SCHOOL CLOSINGS OR LATE STARTSAnnouncements regarding school closings or late starts due to inclementweather or hazardous conditions will be delivered via a Mounds ViewMessaging phone message, accompanied by an e-mail and text message. Mostannouncements will come within 30 minutes of a determination to close schools.Information will also be posted at www.moundsviewschools.org and updated ona recording at 651-621-7669 (SNOW). Families may also access localtelevision and radio outlets, however no guarantee is made that the media willshare our information or deliver it in a timely manner. Families are encouragedto keep their contact information up-to-date with their children’s schools toensure receipt of voicemail and e-mail messages. Families should alwaysassume that schools are open unless they hear differently.

AUTOMATED PHONE MESSAGESOn occasion, Mounds View Public Schools will send voice mail messages tofamilies. These notifications may include news about school closings, helpfulreminders or announcements, and notices regarding student absences and lunchaccount balances. Within the exception of emergency calls, all messages aresent to the family’s primary phone number on record with the school.Emergency calls will often be sent to all of the family’s home, work and cellphone numbers provided to the school. Parents who wish to opt out can do soby sending an email message to [email protected] with thephone number(s) they wish to be blocked.

TRANSPORTATION (Policy and Regulations EG-0103)Mounds View Public Schools complies with all applicable Minnesota Lawrelative to the transportation of students. Riding the bus is a privilege for thosestudents who choose to do so either by residing more than two miles fromschool or by paying a fee for transportation. All K-12 students will be providedwith school bus safety training. It is expected that students will conductthemselves in a respectful manner and not compromise the safety of others onthe bus. Behavior problems on the bus will result in consequences ranging froma warning to loss of riding privileges.

STUDENT ACHIEVEMENT/CURRICULUM AND INSTRUCTION (Policyand Regulations EG-1100 to 1104 and EG-2101)Mounds View Public Schools will seek to fulfill the Mission Statement, andstrive to fully develop the learning potential for all learners and will beaccountable for the measurement, assessment and achievement of studentlearning. Instruction will be based on sound pedagogy, approved Learner

Outcomes and will be modified by evaluation of student achievement.Curriculum and instruction must: Actively engage students in learning; Deliveressential Learner Outcomes; Challenge students to meet high standards ofachievement; Assure that all students possess core knowledge. Specificacademic programs include: Graduation Requirements EG-1102, Credit forLearning Reg. EG 2101, etc.).

HEALTH AND SAFETY (Policy and Regulations EG-3102, 3103 and 3106)Mounds View Public Schools will provide and promote an environment that ishealthy and safe for staff, students and the public. The physical environmentand facilities will be established and maintained in accordance with widelyaccepted standards and practices for use of facilities, and all applicable State andFederal Laws. All individuals, students, staff, parents and the public who are indistrict facilities are expected to conduct themselves in a manner that is notdisruptive or hazardous to the health and safety of self and others, and must notbreak any laws. Conduct which violates the expectations above will beregulated by the District and could result in loss of privilege to access a facilityor school site. A comprehensive School Safety and Crisis Response Plan will bemaintained by the School District and individual school facilities.

School personnel are mandatory reporters under the Maltreatment of MinorsAct, Minnesota Statutes 626.556 et al. We are required to report to child welfareauthorities if we know, or have reason to believe a child is being physically orsexually abused or neglected.

Students who are on medications or are suffering from communicable disease orillness will be treated according to applicable State and Federal law.

STUDENT CONDUCT (Policy and Regulation EG-3101)Student conduct must not be disruptive, hazardous to the health and/or safety ofself or others, or break any laws.

SAFETY AND CRISIS RESPONSE (Policy and Regulation EG-3103)Mounds View Public Schools will develop and maintain a comprehensiveSchool Safety and Crisis Response Plan and system for school-sponsoredactivities as defined:

School-Sponsored Activity: School-sponsored activities include all activitiesscheduled and supported by the Mounds View Public Schools. Examples includeregular classes, interscholastic athletic and fine arts events, intramural programs,classes sponsored by Community Education.

Non- School-Sponsored Activities: Park and Recreation, community groups,non-residents, commercial enterprises, PTO and PTA groups whose programs

schedule and utilize Mounds View Public Schools’ facilities throughCommunity Education are not school-sponsored activities.

The Plan and system will identify and:

a. Define actions to be taken by staff and school administration tominimize potential risk of harm to persons utilizing individualbuildings for school-sponsored activities.b. Assure communication between school administration and local lawenforcement regarding preventive safety strategies and crisis responseplans.c. Assure easy identification of authorized presence of individuals inschools.d. Provide for periodic updating procedures relative to school safety.

STUDENT RIGHTS AND RESPONSIBILITIES (Policy and RegulationEG-3109)Mounds View Public Schools will maintain a respectful and safe learningenvironment that appropriately balances student rights with expectations forstudent behavior. Procedures will be consistent with State and Federal lawsdefining student behavior expectations as well as potential consequences.

HARASSMENT, VIOLENCE, AND HAZING (Policy and RegulationEG-3104)Mounds View Public Schools will maintain a learning and working environmentthat is free from harassment and violence on the basis of race, color, creed,religion, national origin, sex, age, marital status, familial status, status withregard to public assistance, sexual orientation, or disability, and that is free fromhazing. Procedures have been developed consistent with Minnesota StatutesSections 121A.03 and 121A.69 and Chapter 363A to unsure this environment.Contact the District Human Rights Officer, Assistant Superintendent John Wardat 651-621-6005 or [email protected].

MINNESOTA SAFE SCHOOLS INITIATIVE – BULLYING (Policy andRegulation EG-3107)Mounds View Public Schools will maintain a learning environment that is freefrom student to student bullying. Procedures will be developed pursuant toMinnesota Statutes121.031. It will be a violation of EG-3107 for any student toengage in bullying or cyberbullying of another student at school or outside ofschool if that bullying causes a substantial disruption in the normal activities ofthe school day. Individual schools are charged with educational initiativeswhich support positive and respectful student conduct that is consistent with thispolicy and regulation. PROTECTION AND PRIVACY OF STUDENT RECORDS (Policy andRegulation EG-3110)Mounds View Public Schools will abide by all applicable State and Federal lawregarding data privacy of student and employee records. Procedures will beconsistent with State and Federal law to ensure the privacy of these records. Anannual notice will be provided to all parents and guardians with regard to thispolicy and accompanying procedures.

Criminal Background ChecksThe Mounds View Public Schools performs criminal background checks on allemployees including coaches and advisors and volunteer coaches and advisors.Mounds View Public Schools may deny employment or volunteer opportunitiesif anything should appear on the background check or if an applicant fails todisclose the existence of a criminal record where one is present. ELECTRONIC TECHNOLOGY ACCEPTABLE USE AND SAFETY (Policyand Regulation EG 3115)The use of technology by students will be in compliance with Mounds ViewPublic Schools regulations and applicable State and Federal laws.

In making decisions regarding student and employee access to and use ofElectronic Technology, Mounds View Public Schools (District) considers itsown stated educational mission, goals, and objectives. The District strives toprepare all students for the future by providing a challenging education thatbuilds academic competence, develops responsible citizenship, encouragescreativity, promotes lifelong learning, advances critical thinking skills, instills acommitment to personal wellness, and fosters respect for self and others.Electronic information research skills are now fundamental to preparation ofcitizens and future employees. Access to Electronic Technology enablesstudents and employees to explore thousands of libraries, databases, bulletinboards, and other resources while exchanging messages with people around theworld. The District expects that faculty will blend thoughtful use of ElectronicTechnology throughout the curriculum and will provide guidance and instructionto students in their use.

Handbook Information

ACADEMIC EXCELLENCE AWARDSStudents in grades 6-8 will be recognized for academic excellence for achievingor exceeding a 3.6 grade point average for the school year. Sixth and seventhgrade students that qualify will be mailed an academic excellence award withtheir report card in June, and their names will be listed within our “A” honor rollannouncement on the Chippewa Middle School website. A special AcademicExcellence Awards Ceremony will be held for 8th grade students that maintain acumulative grade point average of 3.6 or higher throughout sixth and seventhgrade, and during the fall term of 8th grade.

ACADEMIC HONESTYStudents will do their own work and cite sources when using the work of othersin papers and reports, etc. Students who engage in academic dishonesty will bereferred to the office for consequence and will be required to demonstratemastery of the assessed content (Policy 5500).

ACTIVITY BUSESActivity buses transport students to the general area of their homes and do notprovide the same service as regular buses. Activity buses depart from ChippewaMonday through Thursday at approximately 3:30 pm. Riding the activitybus is a privilege, consequences will be assigned if behavior issues arise.Students must be in a supervised activity to stay after school and ride the activitybus.

A variety of after-school sports and enrichment activities are offered byChippewa and/or the District 621 Community Education Department. Anominal fee will be charged for participation.

AFTER SCHOOL ACADEMIC SUPPORTAfter School Academic Support is a program available several days a weekwhere students are provided a quiet place to focus on their academic work.

ATTENDANCE AND ABSENCESSuccess in school is dependent on being in school every day. Minnesota StateLaw requires that each child between ages seven and eighteen attend school fulltime. Unauthorized absences will be treated as truancies. If a student is absent10 or more school days in a given year, for any reason, a doctor’s note may berequired to excuse future absences. The following are considered the onlyacceptable reasons to miss school:

● Illness● Religious Holidays● Medical/Dental Appointments● Family Emergency

A parent is required to call the attendance office by 7:40 am each day a studentis absent. If a call is not received, the school office will call home. If phonecontact cannot be made, a written excuse from the parent/guardian will berequired when the student returns to school. If a student will be absent morethan one day, please let the attendance office know and only one call will benecessary. Upon parental request, homework will be generated after the secondfull day of illness but most missed work can be found on google classroom andin StudentVUE. .

REPORTING AN ABSENCECall 651-621-6401. A voicemail system is in place to record calls during offhours. Please leave a detailed message with your child’s name (please spelltheir last name), grade, reason for absence, and identify yourself.

ABSENCE WITH ADVANCE NOTICEPlease send a note at least one day in advance to make arrangements for yourstudent to obtain an Advance Report for Excused Absence Form from theattendance office if your student will be absent for an extended period of time.

BUSES (Policy & Regulation EG-0103)Riding the bus is a privilege. Students are expected to treat others respectfully,to keep the bus free from damage, and to ride in a way that does not endangerthe safety of themselves or others. Students are expected to treat the bus driverswith the same respect that they treat staff within the school building. Busproblems will result in consequences ranging from a warning to loss of ridingprivileges. Every student and their parent(s) or legal guardian will be requiredto sign a Bus Safety Pledge Contract prior to the start of school. If criminalconduct occurs, local law enforcement officials and the Department of PublicSafety may become involved. Parents and students should be aware that most ofthe school district’s buses contain video recorders. School buses equipped withvideo recorders contain a notice of the video recorder.

CELL PHONESChippewa Middle School holds high expectations for student behavior,academic integrity, and responsible use of existing and emerging technologies,such as: cellular phones, digital picture/video cameras, and other personalelectronic devices capable of capturing and/or transmitting data or images(including “smart” watches). Students who possess and/or use such devices atschool or school-sponsored events shall demonstrate the greatest respect for theeducational environment and the rights and privacy of all individuals within theschool community. In order to ensure that cell phones are not a disruption to thelearning environment, students will not be allowed to use cell phones in class.Phones must be turned off when students are in class. Students are to keep theircell phones in their locker during PE, as cell phones are not allowed in thelocker room.

CHARGER CAFÉBreakfast is served in the Charger Café before the start of the school day. Astudent may choose from a hot or a cold entrée each day. Students qualifyingfor Free Lunch are eligible for a free breakfast.

Lunch is also served in the Charger Café and is available to all students.Students have their choice of three different school lunch lines. Main Linefeatures our most popular entrée choice from our student body. Grill Linefeatures a choice of pizza or another hot entree; both Main Line and Grill Linecome with a variety of fresh fruits and vegetables and choice of milk. SaladsLine features two options, a hot entree of the day or build your own salad -either one can be rounded off with fresh fruits and vegetables with choice ofmilk.

Students qualifying for free lunch may obtain a school meal at no cost. Forthose students bringing their own lunch, beverages may be purchased separately.Various snack options are also available for purchase in Ala Carte. All food andbeverages purchased must be consumed in the cafeteria.

A computerized system is used to purchase food in the Charger Café. Everystudent will be issued an account accessed by a PIN number. Students maydeposit money into their account by check or cash at school or by credit cardthrough the PayPAMS website. Be sure the student’s name and PIN number isnoted on every payment. A drop box for check and cash payments can be foundin Chippewa’s front entrance and in the Charger Café Ala Carte area.

Mounds View Nutrition Services offers Free Lunch for qualifying families.Contact Mounds View Nutrition Services by calling 651-621-6022.Expected behavior in the Charger Café is as follows:1. Be respectful of others.2. Be responsible for cleaning up after yourself and assist with picking up

items when requested to do so by a supervisor in the cafeteria. Do notthrow food.

Use of the District Computer SystemThe District Computer System consists of all school district computers,software, and communications equipment. This includes internet access, email,voicemail, servers, and projectors. Use of the District Computer System is aprivilege. There is no expectation of privacy. The District’s system may only beused for appropriate educational purposes.Students are prohibited from:

DISMISSALStudents are expected to leave school as soon as their last class is over. Studentsremaining in the building must be in an activity supervised by an adult. Studentsare not allowed to wander through the school unsupervised after regular classhours. Students are not allowed to leave school grounds after school and thenreturn to school to receive a ride home or ride the activity bus. If an activityfinishes early, students must remain in the front lobby.

DRESS CODERationale for student dress code and personal appearance – our main objective isto create a safe and respectful environment where all students have theopportunity to succeed.Student clothing must not hinder the identification of a student.

HEALTH CAREThe licensed school nurse and health paraprofessional are available during theschool day to discuss matters of personal health. They can provide medicalassistance when needed. Approval from a teacher is required to go to the health

office. Students may not leave the school building for health reasons withoutreporting to the health office. Accidents which occur during the school daymust be reported to the health office immediately. Parents must sign ill orinjured students out of school in the office.

HOMEWORKHomework is a regular part of school studies and is expected to be completed.Students should plan time for homework each night. Success for all learners isemphasized at Chippewa. Teachers are willing to provide extra assistance tostudents when needed.

HONOR ROLLAfter each semester, an honor roll list will be posted for students with gradepoint averages of 3.6 or higher (“A” Honor Roll) and students with grade pointaverages of 3.0 to 3.59 (“B” Honor Roll). Honor roll is based on the average ofseven classes. There are no weighted grades.

INSTRUCTIONAL MEDIA CENTER (IMC)The IMC contains library materials including books, reference materials,computers, and magazines. These materials are provided for class assignmentsand recreational reading. No materials will be allowed out of the IMC withoutbeing checked out. Materials may be checked out for one or two weeks and maybe renewed. If students are in need of a reference book, please seek assistancefrom an IMC staff member. Current magazines may not be checked out;however past issues may.

Lockers are considered school property. School administration may checklockers at any time deemed necessary. It is expected that students will maintainlockers in excellent condition. At the end of each school year, school personnel

will inspect all lockers. Students will be assessed fines for damage including:scratches, markings, decals, stickers, and extra clean out if needed.

LOST OR STOLEN ITEMSThe school will make every effort to help find student property that is lost orstolen; however, the school is not responsible for such items. We ask studentsnot to keep money or valuable property in lockers. If students need a safe placeto keep an item, they can bring it to the front office. Lost or stolen items shouldbe reported to the office immediately. There is a lost and found table thatstudents can check to see if their items are present.

MEDICATIONS (Regulation EG-3106)Prescription and over-the-counter medications will be kept in the Health Officeand administered under the supervision of the licensed school nurse. Theparent/guardian is responsible to send a written note or obtain a written orderfrom a physician or licensed health care provider. Medication authorizationforms are available in the Health Office. According to school policies, allmedications are required to be submitted to the Health Office in the originalcontainer, labeled with the student name, medication, strength, date prescribed,reason given, and directions for use. This includes any over-the-countermedications.

Short term prescription medication (10 days or less) may be given asprescribed by a physician or licensed provider with written parent/guardianpermission.

Long term prescription medication (over 10 days) will require a written orderfrom a physician/or licensed health care provider. Authorization must beprovided each year.

All controlled medications, as defined by the Federal Narcotics Act, must bebrought to the Health Office prior to school and will be administered in theHealth Office. Violation of the school’s policy in regard to medications will beconsidered a violation of the Students Right and Responsibilities Policy (5114)and may result in disciplinary action if a student is found with possession..PLEDGE OF ALLEGIANCEAnyone who does not wish to participate in reciting the pledge of allegiance, forany personal reason, may elect not to do so. Students must respect anotherperson’s right to make that choice.

PERMISSION TO LEAVE BUILDINGTo leave the building during the school day the following procedure must befollowed:

● Bring a note from your parent/guardian to the attendance office beforeschool. Attendance will provide a pass for approved dismissal time.

PHYSICAL EDUCATIONPhysical Education is a part of the student’s curriculum. Students are expectedto attend and participate on a daily basis. A written request from a parent will berequired in order to have a student excused from class. Parent requests will behonored for a period of one to three days. For requests of more than threedays, a physician’s statement is required. Students who do not participate inPhysical Education will be have the following actions:

● First time: Parent/guardian contact and loss of participation points● Second time: Dean is contact to determine a plan of support, contact

parent/guardian, and loss of participation points● Third time: Detention/make-up class is assigned and loss of

participation pointsEvery student must change for PE class. Students will also be required tobring a swimsuit and towel during swimming units. Chippewa is notresponsible for items that go missing from the gym locker rooms.

PROTECTIVE DRILLSFire, severe storm, and lockdown drills will be conducted throughout the year.Emergency information is posted in each classroom. During drills, students areexpected to remain quiet and follow directions from staff. Severe Storm andLockdown Drills are announced over the intercom and by an alarm. Fire drillsare sounded by an alarm.

REPORT CARDSReport card grades are issued at the end of the semester. They are available onParentVUE. If you do not have internet access, please contact Student Services(651-621-6406) for a paper copy.

SCHOOL DANCES/ACTIVITY EVENTSSeveral times throughout the school year student groups or ChAC host an afterschool activity from 2:25-4:15 pm. A fee is charged for admittance. Studentsattending the activity must be enrolled at Chippewa and currently taking classesin the building. Guests are not allowed. Appropriate behavior and dress areexpected. If a student chooses to leave school during an activity, they may notreturn. Concessions are available. Students involved in inappropriate behaviormay not be allowed to attend future dances.

StudentVUE and ParentVUEAt Chippewa Middle School, we emphasize student engagement in the learningprocess and believe that school success is dependent on positive communicationbetween the teacher, the student, and the parent.

Students will have access to viewing their grades, assignments, and attendanceinformation through their StudentVUE portal. Similarly, our parent portal,ParentVUE, will serve as an easy to access online database designed to provideparents with information related to their child’s current academic performance,attendance, and standardized test history.

As Chippewa students entering sixth grade, they are each given their ownpassword and student ID number for StudentVUE. We encourage you to look atParentVUE with your child once or twice a week. We want to empower thestudents to advocate for themselves whenever possible.

STUDENT PICTURESStudent pictures will be taken during the Welcome Back Day in August. Pictureretakes will be available at the end of September or early October.

STUDENT STORAGEStudent storage is available next to Room 252. Items such as school projects,sports equipment, or other items which do not fit into lockers should be stored inthis location. The Front Office is not to be used as a storage area.

STUDENT SUPPORT SERVICES (Deans)The deans meet with students several times a year, and with students aboutacademic or personal issues as needed. Appointments are advised. Deans willalso work with students after a behavioral concern at school or on the bus occursto create a positive plan.

TARDIESStudents are expected to show up on time and be prepared for class. Any time astudent is late for a class and does not have a pass, they will be considered tardy.If a student is late for first period class, they need to report to the front office.All other records of tardies will be kept by the classroom teachers. Three timestardy to the same class in a quarter will result in an hour of detention.

TEACHER WEB PAGESChippewa teacher web pages are a good resource for information. Look at theteacher's web page for assignments, worksheets, calendars, supply information,links to resources, and much more. If your student is absent, this is a good wayto find work they have missed. In addition, web pages include the teacher’sphone number and email address, and links to other sites (such as Googleclassroom) if more information is needed.

TELEPHONE USAGEA phone is available for student use before and after school in the front lobby. Ifan emergency arises during the day, students may use the phone in the frontoffice. If a parent calls with an emergency during the day, a message will betaken and given to the student.

TEXTBOOKSStudents will be issued textbooks in some classes. The textbook remains theproperty of the school, but becomes the responsibility of the student. Thecondition of the textbook will be noted when issued to students. Students areexpected to maintain their textbooks in good condition or they may have to paya fine.

TRANSPORTATIONCan my child ride home with a friend or use another bus stop?No. For safety reasons, students may only ride their assigned bus and must geton and off at their assigned stop only.

Does my child need to keep the bus pass all year?Yes. Please have your child keep the pass in his/her backpack. This is helpfulfor substitute drivers and for activity bus drivers at the secondary level. Driverswill also conduct pass checks to verify rider eligibility.

Are sports equipment and musical instruments allowed on the bus?The following is a list of items that students are allowed and not allowed tobring to and from school. This is meant to be a guide from a safety and spaceperspective.

All visitors, as they enter the Chippewa foyer, will need to be let into thebuilding by Chippewa staff during school hours. You will need to show a stateissued ID and a visitor badge will be printed for you. If you are droppingitems off for your student, you may leave items with the front desk and they willbe sure your student gets them. Parents are welcome to visit. We do ask that anappointment be made with the teacher(s) or other personnel you wish to see.

Student Visitors: Requests for student visitors are discouraged and will begranted on very rare occasions and for a limited amount of time, e.g. one classperiod. Requests for visitors can be submitted by parents to theadministration one week in advance. Concerns about student’s safety, classroomspace, and disruptions will prohibit the approval of student visitor requests.

ANNUAL PUBLIC NOTICEDATA PRIVACY

Independent School District No. 621 gives notice to parents of students andstudents currently in attendance in the School District of their rights regardingpupil records.

The Family Educational Rights and Privacy Act (FERPA), 20 U.S.C. 1232g, andthe Minnesota Governmental Data Practices Act, Minn. Stat. 13.01, et seq.,provide parents and students who are over 18 years of age (eligible students)with certain rights with respect to the student’s education records. These rightsinclude:

1. Parents and eligible students have the right to inspect and review thestudent’s educational records. The School District will attempt tohonor a request for access to a student’s education records immediately,if possible, or within five days. If the School District needs additionaltime to respond to a request for access to education records, the SchoolDistrict may request an additional five days to comply with the request.

a. If a parent or eligible student wants to make a request foraccess to the student’s education records, the parent or eligiblestudent must submit a written request to inspect and revieweducation records to the District Data Practices ComplianceOfficial. The request should identify as precisely as possiblethe record or records the requesting party would like to inspectand review.

2. Parents and eligible students have the right to request the amendment ofthe student’s education records that the parent or eligible studentbelieves are incomplete, inaccurate or misleading.

a. If the parent or eligible student wants to amend an educationrecord(s) that they believe are incomplete, inaccurate ormisleading, the parent or eligible student must request anamendment in writing to the District Data PracticesCompliance Official. The request must identify the records(s)that the parent or eligible student seeks to amend and identifythe items contained therein that are believed to be incomplete,inaccurate or misleading and specify the correction that isbeing requested.

b. If the School District does not amend the record(s) asrequested by the parent or eligible student, the School Districtwill notify the parent or eligible student of the decision andadvise them of their right to a hearing regarding the request foramendment. Additional information regarding the hearingprocedures will be provided to the parent or eligible studentwhen notified of the right to a hearing.

3. Parents and eligible students have the right to consent to disclosures ofpersonally identifiable information contained in the student’s educationrecords prior to such disclosure, except to the extent that FERPA, theMinnesota Data Practices Act, or other state or federal law expresslyprovides for disclosure without prior consent.

a. One exception which permits disclosure without prior consentis disclosure to school officials who have legitimateeducational interests. A school official is a person employedby the School District as an administrator, supervisor,instructor, or support staff member (including health ormedical staff and law enforcement unit personnel); a personserving on the School Board; a person or company with whomthe School District has contracted to perform a special task(such as an attorney, auditor, medical consultant, or therapist);or a parent or student serving on an official committee, such asa disciplinary or grievance committee, or assisting anotherschool official in performing his or her tasks.

b. Generally, a school official has a legitimate educationalinterest if the individual needs to review an education recordor have the educational information in order to fulfill his orher professional responsibilities.

c. Upon request, the School District discloses a student’seducation records, without prior consent, to officials ofanother school district in which a student seeks or intends toenroll. The School District forwards education records on

request to a school in which a student seeks or intends toenroll, the School District does not provide further notice toparents or eligible students prior to such a transfer.

4. Parents and eligible students have the right to file a complaint with theU.S. Department of Education concerning alleged failures by theSchool District to comply with the requirements of FERPA. The nameand address of the federal Office that administers FERPA and receivescomplaints is:

"Directory information" includes the following informationrelating to a student: the student's name; address; telephonenumber; date of birth; sex; major field of study; dates enrolledin Mounds View Public Schools; height and weight ofmembers of athletic teams; extra-curricular activities, specialachievements and honors. "Directory information" does notinclude a student’s social security number, studentidentification number, a student ID badge number, personallyidentifying data which references religion, race, color, socialposition, nationality. Director information also does notinclude data collected from nonpublic school students, otherthan those who receive shared time educational services,unless written permission is given by the student’s parent orguardian.

b. Should the parent of a student or the student so desire, any orall of the directory information listed above will not bedisclosed without the parent's or student's prior written consentexcept to school officials as provided under federal law.

c. In order to make any or all of the directory information listedabove "private" (i.e. subject to consent prior to disclosure), theparent or student must make a written request to the buildingprincipal within thirty (30) days after the date of the lastpublication of this notice. This written request must includethe following information.

ANNUAL PUBLIC NOTICESTUDENT SURVEYS

Periodically, our School District will administer student surveys on a variety ofsubjects. In addition, our School District participates in the Minnesota StudentSurvey conducted by the Minnesota Department of Education. The MinnesotaStudent Survey asks information of our 6th, 9th and 12th grade students. Federallaw requires us to notify parents of this activity.

The Minnesota Student Survey is anonymous, confidential and voluntary.Students do not provide their names, school identification numbers or otherpersonal information. No individuals or their families can be identified. Theresults are reported on the School District’s students as a whole. The privacy ofstudents is protected.

There are frank questions on the Minnesota Student Survey about many topicsrelating to young people’s lives. These include use of alcohol or other drugs,safety in and around the schools, physical activity and nutrition, mental oremotional health (stress, feeling bad), illegal or anti-social behavior (theft,chemical use, fighting), and belief systems that affect personal decisions.Questions regarding sexual behavior and attitudes are asked only of 9th and 12th

grade students. If students are not involved in an activity, they skip the relatedquestions and go on to the next section. The Minnesota Student Survey alsoasks about how well school is going, future plans, how adolescents spend theirtime and about how the school, family and community support them.

As part of Protection of Pupil Rights Amendment (PPRA) (20 U.S.C. § 1232h;34 CFR Part 98), you are allowed to review the survey and have the opportunityto elect not to allow your child to participate. If you want to review the surveyor if you choose to have your son or daughter not take the Minnesota StudentSurvey or any other surveys administered during the current school year you can
