    As a Patient it is your Right

    1. To be informed of your rights.

    2. To be given care regardless of your race, color,national origin, religion, age, gender, sexualorientation, ability or disability, or lifestyle.

    3. To be given care that respects your individualcultural, psychosocial, spiritual, personal values,beliefs and preferences and which promotes your

    dignity, privacy, safety and comfort.

    4. To be given care without verbal, physical,sexual or emotional abuse, annoyance or neglect.

    5. To be placed in restraints or isolation onlywhen it is medically necessary to improve your well-being, protect you from harm and/or prevent youfrom harming others and to have you or your familyinformed.

    6. To have your pain well managed, and help indeveloping a plan to manage your pain.

    7. To be told about the condition of your health,including descriptions of your illness and types oftreatment available, their expected results andpossible risks, benefits, alternatives and costs, andto be part of your health care decisions.

    8. To expect that we will talk with you in a waythat you can understand.

    9. To help plan your care and treatment, includingmaking decisions regarding your care. To accept orrefuse recommended tests or treatments or requesttreatment; and to bring up issues of concern aboutyour care with your caregivers.

    10. To get information and get help withadvance directives, so we know what treatment youdo or do not want if you cant tell us yourself.These include a living will, power of attorney forhealth care (where you pick someone to speak foryou) or mental health treatment preferencedeclaration. To expect that your wishes will befollowed within the law and the hospitals ability. Tolet the hospital appoint a person to make decisionsabout your care if you have not already pickedsomeone.

    11. To receive health care that treats the wholeperson by addressing your spiritual, emotional andphysical needs.

    12. To get information about and access toneeded resources and services those protects andhelp you.

    13. To be told the name of the doctor or otherstaff that are responsible for your care.

    14. To have visitors, mail and telephoneservices unless you have been informed that thereare sound medical or institutional reasons to restrictsuch access.

    15. To be told of the risks and benefits of anyproposed research or experimental treatment thatmay be considered in your care, and to agree ordisagree to participate

    16. To express complaints about the quality ofcare or service without fear, and to get a promptand courteous answer to your concerns.

    17. To get information contained in yourhospital records within a reasonable time after you

    ask for them and to be certain that other rightsconcerning your hospital record, as described in thehospitals Notice of Privacy Practices, will berespected.

    18. To have your insurance plan billed for yourhospital services, and to receive financialcounseling. This may include information about anycharges that you may have to pay, an explanationof your bill, if you ask for it, or information onavailable financial assistance.

    19. To be told how to get financial assistance, ifyou need it.

    20. To be told when you are ready to leave thehospital. If you think you are being asked to leavethe hospital too soon, you may ask to talk to yourcaregivers about this.

    21. To have a family member or a person ofyour choice, and you doctor, promptly notified thatyou are in the hospital

    22. To understand that medical and personalinformation will be handled in a private manner.

    23 To be given care in a safe setting that respectsyou and maintains your dignity.

    24. To receive the best and safest care that wecan provide.

    Aspatient it is Your Responsibility

    1. To provide all personal and family healthinformation needed to give you the best carepossible.

    2. To help make the best decisions about yourmedical treatment, to follow instructions and tofollow your plan of care. To accept and

    understand what may happen if you do notfollow your plan of care.

    3. To talk to your doctor or other care provider ifyou desire a transfer of care to anotherdoctor, caregiver or facility.

    4. To ask questions of your doctor or other careproviders when you do not understand informationor instructions.

    5. To show respect and be considerate of othersgetting and providing care.

    6. To follow the rules about smoking, noise andthe number of visitors.

    7. To understand that your health care servicesmust be paid for and to work with thehospital on a payment plan, as described in the

    hospitals Statement of Patient Billing Rightsand Responsibilities.

    8. To tell your doctor or other hospital staff if youneed to apply for financial assistance.
