and remains valid only if the permittee complies with all of these conditions. Any person aggrieved by this decision of the staff may, within 30 days, request that the Commission review the decision. 3. Construction activities authorized in this permit must be substantially started within 2 years of the effective date of this permit and substantially completed within 5 years of the effective date of this permit. If such construction activities are not started and completed within this time limitation. this permit shall lapse and no activities shall then occur unless and until a new permit has been granted by the Commission.

4. Structures authorized under this permit, as well as filled and graded areas and cleared openings created as part of construction activities authorized under this permit, must be located to meet the road, property line, water and weHand setback distances, exterior dimensions and building heights listed in the tables in Sections 4 and 5 and approved by this permit.

5. Cleared openings created as part of construction activities authorized under this permit must be effectively stab~ized and revegetaled.

6. All imported fill material must be free of hazardous or toxic materials and must not contain debris, trash, or rubbish.

7. Upon completion ollhe authorized structures within the terms of th1s permit any existing slrudures to be removed from the lot and other construction debris must be disposed of in a proper manner, in compliance with applicable state and lederal solid waste laws and rules.
