• Control de sangrado. Evitar hipertensión e hipercapnia.• VA difícil: Fijación tubo, prevenir desconexiones.• Cubrimiento antibiótico: Ruptura dural.
Prevención PONV
• Ondansetron 0,15 mg/kg. Dexametasona 0,15 mg/kg + Onsansetron 0,05 mg/kg.
Evitar desplazamiento del injerto.
• PONV, analgesia, sangrado, pérdida de LCR.

• Inducción: Inhalada o IV. Propofol: VA normal.• Fentanil: 1 – 2 mcg/kg. Analgesia.• Mantenimiendo VA: IOT o LMA reforzados.• Normocapnia: Retraso de la Vent. espontánea.• Analgesia: AINES, Tramadol. Evitar Opioides de LDA.
Postoperatorio
• Vómito y Hg: > incidencia primeras 6 hrs.• Hg POP: 1-2% en 24 hrs. 0,06% necesita Anest. gral. hemostasia. • Presencia continua de sangre en vómito: Revisión.• < 4 años: Riesgo de obstrucción 1as 24 hrs.
