The AstraZeneca Award for Innovation in Chronic Disease Management provides a member of NPAO with a $5000 grant to develop strategies to optimize the management of chronic disease and improve quality of life for those living with chronic disease. This year’s award recipient is Susan Shea for the Better Health Project, with the Rideau Community Health Services in Smiths Falls, Ontario. The Better Health Project is an interprofessional clinic utilizing Group Medical Visits as a unique approach to medically care for and promote self-care management skills for participants without a primary care provider. Once the participants complete the program they are transferred to a permanent health care provider. Susan Shea is the Nurse Practitioner lead for this project. Goals of the project include:

To provided health care to people who do not have a primary care provider

To connect participants to a permanent health care provider To provide health education and management of chronic diseases To support participants to learn self-management skills and

develop goals related to their health and wellness To build a medical chart To refer to medical specialists and community programs as

appropriate To work collaboratively with Perth& Smiths Falls Hospital, CCAC

and other community partners. To diminish impact of unnecessary visits to the Hospital ER.

PACE (Pfizer Award for Clinical Excellence) Presentations at 1200 on Friday Nov 11

The Pfizer Award for Clinical Excellence provides recognition and financial support for two NPAO members (one NP-PHC and one Adult or Paediatrics) displaying excellence and best practices in one of three clinical areas: Cardiovascular Care and Wellness (including hypertension, cholesterol, diabetes management or smoking cessation), Pain and Symptom Management or Dementia related care. Each recipient will receive a grant for $2500. Sponsored by Pfizer, the award recognizes best practices and leadership and helps promote the NP role. NPAO is grateful for Pfizer`s support for these two awards.

This year competition for this award was fierce. We received many wonderful applications a testament to the great work NPAO NP members are engaged in throughout the province. Thank you to all of you who applied.

Please join me in congratulating this year’s NP-Adult recipient from London Ontario, providing geriatric consultation and working on her PhD, Jacqueline Crandall. Jacqueline has a varied advanced practice nursing background and her impact to date has spanned hospitals, communities and long term care. She has been instrumental in integrating RNAO Best Practice guidelines in all of these settings; she has been a champion for medication safety. End of life care and building capacity for end of life care have been near and dear to her heart as a palliative care specialist and volunteer. Now engaged in PhD studies in Nursing at the University of Alberta, Jacqueline will link clinical practice issues with research in the area of geriatric palliative care which include pain and symptom management as well as dementia. Please join me in congratulating Jacqueline Crandall, one of this year’s recipients of this year’s PACE (Pfizer Award for Clinical Excellence) Award.

congratulating Joy Parsons-Nicota. She has been a Primary Health Care Nurse Practitioner for the past eleven years and has spent much of her career devoted to working with interdisciplinary teams, participating in research, creating evaluation tools and as an educator on the care of the geriatric patient. She has a long history of demonstrating excellence and leadership in the provision of geriatric care and dementia education and treatment. She has clinical expertise in Primary Health Care with specialities in Gerontology and Mental Health Nursing and is a certified a Gerontological Nurse and a member of the Gerontological Nurse Association. She is also a long time active member of Nurse Practitioners’ Association of Ontario.

Ms. Parsons will use the award for scholarly development to assist her with continuing doctoral studies on ways to use technology to enhance continuing education and research in the areas of Dementia and Geriatric care for Nurse Practitioners and other health care professionals. Please join me in congratulating Joy Parsons-Nicota , the second of this year’s recipients of this year’s PACE (Pfizer Award for Clinical Excellence) Award.

The NPAO bursary awards were developed to support members who are

pursuing NP education or post graduate studies. To date, NPAO has

provided bursaries to 41 members.

Presidents’ Award: Michelle Acorn

Luise: This year a new award was created by the generous donation of Michelle Acorn, NPAO’s current president. The award was created to support NP innovation. Like other awards, many deserving candidates applied. Thank you to all who applied.

Michelle: This year`s award recipient has collaborated with colleagues and leaders within her employment setting and the University of Toronto to design, develop and implement a rotating internship in women’s health for MN/NP students. Now MN/NP students are engaged with NPs through this internship program to learn from practicing NPs in several womens’ health specialties at Women’s College Hospital, including after cancer treatment transition, headache, osteoporosis, endocrinology and diabetes to mention a few. Please join me in congratulating Deidre Bainbridge NP-PHC, the inaugural recipient of NPAO’s Presidents ‘Award

Jerry Gerrow AwardMichelle Acorn

Jerry Gerrow was one of the founding members of the NPAO and this award was created to honour exceptional leadership to the Association and contributions to the development of nurse practitioner practice.

This years`Jerry Gerow award recipient has been working for years to advance the practice of NPs and comes highly recommended and admired by her peers at London Health Sciences Centre. She is described as “a professional, quiet leader who embodies the full scope of the NP-Adult role, demonstrates leadership toward all APN roles and contributes in leadership roles within NAPO.”

For more than ten years, she has worked in the NP role providing consultation to enhance the care of older adults. Currently she completes comprehensive geriatric assessments and assists with collaborative patient care planning in a team setting, liaising with community professionals and resources to ensure continuity of care.

She has been very active within several groups to enhance patient accessibility to care. Within London Health Sciences Centre, she was very involved in the creation and implementation of the Acute Care of the Elderly (ACE) Unit. She is also a key member of the London City Wide 3Ds group participating in and writing a module of the RNAO 3D e-learning package- a learning package that is now available world wide

She has also worked with the College of Nurses to enhance quality assurance within the NP profession.

She is described as an excellent role model for nurse practitioners. Her calm and professional approach combined with the thoroughness of her assessments and interventions makes her an outstanding professional. She can often be seen with students, nurse clinicians and other NP candidates as they move through their education. She is always available for her colleagues to review complex patient situations or assessment challenges. In addition, she regularly provides education to post-graduate students on the topics of professionalism, political practice and the advanced practice nurse role at the University of Western Ontario.

She is described as an excellent communicator.

She is now engaged in new education and is embarking on the journey of pioneering the NP – anesthesia role at London Health Sciences Centre.

Please join me in congratulating Heather Whittle, this year’s recipient of the Jerry Gerrow Award to recognize her outstanding NP leadership.

Pfizer Consumer Health Care Awards – Saturday Nov 12 with opening remarks 0900

Formerly Whitehall Robins, was the very first organization to make a

commitment to support NP education in Ontario. This is the 8th year of the

award which is now called the Pfizer Health Care Consumer Award. This

bursary provides support for NPAO members intending to or currently

practicing in the NP role in various community programs throughout the

province.

This years recipients are Jaqueline Crandall and Deidre Bainbridge

Deidre Bainbridge is a NP- Primary Health Care who provides acute and

follow-up care to victims and survivors of sexual assault/domestic violence

at Womens’ College Hospital. Deidre worked with the Chief Nursing

Executive at the hospital and her Advanced Practice Nursing NP

colleagues and the University of Toronto to design, develop and implement

a rotating internship in women’s health for MN/NP students. Now MN/NP

students are engaged with NPs through this internship program to learn

from practicing NPs in several womens’ specialties such as after cancer

Huronia Award being presented on Saturday at 1030

As a result of the success of HNPN’ s (Huronia Nurse Practitioner Network) Annual Inter-Professional Conference, and in support of their nurse practitioner colleagues, HNPN is offering a $1500 bursary to a NPAO member to support their continuing education. Established in 2000, HNPN is a group of over 30 local NPs who work in a variety of primary and acute care settings and support inter-professional practice, education and evidenced-based practice. This year’s recipient has worked as a Nurse Practitioner since 2004. For the first 6 years she worked in the area of acute stroke care and secondary stroke prevention and then changed focus in September 2010 and now works in Long Term Care (LTC) Outreach in the Simcoe Muskoka LHIN. The LHIN is looking to expand the role to provide extended coverage and to trial our services in retirement home settings. She is registered to start the on-line course on Pharmacotherapeutics through the Council of Ontario University Program. The program will take me approximately 8 months to complete all 13 modules. With our increase in prescriptive authority it is vital that this area of continuing education be part of my learning plan to ensure competency and optimize client care. She plans to use the funding toward course within this program. Please join me in congratulating this year’s Huronia Award recipient, Irene Heinz, NP- Adult.

Paul Carere has worked tirelessly on behalf of Nurse Practitioners for many years. Most recently she has served on the NPAO board of directors as treasurer, president elect, president and past president. She works with what seems unending energy to advance the role of the nurse practitioner in Ontario so more Ontarians can benefit from the care NPs are capable of providing. As president of NPAO in 2010, Paula led with passion and commitment and now continues with energy and collaboration in her role as past president in 2011. She assists the board and its association members to clearly articulate a patient centered approach in all they do - from their work with government, regulatory bodies, other health care professionals, industry and many others. Paula continues to work tirelessly and with finesse to transcend real and perceived barriers, push boundaries and to build partnerships all in an effort to achieve a shared vision for excellent patient centered heath care. She never misses an opportunity to help others understand the unique contributions Nurse Practitioners make to the health of Ontarians across the continuum of care in a variety of community, hospital and long term care settings for children, adults and seniors.

Paula's clinical care was highlighted in the Feb 2011 issue of Canadian Living. It is a pertinent example of how Paula approaches nursing care. “It’s about combining what you as the patient see as your health needs with my knowledge and expertise, to reach the best outcome for you” Paula describes.

Paula has assumed a leadership role to organize NPs starting up Nurse Practitioner Led Clinics this past year. She links these highly motivated innovators and provides support for their collective journey to create a new primary care option for Ontarians through regular teleconferences. Additionally, she also co-chairs with Debra Bournes (Ontario’s Chief Nursing Officer) the Nursing Secretariat’s Nurse Practitioner Led Clinic Steering Group.

We thank Paula for her dedication and commitment in providing both exemplary patient centred care and in leading Ontario nurse practitioners to new relationships and partnerships, to new legislation and regulation, and opportunities for continuing education. Her efforts have helped to ensure that all Ontarians will have access to a new type of health care that truly puts the patient in the centre of the health care team.

Jerry Gerow Nurse Practitioner Leadership Award – Application

Established in 2005, NPAO’s most prestigious award recognizes the significant contributions of Jerry Gerow to the profession as a pioneer in NP practice in Ontario and to the Association as one of the founding members. NPAO provides a $1500 award to a member who has demonstrated exceptional leadership to the Association and in NP practice. Funding is intended to support post-graduate education or attendance at a national or international nursing conference. This is a peer nomination.

As a result of the success of our Annual Conference, NPAO offers four bursaries of $1000 to members of NPAO who are pursuing further education at the graduate or post-graduate level. Over forty members have benefited from this funding to pursue their NP certificate or post-graduate education.

NPAO Presidents Award (New this year) - Application

A new award of $1000 was established this year by the NPAO President to honour NP innovations. NPAO members are invited to apply for this award to support NP innovations.

The AstraZeneca Award for Innovation in Chronic Disease Management – Application

Provides a member of NPAO with a $5000 grant to develop strategies to optimize the management of chronic disease and improve quality of life. Members of NPAO are encouraged to submit proposals that will support implementation of an innovative project that will influence how CDM is integrated into their practice setting or community/region. Initiatives that support the development, implementation or integration of tools, resources or strategies to enhance patient self-management or interprofessional care are encouraged. The project must be completed by November 1, 2011 and the award recipient is expected to participate in at least one dissemination strategy.

Huronia Nurse Practitioner Network Bursary – Application
