Background

Initiative updates: • FY 2014 - 59% of Health centers are recognized as a

PCMH

• 23 % of Health centers hold accreditation

• Continuing support for health centers seeking accreditation and PCMH recognition

• Participation in the Initiative is voluntary

• Notice of Intent (NOI) is needed to participate in the Initiative.

• Look-a-Likes are no longer eligible financial support for accreditation or PCMH recognition

• New procedures for completing and submitting a NOI

Getting Started

Please have the following information before starting:

• Health center grant number or H80 number

• Full name and address of the health center and BHCMIS ID located in box 9 on Notice of Award (NoA)

• BPS site ID number, name and address of each site applying for recognition

• Name of your HRSA project officer

• Alternate contact person and contact information for the recognition process

Things to Remember

• A Notice of Intent (NOI) is necessary to participate in HRSA’s Accreditation and PCMH Recognition Initiative

• Allow 2-3 weeks for HRSA to complete an internal review for organizational readiness

• Effective March 1, 2015 - NOI will be processed through the new system

• Step by Step instructions will be available

• 2015 Base Adjustment Awards for PCMH– Must be recognized on July 1, 2015 to be eligible– Health centers must submit survey tool to NCQA for

review no later than May 1,2015– Accreditation/PCMH on site surveys must be completed

no later that May 31, 2015
