JUDGING PANEL
CLASS PRIZES Qualifying score required for all prizes offered in all classes. A rosette will be awarded for 1st through 4th places in each height division of all Standard,Jumpers with Weaves, and Premier classes. All dogs entered will receive 1 dog treat or toy for the weekend. A dark green qualifying ribbon will be awarded to each dog receiving a qualifying score in any class. A MACH/PACH bar/rosette will be given to any dog completing a MACH/PACH title. A PAX rosette will be given to any dog completing a PAX title. A New Title rosette will be given to any dog completing an agility title. A Double Q ribbon will be given to any dog qualifying in both JWW & Standard.
All prizes are offered by the Hamilton Dog Training Club unless otherwise noted.
All prizes must be claimed at the trial; none will be mailed.
Unclaimed prizes become the property of HDTC.

Make checks or money orders payable to the Hamilton Dog Training Club, or HDTC. Checks will be deposited after final closing date. If you are sending your entry by means other than the regular U.S. Mail, you MUST give permission for delivery without signature. Before sending your entry, please see the page facing the entry form for important in-formation. No refunds will be made for overpayments of $7.00 or less.
ENTRIES & CHECK HANDLING
Wait list after the closing date/time: If entry limits are reached prior to closing, HDTC will offer a Wait List to fill openings created by with-drawn entries after the trial’s closing date/time. A maximum of 25 dogs will be placed on the club’s wait list. All entries, including wait listed entries, must be received by the Trial Secretary prior to event closing (noon—Sept. 5). Any withdrawals that are received by noon on Monday, Sept. 17th, will be replaced by a wait listed entry if any are available. If a withdrawn entry is replaced by a wait listed entry, a full refund will be issued for the withdrawn entry. Any wait listed entry that does not move off the wait list into the trial by noon, Sept. 17th, will receive a full refund.
WAIT LIST
Pre-trial move-ups will be allowed with written notice in the hands of the Trial Secretary by noon pm, Monday, Sept. 17. Move-ups can be submitted on the form contained in the premium list or by email. Email notice must include dog’s call name, owner’s first and last name, and the class moved up to. Overnight move-ups will be accepted for dogs completing titles on Fri-day or Saturday. Move-up forms (available from the Trial Secretary) must be completed and turned in by the completion of judging on the day preceding the move-up.
MOVE UPS
Overnight camping is permitted. Self contained units only. No hook ups and no facilities outside of show hours. Please contact Diana Reeb at reebdm@yahoo.com to request a camping spot. Camping reserva-tions must be made before Monday, Sept. 17th. Openings are strictly limited. To ensure a spot, make your reservation early. When making a reservation, include all details about your camper (size, etc.). --> Absolutely NO DUMPING of ANY waste water at the site ! --> DO NOT just show up and expect to find a space available!!! You MUST reserve a spot by May 4th due to space limitations !!

Trial will be held indoors in a climate-controlled facility. Ring size is 97' x 85'; running surface is 3/4" synthetic turf.
No grooming is permitted inside the building. The handler will be solely responsible for having the dog at ring-
side when scheduled. If a conflict is anticipated, handler must notify the judge/gate steward prior to the start of judging for that class and/or jump height. The judge is not required to wait for any dog holding up a class. Please have consideration for your fellow competitors.
If, because of riots, civil disturbances, or other acts beyond the control of HDTC, it is impossible to open or to complete the tri-al, no refund of entry will be made. HDTC will not be responsi-ble for loss or damage to property while on show site.
Exhibitors should follow their veterinarians’ recommendation to assure their dogs are free of internal and external parasites, any communicable diseases, and have appropriate vaccinations. Ohio law requires proof of current rabies vaccination.
The trial is open to all dogs fifteen (15) months of age or older that are registered with the American Kennel Club or that have AKC Limited Registration, Purebred Alterna-tive Listing/Indefinite Listing Privileges (PAL/ILP), or an AKC Canine Partners listing number, or approved Founda-tion Stock Service (FSS) breeds.
ALL DOGS MUST BE ON LEASH, CRATED, SECURELY PENNED, OR OTHERWISE UNDER RELIABLE PHYSICAL CONTROL AT ALL TIMES WHEN NOT COMPETING OR WARMING UP.
Crating in designated areas only. Plastic covering must be placed under all crates in crating area.

If your dog needs to be measured, you must check the ap-propriate box on the entry form. It is the exhibitor’s responsi-bility to insure that the dog obtains any required measurements. Qualifying scores and awards may be cancelled by AKC for any dog which has not had a required measurement. You no longer need to submit a copy of the height card with your entry. If no VMO is present, a measurement will be done by the judge; this measurement will be valid for all three days for this weekend only. If a measurement is necessary, it is the exhibitor’s responsi-bility to present the dog for measuring to either judge, or VMO - if available.

MEASURING
Bitches in season: all but $3 (for administrative fees) of entry fee will be refunded. Written documentation from a veterinarian must be presented prior to the start of judging on the first day the dog is entered. Serious illness/injury to dog/handler: All but $7 (for ad-ministrative fees) per run will be refunded. Written documentation from veterinarian/physician must be delivered to the Trial Secretary within one (1) week after the trial weekend. Such refunds will be mailed 3-4 weeks after the trial.
REFUND POLICY
HDTC does not agree to arbitrate claims as set forth on the official AKC entry form for these events.
Please print legibly on entry forms, taking special care to note that your email address will be clear to someone who doesn’t al-ready know what it is. Use a valid email account only!
IN CASE ENTRY CAN ONLY BE PARTIALLY FULFILLED, Trial Secre-tary will honor requests to return entries if one of the requested classes is unavailable. Such requests MUST be indicated on the back of the entry form.
Except as noted above, no entry will be accepted which carries any conditions regarding its acceptance.
Entries received without fees will not be accepted. Post-dated checks, unsigned checks, and returned checks do not constitute payment of fees. A $30 service fee will be assessed for any re-turned checks. Make sure checks are payable to HDTC !
Entries MUST be submitted on an official AKC entry form including the COMPLETE “Agreement” page. No phone, fax, or scanned entries will be accepted.
Unsigned, incomplete, or illegible entries WILL be returned, as will entries received prior to 8:00am Monday, March 19, 2018.
FOR MIXED BREED DOGS: Use breed name All-American; no other breed description may be used to identify a mixed breed entry. Enter the Canine Partner listing num-ber and check the AKC No. box on the form.
Entries for a maximum of ten (10) dogs are allowed in each en-velope submitted to the Trial Secretary for random draw. Enve-lopes containing entries for more than the ten (10) dog maxi-mum shall be treated as an invalid entry and shall be “Returned to Sender”.
JUNIOR HANDLERS must complete the information on the front of the entry form, and read the back of the entry form. Junior Handlers must include junior handler number in the box labeled “Handler”. If you do not have a junior number, contact AKC (see back of entry form) or the Trial Secretary for an application for Junior Handler number.
Be sure to clearly and correctly indicate whether you are entering regular classes or preferred. A dog must show either in regular classes only, or preferred classes only throughout the course of the weekend; no day to day changes are permitted by AKC regu-lations.
PLEASE CHECK YOUR ENTRY BEFORE SENDING IT ! Most common problems are missing signatures, missing registration numbers, and not including the “Agreement” page. Any or all of these oversights will result in your entry being returned to you. So PLEASE check carefully!

INSTRUCTIONS
Handlers whose dogs measure into the 8” through 20” regular jump height divisions may opt to enter in a higher or 24 -choice height division for all Regular titling classes, but not in a division lower than their proper height division. Dogs who measure into the 24” regular jump height division may not enter 24-choice. Dogs entered in the Preferred classes must compete at the required jump height
and may not compete in a jump height division higher or lower than their proper height division.
All dogs that have not been officially measured for an Agility Jump Height Card may still compete; however, they must be measured by a Judge of record, an Agility Field Representative, or a Volunteer Measuring Official, prior to running. If measured by the Judge of record, that measurement will be valid for this trial or back-to-back trials only. If a measurement is necessary, it is the exhibitor's respon-
sibility to have their dog(s) measured prior to running.
Dogs listed in the AKC Canine PartnersSM program may be eligible to enter all-breed AKC agility trials at the club’s option. These dogs
should be listed as All American Dog and must include their AKC number on the entry form and check the AKC No. box.

AGREEMENT
I certify that I am the actual owner of the dog, or that I am the duly authorized agent of the actual owner whose name I have entered. In consideration of the acceptance of this entry, I (we) agree to abide by the rules and regulations of The American Kennel Club in effect at the time of this event, and any additional rules and regulations appearing in the premium list of this event and entry form and any decision made in accord with them. I (we) agree that the club holding this event has the right to refuse this entry for cause which the club shall deem sufficient. I (we) certify and represent that the dog entered is not a hazard to persons or other dogs. In consideration of the acceptance of this entry and of the holding of this event and of the opportunity to have the dog judged and to win prizes, ribbons, or trophies, I (we) agree to hold the AKC, the event-giving club, their members, directors, governors, officers, agents, superintendents or event secretary and the owner and/or lessor of the premises and any provider of services that are necessary to hold this event and any employees or volunteers of the aforementioned parties, and any AKC approved judge, judging at this event, harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of this dog while in or about the event premises or grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim; and I (we) further agree to hold the aforementioned parties harmless from any claim for loss, injury or damage to this dog. Additionally, I (we) hereby assume the sole responsibility for and agree to indemnify, defend and save the aforementioned par-ties harmless from any and all loss and expense (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death at any time resulting therefrom, sustained by any person or persons, including myself (ourselves), or on account of damage to property, arising out of or in consequence of my (our) participation in this event, however such, injuries, death or property damage may be caused, and whether or not the same may have been caused or may be alleged to have been caused by the negligence of the aforementioned parties or any of their employees, agents, or any other person. I (WE) AGREE THAT ANY CAUSE OF ACTION, CONTROVERSY OR CLAIM ARIS-ING OUT OF OR RELATED TO THE ENTRY, EXHIBITION OR ATTENDANCE AT THE EVENT BETWEEN THE AKC AND THE EVENT-GIVING CLUB (UNLESS OTHERWISE STATED IN THIS PREMIUM LIST) AND MYSELF (OURSELVES) OR AS TO THE CONSTRUCTION, INTERPRETATION AND EFFECT OF THIS AGREEMENT SHALL BE SETTLED BY ARBITRA-TION PURSUANT TO THE APPLICABLE RULES OF THE AMERICAN ARBITRATION ASSOCIATION. HOWEVER, PRIOR TO ARBITRATION ALL APPLICABLE AKC BYLAWS, RULES, REGULATIONS AND PROCEDURES MUST FIRST BE FOLLOWED AS SET FORTH IN THE AKC CHARTER AND BYLAWS, RULES, REGULATIONS, PUBLISHED POLICIES AND GUIDELINES.
INSTRUCTIONS
Handlers whose dogs measure into the 8” through 20” regular jump height divisions may opt to enter in a higher or 24 -choice height division for all Regular titling classes, but not in a division lower than their proper height division. Dogs who measure into the 24” regular jump height division may not enter 24-choice. Dogs entered in the Preferred classes must compete at the required jump height
and may not compete in a jump height division higher or lower than their proper height division.
All dogs that have not been officially measured for an Agility Jump Height Card may still compete; however, they must be measured by a Judge of record, an Agility Field Representative, or a Volunteer Measuring Official, prior to running. If measured by the Judge of record, that measurement will be valid for this trial or back-to-back trials only. If a measurement is necessary, it is the exhibitor's respon-
sibility to have their dog(s) measured prior to running.
Dogs listed in the AKC Canine PartnersSM program may be eligible to enter all-breed AKC agility trials at the club’s option. These dogs
should be listed as All American Dog and must include their AKC number on the entry form and check the AKC No. box.
Junior Handlers should enter their Junior Handler number on the front of this form. Should you not have your Junior Handler number, it may be obtained from the American Kennel Club (919) 233-9767.
