Phase 1 Environmental Assessment (Attach as exhibit), and summarize important findings with supporting documentation – See attached exhibit #1 for Summary and attached exhibit #1B Phase I Environmental on CD. Any evidence of hazardous substance on site, including underground tanks – There was no evidence of any hazardous substances on the site, including underground storage tanks. Hazardous site history – The history for the property includes farmland and undeveloped land. There was no history found regarding storing hazardous substances on the property. Risks of neighboring properties – Neighboring properties consist of farmland, residential and light industrial. None of these properties has the potential to negatively affect the subject property. Environmental liens – No environmental liens were found for the subject property during the reconnaissance. Drainage issues – The property appears to have good drainage. No flooding or drought was observed during the inspection. 2. Air Quality (Air Attainment) Status at county level; discuss attainment, non-attainment and maintenance status by major source at county level; include maps showing attainment/non-attainment/maintenance status (Attach as exhibit) – Rock County is an attainment area. See attached exhibit #2 and see attached exhibit #2B for air attainment letter. 3. Wetlands Delineation Study-has a study been completed and if so, by whom? Provide a color copy of the study and summarize conclusions (Attach as exhibit) – A wetland study was done for the street corridor for Gateway Boulevard. No wetlands were identified in the corridor south of Eagles Ridge Drive. See attached exhibit #3. 4. Flood Plains is site located within or outside of the 100 and 500-year flood plain? If within, clearly indicate the percentage on map or discuss measures to remove site out of flood plain; provide engineering map or original FEMA flood plain map clearly showing site and location within or outside of known flood plain designation (Attach as exhibit) – The park is not in a 100 or 500-year flood plain. See attached exhibit #4. 5. Endangered Species-submit process or report to confirm that no endangered/threatened species will be impacted from development – See attached exhibit #5 of the Summary of Findings and the complete report on National Historic Preservation Survey on CD attached as exhibit #5B. 6. Archeological or Historic Resources-submit process or report to clear site from any archeological or historical designation (Attach exhibit) – See attached exhibit #5C. 7. Soils Conditions/Stability/Supply Report from an engineering or other qualified firm that discusses the following: Soil type at site – See attached exhibit #6 with the complete report attached as exhibit #6B Geotechnical Investigation Report. Results of sample soil borings taken at site – See attached exhibit #7. Documentation of the following risks factors, including seismic vibration/activity, fault lines, sinkholes, past undermining – Attached exhibit #8 is USGS Earthquake Hazards Program Report, and a report from Best Syndication. Both reports indicate that Rock County is a relatively low risk area for earthquakes. The nearest fault line is the New Madrid fault line in Missouri. Also attached is exhibit #9, USGS Landslide Overview Map indicating that Rock County is at low risk for landslides. There is no evidence of sinkholes or past undermining on the site. 10 year history of tornados in area and intensity level – See attached exhibit #10 tornado history report for Rock County and surrounding counties. Average annual snowfall – 32.7 inches. 8. Other natural disasters if not discussed above Some history (1972, 2008) of Rock River and Turtle Creek flooding in City Center.
