1. Black ink should be used in preparing pupils records / promotional forms.

2. Subject nomenclature (arrangement) must be the same in all forms / records as much as possible (Form 1, Grading Sheets, Form 137, Form 138, and Form 18 – E/A).

3. Total Number of school days must not be less than 193.

3.1 Grades should be computed in accordance with the new rating system by subject area.

3.2 Avoid erasures of grades especially among honor pupils.

3.3 Computerized class record must be supported by actual data.

4.1 Don't forget to write the name of school and division.4.2 Complete all the data needed.4.3 The age as of June in fraction ex. 8 ½4.4 Avoid erasures in grades.4.5 Accomplish the no. of days per month , it should tally with the other pupils forms based on form 1.4.6 Be careful, comments must be stated positively. Example: Iwasan ang pagliban (subalit isa lang ang nakatala sa "card" na liban ng mag-aaral)

4.7 LRN in upper right hand corner must tally in form 1384.8 LRN must be based on reports created by LIS deped.gov.ph website.4.9 For Grades I and 7 - First to fourth grading Period is descriptive - Final rating is descriptive and numerical.

Note: Do not encircle the general average but still with initial.

V. Form 18 - E/AV. Form 18 - E/A

5.1 It should be computerized using the black ink.

5.2 Check alphabetical arrangement, male first then female leaving two spaces between males and females.

5.3 Include names of pupils who transferred in/out and those who dropped in the list arranged alphabetically .

5.4 Use red ink for drop out , retained/failed , students rating, remarks .

5.5 Subject Area listing must be the same with other Forms (138, 137).

Note : Total age of pupils shall be filled up and average must appear at the bottom. Include the average of each subject area at the bottom with subject teachers initial (for elementary only) The teacher-adviser shall put his-her initial below the last number for boys and girls

5.6 For Grade I and 7 Grades in Music, Arts, PE and Health is reflected individually.

5.7 Home address complete.

5.8 Years in school - actual number of years attended from Grade I/ First Year to present curriculum year.

5.9 For PEPT passers (count the total number of years in school).

5.10 AGE is in decimal add 0.75 or 3/4 to the age of June. This will be their age in March, please refer to Form 1. Example: June 10 1/2 plus 0.75 - March 11.25

5.11 Write the sub-total for ages of males and females and the total age below .

5.12 All pages must have heading (front) and back pages which include signatories

5.13 All credentials of pupils/students be submitted in school on or before February 28 to be included in 18 E./A.
