1. I, Mike Seyler, states and declares as follows:

2. I am over the age of 18, competent to testify to the matters set forth herein, and testify

based on my personal knowledge.

3. I am a process server licensed in King County #549883. I was hired by plaintiff on April

27, 2014 to serve John and Amber Du Wors.

4. I reviewed an affidavit of service on John and Amber Du Wors in the Schweickert v. Du

Wors matter, Case No. 2:13-CV-00675 RSM on May 16, 2013 (Exhibit A). In preparing

to serve the Du Wors, I also reviewed a Skip Trace Report from Gary’s Process Service,

(Exhibit B) and John Du Wors’ criminal citation for Unlawful Imprisonment Attempt and

Obstruction Law Enforcement Officer; Online Property Records, and DMV Records.

5. After researching for potential addresses, I followed up with cross referencing the

addresses with other sources that I use to locate people and determined the following

addresses noted below as the most likely locations for his personal abode.

6. On April 29, 2014, I travelled to Bainbridge Island and went to the following addresses to

serve John Du Wors and Amber Du Wors:

a. 8490 NE Meadowmeer Dr, Bainbridge Island, WA 98110

b. 14414 Madison Avenue NE, Bainbridge Island, WA 98110-1156

c. 6574 NE New Brooklyn Rd, Bainbridge Island, WA 98110-4285

7. I traveled to each address on April 29, 2014 and made an attempted service at each

location. I discovered that these addresses were vacant and empty.

8. I was unable to determine a residential location for John and Amber Du Wors.

I declare under penalty of perjury under the laws of the State of Washington that the foregoing is true and correct. EXECUTED this 26th day of June, 2014, at Seattle, Washington.
