FA requests Survey Link for the unit to be surveyed
FA must provide the number of unit survey participants
FA receives the HSOPS link via e-mail
Participant reporting allows for percent complete real time
The link is unique to the unit only; not to any unit members
FA provides the link to the survey participants via e-mail
Surveys are considered complete upon submission
Unit members must complete the survey within 2 weeks

If a user tries to log in with either an invalid username or password, they will receive a notice stating: "The username and/or password you entered is incorrect. Please check your username and password and re-enter. Thank you."
Clicking the "I Forgot My Password" link will send an email containing the user's new password.
Data Entry User Test ScriptsDescription
New users will be sent an email with a default password. The user will be required to change their password the first time they log in, and will be taken to a change password page.
In the "My Units and Forms" tab, view a list of all forms available to be completed for a unit.
After opening a form for data entry, users will be able to both exit with saving and exit without saving any changes.
In the "My Units and Forms" tab, fill out an unlocked ICU Team Checkup Tool form from an existing unit and facility.
In the "My Units and Forms" tab, fill out an unlocked ICU CLABSI form from an existing unit and facility.
In the "My Units and Forms" tab, fill out an unlocked ICU VAP form from an existing unit and facility.
