    Terms and Abbreviations

    Annotation A particular detail of a digitized slide.

    Host User who uploaded the digital slide to CaseCenter and who initiates the Teleconsultation about the slide.

    CaseCenter user group Group created by a CaseCenter administrator.

    LDAP Lightweight Directory Access Protocol – an Internet protocol for accessing distributed directory services that act in accordance with X.500 data and service models.

    OLDAP OpenLDAP – open source implementation of the LDAP

    Participant User invited to participate in the evaluation of a slide stored on a CaseCenter server.

    Teleconsultation Provides access to digitized slides for several users and supports their collaboration in inspecting and evaluating the virtual slide.

    Teleconsultation group Group created by the Host for a Teleconsultation session independently of the user groups that exist on CaseCenter.

    Several Teleconsultation sessions (and therefore several Teleconsultation groups) can be activated on a CaseCenter server in parallel.

    Telepathology Teleconsultation in the field of pathology: pathologists exchange information and evaluate a slide together via Teleconsultation.

    1 Introduction The system supports the following two protocols for accessing the slides:

    1. Microsoft Network protocol (SMB/CIFS), if the connection satisfies the standards below:

    • If using IPV4: RFC 1918 standard, section P r ivate IPV4 ad d ress sp ac es , or the IPs are different only in their class D addresses.

    • If using IPV6: RFC 4193 standard, section Unique local address.

    2. HTTP/HTTPS

    Install the scanner – SU (as an external storage) – client within the same facility, and this order if possible, since the HTTP overhead can highly decrease the speed of scanning and communication.

    It is recommended for you to connect through HTTPS (HyperText Transfer Protocol over SSL) that is a TCP/IP protocol used by Web servers to transfer and display Web content securely. The data transferred is encrypted so that it cannot be read by anyone except the recipient.

    1.1 Structure of the CaseCenter

    CaseCenter is part of the 3DHISTECH product family that supports tasks in the routine and research workflow of pathologists. It is a platform for managing slides, cases, and other documentations. The figure below demonstrates how slides can be stored and managed on a designated CaseCenter server.

    Important!

    Operating system components in different time zones is not supported.

    1.2 HUB

    The HUB itself consists of two main parts: a database and the web surface. Database is for storing metadata of slides, the web surface is through which you can access and manage slide metadata. Accordingly, the VIMMEET.EXE application, which enables the usage of the Teleconsultation function, runs from the HUB as well.

    1.3 Service Unit

    A Service Unit (SU) is an element of CaseCenter where slides can be physically stored and slide processes take place. Storage can be added to the SUs on the network. Users who have registered on the server (registration is performed by the server administrator) can manage and use their slides on the server in the following ways:

    • Upload slides or scan directly to the server.

    • Arrange slides in folders, for example, group slides according to their staining types or your research projects.

    • Share slides with other users who:

    ◦ have account on the same CaseCenter (and can therefore access your slide on the server or if you share the slide with them explicitly).

    ◦ can access the slide via E-School.

    • Invite other users into Teleconsultation sessions to discuss slides.

    • Participate in Teleconsultation sessions initiated by another user.

    • Search for slides.

    1.4 Configuring a Distributed Network System

    The below configuration method of a distributed network system allows you to use Microsoft Server Message Block Protocol within a LAN. This way such a system can be built, where slides will be registered on the HUB, the HUB and the SU communicate with each other via a HTTP or HTTPS connection (for example, they are located at different sites).

    A LAN connection is ensured for the SU–Scanner and the local users; therefore, slides stored on the local SU can be accessed more quickly, and the speed of scanning is increased accordingly.

    There are two ways to configure a distributed network system:

    1. There is no Client #2 (7) or Slide Scanner #2 (8) (no client which should access CaseCenter outside of the LAN):

    NOTE: No special configuration is required. Either NETBIOS name, local IP address, or FQDN (Fully Qualified Domain Name) can be used during the installation procedure of CaseCenter. No Public DNS (9) is required.

    • The HUB (1), the SU (2) and the Slide Storage (3) can be on the same machine, or all other distribution is accepted (for example, SU (2) and Slide Storage (3) on the same machine, and the HUB (1) on an other).

    • To enable LAN slide access instead of HTTP slide access you have to specify all the “Slide store directories” in “Service Unit Settings” on the SU (2) with UNC name. (\\NETBIOS_NAME_OF_THE_STORAGE\ SHARE \further path ). In the UNC name there can be either a local IP address, NETBIOS, or FQDN.

    • You should use NETBIOS names everywhere during the installation and further configuration. This is also recommended on Slide Scanner #1 (5) and Client #1 (6).

    2. There is Client #2 (7) and/or Slide Scanner #2 (8) (there is one or more client which should access CaseCenter outside of the LAN):

    NOTE: FQDN must be used during the installation of CaseCenter.

    • The HUB (1), the SU (2) and the Slide Storage (3) can be on the same machine, or all other distribution is accepted (for example, SU (2) and Slide Storage (3) on the same machine, and the HUB (1) in on an other). Ensure that the Storage is accessible through SMB Protocol from the SU.

    • If the HUB (1) and the SU (2) are on the same machine you must have one public/physical IP address and a registered FQDN for it. If the HUB (1) and the SU (2) are two separate machines you must have two public/physical IP addresses and registered FQDN for them. Storage does not require FQDN and public IP.

    • You must link the one or the two public IP address to the Router (4).

    • You must configure the Router (4) to do “port forwarding” on TCP/HTTP (80) and TCP/HTTPS (443) ports from the public IP address(es) to the local/private IP address(es) of the HUB (1) and the SU (2).

    Important!

    Some Anti-Virus applications (embedded gateway Anti-Virus solutions, such as the Juniper-Kaspersky’s Deep Inspection Firewall) may filter and block packets during the communication between client and server. In such cases, turn off virus scanning of HTTP packets besides allowing incoming connections on TCP port 80.

    • If you already have a public domain name (for example, 3dhistech.com) with a public Domain Name Server (9), you could register HUB (1) and SU (2) as a sub-domain of your existing public domain name.

    • If you do not have a public domain name in a Public Domain Name Server (9), then you have to purchase one for the HUB and one for the SU.

    • If there is a Local DNS server (10), then set local IP addresses of HUB and SU for the FQDN. LAN clients must use this address as primary DNS.

    • If you do not have a Local DNS server (10), you can use the HUB (1) or the SU (2) (Windows Server 2008 R2) as a Local DNS server (10), and you can configure it as shown above. In this case you must ensure that all the clients on the LAN use this DNS as primary DNS. You can do this to simply configure your DHCP server to advertise that DNS as primary DNS. (If you do not have DHCP server on your LAN you can use W2k8 R2 also as a DHCP server).

    • To enable LAN slide access instead of HTTP slide access you have to specify all the “Slide store directories” in “Service Unit Settings” on the SU (2) with a UNC name. (\\NETBIOS_NAME_OF_THE_STORAGE\ SHARE \further path ). In the UNC name there can be either local IP address, NETBIOS, or FQDN.

    • Slide Scanner #1 (5) and the Client #1 (6) can access it via NETBIOS name or local IP, but the Client #2 (7) and the Slide Scanner #2 (8) cannot (regardless of using NETBIOS or FQDN).

    • You must use FQDN everywhere during the installation and further configuration (except in UNC path names of slide store). This is also recommended on the Slide Scanner #1 (5) and the Client #1 (6) (at the client side), and must do on remote clients, Client #2 (7) and the Slide Scanner #2 (8).

    1.5 Folder hierarchy

    The tree structure of the server can be seen on the left side of the panel, where the actual folder is marked with blue background.

    Hierarchically, the Case is a sub-level element within a folder, and the Block is a sub-level element of a Case. You can create several cases within a folder and several blocks in a case. Each folder, case, or block can directly contain slides.

    Important!

    The recommended maximum number of sub-level elements is 200 folders within a parent folder, 200 cases per folder, and 20 blocks per case (50000 elements total).

    For slides the recommended counts are: 50 slides per folder, 10 slides per block, and 100 slides per case (in different blocks).

    1.5.1 Folder Color Markings

    User access rights are presented in different colors of folders in the tree panel. A folder name appears in black characters when it contains slides to which you have read and write access, blue if the slides are read only, and gray if the slides in the folder are hidden because the slides in it are not accessible.

    Folder color marking and the tasks according to the different access rights are listed below:

    • Black – Read/Write access

    • Blue – Read only access

    • Gray – No access permission, but a sub-folder can be visible if you have Read only or Read/Write access rights for that specific sub-folder, case, or block.

    1.5.2 Folder option buttons

    This section describes the folder option buttons above the folder tree panel. Folder option buttons may be inactive depending on folder-related permissions.

    • – New folder

    1. In the folder tree, click the desired folder in which you would like to create the sub-folder, and if active, click this button.

    2. The Create new folder window opens. Type in the Folder name. Add some information in the Description field if necessary.

    3. Select users and/or groups you would like to give access to this folder, and set their permissions. Click the right arrow buttons to add them to the selection list. Click the left arrow button to remove items from the lists.

    By default, users are displayed with the given permissions for the parent folder, and will be marked with red if you modify their permissions.

    NOTE: Only moderator and administrator users are allowed to set user permissions.

    Each user type is marked with an avatar of a unique color:

    • Moderator user is displayed with green

    • Normal user is displayed with blue

    • Restricted user is displayed with red

    • – New case

    Creates a case under the actual folder. Note, that a case cannot be created under an other case or block. Similarly to the Create new folder window, description can be added to the case as well as users and groups and their access rights to the case.

    Click the Accept button to create the case.

    • – New block

    Creates a block under the actual case. Note, that you cannot create a block under an other block. Description can be added to the block as well as determining users and groups and their access rights to the block.

    Click Accept to finalize block creation.

    • – Edit folder

    By default, you can modify the folder description only. Moderator and administrator users are allowed to change user permissions. Administrators are listed with green avatars.

    Click Accept to confirm changes.

    • – Delete folder

    Upon clicking the Delete Folder button a dialog window opens. Select Accept to confirm the removal of the folder.

    NOTE: Only an empty folder can be deleted.

    Creates an exact copy of the selected folder and its content to another location in the tree.

    • – Move folder (for administrator users only)

    Same operation and window as above, except that the original folder and its components will be completely removed from the server.

    *Depending on your storage availability

    Important!

    More than one SUs can be connected to a HUB, but a SU can belong to one HUB only.

    Important!

    By completing the above minimum requirements the minimum operating standards are established only. If raising the number of user/folder/SU, make sure to proportionally size each of the components.

    Important!

    When implementing network-element redundancy, consider the following requirements:

    • HDD – Sub-system under the OS must be of RAID level 1 or higher providing greater data security.

    • Data – RAID level 5 or higher

    • Network – If NIC teaming method is applied, run at least 2 ethernet cables between the ports of the switch and the server

    2.2 Server Configuration

    Before installing any components of the CaseCenter, go through some important steps when using Windows Server 2008 R2.

    1. Open the Windows Server 2008 R2 Initial Configuration Tasks window.

    2. Select Add roles option from the Customize This Server menu.

    3 Managing Users and GroupsThe Users & Groups window displays information about registered users and groups created by administrators. The Users and Groups information is separately displayed on two different panels. For group data click the Groups tab on the left side of the panel.

    To search for users or groups scroll down the list in this window, and you can also narrow search results by entering text in the Quick search textbox.

    Adding or removing users and groups is available for you in these two panels.

    If LDAP settings are correctly set in the Service/Settings panel, then Get LDAP user list and Get LDAP group list buttons are displayed and active on both Users and Groups panels.

    To search for LDAP users and groups in LDAP server click the Get LDAP users or Get LDAP groups buttons respectively. A window is then displayed in which LDAP users/groups can be filtered based on the settings.

    5.1 Add SU

    A SU can be connected to one HUB only!

    After installing the SU, it has to be connected to the HUB through the 3DHISTECH CaseCenter Configuration application, HUB tab panel.

    1. Log in as administrator (user name: admin)

    2. Add HUB address

    3. Chose HUB server protocol (Secure connection means HTTPS)

    4. Add SU name

    5. Add SU self address

    6. Choose used SU protocol

    When it is done, the SU starts communicating with the HUB.

    5.2 Edit SU

    If the SU settings are changed, such as its name, address, or protocol, these values can be set in the SU through the 3DHISTECH CaseCenter Configuration application, HUB tab panel.

    NOTE: you cannot change the SU virtual folder.

    On the SU side, start the 3DHISTECH CaseCenter Configuration application and switch to the HUB tab panel:

    1. Log in as administrator (user name: admin)

    2. Change SU name

    3. Change SU self address

    4. Change SU protocol

    5. Press Change button to save changes

All slide-related data (name, staining, organ, scan date) are stored on the HUB, the unique identifiers (GUID) of slides are stored on the SU. Therefore, in a normal situation only that SU can be unregistered from the HUB, that does not contain slides. You can verify this in the settings tab panel of the 3DHISTECH CaseCenter Configuration application.

    6.1 SU/HUB Server Data Backup

    To initiate SU and HUB server data backup, follow these steps:

    1. Log in as administrator through the CaseCenter web interface.

    2. Open Service window Database Backup panel.

    3. Click Start Backup button.

    For more information on initiating the backup, see section 7.5.10 Database Backup; on configuring the backup folder, see section 7.5.8 Settings.

    6.1.1 Slide-related information and data backup

    During the process, slide locations and names, and personal settings of the application for their handling will be stored at the specified location. Because of security reasons, backup of data stored in the SU database is transferred in a converted format to the exact location.

    Important!

    During backup, slides are not being saved in the backup folder.

    6.1.2 Result

    After the process is finished, an additional folder with a prefix Backup_ is created with a timestamp suffix, in which the following files are stored:

    1. Folders

    • Attachments – container for attachments located on the HUB server

    • Thumbnails – container for thumbnail images located on the HUB server

    2. Files

    • Attachments.copy.log

    • Thumbnails.copy.log

    These log files list records of preformed activities. Search for error logs in these log files in case any fault has occurred during the backup process.

    6.2 Restoring the HUB Database Backup

    When restoring, the usage of CaseCenter is strictly avoidable, and the person doing the upgrade is responsible for ensuring technical conditions (for example, making the client inaccessible for users) and taking necessary precautionary measures.

    Restoration of previously saved data can be performed manually. If restoring a backed up CaseCenter data is needed because the version in use became unavailable, you must begin the process by installing a new release of the product. Make sure to read compatibility information regarding the backup of the specific version.

    After performing installation in accordance with the instructions, an empty database with a basic configuration will be available. Restore can only be preformed after creating an empty database. Act according to the situation in any non-ordinary case.

    6.2.1 Copying folders

    Copy the two resulting folders of the backup process (Attachments and Thumbnails) together with the included elements to the Web sub-folder of the CaseCenter application. By default the installation wizard sets: „C:\Program Files (x86)\3DHISTECH\CaseCenter\Web\” as destination folder.

    6.2.2 Restoring the database

    For this operation the command line interface to the MySQL database manager (installed together with the CaseCenter) is needed.

    1. Launch the interface from the start menu by typing mysql.exe -u root -p to the command line.

    NOTE: In place of root type the user name used for the database that has been set during installation. Only user cc_install is permitted to perform this action, as cc_user having a set of minimally required permissions cannot create the database.

    2. You are asked for the password for the database-server.

    3. After logging in successfully, remove the present CaseCenter database with the following command: drop database cc_hub;

    NOTE: Do not forget to put a semicolon (;) to the end of the line, and write the name of the database set during installation in place of cc_hub.

    4. Leave MySQL command interface by typing exit to the command line.

    5. Type cmd to the command line, then navigate to the folder created during backup.

    6. Run the restore process with the following command (entering the database name used previously in place of cc_hub, and the password identical with the one used above instead of using the label ): RestoreSQL.cmd 127.0.0.1 3306 root cc_hub HUB.sql

    NOTE: In case the database is not locally installed, replace 127.0.0.1 to the name of the host from which the MySQL HUB database is run.

    7. A loading procedure may take few minutes, and restore is fully completed when finished.

    8. Finally, modify HUB-SU connection settings in the CaseCenter Configuration window by entering a valid name and IP address.

    1. Transfer each slide manually to the new location (to another computer, or IP destination).NOTE: This location is not the final destination of slides.

    2. Install CaseCenter to the new machine with the new IP address (as HOST).

    3. Install necessary Windows updates.

    4. Start with restoring the HUB first, then the SUs one after the other.

    For more information on HUB restoration follow steps in section 6.2, and on SU restoration see section 6.3.

    5. Finally, launch CaseCenter Configuration software, and modify HUB-SU connection settings – rewrite old IP address and name to the new ones.

    7 Manage Slides

    7.1 SU storage management

    The average size of a digital slide is 1GB (Z-Stack slides require multiple times more space). The size of a specific slide depends on the size of the scanned area, the scanning method (Brightfield, Fluorescent, Z-Stack), the selected compression algorithm, and the rate of the algorithm (JPG, JPG2000, BMP). For example, a Brightfield Z-Stack whole slide smear digitized in JPG 80 may require more than 30 GB space on the storage drive.

    CaseCenter does not perform backup of slides (because of the huge amount of data) so it is the user's responsibility to ensure that storage of slides is secure. The most obvious solution is to use storage drives conform with security standards.

    Data security level can be increased if a backup copy of slides to a 3rd party device is created regularly. Expectation towards the storage device that is has to be compatible with Microsoft Windows Server 2008 R2 (operation system of CaseCenter Host).

    Storage connection type is either NAS (Network Attached Storage), DAS (Direct Attached Storage) or SAN (Storage Area Network).

    If NAS connection is established, communication through a Windows SMB/CIFS compatible protocol. DAS or SAN requires a proper Windows driver as the bare minimum.

    Ensure that storage server does not provide SMB file or printer share function towards CaseCenter clients, otherwise clients connected to storage server can access slides from CaseCenter through HTTP/HTTPS only.

    Outer storage has to be connected to SU (if NAS is preferred, the outer storage is connected to the LAN that includes the SU).

    NAS storage is in direct connection with clients of the same LAN (through SMB protocol) only if SMB access is enabled on the SU (enabled by default). If SAN or DAS is used, clients access storage through the SU. In such cases, slide storage folder on the SU must be shared (Windows Share), thus making it accessible for clients on the LAN (that includes the SU) through SMB.

    Storage drives can be set in Settings tab panel in CaseCenter Configuration application.

    On the Settings panel of the Service Unit Settings window, the following columns are displayed:

    • Storage path: slide storage directory

    • State1: usability status of storage (modifiable by administrator)

    Values:

    • Active: new slide can be placed on the storage

    • Archived: additional slide cannot be placed on the storage, slides already present on the drive can be read/written

    • State2: accessibility status of storage (shifts automatically)

    Values:

    • Available: storage can be accessed

    • Unavailable: storage is temporarily inaccessible (for example, because of a network error)

    • # of slides: number of slides momentarily stored on the storage

    • Allocated space: total size of slides on the storage

    • Reserved space for other SW's: smallest reservable free space on the storage

    • User Name: a valid user, that either exists on a domain server which includes SERVER_NAME and SU as members, or a local user on the SERVER_NAME machine. This user must have Full Control NTFS access permission to \\SERVER_NAME\SHARE\SUBFOLDER folder and to all of its sub-folders, as well as to \\SERVER_NAME\SHARE SMB share. Permission setting must be performed prior to adding the storage by the storage administrator. Permission must be registered for the user explicitly, group permission is invalid.

    • If several Storage Path is added to the system, each of them has to be set with the same User Name.

    7.1.3 DAS, SAN or local storage usage

    • The folder for storing slides must be shared (Windows share). Specification for the share created this way is the same as in section NAS storage usage.

    • Although the system allows us to use local folder path as slide storage (for example, D\:Slides), slides stored in such folder can only be accessed through HTTP/HTTPS even from clients within the LAN of SU, that drastically decreases the speed of transfer, and has dramatic effect especially on the scanning speed.

    7.1.5 Requirements and and recommendations

    Each system component of CaseCenter (storage as well) is within the same – solely – Windows domain, in other words from network access, name resolution, and handling of user privileges points of view, are included in the scope of the same administration area; so they are mutually accessible and interoperable (including deactivation of occurrent bottlenecks such as firewalls, anti-virus applications, or security system limitations; otherwise – if these components are enabled – assure an absolutely transparent yet standard software interoperability.

    • All of the storages of the specific SU must be connected with the same (and the only one) domain user. For this user must have explicitly-granted full access permission (in addition to effective rights related to group membership) both for shares and the file system.

    Accordingly, it is the person's responsibility who performs the installation, to ensure that the target computer of the storage is inaccessible by other users using a different user credentials via the SU (neither for establishing connection related to storage, software, nor services) than the one selected.

    • If possible, the connection of storages to the SU units must be performed when putting CaseCenter in operation, but their modification during operation is avoidable. Any alteration requires special circumspection and attention, be performed on a system unit out of service, and in accordance with the guidance within the topic.

    • Permissions required for the connection and usage of the SU unit must be guaranteed on connectable storage prior putting them in operation, and then continuously during their operation time (while in use).

    • The storage connected to SU must be at service non-stop, and with great reliability during operation.

    • Connection of more than one storage from the same volume (drive or file-system-level logical unit) to an SU is not supported.

    • The folder used as slide storage neither can be a root folder of a drive (for example, D:\) nor a root folder of a network share (for example, D:\) or a DFS path pointing to a root folder (for example, \\server\share). NOTE: HUB/SU backup could not be performed from these locations.

    • Do not connect a storage through an administrative Windows share, exclusively by granting appropriate permissions on an explicitly shared folder (for example, \\\C$).

    To assure convenient operation performance, we suggest to designate a storage in such a way that it is located on an independent physical drive using different code than any other operating unit of the CaseCenter runs. To meet higher performance needs, further (inner – from system point-of-view) resource decoupling or exclusive use might be necessary.

    7.2 Scan to Server

    The relevant feature documentation can be found in the device-specific User's Guide for Pannoramic scanners.

    7.3 Upload slides with Pannoramic Viewer

    You need Pannoramic Viewer to upload slides to the server.

    Tip!

    Since the upload may take a significant amount of time – depending on the slide size – it is recommended that you upload the slide to the server before starting a Teleconsultation session.

    You can only upload one slide at a time from Pannoramic Viewer. Administrators can perform bulk uploads via file transfer.

    1. Launch Pannoramic Viewer on your computer.

    2. To connect to the CaseCenter server, click in the toolbar.

    Slide import is a method of placing slides to the server but not via uploading. Import has to be initiated from the SU side, as the process cannot be launched through the web interface – because of security reasons the web interface does not have access to any data stored on remote computers.
