PDGM PROVIDER ACTION PLANPurpose: To effectively plan for PDGM, every provider must understand its agency-wide strengths, weaknesses, market opportunities and business threats based on an analysis of market data, clinical data, operating history, financial condition and components of the payment structure that will most affect operations. Based on analysis of agency characteristics, a work plan with measurable goals must be developed and adopted.

Analyze SNF discharges and LOS, determine SNFs with high readmits and target them.

Identify operating hurdles and contingency plans.
