Executive Summary The Healthy Challenge is a team-based wellness program that encourages healthy behaviors through group support and gentle competition. A six-week Healthy Challenge sponsored by the Center for Sustainable Health at the Biodesign Institute began on October 7 and ended on November 17. To generate interest, a video was prepared and sent to all Biodesign Institute employees. The video distribution and other email messages did their job; the Healthy Challenge was very popular. Initially, 60 teams of 384 individuals signed up to participate and 58 teams completed the Challenge! To publicize the program, a Healthy Challenge Kick-off Event & Wellness Fair was held at the Biodesign Institute on September 26, 2013. It included information tables with displays and hands-on activities from many campus entities. All participants also received a Healthy Challenge t-shirt. A video crew was on hand to capture the event. The event also offered free health mini-screenings for blood pressure, cholesterol panel, BMI, body fat measurement, osteoporosis bone density, Visiderm skin sun damage, and blood glucose. Approximately 80 people participated in these pre-program screenings. A post-screening survey of participants

revealed that they appreciated the convenience of workplace screenings and the immediate feedback they received. Within the first week of the Healthy Challenge, participants were asked to complete an online survey about the program. Results showed that 66% of respondents were seeking motivation to adopt healthier behaviors. Over two-thirds set a personal goal and the top three, in order were general healthy behavior, weight concerns and exercise. Approximately 64% had never participated in a workplace wellness program before. To maintain motivation and engagement, a “Monday Motivator” email was sent to all participants. It contained

information about team standings, any bonus point activities for the week, a healthy recipe, and links to one or two health-related articles. At the conclusion of the Healthy Challenge, a modest awards ceremony was held at the Biodesign Institute. Once again, free health mini-screenings were available and 50 people took advantage of the opportunity. After the program was over, participants were again asked to complete an online survey. The results indicate strong support for the Healthy Challenge, with almost 94% reporting that they thought it was a valuable experience. When asked if they planned to continue any of the Healthy Challenge behaviors

beyond the end of the program, about 84% said that they do. Finally, 80% reported feeling more positively towards their employer for having offered the Healthy Challenge. Excluding personnel hours, the total direct cost for the Healthy Challenge amounted to $15,153.00. Costs were incurred for the purchase of the participant t-shirts, the development of the Healthy Challenge logo, the production of the Kick-off Event video, participant certificates, and small prizes for winning teams. Throughout the Healthy Challenge, participants shared anecdotes about their experiences. Accomplishments included reports of weight loss, more sleep, a better diet, and even cleaner teeth! There were a number of inspiring stories from current participants who were still maintaining good habits from the 2011 Healthy Challenge!

Lessons learned from this Healthy Challenge:

It is difficult to offer program activities that are location-specific when teams are located at different campuses. Activities such as health screenings, the wellness fair and t-shirt distribution were made difficult by the multiple locations of teams. In the future, team-building activities that could include all Healthy Challenge participants need to be considered.

If the program is going to scale to larger populations, point-tracking and score-keeping functions need to be web-based tools that can be used on different platforms (computers, smartphones, tablets).

The program works well now because it is broadly defined. It does not mandate one kind of exercise over another and it uses the carrot of points awarded for good behaviors rather than the stick of subtracting points for poor choices.

Keep communications short and direct. Many people don’t take the time to read – or thoroughly read – emailed instructions.

A six-week program is a little too short. The program could easily be 8-12 weeks for greater effectiveness.

Social media is worth considering for future Healthy Challenges so that photos, messages and challenges between teams, success stories, and encouragement could be shared among participants.

Background The Healthy Challenge is a team-based wellness program that encourages the adoption of healthy behaviors through group support and gentle competition. The goals can be captured by five words: education, motivation, team-building, empowerment, and FUN! The program aims to:

Educate individuals about aspects of personal health and wellness, Motivate through gentle competition, Build work-group and organizational teams, Empower individuals to take responsibility for their own health, and Achieve measurable results while having FUN!

The Healthy Challenge program has a ten-year-long history at the Fred Hutchinson Cancer Research Center in Seattle, Washington. In 2011, the program was brought to the Biodesign Institute by leadership at the Center for Sustainable Health. For that first Healthy Challenge, approximately 12 teams (70-75 individuals) participated in the 12-week Challenge and some of those original participants were eager to participate again.

How does it work? The Healthy Challenge program consists of teams with three to eight members, including a captain who reports the team’s weekly scores to the Healthy Challenge administrator. With the encouragement and support of your team members, participants earn points for:

Daily physical exercise, 1 point/15 minutes, up to one hour each day, maximum of 24 points a week. Each participant determines the activity or mix of activities.

Mental health 15-minute stress break, 1 point/day for any activity that allows a break from tasks and is refreshing.

Healthy breakfast, 1 point/day. A healthy breakfast provides good fuel for the day’s activities.

Food choices: 5+ servings of fruits and veggies, 1 point/day; 1+ serving of whole grains, 1 point/day; meatless dinner, 1 point/week.

Sleep: 7+ hours of sleep each night, 1 point per day.

Teeth flossing once a day, 1 point per day.

Each team member receives an Excel worksheet to track their weekly points and forwards their weekly score to the team captain. The team captain enters it onto a self-calculating Excel spreadsheet that generates a weekly team average score that is forwarded to the Healthy Challenge administrator. At the end of the six weeks, the teams with the highest scores receive modest prizes for 1st, 2nd, and 3rd place.

Healthy Challenge Publicity Video Prior to the beginning of the Healthy Challenge, a video was produced and sent to all Biodesign Institute employees. In it, participants of previous Healthy Challenges talked about their experiences with the program. For some, the results were obvious and transformational, such as a significant weight loss. Others talked about changes in daily activity that continued beyond the end of the Healthy Challenge. All were enthusiastic about participating in a new Healthy Challenge at the Biodesign Institute. The video is an inspiring piece that helped people make a decision about their own participation.

Healthy Challenge T-shirts All Healthy Challenge participants received a free t-shirt. The shirts were distributed at various times at the Biodesign Institute as well as at the Downtown ASU campus. The design was apparently compelling enough that one person formed a Healthy Challenge team just so she could acquire a t-shirt!

Kick-off Event – September 26, 2013 The Healthy Challenge Kick-off Event & Wellness Fair was held in the Biodesign Lower-level Atrium from 9:00 a.m. to 1:00 p.m. The event was open to all interested parties, whether or not they intended to participate in the Healthy Challenge, and attendees came from other ASU campus locations as well. The event featured tables from the following campus organizations:

ASU Health Services – free chair massages ASU Health and Safety – lab safety information and ergonomic office equipment demonstrations ASU Health Services Dietician – models of proper food portions and nutritional information ASU Campus Police – bicycle safety and introduction to the new Tempe campus walk zone ASU Farmers Market – information about market vendors and dates ASU Employee Assistance Program – information about the many programs available ASU Sun Devil Fitness Center – class information and make-your-own stress ball activity Demonstration of the Breezing Metabolism Device developed at Biodesign Sign-up table for Y Race Phoenix Sign-up table for Healthy Challenge teams and participant t-shirt distribution

The event also offered free health mini-screenings provided by Healthwaves, a company that provides this service to ASU employees. Tests were available for blood pressure, cholesterol panel, BMI, body fat measurement, osteoporosis bone density, Visiderm skin sun damage, and blood glucose. Approximately 80 people participated in these pre-Healthy Challenge screenings. Healthwaves always sends an online survey to participants as a follow-up to the screening. Of the 80 people who were screened, 41 people completed the survey. Just over 80% of respondents appreciated the immediate feedback they received. The top two reasons for participating in the screenings were to check their health status and because the process was so convenient. To capture the activities of the Kick-off event, a video crew filmed participants – vendors, Healthy Challenge team members and the merely curious. Additional publicity items were developed with the Biodesign Institute Communications team. They included poster signs and table tent cards as well as email blasts to interested non-Tempe campus groups. Throughout the entire Healthy Challenge, the Communications Team also developed and posted Healthy Challenge messages on the Biodesign monitor screens. Since the Biodesign Institute is a secure facility, support for the event was provided by Biodesign Security, who issued temporary badges to guests. All set-up and breakdown tasks were performed by the Biodesign facilities team and all audiovisual requirements were handled by Biodesign IT.

Healthy Challenge Teams The video distribution, other email messages and the Kick-off Event & Wellness Fair all did their job in publicizing the Healthy Challenge. As a result, interest in the program was high and the Healthy Challenge was very popular. Initially, 60 teams of 383 individuals signed up to participate in the Healthy

Challenge. Of those, 58 teams completed the program. Teams from ASU’s downtown campus came primarily

from two groups: the College of Nursing & Health

Innovation, and the School of Nutrition and Health

Promotion, although one downtown team came from the

campus library!

Of the Tempe campus teams that were not part of the

Biodesign Institute, the majority came from various

departments in the Office of Knowledge Enterprise

Development (OKED) located at Centerpoint. And three

teams came from Fervor Creative, the design firm that designed the Healthy Challenge logo and

supporting materials.

Teams are encouraged to be creative when naming their team and there were quite a number of very

inventive ones! By popular vote, “Wine Is Healthy, Right?” was chosen as the best team name!

First Survey of Participants

To learn more about the motivations and expectations of program participants, a short survey was developed and emailed to everyone who was on a Healthy Challenge team. It was sent mid-way through the first program week and 161 participants (approximately 41%) responded. The first question was “What aspects of the Healthy Challenge are you looking forward to the most?” Almost 67% were looking for the motivation to adopt healthier behaviors. The next three choices, in order, were friendly competition among colleagues, greater awareness about behavior, and working in a team with colleagues. Over half of the survey respondents reported having no concerns about their participation in the Healthy Challenge. However, about 25% said they had some concern about the time commitment they thought would be required by their participation. Other concerns included letting their team down or the admission that the anticipated behavior change was “a little nerve-racking.”

When asked about personal goals for the Healthy Challenge, 77% said that they had set a personal goal. Weight concerns came in as the number one goal, followed very closely by the desire to improve general healthy behavior and the desire to exercise. Two-thirds of the respondents reported that they had participated in other workplace wellness programs but for about 37%, it was a new activity.

Monday Motivator Email Blasts Once a week, Healthy Challenge participants would receive a short, informative email that would contain weekly team standings, and links to a vegetarian recipe and one or two health-related articles. To facilitate distribution and track reader clicks, Campaign Monitor software was used. The cost to send an email to 383 participants was less than $25 each week.

Bonus Point Activities The Healthy Challenge included three bonus point activities, which were intended to shake up the competition among teams and also to add interest and some additional fun to the program. The first two bonus point activities were photos and you can see the results throughout this report. For the first bonus point activity, all participants could submit one photo of themselves doing some physical activity, preferably one that they would use for their daily exercise points. Each person who submitted a photo earned five bonus points. The second bonus point opportunity was a team photo. Each team could earn extra bonus points by submitting a photo of all team members. If any team members were unavailable for the photo, they could be represented by a hand-held photo or drawing, a physical stand-in or a Photoshopped photo of them added to the group photo before submission for points. The third and final bonus point activity was quite exciting! The daily and weekly exercise point limit was dropped so those who could exercise more would get credit for all exercise performed. This had the potential of really boosting each team’s score.

Award Ceremony – November 21, 2013 To celebrate the many accomplishments of all the Healthy Challenge participants and to reward the top-scoring teams, an award ceremony was held in the Lower Level Atrium at the Biodesign Institute. Similar to the Kick-off Event & Wellness Fair, health mini-screenings were again available. People could receive tests for blood pressure, cholesterol panel, BMI, body fat measurement, osteoporosis bone density, Visiderm skin sun damage, and blood glucose. Fifty people participated. Every participant received a certificate and small trophies were given to the 1st, 2nd and 3rd place winning teams, as well as certificates for some additional awards.

Post-Healthy Challenge Survey Once the Healthy Challenge was completed and people could reflect on their experiences, another survey was emailed to participants. When asked how they would describe their Healthy Challenge experience, 81% felt it was fun and 63% felt it was challenging in a good way. Also, over 93% considered it a valuable experience. Motivation to exercise more and eat healthier were chosen as the most impactful activities. Fully 85% of respondents said they intend to continue one or more Healthy Challenge behaviors beyond the length of the Challenge itself. A little over half of the respondents reported that they met their personal goal for the Healthy Challenge and 98% said they would recommend a Healthy Challenge to colleagues thinking about participating in another one. Finally, 79% said that they thought more positively about their employer for having offered the Healthy Challenge.

Budget The costs for the Healthy Challenge include the branding items that were developed by Fervor Creative (logo, t-shirt design, stationery, scoresheet design and e-blast fees), the filming and production costs for the Kick-off Event video, and the purchase of the participant t-shirts. This does not include employee hours for development and administration of the program.

Testimonials At the Award Ceremony, people had an opportunity to share their experiences and results from the Healthy Challenge. One person shared that she lost 10 pounds while doing the Healthy Challenge and another shared how he had changed his sleep habits so that he is getting more rest. One laboratory employee said that the most positive result from her standpoint was the team building that occurred because of participation in the Healthy Challenge.

Lessons Learned Here are some suggestions, based on past experience with this (and other) Healthy Challenges:

Complications from multi-location teams

It is very difficult to offer program activities that are location-specific when teams are located at different campuses. Kick-off activities like the health screenings or the wellness fair did not receive as many non-Biodesign attendees because of the travel time. T-shirt distribution for the downtown teams entailed a special trip or the use of campus mail. Team-building events for all participants, like climbing up A Mountain, would not work for all Healthy Challenge participants. It will require a re-thinking of

these types of activities and probably some extra staffing if future Healthy Challenges include teams at many different locations.

Web-based tools

If the program is going to scale up to larger populations, point-tracking and score-keeping functions need to be web-based tools that can be used on different platforms (computers, smartphones, tablets). Right now everything requires too much effort for the administrator as well as the participants.

Resist tinkering

The program works well now because it is broadly defined. It does not mandate one kind of exercise over another and it

uses the carrot of points awarded for good behaviors rather than the stick of subtracting points for poor choices. Many people would like to add on items (points for wearing sunglasses or sunscreen; points removed for poor food choices; adding components for sustainable practices like recycling, etc., etc.). Any additions to the program should be viewed very critically. If too many items are added on, there is a concern that daily scorekeeping will become too complicated or bothersome for participants.

Simple messages

Length of the Healthy Challenge

A six-week program is too short. The competition might be more interesting if the Challenge lasted longer than six weeks. Also, true behavior change might be better reinforced with a longer Challenge. The program could easily be 8-12 weeks for greater effectiveness.

Social media

Social media is worth considering for future Healthy Challenges so that photos, messages and challenges between teams, success stories, and encouragement could be shared among participants.

In Conclusion The Healthy Challenge is an effective vehicle for building workplace teams and promoting better health behaviors among co-workers. The majority of participants enjoyed the Healthy Challenge and would recommend it to their friends and co-workers. As was mentioned earlier, 79% of post-Healthy Challenge survey respondents reported feeling positive about their employer because of their sponsorship of the Healthy Challenge. A few participants mentioned that they were forming unofficial teams with family members so that the entire family could benefit from the structure and support of the Healthy Challenge program. Changing the team concept to include home teams might be another step that would reinforce the adoption of the healthy behaviors that are integral to the Healthy Challenge. Finally, there was tremendous demand for the Healthy Challenge. People were happy to hear about the Challenge and eager to join. Many participants suggested that the program should be offered throughout ASU to all employees at all campuses and there is already anticipation about a future Healthy Challenge.
