During the year 2015-2016, there hasbeen significant activity addressingthe Strategic Themes of the Anne

Johnston Health Station’s Strategic Plan.The Strategic Themes, namely PromoteHealth and Wellbeing; Support Clientsthrough Life Transitions; Deliver Care Better;and Deliver Care in and With the Community,are the basis for all programs and services atthe AJHS.

There have been many accomplishments toCelebrate, many Opportunities to consider,many Challenges to face, and many Changesto address. This annual report highlights ourCelebrations, our Opportunities, ourChallenges, and Change.

In November 2015, the Anne Johnston HealthStation participated in an accreditationreview by the Canadian Centre forAccreditation. Accreditation is an importantundertaking for an organization to ensurepolicies, procedures, safeguards, andprograms and services are in place to meetthe needs of all clients, staff, and volunteers.A full four-year accreditation status wasachieved and truly a time for congratulationsand Celebration.

The Anne Johnston Health Station continuallypursues Opportunities to better serve ourclients. Examples of such opportunitiesinclude the strengthening of the Spina BifidaTransition Program for adolescents movingto adult primary health care; partnering withDelisle Youth Services to address youth andmental health concerns; providing access toprimary care for seniors living in supportivehousing; and supporting education andcounselling for people with Diabetes.

Working within the health care environmentis not without its Challenges. Doing morewith the same requires a critical look atefficiencies, effectiveness, staff recruitmentand retention, and ability to partner. Staffhave now been gratefully offered participationin the Healthcare of Ontario Pension Plan(HOOPP) which provides a secure, long termfinancial benefit for all participants.

This past year, the AJHS has been consideringintegration with another organization.Integration can bring increased access toservices for more clients, better healthoutcomes, and opportunities for people tolive independently and age at home. TheBoard and staff have been involved inengaging clients, staff, and volunteers inthese integration discussions. This may meanreal and positive Change.

This report is an important way to say thankyou to clients, staff, Board members, andvolunteers – old and new. We look forward tothe upcoming year to Celebrate, to considerOpportunities, to address Challenges, andto embrace Change.

ProgramHighlights

Volunteer HubFunding support from the Ontario Trillium Foundationhas allowed us to create an inclusive volunteerprogram. Accessible computers, training and resourcematerials, and a central location have enhanced ourcapacity to provide meaningful opportunities for oureager volunteers. As a result, we have been able tofoster a greater sense of community between ourvolunteers, clients, and staff.

Boccia BallThe Boccia Ball Club is one of the newest programs atthe AJHS. This program enhances health and wellness,and provides an accessible program that supportsphysical activity, social interaction, and of course FUN!We have received very positive feedback and are lookingforward to offering Boccia Ball at additional times.

Mid-Toronto Diabetes ProgramThe Mid-Toronto Diabetes Program provides freediabetes self-management and preventative educationfor adults with type 2 diabetes, pre-diabetes, and forthose at high risk of developing diabetes. OurRegistered Nurse and Registered Dietitian are bothCertified Diabetes Educators and facilitate groupeducation, workshops, and provide individualcounselling sessions.

Gentle ExerciseThis low impact class for seniors consists of musclestrengthening and flexibility exercise. The goal is to getfit, meet other seniors, and have fun. Gentle Exercise isoffered Monday, Tuesday, and Thursday mornings; andinterested participants must register prior to attending.In addition, an exercise program for people with physicaldisabilities is available that encourages healthy andactive living by incorporating aerobic and stretchingexercises, and resistance training.
