Your Name and Contact Information• Your name• Your address• One phone number• One professional email address• Your Social Security Number (memorize it)
I-9 Documents
• Must have:• A document to establish Identity, such as a State
issued photo ID or Drivers License, or a School ID
AND• A document to establish Right to Work, such as a
Social Security card, Birth Certificate, US Citizen ID
OR
• A document that establishes both Identity and Right to Work, such as a US Passport or Permanent Resident card
Work History
• Job Title• Company• Address / Phone Number• Name of Supervisor• Start date / End date• Starting pay / Ending pay• 3-5 Duties / Responsibilities / Skills• Reason for Leaving• May we contact this employer? (Yes or No)
Self-employed
• Includes Odd Jobs (babysitting, dog walking, yard work, etc.)
• YOU are the employer, “Self-Employed”• Include your Customers as “References”
