Appendix A: SWPPP Template Instructions

To help you develop the narrative section of your construction site SWPPP, EPA has created this electronic SWPPP template. The template is designed to help guide you through the SWPPP development process and help ensure that your SWPPP addresses all the necessary elements stated in your construction general permit. You should use this template with EPA’s guidance on Developing Your Stormwater Pollution Prevention Plan. Both are available on EPA’s website at www.epa.gov/npdes/swpppguide

This template covers the SWPPP elements that most state construction general permits require, however, you are strongly encouraged to customize this template. There are two major reasons to customize this template:

• To reflect the terms and conditions of your construction general permit

1.3 Nature and Sequence of Construction Activity

This project is proposing to fill and grade less than six acres of an undeveloped out-lot. The proposed sequencing of the construction activities is as follows:

1. Installation of soil erosion and sediment control SE/SC measures:

a.) Selective vegetation removal for silt fence installation.

b.) Silt fence installation.

c.) Construction fencing around areas not to be disturbed.

d.) Stabilized construction entrance.

2. Tree and shrub removal where necessary (clear & grub).

3. Construct sediment trapping devices (sediment traps).

4. Expose existing tile and prepare proposed tile plan to be submitted to the Village within 30 days after completion of items 1, 2 & 3 above.

5. Strip topsoil, stockpile topsoil & grade site.

6. Temporarily stabilize topsoil stockpiles (seed and silt fence around toe of slope).

7. Install storm sewer, sanitary sewer, water and associated inlet & outlet protection.

8. Temporarily stabilize all areas including areas of the site that have reached temporary grade.

9. Install structure(s) and complete final grading/topsoil spreading of all areas.

10. Permanently stabilize all areas.

12. Remove all temporary SE/SC measures after the site is stabilized with 70% vegetation.

1.4 Soils, Slopes, Vegetation, and Current Drainage Patterns

Soil type(s):

• Beecher silt loam (298B)

• Ashkum silty clay loam (232)

• Markham silt loam (531C2)

Slopes (describe current slopes and note any changes due to grading or fill activities):

• The current slopes are 0.3-6 percent.

• Post construction will grade the crown of the fill pad at 1-2 percent. All overland drainage routes shall be a minimum of 1.5 percent.

Drainage Patterns (describe current drainage patterns and note any changes dues to grading or fill activities):

• In the current condition the property drainage is split north (3.53ac) and south (2.38ac). The north is conveyed to a storm sewer system and the south is conveyed to culvert under Regan Lane and into a subdivision lake/detention basin.

• Proposed conditions split the property drainage north (2.47ac) and south (3.46ac). Outfall conditions remain the same from existing to proposed condition.

Vegetation:

• Current ground cover is lawn with wet areas that do not sustain good ground cover. Many areas of the site are poorly drained and vegetation is poor.

• The proposed plan will establish good ground cover throughout the property. Slopes of drainage swales will be graded in a manner to allow good vegetative growth.

Description of receiving waters: Runoff is tributary to the Squaw C reek sub-watershed and then flows into the Fox River Watershed.

Description of storm sewer systems: The north area is tributary to a storm sewer system that conveys drainage south to Countryside Glen Lake. The south area is conveyed by swale to a culvert under Regan Lane and then into Countryside Glen Lake.

SECTION 2: EROSION AND SEDIMENT CONTROL BMPS A Designated Erosion Control Inspector (DECI) for the Development shall complete inspections a minimum of every 7 calendar days, after a 0.5” rainfall event, upon completion of the installation of the sediment and runoff control measures and after the completion of each phase of the construction process. The practices shall be maintained as deemed necessary by the DECI for the Development.

2.1 Minimize Disturbed Area and Protect Natural Features and Soil: Silt fence is proposed at the limits of disturbance and therefore shall protect areas that are not to be disturbed. Topsoil stock pile has a designated area and shall be wrapped at the perimeter with silt fence and stabilized with temporary seeding when not actively being used. Inspections shall be a minimum of every 7 calendar days, after a 0.5” rainfall event, upon completion of the installation of the sediment and runoff control measures and after the completion of each phase of the construction process.

2.2. Phase Construction Activity: • Phase I

The installation of perimeter controls, stabilized construction entrance, inlet protection, ditch checks and sediment traps.

Anticipated start 9-1-08: end date: Beginning of Phase II. BMP’s shall be as indicated on the Soil Erosion and Sediment Control

Plan(s) Temporary stabilization shall occur in all areas of disturbance within 14

days in areas of the site that will not be worked within 21 days. • Phase II

This phase shall include mass grading and filling of the property and installation of the storm sewer.

Duration of phase; start date: End of Phase I; end date: beginning of Phase III

BMP’s shall be as indicated on the Soil Erosion and Sediment Control Plan(s)

Temporary stabilization shall occur in all areas of disturbance within 14 days in areas of the site that will not be worked within 21 days. All areas where construction/grading has been completed shall be re-spread with topsoil and permanently stabilized.

• Phase III

Topsoil re-spread and permanent stabilization of all disturbed areas. Removal of all temporary sediment control measures not necessary to prevent sediment from leaving the site.

Duration of phase; start date: End of Phase II; end date: 11-1-08

Upon establishing 70% coverage of all disturbed areas the remaining temporary control measures shall be removed and all disturbance stabilized.

2.3 Control Stormwater Flowing Onto and Through the Project:

• BMP Description: Silt fence

• Installation Schedule: Phase I (9-1-08)

• Maintenance and Inspection: Repair construction damaged and knocked down areas. Remove and replace silt fence that has deteriorated from sunlight. Remove sediment as it accumulates. Inspections shall be a minimum of every 7 calendar days, after a 0.5” rainfall event, upon completion of the installation of the sediment and runoff control measures and after the completion of each phase of the construction process.

• Responsible Staff: Earthwork Contractor

2.4 Stabilize Soils:

• BMP Description: Temporary seeding – Used to provide a stabilized ground cover prior to final grading and stabilization.

Permanent Temporary

• Installation Schedule: Following mass grading when areas will not be reworked within 21 days of the last hydrologic disturbance.

• Maintenance and Inspection: The temporarily stabilized areas shall be inspected a minimum of every 7 calendar days and after a 0.5” rainfall event. Maintenance shall be re-seeding as necessary for those areas that vegetation does not establish.

• Responsible Staff: Grading Contractor

• BMP Description: Erosion Control Blanket – To be installed in the bottom of swales as indicated on the Erosion Control Plan.

Permanent Temporary

• Installation Schedule: Immediately upon completion of the final grading of the swales.

• Maintenance and Inspection: Inspection shall be a minimum of every 7 calendar days, after a 0.5” rainfall event. Damage such as lose ends,

• Installation Schedule: At completion of mass grading and after topsoil re-spread and seeding.

• Maintenance and Inspection: Maintenance shall include the reapplication of mulch that has been displaced. Inspection shall be a minimum of every 7 calendar days, after a 0.5” rainfall event and after wind storms.

• Responsible Staff: Grading Contractor

• BMP Description: Permanent Seeding – Where final grading has been completed permanent seeding accomplished.

Permanent Temporary

• Installation Schedule: At completion of final grading and after topsoil re-spread.

• Maintenance and Inspection: Maintenance shall include the reapplication of seed for areas that have not established cover. Inspection shall be a minimum of every 7 calendar days, after a 0.5” rainfall event and after wind storms.

• Responsible Staff: Grading Contractor

2.5 Protect Slopes:

• BMP Description: All slopes will be graded at 4(H) to 1(V) or flatter so as to prevent erosive slopes. Grading/construction of the slopes shall be completed in a transverse direction to the slope.

• Installation Schedule: Slopes to be constructed during mass grading.

• Maintenance and Inspection: Slopes shall be monitored for erosion and re-grading completed as necessary. Inspection shall be a minimum of every 7 calendar days, after a 0.5” rainfall event and after wind storms.

• Responsible Staff: Grading Contractor

2.6 Protect Storm Drain Inlets:

• BMP Description: Inlet Baskets

• Installation Schedule: Phase I - 9-1-08 (to be installed prior to commencing of earthwork)

• Maintenance and Inspection: Maintenance shall include the removal of sediment from the product or replace fabric as necessary. Inspection shall be a minimum of every 7 calendar days, after a 0.5” rainfall event.

• Responsible Staff: Grading Contractor

2.7 Establish perimeter controls and sediment barriers:

• BMP Description: Check Dams

• Installation Schedule: Following mass grading when areas will not be reworked within 21 days of the last hydrologic disturbance.

• Maintenance and Inspection: Maintenance shall include the removal of sediment as it accumulates from behind the ditch check and replacement of the ditch check if needed. Inspection shall be a minimum of every 7 calendar days, after a 0.5” rainfall event.

• Responsible Staff: Grading Contractor

• BMP Description: Silt Fence – Trenched geotextile fabric upheld by posts to intercept sediment from sheet flow runoff.

• Installation Schedule: Phase I - 9-1-08. To be installed prior to commencing of earthwork and additional Silt Fence installed as needed during construction.

• Maintenance and Inspection: Remove and replace any areas that have been deteriorated by sunlight. Repair blowout areas, wind blown and torn, knocked down and undercut areas. Inspection shall be a minimum of every 7 calendar days, after a 0.5” rainfall event and after wind storms.

• Responsible Staff: Grading Contractor

2.8 Retain Sediment On-Site and Control Dewatering Practices:

• BMP Description: Sediment Trap – An excavated or earthen berm constructed area to hold sediment laden stormwater runoff and allow sediment removal.

• Installation Schedule: To be installed during the mass grading or immediately following earthwork as necessary.

• BMP Description: Dewatering Structure Perforated Riser – Perforated PVC with non-woven filter fabric wrapped around mesh (chicken wire or hardware cloth).

• Installation Schedule: To be installed during the mass grading or immediately following earthwork.

• Maintenance and Inspection: Remove sediment as need to maintain necessary volume. Replace filter fabric and stone as necessary. Inspection shall be a minimum of every 7 calendar days, after a 0.5” rainfall event

• Responsible Staff: Grading Contractor

• BMP Description: Street Cleaning

• Installation Schedule: To be completed as necessary during the entire construction process.

• Maintenance and Inspection: All public roads where access is taken shall be inspected at the start and end of each active construction day. Sediment shall be removed as needed. During non-active construction phases inspection shall be a minimum of every 7 calendar days and after a 0.5” rainfall event.

2.9 Establish Stabilized Construction Exits:

• BMP Description: Stabilized Construction Entrance – Filter Fabric with coarse stone/aggregate is placed at the egress/ingress of the construction traffic to remove sediment and prevent off-site tracking.

• Installation Schedule: Phase I - 9-1-08 To be installed prior to commencing of earthwork/mass grading.

• Maintenance and Inspection: The condition and the functionality shall be inspected at the start and end of each active construction day. Mr. Nick Philippas will conduct inspections for all areas of the site disturbed by construction activities, areas used for storage of materials that are exposed to precipitation, discharge points, and construction exits.

• In absence of Mr. Nick Philippas, an associate will complete the necessary inspections

• If corrective actions are identified by Mr. Nick Philippas during the inspection, he will prepare an inspection report take corrective actions within 24 hours of the report and completing maintenance as soon as possible or before the next storm event. For any corrective actions requiring a SWPPP amendment or change to a stormwater conveyance or control design, Mr. Philippas will notify Reed Engineering soon as possible, before initiating the corrective action.
