transitional housing and permanent supportive housing pro-
grams that offer opportunities for men and women experiencing
homelessness to become emotionally, mentally, and physically
more stable.

Our
Services
Include
Emergency Shelter for adult men and women, with 78 beds (35
for men, 26 for women and 17 for veterans), plus 4 overflow
mats, for a total capacity of 82.
Shelter open 7 days a week, 24 hours a day, 365 days a year.
Computer Lab open to participants and the public for classes,
job searches and resume building.
Day Shelter open from 8am to 6pm Monday through Friday.
Day Shelter provides both participants and community mem-
bers with a safe place to come in from the cold or heat.
Service Coordination
At the core of all HMSI programs is the philosophy of Service
Coordination. HMSI Service Coordinators work with program
participants to help them develop individual service plans focused
on goals designed to increase self-determination, increase skills
and income, and obtain and remain in permanent housing. Par-
ticipants and Service Coordinators work as partners to assess the
participant’s strengths, obstacles, and resources, and use that in-
formation to help the participant achieve their goals.
Shelter Service Coordination
The Shelter Service Coordination program serves homeless adults
staying at Humility of Mary Shelter, Inc. This program helps
participants address the most urgent issues that caused them to
experience homelessness, including needs for identification
documents, transportation, mental and physical health care, and
income.
Transitional Housing Program
The 17-bed Transitional Housing Program provides up to 24
months of service coordination and housing to homeless persons
with
disabilities or other special needs that make it more difficult for
them to find appropriate housing. Participants benefit from
bi-weekly program meetings, support groups, and mental health
counseling.
Veterans Transitional Housing Program
The 17-bed Veterans Transitional Housing Program provides ser-
vice
coordination and housing to homeless veterans for up to 24
months.
Working with the local Veterans Affairs Homeless Outreach Cen-
ter, the program allows qualifying veterans to take advantage of
services such as case management, education, job training, crisis
intervention and counseling.
Veterans Accessing Long-term
Opportunities and Resources (VALOR)
The VALOR program serves very low-income veterans from Scott,
Muscatine and Clinton Counties, Iowa and Rock Island, Henry,

Whiteside and Mercer Counties, Illinois who are homeless or at
imminent risk of becoming so.
Participants work closely with staff to rapidly return to permanent
housing, or to maintain their current housing, while addressing the
issues that led to their housing crisis. The VALOR program is a
natural extension of our successful Veterans Transitional Housing pro-
gram. The VALOR office is located within Cobblestone Place at 1212
West 3rd Street, Davenport.
Permanent Housing Program (PH)
The Permanent Housing Program assists homeless persons who have
a disabling condition which impedes their ability to live independent-
ly. Eligible participants may include those experiencing challenges
with severe and persistent mental illness, persons in recovery from
drug or alcohol dependence, and persons with chronic illness or phys-

A 51-year-old, veteran who had experienced homelessness multiple times, finally found comfort and safety at
HMSI. Together with his Service Coordinator he worked to obtain disability income which subsequently opened the
door for him to have a relationship with his child that he had not had in 7 years.

The VALOR Program helped a single parent veteran connect with the HUD VASH Program,
which provides
chronically homeless veterans with Housing Choice Vouchers. This veteran is the mother of six
minor children and therefore finding affordable housing was a significant barrier. At the time
she enrolled in the VALOR Program, she was staying in a local emergency shelter. This veteran
obtained her Housing Choice Voucher and was then able to move into a 3 bedroom apartment
in Davenport with her children where her family could begin to find stability.
Melissa checked in to HMSI in the middle of a very severe winter. She was immediately
connected to her Service Coordinator and together they addressed some of the unique barriers
she had to obtaining stable housing. Melissa was able to find long term support in our
Permanent Housing Program, where she been able to focus on increasing her quality of life.

A young woman entered the Shelter Service Coordination pro-
gram very determined to enroll in an Illinois college.
Together, she and her Service Coordinator found stable housing
and a program that assists youth. Both were near her school of
choice! On a beautiful, sunny morning, her Service Coordinator
was able to assist her in moving from
homelessness to housed!
Jack,’ a 44-year-old single male, was in the process of being evicted from his apartment, when HMSI staff enrolled
him in our VALOR Program. ‘Jack’ had been unable to work in four years, due to the severity of injuries he received
during active military service. VALOR staff not only helped him move into an affordable apartment in the Quad Cit-
ies, they also assisted with expediting his current “stalled” VA claim. With a service connection claim awarded, Jack
can now pay for his apartment on his own, without assistance from his family.

The Humility of Mary Shelter, Inc. (HMSI) participant profile presents basic demographics as well as
many of the challenges faced by participants when they entered HMSI.
From July 1, 2013 to June 30, 2014, HMSI provided shelter, housing, and/or services to 729 individuals (exclusive of HMSI’s VALOR
program). Each night, an average of 113 individuals were served. Of the 729 individuals, 130 were children under 18 years of age and 4
of those children were not accompanied by adults. HMSI provides the children and their families with supportive services through a
collaborative effort with The Salvation Army.
Forty percent of participants were female and one participant identified as transgendered. Approximately 11% of participants were
veterans. Fifty percent identified as Caucasian, 38% identified as Black or African-American, and 11% identified as being Asian, Native
American, or multiple races. Six percent identified
as being of Latino/Hispanic ethnicity.
Forty percent of participants had a mental illness
at entry. Almost 35% had experienced drug and/
or alcohol abuse; 23% had a chronic health
condition, HIV/AIDS, or a developmental
disability; 25% had a physical disability. Thirty-
eight percent of participants had more than one
known physical or mental health condition at
entry. Thirty-three percent of participants had
been victims of domestic violence; of those, 13%
had experienced domestic violence in the past 12
months.
Twenty-five percent of participants were earning
an income at entry and 32% had no income at
entry. Of those who left the program, 49% were earning an income at exit. The average length of

Humility of Mary Shelter, Inc. (HMSI) provides emergency shelter, transitional housing and permanent supportive housing programs
that offer opportunities for men and women experiencing homelessness to become emotionally, mentally, and physically more stable.
HMSI
formally evaluates Program Outcomes on an annual basis and uses the results to report on how program participants are progressing
and succeeding through the programs. Outcomes are also used to improve existing services, distinguish program strengths, identify
staff and program needs, justify budgets, and assist in developing long-range agency plans. HMSI’s Program Outcomes are based on
three main goals:
1. Program participants access mainstream and/or self-enrichment resources,
2. Program participants with education/job training or employment goals make progress on those goals, and
3. Program participants enter housing of their choice at departure.
Program participants in this report were adult men and women actively enrolled in an HMSI program between April 1, 2013 and March
31, 2014. Each participant completed a maximum of three Outcome Assessment Surveys with their Service Coordinator at program en-
try, during the course of the program, and at program departure.
The HMSI Programs
Program Outcomes are measured on four HMSI programs, two of which are Permanent Supportive Housing (PSH) programs: Perma-
nent Supportive Housing for persons living with disabilities and Housing First for persons who are chronically homeless. Each of the
PSH
programs provides long-term support for its residents.
The third program is a Shelter Service Coordination – Supportive Services Only program providing individuals with support services for
approximately 30 to 60 days. HMSI Service Coordinators provide supportive services to persons living at HMSI’s emergency shelter and
to individual and family emergency shelter residents of The Salvation Army. The short-term nature of this program allows a higher
number of

participants to be served over the course of a year.
The fourth program is a Transitional Housing (TH) program that provides persons in emergency shelter with structured programming
and support services for up to 24 months. This program benefits participants who need more time to reach their goals. HMSI also pro-
vides a TH program exclusively for the military veteran population with funding from the Department of Veterans Affairs.
Participant Overview
Between April 1, 2013 and March 31, 2014, 757 persons requested and received emergency shelter at HMSI, and 569 individuals and
families entered HMSI programs. Of those entering programs, 87% participated in the emergency shelter's Shelter Service Coordina-
tion program (11% were families), 10% participated in the Transitional Housing program (5% were in the veterans program), and 3%
participated in the Permanent Supportive Housing and Housing First programs.
Of those in HMSI programs, 32% of individuals were female. The median age of persons entering the HMSI programs was 44 years
(min=18 years old, max=74 years old); the lower 10% of ages ranged from 18-24 years old and the top 10% of ages ranged from 58-74
years old. Races included Native American/Alaskan (5%), Asian (1%), African American/Black (37%), Pacific Islander/Hawaiian (<1%),
and Caucasian/White (61%). Seven percent identified with Hispanic/Latino ethnicity.
At program entry, 99% of individuals entering HMSI programs were literally homeless and less than 1% were at imminent risk of losing
their housing. Thirty-eight percent of participants reported having never experienced homelessness before this episode, and 37% re-
ported that this episode was their 2nd
or 3rd
episode of homelessness. Approximately 25% of program participants were experiencing
chronic homelessness (defined as four or more homeless episodes in the past three years, or at least one year of continuous homeless-
ness; 12% of participants were experiencing continuous homelessness for one year or longer). Nearly 67% of participants had at least a
high school
diploma or GED and 46% had no income over the past month. Forty-two percent of participants were survivors of domestic violence,
with 27% suffering a domestic violence episode within the past three months. Seventy-three percent of participants had a long-term
disability involving one or more of the following: mental illness (47%), physical mobility (31%), chronic health condition (21%), alcohol
abuse (18%), drug abuse (9%), or developmental disability (3%).
Nineteen percent of individuals had served in the military. Of those, 54% served in the Army, 21% served in the Navy, 17% served in

the Marines, and 6% served in the Air Force. One percent served between WWII and the Korean War (August 1947 to May 1950), 3%
served during the period between the Korean War up to the Vietnam War (February 1955 to July 1964), 21% served in the Vietnam era
(August 1964 to April 1975), 53% served in the post-Vietnam era (May 1975 to July 1991), 12% served in the Persian Gulf era to present
(August 1991 to August 2011) and 11% served since September 2011.
Sixty-two percent of program participants exited because they had completed the program, 16% of participants had reached the maxi-
mum time allowed in the program and were required to exit before program completion, 14% were asked to leave the program due to
noncompliance with rules, and 8% left the program suddenly and without warning.
Program Outcomes Evaluation Results
1) Program participants access mainstream and/or self-enrichment resources Measure:
At least 65% of program participants will access mainstream and/or self-enrichment resources.
Result:
Goal was met.
Summary of findings:
Mainstream resources are defined as general state and federal benefits available to qualified individuals. Self-enrichment resources in-
clude, but are not limited to, services such as life skills training, alcohol/drug services, HIV services, physical health services, and mental
health services. Eighty-five percent of participants accessed at least one self-enrichment resource while in an HMSI program and 81%
accessed mainstream benefits (see Table 1 on page 10).
Accessing assistance with these resources resulted in a significant increase of participants’ ability to continue meeting basic needs by
program exit. For example, participants significantly increased their earned income from program entry to program exit (p<.001),
participants significantly increased receipt of SSI from program entry to program exit (p=.03), and more participants were receiving
food stamps at program exit than at program entry (p=.02).

2) Program participants with education/job training or employment goals make progress on those goals Measure:
At least 50% of program participants with education, job training, or employment goals will make progress on those goals, and 20% of
participants will be employed at program departure.
Result:
Goal was met.
Summary of findings:
Seventy-six percent of all participants reported an education/job training goal (40%), employment goal (71%), or both, with 56% mak-
ing progress on their goals (Table 1). Participants with education/job training goals were enrolled in GED courses, a certificate program,
vocational school, or college. Participants with employment goals were involved in preparing resumes, submitting applications, obtain-
ing interview clothing, role playing/practicing interviews, interviewing, and test preparation. Twenty-nine percent of participants who
were
unemployed at program entry obtained employment while in an HMSI program and 41% of all program participants were employed at
program departure.
3) Program participants enter housing of their choice at departure Measure:
At least 80% of program participants who enter either Permanent Supportive Housing or Transitional Housing will remain in the pro-
gram at least six months, and at least 65% of participants will enter housing of their choice at program departure.
Result:
Goal was partially met.
Summary of findings:
Ninety-three percent of participants remained in the Permanent Supportive Housing program at least six months. For Transitional
Housing participants, 30% remained in the program at least six months. Of exiting Transitional Housing participants, 74% entered
housing of their choice at program departure, including non-subsidized housing (25%), subsidized housing (23%), permanent move
with friends or family (19%), temporary move with friends or family (4%), and psychiatric or substance abuse treatment facility (2%).
More than 83% of those

exiting Permanent Supportive Housing moved to housing of their choice, including rental housing (50%) and permanently living with
friends or family (33%). Across all programs, 67% of program participants entered housing of their choice at program departure (Table
1).
Summary Participants of HMSI met most of the targeted outcomes goals between April 1, 2013 and March 31, 2014 (Table 1). During this time,
569 individuals and families entered and exited HMSI programs. Of persons served, 87% participated in the emergency shelter's Shelter
Service Coordination program, 10% participated in the Transitional Housing program, and 3% participated in the Permanent Support-
ive Housing and Housing First programs.

Homelessness Point In Time Count in Iowa By Vanessa Reyes, student intern from Augustana College
The Point-in-Time (PIT) Count is a count of all homeless individuals on a single night. The count includes persons staying in Emergency Shelters, individuals in Transitional Housing, and those sleeping on the streets or in places not meant for habitation. PIT also takes a housing inventory count of beds in Emergency Shelters, Transitional Housing, and Permanent Housing (which includes Permanent Supportive Housing and Rapid Rehousing Programs). The U.S. Department of Housing and Urban Development requires that each Continuum of Care (CoC) conduct a PIT Count every year during the last 10 days of January. In Iowa, there are three CoCs: Des Moines/Polk County, Sioux City/Dakota County, and Balance of State, which includes the remaining 96 counties in the state. The latest PIT count for all three Iowa CoCs was conducted on January 29, 2014. The January 2014 PIT Count for the Balance of State CoC clients totaled 1,874 persons, an increase of 15 people from the previous year. The housing inventory count for emergency shelter and transitional housing totaled 3,173 beds, an increase of 141 from 2013. There was a net decrease of 29 persons in the chronically homeless population, which could be due to an increase in the permanent supportive housing beds. While this was the fourth year in which an increase in the total sheltered and unsheltered population occurred, the rate of increase has decreased each year for the past four years. For the Balance of State CoC, there was an increase of 106 people living in emergency shelter, a decrease of 91 individuals living in transitional housing (attributed to the closing of several transitional housing programs), and an increase of 4 unsheltered persons. Females made up 43% of individuals in emergency shelter, 55% of persons in transitional housing, and 25% of people unsheltered. More than 7% of those in emergency shelter identified as Hispanic, as did 10% of those in transitional housing and 2% of unsheltered individuals. One hundred twelve persons in the Balance of State PIT Count were chronically homeless, 328 were severely mentally ill, 290 had experienced chronic substance abuse, and 383 were victims of domestic violence. One hundred persons were veterans. Statewide, 51% of the total count of individuals were in families, 8% were chronically homeless, and 7% were veterans.
The Humility of Mary Shelter Outreach Team participates in the PIT
Count twice each year, in January and July. They help canvass Scott
County and collect data from individuals experiencing homelessness.

Notes from the Finance Department Fiscal year 2014 was a record year for community support! We thank everyone for their generosity. A special thanks goes to the Sisters of the Congregation of the Humility of Mary, for their successful Challenge Grant. The Sisters met their goal of raising $250,000, with the first $100,000 matched by the Congregation. Here are a few ways your donations have helped Humility of Mary Shelter, Inc. succeed:
Cash flow – Most government grants require us to incur expenses upfront, and then be reimbursed for them later. Sometimes these reimbursements are delayed by several months, due to issues beyond our control. Last fall’s government shutdown also impacted our cash stream. Our grant income for FY14 includes several large receivables
from various government agencies. Without the generous financial support of our community, we would have periods where we have no cash coming in.
Cash reserves – As a homeless shelter, we know that we will have improvements and/or repairs to make to our shelter
building in order to remain open. Since these are sometimes unexpected, and must be addressed immediately, it can be difficult to have grant funding available to pay for them. Your support allows us to set aside cash reserves to help pay for maintenance and equipment needs such as a new furnace, water heater, or replacement plumbing.
Program improvements – Increased cash donations help us to improve and expand our programs, to better serve our shelter guests and program participants. Many items that our guests need, such as basic hygiene supplies, are not covered by our major grants. Your donations help us provide these vital items to the individuals we serve.

Night at the Shelter is our year round fundraising campaign that is designed to involve the community and en-
courage you to assist in our mission to help the homeless in the Quad City Area. Our goal is to find a sponsor for
82 beds each night of the year. Let’s Do the Math…

The 2013 Night at the Shelter SleepOut raised $11,801. The event achieved a 27% increase in fundraising and double the
participation of 2012. The evening was a cool 71° and overcast;
approximately 100 attendees slept in their cardboard temporary
homes on the field of Modern Woodmen Park.

Humility of Mary Shelter, Inc. partnered with ComedySportz
Quad Cities on May 4, 2014, providing the community with an
evening of comedy for a cause. The night featured a stand-up
performance by comedian Chris Schlicting followed by a Come-
dySportz match!
Thrivent Financial and Ruth Ahnen, Financial Representative from
Modern Woodmen Fraternal Financial provided support by
matching a portion of the proceeds.
