UNMC orthopaedic surgeons lend a helping hand in Haiti

The 7.0 magnitude earthquake that ravaged the nation of Haiti on January 12, 2010, occurred approximately 25 miles west of the capital city of Port-au-Prince and lasted only about 35 seconds, but left a path of death and destruction that the world will likely never forget. It’s something that orthopaedic surgeons Drs. Miguel Daccarett and Sean McGarry will never forget either.

Drs. Daccarett and McGarry, both assistant professors in the Department of Orthopaedic Surgery at UNMC, recently traveled to Haiti and utilized their skills to help survivors of this horrible tragedy.

Dr. Daccarett was among the first wave of volunteers sent to Haiti through UNMC and Medi-Share, a 13-member team of healthcare workers including six physicians and seven nurses that deployed to Port-au-Prince on January 23rd. The team of volunteers began work at L’Hôpital de la Communauté Haïtienne shortly after their arrival. During that week they performed more than 30 operations on 19 patients.

After a week of non-stop surgery and little sleep, the group headed back to Nebraska. Dr. Daccarett had been home for only a few days when he was called for a second deployment through the Institute for Latin American Concern (ILAC), Creighton University, and the International Medical Alliance of Tennessee (IMA). Update: John F. Connolly Resident Excellence FundThe department has established a goal to raise $150,000, to be donated to the John F. Connolly Resident Excellence Fund in honor of the late Dr. John Connolly (chair of Orthopaedic Surgery 1974-1990). The purpose of this fund is to provide financial support for resident projects during their years in training, in an effort to continue the legacy of resident research and education that Dr. Connolly fostered.

As promised, we will continue to show our progress in each subsequent issue of Breaking News, until the goal has been reached.

Message from the chair

A year of firsts.

The year 2010 is turning out to be an exciting one for our department in many different ways. First, we hope you enjoy the new look of Breaking News. The appearance of this issue represents an exciting change that we will be implementing into multiple aspects of our department communications over the next few months, including our department web site, as well as our biennial report due out in late July of this year. Watch for it to arrive soon.

Our communications are not the only thing in the department being revitalized. If you have been into the orthopaedic clinic recently, you hopefully noticed the long overdue renovations that have taken place. Some fresh paint, new carpet and modern furniture have made a dramatic impact on the look and feel of the department. We are excited about the changes and welcome those of you who have not seen it to stop by for a visit.

Another ‘first’ this year is our decision to start Operation Walk – Team Omaha. During last year’s graduation, we listened to Operation Walk’s founder, Dr. Lawrence Dorr, give a motivating account of his volunteer experiences with Operation Walk – Los Angeles and it spurred us into action. We are thrilled to be a part of such worthy cause and will keep you abreast of the latest news as we continue on our journey.

on the border of the Dominican Republic, near Haiti. There, Dr. Daccarett served as a team leader and orthopaedic trauma surgeon.

On February 20th, Dr. McGarry traveled to Haiti through Medi-Share and the University of Miami. Their team of about 20 physicians was stationed at a hospital near the Port-au-Prince airport. The group was comprised of eight orthopaedic surgeons, as well as physicians from plastic surgery, internal medicine, emergency medicine, ob-gyn and pathology.

The hospital consisted of four large “M.A.S.H. style” tents that were each designated for a specific purpose, including: 1) sleeping quarters, 2) adult care 3) pediatric care, an intensive care unit, and four surgical areas, and 4) supplies.

For seven days it was basically the same routine. They rose early for a 7:30 a.m. staff meeting with the orthopaedic team, gave surgery and outpatient assignments, and as a team saw 160-180 patients each day. They had to round on all of the patients each day, which meant 30-40 per physician.

While the team did treat a number of acute trauma cases, arriving six weeks post-quake presented Dr. McGarry’s team with a different dynamic than those teams who had arrived immediately following the quake. Well over half of the injuries seen were orthopaedic in nature and required follow-up care including new x-rays, sometimes casting, and often rehabilitation instruction.

With so many spinal cord injuries (quadriplegics and paraplegics), the hospital was evolving into primarily a rehabilitation facility, said Dr. McGarry, which would be a vital component of their recovery. Family members were taught how to mobilize and transfer patients and were responsible for helping with much of that care.

Also surprising, were the number of acute injuries that came into the hospital that were unrelated to the quake. Because so much of the medical infrastructure had been damaged, there was nowhere else to treat these patients.

“It was definitely an inspiring experience . . . a once-in-a-lifetime opportunity to use my skills as an orthopaedic surgeon in this type of setting,” said Dr. McGarry.

Dr. Daccarett and his team spot a seemingly unharmed anesthesia machine amidst the ruins of a local hospital in Haiti. Although they were operating without one, with no way to move it, they were forced to leave it behind.

All supplies - joint implants, sterilization and radiology equipment, durable medical equipment, antibiotics, and drugs and dressings – are gathered through donations. Team Omaha professionals donate their services and skills, but funds must be raised for travel expenses, meals and hotels, and shipping of the medical supplies, averaging $100,000 per trip. In January, Dr. Garvin and Mary Peterson, RN, departed for Ho Chi Minh City, on a trip with Dr. Lawrence Dorr and his team during Operation Walk - Vietnam. Dr. Dorr is founder and medical director of the Dorr Arthritis Institute in Los Angeles, CA, as well as founder of Operation Walk.

The trip is designed to allow each new team an opportunity to understand the reality of the process prior to being responsible for their first mission. Over three long, but gratifying days of surgery, approximately 60 total joint replacements were completed.

“It was a rewarding and motivating experience,” said Dr. Garvin, “and we are definitely looking forward to our upcoming mission in Antiqua.”

In July of this year, Dr. Garvin and Peterson will join Team Canada on a pre-trip to Antigua to ‘scout’ for the November mission, do post-op exams on patients from a previous trip, obtain x-rays and complete pre-op exams on possible patients for the next mission.

Operation Walk is a private, not-for-profit volunteer medical organization that provides comprehensive, state-of-the-art free hip and knee replacements for impoverished patients with debilitating joint disease, while educating local healthcare professionals on the most advanced treatments for these conditions.

Started in 1995 by Lawrence D. Dorr, M.D., a leading joint replacement surgeon in the United States, Operation Walk has traveled to Cuba, China, Nepal, the Philippines, Nicaragua, Guatemala, Peru, El Salvador, Mexico and Kenya, and has operated on over 2500 patients in ten countries.

New Orleans, Louisiana was the host city of this year’s annual meeting of the American Academy of Orthopaedic Surgeons (AAOS), held March 9-13th, and the Nebraska Orthopaedic Alumni Reception on March 12th. About 30 Nebraska orthopaedic alumni and their guests gathered for the reception at the Hard Rock Cafe to catch up with other alumni and friends.

During this year’s AAOS meetings, faculty, residents and staff gave three podium presentations, four poster presentations, and one scientific exhibit, participated in five instructional course lectures, spoke at one symposium and four specialty day symposiums, and served as moderator of one session. The department is proud to announce Dr. Brian D. Adams as the featured speaker for this year’s graduation ceremonies to be held June 18th and 19th. Dr. Adams is a professor in the Department of Orthopaedics and Rehabilitation and the program director of the hand surgery fellowship at the University of Iowa.

Dr. Adams attended medical school at the University of Nebraska Medical Center. He completed orthopedic surgery residency training at the University of Iowa and a fellowship in hand surgery at Loma Linda University in California.

His clinical specialties are in hand, wrist, elbow and shoulder surgery. His research interests are focused on biomechanics, orthopedic implant development, and clinical outcomes of upper extremity reconstructive surgery.

He is a member of the American Society for Surgery of the Hand, American Academy of Orthopaedic Surgeons, American Association for Hand Surgery, American Orthopedic Association, Mid-America Orthopedic Association, and the Association of Bone and Joint Surgeons, among others. He has served on many organizational and journal editorial boards.

Orthopaedic celebration goes off with a “bang!”It was a night of “Murder, Mystery & Mayhem” at this year’s Orthopaedic Department celebration, held at the Omaha Press Club on January 23, 2010. There was Vegas-style casino fun to be had with a murder-mystery twist! Several faculty and staff volunteered to get into character, both dressing and acting the part. At 10:00 p.m. sharp, the lights flickered on and off, and revealed the victim to be…Ace High, the high-rolling gambler, aka Dr. Kevin Garvin, chair of Orthopaedic Surgery! (Really, that wasn’t planned.) Guests then followed the clues to identify the murderer as gambler Hustling Hailey, aka Heather Oksey, MA.

‘Smile and pretend to be innocent!’ Gambler Hustling Hailey (aka Heather Oskey, MA) and cocktail waitress Mimi Martini (aka Susan Siebler, business associate) hatch a plan to overtake the rest of the high-rolling gamblers.

Lori Dawson, MA, completed an impressive 41 push-ups during “Push-ups for Haiti”, which raised money for the UNMC Haitian Relief and Outreach fund. Sixteen participants did a total of 808 push-ups and raised a total of $8710 towards the overall goal of $100,000.

Congratulations to Dr. Brian Hasley and his wife, Katie, who celebrated the birth of Luke William Hasley on December 5th, 2009. Dr. Hasley is an assistant professor in the Department of Orthopaedic Surgery and Rehabilitation.

AWARDS AND ACHIEVEMENTS:Congratulations to Dr. Susan Scherl, who was among the first group of physicians awarded with the American Academy of Orthopaedic Surgeons Achievement Award in

March of this year. The new program was designed to thank AAOS members for their many contributions to education, research, and advocacy in orthopaedics, while creating an incentive for others to give back in a similar capacity.

During the College of Medicine’s AOA/Faculty Honors Convocation in March of this year, Dr. Edward Fehringer was presented with a Special Professional Achievement Award for receiving the Creighton University/University of Nebraska Health Foundation’s Orthopaedic Surgery Residency Program Out-standing Instructor Award, as well as serving as President-Elect of the Nebraska State Orthopaedic Society.

In 2009, Dr. Kevin Garvin passed the 2009 Recertification Examination and is now certified by the American Board of Orthopaedic Surgery through 2020.
