Our Mission at Almost Home Kids is to provide transitional care in a home-like setting to children with complicated health needs, training for their families and respite care.

Dear Friends, In fiscal year 2011, AHK continued to help children and their families. Despite a very challenging funding climate, we experienced a time of growth. We are thrilled about our progress as we expand our home in DuPage County and have signed our lease to open a second facility in 2012 in downtown Chicago. It is an exciting time for us and we hope you will embrace the opportunity and become part of our family. Help us “share the care”.

Under the direction of our Medical Director, Mary Keen, M.D., we serve as a leader in providing care and compassion for children who are medically fragile. Our home medical model is recognized by hospitals, clinics and insurance companies as a cost effective alternative. In September of 2011, our Board of Directors revisited the vision statement by asking the question – what does success look like in the future? The resulting vision statement: The AHK model is present in any place there is a need without regard to geographical boundaries. All key community and policy stakeholders are fully committed to the AHK model and recognize it as the premiere standard of care.

We are thankful to have a dedicated staff, committed Board of Directors, volunteer network and other stakeholders who generously support our mission. We thank all the individuals, civic groups, government, corporate and philanthropic partners whose contributions allow us to achieve our goals.

Caring for Our Children There is a growing need for facilities dedicated to the care of children who are medically fragile, which means the child is in a stable condition, but is dependent on life sustaining medications, treatments, equipment and has need for assistance with activities of daily living.

For the past two years, Almost Home Kids has been forced to turn away more that 50 children and families because of lack of space at the Naperville facility. Now we will soon be able to welcome more children into the Almost Home Kids family with a second location in downtown Chicago, scheduled to open in the summer of 2012.

How We Do ItTransitional Care — A bridge from hospital to home

AHK provides a Transitional Care Program for children who are ready to be discharged from a hospital but for whom preparations at home need to be made. In a comforting environment, children receive 24-hour medical and nurs-ing support from skilled pediatric nurses. AHK trains families on how to both care for their children who are medically fragile and prepare the home for their child.

Respite Care

The Respite Care Program at AHK offers children a comfortable place to stay with access to medical and nursing support while their

parents/caregivers tend to personal or family issues. The Respite Transportation Program provides roundtrip transportation for a child from their home to an AHK facility.

Our model for providing transitional and short-term respite care can be replicated throughout the country. Where the need exists, the organiza-tion will help facilitate development of other Almost Home Kids locations.

Community Support

In addition to training and care, AHK provides community resources and activities that become a support system the family can depend on for years to come.

Medical Home Model

Hospital is ready to release a child with complicated health needs but caregivers have no experience in handling the care.

Child is diagnosed with complicated health needs and treated at hospital.

Transitional Care offers short-term care for the child, care coordination and training for caregivers.

Respite Care offers a safety net in the continuum of care.

Trained caregiver takes the child back to the prepared home environment.

Caregiver has to take a temporary leave and requires support for the child’s care.

Funding SourcesAHK is licensed by the Illinois Department of Public Health and operates under the Alternative Health Care Delivery Act as a Children’s Community-Based Health Care Center. Three out of four of the families served at AHK live at or below the federal poverty level. Most qualify for medical care support from the Illinois Department of Healthcare and Family Services and many are eligible for children’s waiver services through the Department of Specialized Care for Children. Neither of these governmental programs covers the complete cost to care for the children. AHK absorbs the unreimbursed medical expenses by turning to philanthropy.

How We Spend FundsIt is 500% more expensive to provide continuous clinical care for a child in the hospital rather than an AHK facility. Each day spent at AHK represents a tremendous financial savings to the State of Illinois. During FY 2010 and 2011, AHK saved Illinois taxpayers over $10 million in Medicaid dollars.

■ Almost Home Kids applies 85 cents of every dollar raised to its mission.

■ Almost Home Kids harnesses the power of volunteers on a daily basis. Over 500 volunteers give more than 3,500 hours to AHK annually. This represents 2.5 full time employees over the course of a given year.

■ Almost Home Kids benefits the community by offering resident and fellow training, advocacy efforts on behalf of children and family-centered support services and programs.

Almost Home Kids in Chicago AHK will lease the 6th floor of a new Ronald McDonald House in Chicago. This building, situated near four world-class hospitals, is scheduled to open in summer 2012. The partnership between AHK and Ronald McDonald House Charities of Chicagoland and Northwest Indiana (RMHC-CNI) is the first of its kind. By combining the care and services for children who are medically fragile with RMHC-CNI, families will be reunited sooner and the partnership will keep families together and strong.

AHK is poised to be a national leader in treating children that are medically fragile, with a cost-effective business model that can easily be replicated in any community across the country.

We Need Your SupportAHK relies on philanthropy to deliver critical care to its children and their families. Support comes from individuals, civic groups, government, corporate and philanthropic partners. Together, we “share the care” by providing medical care and compassion for children with complicated medical needs.

Almost Home Kids Chicago will enjoy 8,000 square feet comprised of six bedrooms and one playroom so families may spend time together while transitioning from hospital to home. The floor plan also includes a therapy room, a work station for nursing staff, laundry room and, of course, a kitchen for families.

Total cost for construction, furnishing, equipment and start up costs is $3.5 million and the operational cost for the two facilities (Naperville and Chicago) over the next five years is $4.5 million. With enthusiastic support from its Board of Directors, Almost Home Kids is seeking public grants and private donations to fund the $8 million project.
